目的本研究旨在分析P2Y12受体抑制剂联合质子泵抑制剂（PPI）治疗对经皮冠状动脉介入（PCI）术后的急性冠脉综合征患者缺血事件的影响。方法基于国际多中心回顾性注册登记研究，纳入2003至2014年因急性冠脉综合征人院行PCI术的患者，分为PPI组及非PPI组并随访1年，主要临床终点为全因死亡／再发心肌梗死的复合终点。根据P2Y12受体抑制剂种类，将入组患者分为氯吡格雷组及替格瑞洛组，并比较不同药物与PPI联用发生临床终点事件的风险。结果研究入选9429例患者，PPI组占54．8％，具有更多高危因素。Cox回归结果提示PPI组较非PPI组全因死亡／再发心肌梗死复合事件的发生差异无统计学意义（HR1．00，95％CI 0．86—1．18）。根据P2Y12抑制剂种类不同分为氯吡格雷组和替格瑞洛组，不同P2Y12受体抑制剂联用PPI较未联用PPI患者的临床终点无差异，联用PPI的氯吡格雷组与替格瑞洛组的临床终点差异也无统计学意义。结论急性冠脉综合征患者PPI与P2Y12受体抑制剂联用不增加全因死亡和再发心肌梗死风险，尤其PPI联用氯吡格雷在患者的缺血事件上与替格瑞洛比较差异无统计学意义。

尽管脑血管病的影响广泛且对其病理生理学的理解已取得一些进展，但“卒中”一词在临床实践、临床研究和公共卫生评价中的定义并不统一。经典的卒中定义主要是临床定义，不能反映科学技术的进步。因此，美国心脏协会卒中委员会／美国卒中协会召集了一个写作组，旨在对21世纪的卒中新定义制定一份专家共识文件。中枢神经系统（centralnervoussystem，CNS）梗死被定义为缺血导致的脑、脊髓或视网膜细胞死亡，并且具有永久性损伤的神经病理学、神经影像学和（或）临床证据。CNS梗死具有一系列临床表现：缺血性卒中特指伴有明显症状的CNS梗死，而无症状梗死则顾名思义不引起明显症状。广义的卒中还包括脑出血和蛛网膜下腔出血。卒中的新定义结合了临床和组织学标准，因此能被融入临床实践、研究和公共卫生评价。

Objective:To describe the prognostic and clinical profile of hospitalized patients with chikungunya virus(CHIKV)infection focusing on renal ***:This is a cross-sectional study including all patients with confirmed chikungunya fever(CHIKF)admitted to 3 different highcomplexity hospitals in Fortaleza,Brazil between january 2016 and june *** analysis was carried out to evaluate correlation between clinical profile and ***:Fifty-five patients were included,with a median age of 77(IQR=21)years,and 23(41.82%)were ***-five patients(45.45%,25/55)developed acute kidney injury(AKI),and 15(60.00%,15/25)were classified as KDIGO 1,1(4.00%)as KDIGO 2,and 9(36.00%)as KDIGO *** overall mortality was 34.54%whilst AKI-related mortality was 64.00%(16/25).Both AKI and encephalitis were associated with higher *** who died were significantly older[82(IQR=12)years vs.70(IQR=28.75)years,P<0.001].In the multivariate analysis,abdominal pain was associated with an increased risk of severe AKI(OR=5.33,95%CI=1.11–25.64,P=0.037)and AKI was an independent risk factor of death(OR=12.06,95%CI=2.55–57.15,P=0.002).Recovery of renal function was similar among the different age ***:AKI is present in half of the study population and is an independent risk factor of ***,renal function should be carefully monitored in hospitalized patients with CHIKV infection.

首次心肌梗死前二尖瓣反流对预后的影响

Aims Mitral regurgitation(MR) following an acute myocardial infarction(AMI) confers an adverse prognosis during long term follow- up. There are no studies evaluating the influence of pre- AMI MR in the short- and long- term prognosis of such patients. Our aim was to assess the prognostic value of pre- AMI MR in the short and long- term follow- up of patients who suffered a first AMI and to assess its influence on left ventricular haemodynamics. Methods and results Sixty- eight consecutive patients with a first AMI and an echocardiographic study before AMI(< 3 months) were included in the study. The pre- AMI echo was performed for various reasons. Of these 68 patients, 42 had pre- AMI MR(Group 1) and 26 showed no pre- AMI MR(Group 2). The presence of degenerative changes at the level of the mitral valve was confirmed in all cases. Patients with any other cause of MR were excluded. Clinical and echocardiographic variables for both phases(pre- AMI and post- AMI) were analysed and patients were followed up. Mean age was 75.5± 9.5 years; there were 38 males(55.9% ). There were no statistical differences in baseline clinical variables between the groups, except for the presence of pre- AMI atrial fibrillation, which was more frequent in Group 1(21.4 vs. 0% ; P=0.01 ). After AMI, only end- diastolic left ventricular diameter was significantly larger in Group 1(54.9± 4.7 vs. 48.1± 5.6 mm; P< 0.001). During long- term follow- up, median survival times were 912 days(interquartile range: 690 days) in Group 1 and 1423 days(interquartile range: 520 days) in Group 2(Log- rank P=0.02). The multi- variable analysis showed that the presence of pre- AMI MR relates to a statistically significant relationship with a worse post- AMI evolution[relative risk(95% confidence interval): 3.8(1.1- 13.1); P=0.037]. Conclusion The present study shows that the presence of pre- AMI MR is an independent prognostic marker among those patients suffering a first AMI.
