Quality of life before and after the Use of Vaginal Estriol in Postmenopausal Women with lower Urinary Tract Symptoms

Background: Nowadays, a substantial number of women spend more than one-third of their lives in the postmenopausal period. It is characterized by a marked decrease in the production of estrogen, which leads to urogenital atrophy. The symptoms stemming from vulvovaginal atrophy and the lower urinary tract are currently referred to as the Genitourinary Syndrome of Menopause (GSM), which can have a negative impact on the quality of life (Qol). Estrogen replacement is the ideal treatment for GSM, and vaginal administration is the most recommended. Objective: To assess the impact of the use of topical vaginal estriol on the quality of life (Qol) of postmenopausal women with lower urinary tract symptoms (lUTS). Methods: This is an interventional, prospective study, performed in 49 women at the Antonio Pedro Hospital, at the Universidade Federal Fluminense, in Niterói, Brazil, from August 2014 to April 2015. It included postmenopausal women with lower urinary tract symptoms who were not using any estrogen hormone therapy, and it excluded those with contraindications for the use of estriol. A specific questionnaire on Qol and urinary tract symptoms, the King’s Health Questionnaire (KHQ), which identifies lUTS and assesses to what extent those symptoms interfere with Qol, has been used as a research tool. Results: The average age was 62.24 years. Urinary urgency and urge incontinence were reported by 91.8% of women. The average scores of the domains of the KHQ decreased in the domains General Health Perception (before: 46.42 ± 21.65;after: 40.81 ± 22.64), Incontinence Impact (before: 74.82 ± 27.66;after: 41.49 ± 30.83), Role limitations (before: 43.20 ± 32.80;after: 21.09 ± 24.71), Physical limitations (before: 38.09 ± 32.09;after: 14.62 ± 24.20), Social limitations (before: 30.38 ± 28.75;after: 12.62 ± 19.85), Emotions (before: 43.31 ± 32.96;after: 20.18 ± 26.41), Sleep/Energy (before: 41.48 ± 37.74;after: 15.98 ± 23.31) and Severity Measures (before: 48.02 ± 24.68;after: 2

Prediction Equation of Body Weight of Amazonian Manatee (Trichechus inunguis) Calves in Captivity Using Biometry

The objective of the study was to determine a prediction equation of body weight of Amazonian manatee calves in captivity using their biometry. It was conducted out with 7 calves (4 males and 3 females) of approximately 8 months of age and average body weight of 29.94 ± 0.055 kg, arranged in pools of sufficient size. Biometry and weighing were performed periodically, with the following measurements: body weight (BW), total curved length (TCl), total length (Tl), circumference (CIR), fin width (FW), tail width (TW) and peduncle (PED). Data were subjected to Pearson correlation analysis and linear regression, using the statistical software IBM SPSS 24.0. The results showed a significant correlation (P R2 = 0.855, R2aj = 0.852). In addition, three linear multiple regression equations of BW were calculated using the predictor variables previously analyzed by Person correlation analysis. The equation that used all biometric measurements (TCl, Tl, FW, TW, CIR and PED) had the highest coefficient of determination and the lowest estimation error to predict BW. In conclusion, the biometric measurements of TCl, Tl, FW, TW, CIR and PED showed a high correlation with the BW and can be used as predictive variables of BW of manatee calves, as they are easy to be measured.

Implementation of a nurse-led protocol for early extubation after cardiac surgery: A pilot study

BACKGROUND Protocols for nurse-led extubation are as safe as a physician-guided weaning in general intensive care unit(ICU).Early extubation is a cornerstone of fast-track cardiac surgery,and it has been mainly implemented in post-anaesthesia care *** a nurse-led extubation protocol may lead to reduced extubation *** To investigate results of the implementation of a nurse-led protocol for early extubation after elective cardiac surgery,aiming at higher extubation rates by the third postoperative *** A single centre prospective study in an 18-bed,consultant-led Cardiothoracic ICU,with a 1:1 nurse-to-patient *** a 3-wk period,the protocol was implemented with:(1)Structured teaching sessions at nurse handover and at bed-space(all staff received teaching,over 90%were exposed at least twice;(2)Email;and(3)laminated sheets at *** compared“standard practice”and“intervention”periods before and after the protocol implementation,measuring extubation rates at several time-points from the third until the 24th postoperative *** Of 122 cardiac surgery patients admitted to ICU,13 were excluded as early weaning was considered ***,109 patients were included,54 in the standard and 55 in the intervention *** of surgical interventions and baseline left ventricular function were similar between *** the third to the 12th post-operative hour,the intervention group displayed a higher proportion of patients extubated compared to the standard ***,results were significant only at the sixth hour(58%vs 37%,P=0.04),and not different at the third hour(13%vs 6%,P=0.33).From the 12th post-operative hour time-point onward,extubation rates became almost identical between groups(83%in standard vs 83%in intervention period).CONClUSION The implementation of a nurse-led protocol for early extubation after cardiac surgery in ICU may gradually lead to higher rates of early extubation.

Dietary Monosodium Glutamate Does Not Affect the Electrocardiographic Profiles of Diabetic and Nondiabetic Wistar Rats

Background and Aims: Some studies have recently indicated that dietary monosodium glutamate (MSG) is linked to obesity and the development of diabetes. Both diseases induce cardiovascular changes, such as increases in blood pressure and arrhythmias, including ventricular fibrillation, which may result in sudden death. Here, we aimed to investigate the effects of oral MSG administration on the electrical conduction and histological dysfunctions of the heart in control and diabetic Wistar rats. Methods and Results: Twenty-one-day-old Wistar rats were fed diets containing 0.0%, 1.0%, 2.5% or 5.0% MSG for 70 days. After this period, diabetes was induced with streptozotocin (STZ;50 mg/kg bw) in half the rats and after an additional 21 days period;the electrocardiographic parameters and heart histology were evaluated. Diabetic rats demonstrated a reduction in heart rate as well as an enlargement of the QRS complex and QT and QTc intervals. Nevertheless, those changes are typical of STZ-induced diabetes, mainly because of electrolyte disturbances. The presence of MSG in the diet did not change the parameters evaluated between the group that received MSG and the group that did not receive MSG. Moreover, no histological alterations in the heart were observed due to MSG ingestion. Conclusion: Based on this evidence, diets containing MSG did not interfere with cardiovascular changes due to diabetes;there were no differences in the electrocardiographic and histological characteristics of the hearts of rats treated with MSG.

Demonstration of uncertainty resulting from MODerate resolution Imaging Spectroradiometer-like geometries:an albedo case study

This research examines uncertainty in MODerate resolution Imaging Spectroradiometer(MODIS)observations,and demonstrates the direct influence of geometric distortions resulting from the standard practice of geolocating swath *** observations vary dependent on the ground sample distance,which varies dependent on the view zenith angle that changes with every *** level 2G(l2G)land products are generated by applying a geolocation algorithm that resamples the variable observation geometries to a consistent grid of fixed pixel size and location,a process which itself introduces variability associated with the changing observational *** this study,broadband albedo was simulated for five validation sites,representing five distinct land cover types,exhibiting quantifiable variability,with additional seasonal variability exhibited in some *** site simulations exhibit compounded uncertainty attributable to the geometric distortion sufficient to influence climate models(*** from 0.01 to 0.045 albedo).These results indicate there is a minimum level of uncertainty associated with the variable geometry that should be factored into l2Gbased products,particularly for nominal 250 m band *** the data to coarser resolutions and smoothing the data through average resampling can mitigate the uncertainty.

Comparison of p53 and prohibitin expression in the spectrum of hepatitis, cirrhosis, and hepatocellular carcinoma

Aim:To investigate the correlation of p53 and prohibitin (PHB) expression in the spectrum of hepatitis, cirrhosis, and human hepatocellular carcinoma (HCC).Methods:Hepatic biopsies from patients with HCC (n = 60), cirrhosis (CIR,n= 30), hepatitis C virus (HCV,n = 30), and normal livers (Nl,n = 20) were examined for immunohistochemical expression of RelA/p65, tumor necrosis factor receptor-1 (TNFR1), TNF-related apoptosis-inducing ligand (TRAIl), p53, and PHB. The samples were also analysed by nuclear factor kappa B (NFκB) Southwestern histochemistry and a transferase-mediated dUTP-biotin nick-end labelling ***: Expression of NFκB and RelA/p65 was detected increasingly from Nl to CIR, but had a diminished labelling in the HCC cases (P levels of TNFR1 and TRAIl followed the same pattern (P ly reduced from CIR to Nl (P lear expressions were ampliifed in cases of HCC, but diminished in Nl, HCV, and CIR (Plusion: These results suggest that in addition to well-understood sequences of proinlfammatory events such as TNF-induced NFκB activation and NFκB/TRAIl pathway-mediated apoptosis, development of HCC is also inlfuenced by regulation ofp53 andPHB tumour suppressor function. Additional studies are necessary to explain the contradictory mechanisms of the tumour microenvironment observed in the sequence of HCV, CIR, and HCC.

Teaching Methodology for the Training of a laparoscopic Technique: Intestinal Anastomosis

Traditionally surgeons learned surgical techniques by training them directly in the patient by imitating the actions of the surgeon with more experience. Today this methodology is difficult to apply, since there are multiple factors that hinder its use. The limited time of training systems, scarcity of resources, patient safety or the need to learn new techniques of minimally invasive surgery, make the training of surgeons require a different learning methodology. This study has designed a new methodology for the learning of surgical techniques that minimizes the impact of all the previous factors on the training of surgeons. Its effectiveness in the teaching of laparoscopic manual intestinal anastomosis is analyzed. It has been asked a group of 24 expert surgeons teaching in intestinal anastomoses, which are the steps that must include the teaching of the technique. The phases evaluated with the highest score were included in the methodology. Once the steps of the methodology were designed, 25 general surgeons were chosen and divided into three groups according to their level of experience (experts, trained and beginners). Each participant performed five training sessions following the new methodology. The technical quality of the anastomosis, the overall development of the operation, as well as the feedback developed between the participants and the instructor was analyzed. After analyzing the results it was observed that structured feedback between the instructor and the participant increases the involvement of the participant in the learning process and reduces the time needed to acquire the competence. With this methodology the inexperienced participants perform the fifth anastomosis with quality parameters close to those obtained by the expert participants. In addition, this methodology allows to individualize the learning according to the needs of each participant.
