COVID-19 as a trigger of irritable bowel syndrome:A review of potential mechanisms

In December 2019 a novel coronavirus disease 2019(COVID-19),caused by the severe acute respiratory syndrome coronavirus-2(SARS-CoV-2),started spreading from Wuhan city of Chinese Hubei province and rapidly became a global *** symptoms of the disease range from paucisymptomatic disease to a much more severe *** symptoms of the initial phase include fever and cough,with possible progression to acute respiratory distress *** manifestations such as diarrhoea,vomiting and abdominal pain are reported in a considerable number of affected individuals and may be due to the SARS-CoV-2 tropism for the peptidase angiotensin receptor *** intestinal homeostasis and microenvironment appear to play a major role in the pathogenesis of COVID-19 and in the enhancement of the systemic inflammatory ***-term consequences of COVID-19 include respiratory disturbances and other disabling manifestations,such as fatigue and psychological *** date,there is a paucity of data on the gastrointestinal sequelae of SARS-CoV-2 *** COVID-19 can directly or indirectly affect the gut physiology in different ways,it is plausible that functional bowel diseases may occur after the recovery because of potential pathophysiological alterations(dysbiosis,disruption of the intestinal barrier,mucosal microinflammation,post-infectious states,immune dysregulation and psychological stress).In this review we speculate that COVID-19 can trigger irritable bowel syndrome and we discuss the potential mechanisms.

Coronavirus disease 2019(COVID-19)has several extrapulmonary ***(GI)symptoms are among the most frequent clinical manifestations of COVID-19,with severe consequences reported in elderly ***,the impact of COVID-19 on patients with pre-existing digestive diseases still needs to be fully elucidated,particularly in the older *** review aimed to investigate the impact of COVID-19 on the GI tract,liver,and pancreas in individuals with and without previous digestive diseases,with a particular focus on the elderly,highlighting the distinctive characteristics observed in this ***,the effectiveness and adverse events of the anti-COVID-19 vaccination in patients with digestive disorders and the peculiarities found in the elderly are discussed.

Predicting lower limb periprosthetic joint infections: A review of risk factors and their classification

AIM To undertook a systematic review to determine factors that increase a patient's risk of developing lower limb periprosthetic joint infections(PJI).METHODS This systematic review included full-text studies that reviewed risk factors of developing either a hip or knee PJI following a primary arthroplasty published from January 1998 to November 2016. A variety of keywords were used to identify studies through international databases referencing hip arthroplasty, knee arthroplasty, infection, and risk factors. Studies were only included if they included greater than 20 patients in their study cohort, and there was clear documentation of the statistical parameter used; specifically P-value, hazard ratio, relative risk, or/and odds ratio(OR). Furthermore a quality assessment criteria for the individual studies was undertaken to evaluate the presence of record and reporting bias. RESULTS Twenty-seven original studies reviewing risk factors relating to primary total hip and knee arthroplasty infections were included. Four studies(14.8%) reviewed PJI of the hip, 3(11.21%) of the knee, and 20(74.1%) reviewed both joints. Nineteen studies(70.4%) were retrospective and 8(29.6%) prospective. Record bias was identified in the majority of studies(66.7%). The definition of PJI varied amongst the studies but there was a general consensus to define infection by previously validated methods. The most significant risks were the use of preoperative high dose steroids(OR = 21.0, 95%CI: 3.5-127.2, P < 0.001), a BMI above 50(OR = 18.3, P < 0.001), tobacco use(OR = 12.76, 95%CI: 2.47-66.16, P= 0.017), body mass index below 20(OR = 6.00, 95%CI: 1.2-30.9, P = 0.033), diabetes(OR = 5.47, 95%CI: 1.77-16.97, P = 0.003), and coronary artery disease(OR = 5.10, 95%CI: 1.3-19.8, P = 0.017).CONCLUSION We have highlighted the need for the provider to optimise modifiable risk factors, and develop strategies to limit the impact of non-modifiable factors.

Upsampling Monte carlo neutron transport simulation tallies using a convolutional neural network

The physical quantities calculated by nuclear reactor Monte carlo simulations are typically recorded on a grid of two or three spatial dimensions and one dimension of neutron *** of this,increasing the resolution of the calculated quantities can have a significant impact on the memory and CPU time required to run a *** neural networks have been shown to accurately upsample coarse-resolution photo-graphic images to resolutions multiple times finer than the *** we show that a convolutional neural network can accurately upsample flux tallies in a Monte carlo neutron transport simulation by a factor of two along the spatial and energy *** flux tallies in pressurized water reactor assemblies were calculated using OpenMC at a 64×64 pixel spatial resolution and 8 neutron energy groups for input to the neural *** network upsamples the low-resolution neutron flux to 128×128 pixel spatial resolution and 16 neutron energy ***-resolution neutron flux tallies and their uncertainties were also calculated with OpenMC and compared with the network’s *** upsampled data and the high-resolution tally results agree to within the statistical uncertainty calculated by OpenMC.

In the last decade trans-arterial radioembolization has given promising results in the treatment of patients with intermediate or advanced stage hepatocellular carcinoma(HCC),both in terms of disease control and tolerability *** technique consists of the selective intra-arterial administration of microspheres loaded with a radioactive compound(usually Yttrium90),and exerts its therapeutic effect through the radiation carried by these microspheres.A careful and meticulous selection of patients is crucial before performing the radioembolization to correctly perform the procedure and reduce the incidence of *** is a technically complex and expensive technique,which has only recently entered clinical practice and is supported by scant results from phase Ⅲ clinical ***,it may represent a valid alternative to transarterial chemoembolization(TACE) in the treatment of intermediate-stage HCC patients,as shown by a comparative retrospective assessment that reported a longer time to progression,but not of overall survival,and a more favorable safety profile for *** addition,this treatment has reported a higher percentage of tumor shrinkage,if compared to TACE,for pre-transplant downsizing and it represents a promising therapeutic option in patients with large extent of disease and insufficient residual liver volume who are not immediately eligible for *** might also be a suitable companion to sorafenib in advanced HCC or it can be used as a potential alternative to this treatment in patients who are not responding or do not tolerate sorafenib.

The brokering approach can be successfully used to overcome the crucial question of searching among enormous amount of data （raw and/or processed） produced and stored in different information systems. In this paper, authors describe the Data Management System the DMS （Data Management System） developed by INGV （Istituto Nazionale di Geofisica e Vulcanologia） to support the brokering system GEOSS （Global Earth Observation System of Systems） adopted for the ARCA （Arctic Present Climate Change and Past Extreme Events） project. This DMS includes heterogeneous data that contributes to the ARCA objective （***） focusing on multi-parametric and multi-disciplinary studies on the mechanism （s） behind the release of large volumes of cold and fresh water from melting of ice caps. The use of anticoagulation therapy could prove to be controversial when trying to balance ischemic stroke and intracranial bleeding risks in patients with concurrent cerebral amyloid angiopathy(CAA)and atrial fibrillation(AF).In fact,CAA is an age-related cerebral vasculopathy that predisposes patients to intracerebral ***,many AF patients require oral systemic dose-adjusted warfarin,direct oral anticoagulants(such as factor Xa inhibitors)or direct thrombin inhibitors to control often associated with cardioembolic stroke *** prevalence of both CAA and AF is expected to rise,due to the aging of the *** clinical dilemma is becoming increasingly *** patients with coexisting AF and CAA,the risks/benefits profile of anticoagulant therapy must be assessed for each patient individually due to the lack of a clear-cut consensus with regard to its risks in scientific *** review aims to provide an overview of the management of patients with concomitant AF and CAA and proposes the implementation of a risk-based decision-making algorithm.

In this White Paper we present the potential of the enhanced X-ray Timing and Polarimetry(eXTP) mission for studies related to Observatory Science targets. These include flaring stars, supernova remnants, accreting white dwarfs, low and high mass X-ray binaries, radio quiet and radio loud active galactic nuclei, tidal disruption events, and gamma-ray bursts. eXTP will be excellently suited to study one common aspect of these objects: their often transient nature. Developed by an international Consortium led by the Institute of High Energy Physics of the Chinese Academy of Science, the eXTP mission is expected to be launched in the mid 2020s.
