BACKGROUND Neoadjuvant treatment(NT)with chemotherapy(Ch)is a standard option for resectable stage III(N2)*** studies have suggested benefits with the addition of radiotherapy(RT)to NT *** International Association for the Study of Lung Cancer(IASLC)published recommendations for the pathological response(PHR)of NSCLC resection specimens after *** To contribute to the IASLC recommendations showing our results of PHR to NT Ch vs NT chemoradiotherapy(ChRT).METHODS We analyzed 67 consecutive patients with resectable stage III NSCLC with positive mediastinal nodes treated with surgery after NT Ch or NT ChRT between 2013 and *** NT,all patients were evaluated for radiological response(RR)according to Response Evaluation Criteria in Solid Tumours criteria and evaluated for surgery by a specialized group of thoracic *** histological samples were examined by the same two *** was evaluated by the percentage of viable cells in the tumor and the resected lymph *** Forty patients underwent NT ChRT and 27 NT ***-six(83.6%)patients underwent surgery(35 ChRT and 21 Ch).The median time from ChRT to surgery was 6 wk(3-19)and 8 wk(3-21)for Ch *** observed significant differences in RR,with disease progression in 2.5%and 14.8%of patients with ChRT and Ch,respectively,and partial response in 62.5%ChRT vs 29.6%Ch(P=0.025).In PHR we observed≤10%viable cells in the tumor in 19(54.4%)and 2 cases(9.5%),and in the resected lymph nodes(RLN)30(85.7%)and 7(33.3%)in ChRT and Ch,respectively(P=0.001).Downstaging was greater in the ChRT compared to the Ch group(80%vs 33.3%;P=0.002).In the univariate analysis,NT ChRT had a significant impact on partial RR[odds ratio(OR)12.5;95%confidence interval(CI):1.21-128.61;P=0.034],a decreased risk of persistence of cancer cells in the tumor and RLN and an 87.5%increased probability for achieving downstaging(OR 8;95%CI:2.34-27.32;P=0.001).CONCLUSION We found significant benefits in RR and PHR

Difficulties Faced by Physicians in the Diagnosis of Endometriosis in the Brazilian Public Health System

Endometriosis is a clinical and recurrent condition characterized by the presence of functional endometrial tissue outside the uterine cavity and myometrium. It is estimated that 6% to 10% of women of reproductive age, 50% to 60% of adolescents and adults with pelvic pain, and up to 50% of women with infertility are pregnant from the condition. However, this proportion tends to be even greater due to the difficulty in diagnosing it: because it presents different symptoms, lack of information from health professionals, and a high-cost diagnostic test. Another aggravating factor is the average time from the onset of symptoms to the medical diagnosis, which even today ranges from five to ten years;this delay has consequences for disease progression, as it prevents early treatment, which is important for improving pain levels and physical and psychological well-being. Aware of these mishaps, it is therefore important to assess the difficulties faced by doctors in the diagnosis of endometriosis in the public health system, in Brazil, the Unified Health System (SUS). With this purpose, a cross sectional was carried out, developed in the virtual environment using traffic in Google forms, with the difficulties quantified in the Likert scale. Participating physicians pointed out a medium difficulty for patients with endometriosis to access the consultation (58%) and moderate difficulty (52%) to perform an ultrasound examination;also signaled great difficulty in performing magnetic resonance imaging (77%) and laparotomy (61%) in these patients. It is hoped that knowledge about the difficulties faced by physicians in the diagnosis of endometriosis in the SUS will give these physicians a voice and corroborate, even if minimally, so that there is more research that provokes the search for these difficulties, in order to improving the quality of life of these women.

Risk Factors Associated with Acinetobacter baumannii Infections in Patients in an Intensive Care Unit of a Public Hospital in Paraná

To identify risk factors for A. baumannii infection in patients hospitalized in an Intensive Care Unit (ICU) of a tertiary public hospital in Paraná, Brazil, a retrospective paired case-control study (ratio 1:2) was conducted from January 2018 to December 2020. Patients in the case group were hospitalized in the ICU with A. baumanni (n = 68 cases) and were compared with patients in the control group, without infection by A. baumannii (n = 136). Both were matched by age (±10 years), sex, and ICU stay (±5 days). Conditional multiple logistic regression was used to determine statistically significant risk factors based on the results of bivariate analyses. Mortality was higher in infected (cases) than in non-infected patients (51.5% vs. 39.7%). The incidence and bacterial resistance increased annually. At bivariate analysis, cases had longer hospital stays (median 35 vs. 22 days, p A. baumannii and antimicrobial resistance. There is need for surveillance, and constant evaluation of control actions. Risk factors were colonization, previous hospitalization, and hospitalization time. This is essential for the decision-making of professionals and optimization of prevention, control, and therapeutic management actions.

The Human Papillomavirus (HPV) is responsible for almost 99% of cervical cancers. Cervical cancer is a public health problem, being the fourth most common type of tumor among women worldwide and the third most common cancer among women in Brazil. It is known that cervical cancer is one of the most easily preventable forms of cancer, as there is a highly effective vaccine against HPV, however, morbidity and mortality rates related to human papillomavirus are still frightening, especially in less developed countries. Low vaccination adherence rates, especially the second dose, are probably a reflection of the lack of knowledge about this virus, its clinical repercussions, and the vaccine. A clinical trial was carried out in a higher education institution in Alagoas (Brazil), with students of the medical course to whom a validated questionnaire was applied on HPV in the virtual environment (google forms). Data collection was carried out throughout the month of May of the year 2023. It was shown that the knowledge of medical students about the human papillomavirus was less than what would be desired for future doctors. The alert for the acute need to assess health illiteracy is clear, especially regarding cancer that can be prevented by a highly effective vaccine.

Study and Evaluation of Aluminum Capsules to Irradiation of Gaseous Samples in Nuclear Reactor

Gas irradiation in research nuclear reactors is an important way to produce radionuclides. Although some nuclear reactors centers offer this type of service, there are few publications about capsules to irradiation of gaseous samples. This paper describes a method to fabricate and evaluate aluminum capsules to irradiate gaseous samples in nuclear reactor. A semi-circular slotted die from a hydraulic presshead was modified to seal aluminum tubes. The aluminum capsules were subjected to leak detection tests, which demonstrated the accordance with standard ISO 9978.

人类面部形态由神经外胚层发育而来,是区别个体的最显著特征之一。研究人面部特征的遗传机制可为生物医学、司法鉴定等应用领域提供依据。虽然目前已经在正常人群和患病人群中发现了一些与面部特征相关的基因,但是还有许多面部特征的基因有待发现。本研究基于拉美人群进化和多样性研究联合会(consortium for the analysis of the diversity and evolution of Latin America,CANDELA)队列中的1140个秘鲁健康人群,通过标定其面部的47个特征点来计算中部和下部脸部面积,并计算47个特征点之间的欧式距离,以其结果作为表型;与约七十万个单核苷酸多态性(single nucleotide polymorphisms,SNPs)的全基因组分型数据进行全基因组关联分析(genome-wide association study,GWAS),并在CANDELA队列中的384个巴西和智利的独立健康人群中进行结果验证。本研究用一个加性遗传模型来进行线性回归关联检验,其中因变量为表型的数量性状,协变量为年龄、性别、身体质量指数(body mass index,BMI)及基因组群体分层分析中的前5个主成分(principal components,PCs)。研究结果表明,位于LD基因区(EDAR,LIMS1,RANBP2,SULT1C4,CCDC138,GCC2)的17个单核苷酸多态性与7个数量表型有很强的相关性;其中EDAR作为影响头面发育的一因多效性基因,在本研究中影响中部颅面形态。

The American Kennel Club and the Fédération Cynologique Internationale recognize two phenotypic variants of Great Dane breed denominated American and European varieties. Historically, these varieties have been segregated according to morphological characteristics. In an attempt to obtain a better characterization, breeders have been interested in the genetic parameters that could evaluate the within-breed diversity. In this document, we studied the genetic structure of Great Dane breed with 10 STR markers in 88 dogs using capillary electrophoresis. Cluster analysis, population differentiation and phylogenetic analyses revealed that American and European varieties are genetically independent. Nevertheless, within the American variety a genetic stratification was found. Additionally, a high misclassification (28%) was detected, which could be due to wrong registration or false paternity. Our results support the importance to deem genetic markers as useful tools in breeding control. Similarly, these studies serve as an accurate reference to establish standards by dog breeding associations and for choosing among dogs for inter-breeding. Nevertheless, genetic tools are only a complement of morphological methods, since both are reshuffling the control over the breeding line. Notwithstanding, this database provides an overall and scape concerning the impact of genetic diversity within-breed. Unmistakably, more databases are needed to increase the quality of the breeding line as well as the number of STR in order to study, with more detail, the genetic structure in the Great Dane race.

Cognition and Functional Capacity After Thrombolysis： Preliminary Results in a Brazilian Sample

Evidences support that thrombolysis is associated to a better functional outcome but not to intact cognition after subacute stroke. The purpose of this study is to compare cognition and functional capacity, 6?10 months after stroke, in patients treated with thrombolysis with rt-PA (tissular plasminogen activator) and those who were not. From October 2008 to March 2010, 15 patients treated with thrombolysis with rt-PA and 30 patients who did not receive this treatment, underwent an extensive neuropsychological assessment and answered questionnaire about functionality. General cognitive performance, measured through the mean score of each cognitive domain, showed impairment with z score ≥ -1.50. Functional capacity was verified using PFAQ (Pfeffer Functional Activities Questionnaire) relative to instrumental activities of daily living. Five or more points in PFAQ demonstrated functional impairment. Continuous variables were analyzed by students" t-test and categorical ones by Chi-square. Cognitive deficits were present in 20% of all subjects. In the group who received treatment with thrombolysis with rt-PA, 46.7% had language impairment, 33.3% had visuospatial impairment, and 26.7% had visual memory impairment. This group presented 20% of patients with functional impairment. General cognitive performance (p = 0.624) and functional capacity (p = 0.664) showed no difference between groups. There was a statistically significant difference between groups concerning neglect (p = 0.004) and sustained visual attention (p = 0.028). Findings suggest that patients who received thrombolysis presented less neglect and less visual attention deficits but did not show better general cognitive functioning or functional capacity.
