How are children with medical complexity being identified in epidemiological studies?A systematic review

Background There are different definitions to identify/classify children with medical complexity(cMc).We aimed to investigate and describe the definitions used to classify cMc in epidemiological *** PubMed,SciELO,LILAcS,and EMBASE were searched from 2015 to 2020(last updated September 15th,2020)for original studies that presented the definition used to classify/identify cMc in the scientific research *** applied the Preferred Reporting Items for Systematic Reviews and Meta-Analyses *** the included studies,the following were identified:first author,year of publication,design,population,study period,the definition of cMc used,limitations,and *** Nine hundred and sixty-seven records were identified in the searched databases,and 42 met the inclusion *** the 42 studies included,the four most frequent definitions used in the articles included in this review were classification of cMc into nine diagnostic categories based on the International classification of Diseases,Ninth Revision(IcD-9)(35.7%,15 articles);update of the previous classification for IcD-10 codes with the inclusion of other conditions in the definition(21.4%,nine articles);definition based on a medical complexity algorithm for classification(16.7%,seven articles);and a risk rating system(7.1%,three articles).conclusions cMc definitions using diagnostic codes were more ***,several limitations were found in its *** research highlighted the need to improve health information systems to accurately characterize the cMc population and promote the provision of comprehensive care.

The application of a gate voltage to control the superconducting current flowing through a nanoscale superconducting constriction,named as gate-controlled supercurrent(GcS),has raised great interest for fundamental and technological *** gain a deeper understanding of this effect and develop superconducting technologies based on it,the material and physical parameters crucial for the GcS effect must be ***-down fabrication protocols should also be optimized to increase device scalability,although studies suggest that top-down fabricated devices are more resilient to show a ***,we investigate gated superconducting nanobridges made with a top-down fabrication process from thin films of the noncentrosymmetric superconductor niobium rhenium with varying ratios of the constituents(NbRe).Unlike other devices previously reported and made with a top-down approach,our NbRe devices systematically exhibit a GcS effect when they were fabricated from NbRe thin films with small grain size and etched in specific *** observations pave the way for the realization of top-down-made GcS devices with high *** results also imply that physical parameters like structural disorder and surface physical properties of the nanobridges,which can be in turn modified by the fabrication process,are crucial for a GcS observation,providing therefore also important insights into the physics underlying the GcS effect.

The emerging role of mesenchymal stem cell-derived extracellular vesicles to ameliorate hippocampal NLRP3 inflammation induced by binge-like ethanol treatment in adolescence

Our previous studies have reported that activation of the NLRP3(NOD-,LRR-and pyrin domain-containing protein 3)-inflammasome complex in ethanol-treated astrocytes and chronic alcohol-fed mice could be associated with neuroinflammation and brain *** stem cell-derived extracellular vesicles(MSc-EVs)have been shown to restore the neuroinflammatory response,along with myelin and synaptic structural alterations in the prefrontal cortex,and alleviate cognitive and memory dysfunctions induced by binge-like ethanol treatment in adolescent *** the therapeutic role of the molecules contained in mesenchymal stem cell-derived extracellular vesicles,the present study analyzed whether the administration of mesenchymal stem cell-derived extracellular vesicles isolated from adipose tissue,which inhibited the activation of the NLRP3 inflammasome,was capable of reducing hippocampal neuroinflammation in adolescent mice treated with binge *** demonstrated that the administration of mesenchymal stem cell-derived extracellular vesicles ameliorated the activation of the hippocampal NLRP3 inflammasome complex and other NLRs inflammasomes(e.g.,pyrin domain-containing 1,caspase recruitment domain-containing 4,and absent in melanoma 2,as well as the alterations in inflammatory genes(interleukin-1β,interleukin-18,inducible nitric oxide synthase,nuclear factor-kappa B,monocyte chemoattractant protein-1,and c–X3–c motif chemokine ligand 1)and miRNAs(miR-21a-5p,miR-146a-5p,and miR-141-5p)induced by binge-like ethanol treatment in adolescent *** analysis further revealed the involvement of miR-21a-5p and miR-146a-5p with inflammatory target genes and NOD-like receptor signaling *** together,these findings provide novel evidence of the therapeutic potential of MSc-derived EVs to ameliorate the hippocampal neuroinflammatory response associated with NLRP3 inflammasome activation induced by binge drinking in adolescence.

Eosinophilic esophagitis （EE） is the most frequent con- dition found in a group of gastrointestinal disorders called eosinophilic gastrointestinal diseases. The hypo- thetical pathophysiological mechanism is related to a hypersensitivity reaction. Gastroesophageal reflux dis- ease-like complaints not ameliorated by acid blockade or occasional symptoms of dysphagia or food impac- tion are likely presentations of EE. Due to its unclear pathogenesis and unspecific symptoms, it is difficult to diagnose EE without a strong suspicion. Although histo- togical criteria are necessary to diagnosis EE, there are some characteristic endoscopic features. We present the case of a healthy 55-year-old woman with dyspha- gia and several episodes of esophageal food impaction over the last six months. This case report stresses the most distinguishing endoscopic findings-mucosa rings, white exudative plaques and linear furrows-that can help in the prompt recognition of this condition.

Standardness and Standard Automorphisms of chevalley Groups,Ⅰ:the case of Rank at Least Two

The author studies the linkage between the standardness and the standard automorphisms of chevMley groups over rings. It is proved that if H is any standard subgroup of G（R）, then each of its automorphisms can be extended to an automorphism of G（R, I）, restricted to an automorphism of E（R, I）, and an automorphism of E（R, I） can be extended to one of G（R, I）. The case of chevalley groups of rank at least two is treated in this paper. Further results about the case of chevalley groups of rank one, the case of nomcommutative ground ring and some others exceptions will appear elsewhere.

According to the World Health Organization （WHO）, a total of 35.6 million cases of dementia were estimated in 2010, with close to 7.7 million new cases each year. In 2008, WHO declared dementia a priority condition. 90% of all dementia cases are considered to be Alzheimer＇s disease （AD）. Therefore, great effort was made to understand the etiology of the disease and stop or slow down AD progression.
