Timely completion of multiple life-saving interventions for traumatic haemorrhagic shock: a retrospective cohort study

Background:Early control of haemorrhage and optimisation of physiology are guiding principles of resuscitation after *** outcomes have been previously associated with single,timely *** aim of this study was to assess the association between multiple timely life-saving interventions(LSIs)and outcomes of traumatic haemorrhagic shock ***:A retrospective cohort study was undertaken of injured patients with haemorrhagic shock who presented to Alfered Emergency&Trauma Centre between July 01,2010 and July 31,*** studied included chest decompression,control of external haemorrhage,pelvic binder application,transfusion of red cells and coagulation products and surgical control of bleeding through angio-embolisation or operative *** primary exposure variable was timely initiation of≥50%of the indicated *** association between the primary exposure variable and outcome of death at hospital discharge was adjusted for potential confounders using multivariable logistic regression *** association between total pre-hospital times and pre-hospital care times(time from ambulance at scene to trauma centre),in-hospital mortality and timely initiation of≥50%of the indicated interventions were ***:Of the 168 patients,54(32.1%)patients had≥50%of indicated LSI completed within the specified time *** delivery of LSI was independently associated with improved survival to hospital discharge(adjusted odds ratio(OR)for in-hospital death 0.17;95%confidence interval(CI)0.03–0.83;p=0.028).This association was independent of patient age,pre-hospital care time,injury severity score,initial serum lactate levels and *** patients with pre-hospital time of≥2 h,2(3.6%)received timely ***-hospital care times of≥2 h were associated with delayed LSIs and with in-hospital death(unadjusted OR 4.3;95%CI 1.4–13.0).Conclusions:Timely completion of LSI when indicated was completed in a small

Heart rate is an independent predictor of all-cause mortality in individuals with type 2 diabetes: The diabetes heart study

AIM To assess the association of resting heart rate with allcause and cardiovascular disease(CVD) mortality in the Diabetes Heart Study(DHS).METHODS Out of a total of 1443 participants recruited into the DHS, 1315 participants with type 2 diabetes who were free of atrial fibrillation and supraventricular tachycardia during the baseline exam were included in this analysis. Heart rate was collected from baseline resting electrocardiogram and mortality(all-cause and CVD) was obtained from state and national death registry. Kaplan-Meier(K-M) and Cox proportional hazard analyses were used to assess the *** The mean age, body mass index(BMI) and systolic blood pressure(SBP) of the cohort were 61.4 ± 9.2 years, 32.0 ± 6.6 kg/m2, and 139.4 ± 19.4 mmHg respectively. Fiftysix percent were females, 85% were whites, 15% were blacks, 18% were smokers. The mean ± SD heart rate was 69.8(11.9) beats per minute(bpm). After a median follow-up time of 8.5 years(maximum follow-up time is 14.0 years), 258 participants were deceased. In K-M analysis, participants with heart rate above the median had a significantly higher event rate compared with those below the median(log-rank P = 0.0223). A one standard deviation increase in heart rate was associated with allcause mortality in unadjusted(hazard ratio 1.16, 95%CI: 1.03-1.31) and adjusted(hazard ratio 1.20, 95%CI: 1.05-1.37) models. Similar results were obtained with CVD mortality as the outcome of *** Heart rate is an independent predictor of all-cause mortality in this population with type 2 diabetes. In this study, a 1-SD increase in heart rate was associated with a 20% increase in risk suggesting that additional prognostic information may be gleaned from this ubiquitously collected vital sign.

The utility of coronary computed tomography angiography in elderly patients

Background Coronary computed tomography angiography(CCTA)is often avoided in elderly patients due to a presumption that a high proportion of patients will have heavily calcified plaque limiting an accurate *** sought to assess the image quality,luminal stenosis and utility of CCTA in elderly patients with suspected coronary artery disease(CAD)and stable chest *** Retrospective analysis of elderly patients(>75 years)who underwent 320-detector row CCTA between 2012–2017 at *** CCTA was analysed for degree maximal coronary stenosis by CAD-RADS classification,image quality by a 5-point Likert score(1-poor,2-adequate,3-good,4-very good,5-excellent)and presence of artefact limiting *** 1011 elderly patients(62%females,78.8±3.3 years)were *** risk factor prevalence included:hypertension(65%),hyperlipidaemia(48%),diabetes(19%)and smoking(21%).The CCTA was evaluable in 68%of patients which included 52%with non-obstructive CAD(50%)*** Likert score was 3.1±0.6 corresponding to good image *** the 323(32%)of patients with a non-interpretable CCTA,80%were due to calcified plaque and 20%due to motion *** gender(P=0.009),age(P=0.02),excess motion(P78 years of age are significantly less likely to have interpretable scans.

Resilient and Inclusive Prosperity within Planetary Boundaries

The current model of economic growth generated unprecedented increases in human wealth and prosperity during the 19th and 2Oth centuries. The main mechanisms have been the rapid pace of technological and social innovation, human capital accumulation, and the conversion of resources and natural capital into more valuable forms of produced capital. However, there is evidence emerging that this model may be approaching environmental limits and planetary boundaries, and that the conversion of natural capital needs to slow down rapidly and then be reversed Some commentators have asserted that in order for this to occur, we will need to stop growing altogether and, instead, seek prosperity without growth. Others argue that environmental concerns are low-priority luxuries to be contemplated once global growth has properly returned to levels observed prior to the 2008 financial crisis. A third group argues that there is no trade-off and, instead,, promotes green growth： the （politically appealing） idea is that we can simultaneously grow and address our environmental problems. This paper provides a critical perspective on this debate and suggests that asubstantial researc＇h agenda is required to come to grips with these challenges. One place to start is with the relevant metrics： measures of per-capitawealth, and, eventually, quantitative measures of prosperity, alongside a dashboard of other sustainability indicators. A public andpoliticalfocus on wealth （a stock）, and its annual changes, could realistically complement the current focus on market-based gross output as measured by GDP （a flow）. This could have important policy implications, but deeper changes to governance and business models will be required.

Complete transection of peripheral mixed nerves immediately produces loss of sensory perception,muscle contractions and voluntary behavior mediated by the severed distal *** contrast to natural regeneration(~1 mm/d)of proximal axons that may eventually reinnervate denervated targets,re-innervation is restored within minutes by PEG-fusion that consists of neurorrhaphy and a sequence of well specified hypo-and isotonic calcium-free or calcium-containing solutions,the anti-oxidant methylene blue(MB)and the membrane fusogen polyethylene glycol(PEG).In this study,we examined the relative efficacy of PEG-fusion with no MB(0%),0.5%MB,or 1%MB on the recovery of voluntary behaviors by female Sprague-Dawley rats with a complete mid-thigh severance of their sciatic nerve bathed in extracellular fluid or calcium-containing isotonic *** recovery of voluntary behaviors is the most relevant measure of success of any technique to repair peripheral nerve *** assessed recovery by the sciatic functional index,a commonly used measure of voluntary hindlimb behaviors following complete sciatic *** reported that both 1%MB and 0.5%MB in sterile distilled water in our PEG-fusion protocol with neurorrhaphy significantly increased the rate and extent of behavioral recovery compared to PEG plus neurorrhaphy ***,0.5%MB was as effective as 1%MB in voluntary behavioral recovery as assessed by the sciatic functional *** sterile 1%MB is no longer clinically available,we therefore recommend that 0.5%MB be included in upcoming human clinical trials to evaluate the safety and efficacy of *** animal procedures were approved by the University of Texas Institutional Animal Care and Use Committee(AUP-2019-00225)on September 9,2020.

Divergence and constraint in the thermal sensitivity of aquatic insect swimming performance

Environmental temperature variation may play a significant role in the adaptive evolutionary divergence of ectotherm thermal performance curves(TPCs).However,divergence in TPCs may also be constrained due to various ***,we measured TPCs for swimming velocity of temperate and tropical mayflies(Family:Baetidae)and their stonefly predators(Family:Perlidae)from different *** predicted that differences in seasonal climatic regimes would drive divergence in TPCs between temperate and tropical *** tropical temperatures should favor the evolution of"specialists"that perform well across a narrow range of ***,variable temperatures in temperate zones,however,should favor"generalists"that perform well across a broad range of *** phylogenetically paired comparisons of mayflies and stoneflies,swimming speed was generally unaffected by experimental temperature and did not differ among populations between latitudes,suggesting a maintenance of performance breadth across elevation and *** exception was found between temperate and tropical mayflies at low elevation where climatic differences between latitudes are *** addition,TPCs did not differ between mayflies and their stonefly predators,except at tropical low *** results indicate that divergence in TPCs may be con strai ned in aquatic in sects except under the most differe nt ther・mal regimes,perhaps because of trade-offs that reduce thermal sensitivity and increase performance breadth.

Discovery of Japalura chapaensis Bourret,1937(Reptilia:Squamata:Agamidae)from Southeast Yunnan Province,China

Due to a paucity of surveys in northern Indochina and lack of international collaborations among neighboring countries, recognized distributional ranges for many amphibian and reptile species end at the political borders for some countries, despite seemingly continuous suitable habitat spanning the region. Combining both morphological and genetic data, we report the first discovery of Japalura chapaensis, a rare agamid lizard believed previously to be endemic to northern Vietnam only, along the border region of southeastern Yunnan Province,China. To facilitate future research on the genus Japalura sensu lato in Indochina, we provide detailed descriptions of additional specimens of this rare species, including the first description of coloration in life and an expanded diagnosis, and discuss the species boundary of J. chapaensis with respect to its congeners.

数字化时代的到来给史学研究者带来研究方法的变革,越来越多专题性历史数据库的涌现,对人文学者掌握和利用数据库的技术手段方面也带来新的挑战。在将清代缙绅录建设为史学量化数据库的过程中,利用人物姓名、籍贯等关键信息构建有效的内部连接是追踪官员仕途迁转的关键,而跨数据的外部连接能够进一步拓展数据分析维度,补充科考名次、出生年月、家族背景等影响仕途的重要因素。利用明经通谱中的拔贡名册和缙绅录连接比较,可以得到各科拔贡生员入仕的人数比例。在分析拔贡不同的入仕途径过程中,不能忽视履历、方志等史料中对官员捐纳等信息的搜集,同时在任职时间等方面,这些信息可与缙绅录数据库中的信息互补,勾勒出更完整的官员仕途路径。在团队跨学科合作方面,缙绅录就开发检索平台、互动式可视化界面都进行过尝试,但存在成果难以维护和共享利用等问题,为数字人文在史学领域的发展提供思考借鉴。
