Exsanguination from trauma, gastrointestinal bleeding,and obstetric hemorrhage remains a major source of mortality across the planet. Continued research into resuscitation strategies and evolving technology and blood product storage has allowed for patient to undergo very large volume transfusions, even to the point of replacing a patient's blood volume several times over. As massive transfusions have become more common, more studies have been performed delineating the exact patient population that would benefit, start-and stop-points of transfusions, complications and avoidance of the same. We discuss these points and provide information and strategies for massive transfusion.

Individuals having sustained traumatic spinal cord injury(TSCI)in the United States are living longer as compared to historical trends,thanks to an ever-evolving understanding of the nature of this *** this,multiple barriers to care for TSCI patients remain including variations in government-issued veteran insurance,privatized insurance,and among uninsured *** United States alone experiences 12,000 new TSCI cases every year,many of these are found to occur in a growing proportion of elderly *** is crucial to understand both the short-term direct costs as wells as the long-term rehabilitation costs required by these TSCI *** lifetime financial burden for those having sustained a TSCI can be immense for patients,insurance companies,and hospital systems *** those with TSCI,re-hospitalization rates are high,leading to increased healthcare resource utilization within this specific patient *** can quickly balloon into hundreds of thousands of dollars and cause a profound financial burden for these *** review article seeks to communicate an understanding of the current financial landscape surrounding TSCI *** authors will also examine the costs of acute emergency room surgical care such as American spinal injury association grade,hospital length of stay,as well as the timing delay between injury and surgical ***-term costs associated with TSCI such as rehabilitation,care of secondary comorbidities,and post-injury employment prospects will be examined as *** costs will be framed from the patient's perspective as well as from both the hospital and insurance company's *** is hoped a complete understanding as to what makes TSCI such a medically and financially burdensome injury will allow for improved healthcare resource utilization in this population.

Coronavirus Disease-2019 (COVID-19) refers to a clinical disorder caused by a new coronavirus that emerged in late 2019. According to the European Centre for Disease Prevention and Control, as of June 23 2020, over 15 million confirmed cases of patients with COVID-19 have been reported globally, out of which over 600,000 deaths were recorded. The management of patients with COVID-19 is critical. This position paper was prepared through online search for the available guideline in the literature to serve as treatment guidelines for Nigerian physiotherapists and possibly physiotherapists in Sub-Saharan Africa region.

Background:The expression,localization,and function of the endocannabinoid system has been well characterized in recent years in the monkey retina and in the primary thalamic relay,the lateral geniculate nucleus(dLGN).Few data are available on cortical recipients’structures of the dLGN,namely the primary visual cortex(V1).The goal of this study is to characterize the expression and localization of the metabotropic cannabinoid receptor type 1(CB1R),the synthesizing enzyme N-acyl phosphatidyl-ethanolamine phospholipase D(NAPE-PLD),and the degradation enzyme fatty acid amide hydrolase(FAAH)in the vervet monkey area ***:Using Western blots and immunohistochemistry,we investigated the expression patterns of CB1R,NAPE-PLD,and FAAH in the vervet monkey primary visual ***:CB1R,NAPE-PLD,and FAAH were expressed in the primary visual cortex throughout the rostro-caudal ***1R showed very low levels of staining in cortical layer 4,with higher expressions in all other cortical layers,especially layer ***-PLD and FAAH expressions were highest in layers 1,2 and 3,and lowest in layer ***:Interestingly enough,CB1R was very low in layer 4 of V1 in comparison to the other cortical *** visual information coming from the dLGN and entering layer 4Calpha(magno cells)and 4Cbeta(parvo cells)may be therefore modulated by the higher expression levels of CB1R in cortical layers 2 and 3 on the way to the dorsal and ventral visual *** is further supported by the higher expression of NAPE-PLD and FAAH in the outer cortical *** data indicate that CB1R system can influence the network of activity patterns in the visual stream after the visual information has reached area *** novel results provide insights for understanding the role of the endocannabinoids in the modulation of cortical visual inputs,and hence,visual perception.

Feasibility study of first-year medical students identifying cardiac anatomy using ultrasound in rural Panama

BACKGROUND: There are over 15 million children who have cardiac anomalies around the world, resulting in a significant morbidity and mortality. Early recognition and treatment can improve the outcomes and lengthen life-expectancy of these patients. The NIH and WHO have promoted guidelines for screening for congenital cardiac anomalies using ultrasound in rural ***: Our study took place in Bocas Del Toro, Panama where a mobile clinic was established for community healthcare screening and ultrasonographic evaluation by medical student volunteers and volunteer clinical faculty. This was a non-blinded, investigational study utilizing a convenience sample of pediatric patients presenting for voluntary evaluation. Seven first-year medical students were recruited for the study. These students underwent a training program for advanced cardiac ultrasound instruction, termed "Pediatric Echocardiography Cardiac Screening(PECS)".RESULTS: Ten patients were enrolled in the study. Nine patients had adequate images as defined by the PECS criteria and were all classified as normal cardiac pathology by the medical students, resulting in a sensitivity and specificity of 100%. A single patient was identified by medical students as having a pathologic pulmonic stenosis. This was confirmed as correct by a blinded ***: In this pilot study, the first-year medical students were able to correctly identify pediatric cardiac anatomy and pathology in rural Panama after undergoing a 12-hour ultrasound PECS training session. We believe that with this knowledge, minimally trained practitioners can be used to screen for cardiac anomalies in rural Panama using ultrasound.

提出一种基于机器学习的食双星光变曲线自动分类算法。首先对数据进行预处理,将食双星光变曲线数据归一化,并通过滤波/插值降低噪声;其次使用快速傅里叶变换提取频率信号作为特征向量;最后利用特征向量训练支持向量机获得自动分类模型。使用Python实现算法并抓取caleb和GCVS数据验证,分析特征向量、支持向量机核函数与惩罚系数对分类正确率的影响,优化后所得分类模型正确率达到92.8%(训练集)和89.0%(测试集),最后使用所得分类模型对第3方数据进行分类,正确率为88.8%,结果证明提出的分类算法的有效性。
