Electromagnetic Fields and calcium Signaling by the Voltage Dependent Anion channel

Electromagnetic fields (EMFs) can interact with biological tissues exerting positive as well as negative effects on cell viability, but the underlying sensing and signaling mechanisms are largely unknown. So far in excitable cells EMF exposure was postulated to cause ca2+ influx through voltage-dependent ca channels (VDcc) leading to cell activation and an antioxidant response. Upon further activation oxidative stress causing DNA damage or cell death may follow. Here we report collected evidence from literature that voltage dependent anion channels (VDAc) located not only in the outer microsomal membrane but also in the cytoplasmic membrane convert to ca2+ conducting channels of varying capacities upon subtle changes of the applied EMF even in non-excitable cells like erythrocytes. Thus, VDAc can be targeted by external EMF in both types of membranes to release cace:normal;">2+ into the cytosol. The role of frequency, pulse modulation or polarization remains to be investigated in suitable cellular models. VDAcs are associated with several other proteins, among which the 18 kDa translocator (TSPO) is of specific interest since it was characterized as the central benzodiazepine receptor in neurons. Exhibiting structural similarities with magnetoreceptors we propose that TSPO could sense the magnetic component of the EMF and thus together with VDAc could trigger physiological as well as pathological cellular responses. Photonic bound states in the continuum(BIcs)are spatially localized modes with infinitely long lifetimes,which exist within a radiation continuum at discrete energy *** states have been explored in various systems,including photonic and phononic crystal slabs,metasurfaces,waveguides,and integrated *** and availability of the BIcs are important aspects for fully taming the BIcs toward practical ***,we propose a generic mechanism to realize BIcs that exist by first principles free of fine parameter tuning based on non-Maxwellian double-net metamaterials(DNMs).An ideal warm hydrodynamic double plasma(HDP)fluid model provides a homogenized description of DNMs and explains the robustness of the BIcs found *** the HDP model,these are standing wave formations made of electron acoustic waves(EAWs),which are pure charge oscillations with vanishing electromagnetic *** BIcs have various advantages,such as(i)frequency-comb-like collection of BIcs free from normal resonances;(ii)robustness to symmetry-breaking perturbations and formation of quasi-BIcs with an ultrahigh Q-factor even if subject to disorder;and(iii)giving rise to subwavelength microcavity resonators hosting quasi-BIc modes with an ultrahigh Q-factor.

Balance of concerns: Satisfactory Pre-Anesthetic Patient Education and the Extent of Patient Worries

controversy exists whether or not patient’s concerns regarding anesthetics can be reduced by face-to-face pre-assessment with an anesthetist. Thus we were looking at concerns patients had before and after such a consultation. Patient satisfaction was rated by a validated questionnaire. A four-staged Likert-scale was used to quantify the extent of patients concerns. The totaling 461 patients were overall highly satisfied. 448 patients had “nil” to “minimal” concerns. After the assessment, 106 patients stated their concerns had been lessened (p 0.001). Having gone through the anesthetic pre-assessment center, 99.1% of all patients reported no considerable concerns regarding anesthetics whatsoever. A high level of patient satisfaction does not constitute a low level of concerns patients may hold over anesthetics, although a pre-operative consultation mitigated these concerns by 23%, whilst for 9% of all patients this pre-assessment led to a higher level of concerns.

No relationship between biopsy sites near the main testicular vessels or rete testis and successful sperm retrieval using conventional or microdissection biopsies in 220 non-obstructive azoospermic men

In 220 consecutive patients with non-obstructive azoospermia, sperm retrieval was attempted by a combination of conventional and microdissection testicular sperm extraction （TESE）. For sperm retrieval, 2-3 conventional biopsies were performed followed by a microdissection TESE in cases of negative conventional biopsies. During the surgery, the vasculature of the testis was assessed using the operative microscope, and the location of positive biopsies was registered in relation to the blood supply. The overall sperm retrieval rate was 58.2%. From the initial conventional biopsies, sperm could be retrieved in 46.8% of the patients. With microdissection TESE, sperm could be retrieved from an additional 11.4% of the patients. The further use of microdissection TESE improved the sperm retrieval rate significantly （P=0.017）. No significant accumulation of positive biopsies was found towards the rete testis or the main testicular vessels.

Traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease are three distinct neurological disorders that share common pathophysiological mechanisms involving neuroinflammation. One sequela of neuroinflammation includes the pathologic hyperphosphorylation of tau protein, an endogenous microtubule-associated protein that protects the integrity of neuronal cytoskeletons. Tau hyperphosphorylation results in protein misfolding and subsequent accumulation of tau tangles forming neurotoxic aggregates. These misfolded proteins are characteristic of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease and can lead to downstream neuroinflammatory processes, including assembly and activation of the inflammasome complex. Inflammasomes refer to a family of multimeric protein units that, upon activation, release a cascade of signaling molecules resulting in caspase-induced cell death and inflammation mediated by the release of interleukin-1β cytokine. One specific inflammasome, the NOD-like receptor protein 3, has been proposed to be a key regulator of tau phosphorylation where it has been shown that prolonged NOD-like receptor protein 3 activation acts as a causal factor in pathological tau accumulation and spreading. This review begins by describing the epidemiology and pathophysiology of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease. Next, we highlight neuroinflammation as an overriding theme and discuss the role of the NOD-like receptor protein 3 inflammasome in the formation of tau deposits and how such tauopathic entities spread throughout the brain. We then propose a novel framework linking traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease as inflammasomedependent pathologies that exist along a temporal continuum. Finally, we discuss potential therapeutic targets that may intercept this pathway and ultimately minimize long-term neurological decline.

context: current clinical practice relies on hemoglobin to detect iron deficiency, which misses infants not yet anemic and places them at higher risk for neurocognitive impairment. Reticulocyte hemoglobin content (cHr) has never been compared with hemoglobin for screening healthy infants. Objectives: To evaluate cHr for detecting iron deficiency without anemia in healthy 9-to 12-month-old infants and to compare cHr with hemoglobin in screening for iron deficiency in this population. A secondary objective was to explore the association between cHr and subsequent development of anemia. Design, Setting, and Patients: A prospective observational cohort study of 202 healthy 9-to 12-monthold infants from an urban, hospital-based, primary care clinic in Boston, Mass, who were screened for iron deficiency between June 2000 and April 2003, and followed up for a median of 5.6 months. Main Outcome Measures: Iron deficiency (transferrin saturation ciency and 6 (3%) had iron deficiency and anemia. Iron-deficient and non-iron-deficient infants had significantly different values for all measured hematological and biochemical markers for iron deficiency. Optimal cHr cutoff for detecting iron deficiency was 27.5 pg (sensitivity, 83%and specificity, 72%); a hemoglobin level of less than 11 g/dL resulted in a sensitivity of 26%and a specificity of 95%. Reticulocyte hemoglobin content was more accurate overall than hemoglobin was for detecting iron deficiency (area under the receiver operating characteristic curve, 0.85 vs 0.73; P=.007). A cHr of less than 27.5 pg without anemia at initial screening was associated with subsequent anemia when screened again in the second year of life (risk ratio, 9.1; 95%confidence interval, 1.04-78.9; P=.01). conclusions: A cHr of less than 27.5 pg is a more accurate hematological indicator of iron deficiency compared with hemoglobin of less than 11 g/dL
