Pleiotropic effects of apolipoprotein A-Ⅱ on high-density lipoprotein functionality, adipose tissue metabolic activity and plasma glucose homeostasis

Apolipoprotein A-Ⅱ(APOA-Ⅱ) is the second most abundant apolipoprotein of high-density lipoprotein(HDL)synthesized mainly by the liver and to a much lesser extent by the intestine. Transgenic mice overexpressing human APOA-Ⅱ present abnormal lipoprotein composition and are prone to atherosclerosis, though in humans the role for APOA-Ⅱ in coronary heart disease remains controversial. Here, we investigated the effects of overexpressed APOA-Ⅱ on HDL structure and function, adipose tissue metabolic activity, glucose tolerance and insulin sensitivity. c57BL/6 mice were infected with an adenovirus expressing human APOA-Ⅱ or a control adenovirus Ad GFP, and five days post-infection blood and tissue samples were isolated. APOA-Ⅱ expression resulted in distinct changes in HDL apoproteome that correlated with increased antioxidant and anti-inflammatory activities. No effects on cholesterol efflux from RAW 264.7 macrophages were observed. Molecular analyses in white adipose tissue(WAT) indicated a stimulation of oxidative phosphorylation coupled with respiration for ATP production in mice overexpressing APOA-Ⅱ. Finally, overexpressed APOA-Ⅱ improved glucose tolerance of mice but had no effect on the response to exogenously administered insulin. In summary, expression of APOA-Ⅱ in c57BL/6 mice results in pleiotropic effects with respect to HDL functionality, adipose tissue metabolism and glucose utilization, many of which are beneficial to health.

TO THE EDITOR　　A 61 year old man with cirrhosis and hepatocellular carcinoma developed on the background of chronic hepatitis B was admitted because of acute and gradually intensified right thigh pain and swelling, which, within a few hours, was expanded to his right foot. The patient,due to ascites refractory to diuretics, was almost weekly subjected to large volume paracentesis of ascitic fluid for the last six months; meanwhile he has developed a left inguinal, a right femoral and an umbilical hernia. It should be noted that 48 h before admission, the patient was referred to the emergency room because of dyspnoea and 5 L of ascitic fluid were removed.

Hepatocellular carcinoma(Hcc)is the sixth most common *** main risk factors associated with Hcc development include hepatitis B virus,hepatitis c virus,alcohol consumption,aflatoxin B1,and nonalcoholic fatty liver ***,hepatocarcinogenesis is a complex multistep *** factors lead to hepatocyte malignant transformation and Hcc *** and surveillance of Hcc can be made with the use of liver ultrasound(US)every 6 ***,the sensitivity of this imaging method to detect Hcc in a cirrhotic liver is limited,due to the abnormal liver *** tomography(cT)and magnetic resonance imaging(MRI)are considered to be most useful tools for at-risk patients or patients with inadequate *** biopsy is still used for diagnosis and prognosis of Hcc in specific nodules that cannot be definitely characterized as Hcc by *** the American college of Radiology designed the Liver Imaging Reporting and Data System(LI-RADS),which is a comprehensive system for standardized interpretation of cT and MRI liver examinations that was first proposed in *** 2018,it was integrated into the American Association for the Study of Liver Diseases guidance statement for ***-RADS is designed to ensure high sensitivity,precise categorization,and high positive predictive value for the diagnosis of Hcc and is applied to“highrisk populations”according to specific *** importantly LI-RADS criteria achieved international collaboration and consensus among liver experts around the world on the best practices for caring for patients with or at risk for Hcc.

coronary atherosclerosis burden is not advanced in patients with β-thalassemia despite premature extracardiac atherosclerosis： a coronary artery calcium score and carotid intima-media thickness study

Background Thalassemic patients demonstrate an increased rate of extracardiae vascular complications and increased carotid wall intima-media thickness （cIMT）, but very low prevalence of coronary artery disease （cAD）. We investigated the atheroma burden by assessing the coronary artery calcium （cAc） and elMT in these patients. Methods We examined 37 patients with β-thalassemia and 150 healthy control volunteers with multi-detector computer tomography （cT） and ultrasonography to determine cAc score and cIMT, respectively. Results Propensity score matching （c-statistic： 0.88; 95% cI： 0.83-0.93） resulted in 27 pairs of patients; severe cAc was observed in 2 （7.4%） and 0 of β-thalassemia patients and healthy volunteers respectively （P = 0.5）. Median calcium score was 0 （0-0） in β-thalassemia patients and 0 （0-4） in healthy volunteers （P = 0.8）. Median intima-media thickness was higher in β-thalassemia patients compared to control group [0.45 （0.06-0.65） vs. 0.062 （0.054-0.086）; P = 0.04]. conclusions Patients with β-thalassemia in comparison with healthy control subjects exhibit similar cAc score and increased cIMT. Our findings indicate a disparate rate of progression of atherosclerosis between coronary and extracardiac arteries in these patients lending support to the epidemiological evidence.
