Improved hemostasis with major hepatic resection in the current surgical era

Background: Major hepatic resection, predominantly performed for oncologic intent, is a complex procedure with the potential for severe intraoperative hemorrhage. The current surgical era has the ability to improve hemostasis throughout the performance of major hepatic resections which decreases blood transfusions and the detrimental effects associated with transfusion. we evaluated hemostasis and outcomes in the current surgical era of performing hepatic resections. Methods: Utilizing the American college of Surgeons (AcS) National Surgical Quality Improvement Program (NSQIP) database all major hepatic resections performed between 2012 and 2016 were analyzed in regards to hemostasis. Hemostasis was evaluated by the need for and magnitude of blood transfusions. Additional perioperative variables (including operative time, length of hospital stay, and mortality rates) were analyzed to assess for outcomes with hemostasis. The NSQIP results were compared to previous publications involving major hepatic resections to detect improvement in hemostasis and outcomes in the current surgical era. Results: A total of 22777 major hepatic resections met the inclusion criteria for analysis in the NSQIP database. An additional 21198 cases were compiled within the selected publications for comparative analysis. The transfusion rate in the current surgical era was 13.3% versus 38.7% in the previous era ( P = 0.0001). when a transfusion was required in the current surgical era there was a two-fold reduction in the number of units transfused (1.5 U vs. 3.8 U, P = 0.0001). Statistically significant improvements in operative time and length of hospital stay were presented within the current surgical era ( P = 0.0001). when a transfusion was required there was an increased relative risk score of 7 for mortality (4.9% vs. 0.7%, P = 0.0001), however, improvement in mortality rates did not reach statistical significance across surgical eras (1.3% vs. 4.0%, P = 0.0001). conclusions: The cond

Osteopontin （OPN; also known as Secreted Phosphoprotein 1, SPP1） is a secreted extra-cellular matrix （EcM） protein that binds to a variety of cell surface integrins to stimulate cell-cell and cell-EcM adhesion and communication. It is generally accepted that OPN interacts with apically expressed integrin receptors on the uterine luminal epithelium （LE） and conceptus trophectoderm to attach the conceptus to the uterus for implantation. Research conducted with pigs and sheep has significantly advanced understanding of the role（s） of OPN during implantation through exploitation of the prolonged peri-implantation period of pregnancy when elongating conceptuses are free within the uterine lumen requiring extensive paracrine signaling between conceptus and endometrium. This is followed by a protracted and incremental attachment cascade of trophectoderm to uterine LE during implantation, and development of a true epitheliochorial or synepitheliochorial placenta exhibited by pigs and sheep, respectively. In pigs, implanting conceptuses secrete estrogens which induce the synthesis and secretion of OPN in adjacent uterine LE. OPN then binds to αvβ6 integrin receptors on trophectoderm, and the αvβ3 integrin receptors on uterine LE to bridge conceptus attachment to uterine LE for implantation. In sheep, implanting conceptuses secrete interferon tau that prolongs the lifespan of cL. Progesterone released by cL then induces OPN synthesis and secretion from the endometrial GE into the uterine lumen where OPN binds integrins expressed on trophectoderm （αvβ3） and uterine LE （identity of specific integrins unknown） to adhere the conceptus to the uterus for implantation. OPN binding to the αvβ3 integrin receptor on ovine trophectoderm cells induces in vitro focal adhesion assembly, a prerequisite for adhesion and migration of trophectoderm, through activation of： 1） P70S6K via crosstalk between FRAP1/MTOR and MAPK pathways; 2） MTOR, PI3K, MAPK3/MAPK1 （Erk1/2） and MAPK14 （p38） signalin

Assays that measure steroid hormones in patient care, public health, and research need to be both accurate and precise, as these criteria help to ensure comparability across all clinical and research applications. This review addresses major issues relevant to assay variability and describes recent activities by the US centers for Disease control and Prevention （cDc） to improve assay performance. currently, high degrees of accuracy and precision are not always met for testosterone and estradiol measurements; although technologies for steroid hormone measurement have advanced significantly, measurement variability within and across laboratories has not improved accordingly. Differences in calibration and specificity are discussed as sources of variability in measurement accuracy. Ultimately, a combination of factors appears to cause inaccuracy of steroid hormone measurements, with nonuniform assay calibration and lack of specificity being two major contributors to assay variability. within-assay variability for current assays is generally high, especially at low analyte concentrations. The cDc Hormone Standardization （HoSt） Program is improving clinical assays, as evidenced by a 50% decline in mean absolute bias between mass spectrometry assays and the cDc reference method from 2007 to 2011. This program provides the measurement traceability to cDc reference methods and helps to minimize factors affecting measurement variability.

The increasing global environmental awareness, evidenced by recent worldwide calls for control of climate change and greenhouse emissions, has placed significant new technical mandates for automotives to improve engine efficiency, which is directly related to the production of carbon dioxide, a major greenhouse gas. Reduction of parasitic losses of the vehicle, powertrain and the engine systems is a key component of energy conservation. For engine efficiency improvement, various approaches include improvements in advanced combustion systems, component system design and handling—such as down-sizing, boosting, and electrification—as well as waste heat recovery systems etc. Among these approaches, engine friction reduction is a key and relatively cost-effective approach, which has been receiving significant attention from tribologists and lubricant-lubrication engineers alike. In this paper, the fundamentals of friction specific to the environments of engine components tribology are reviewed, together with discussions on the impact of developing vehicle powertrain technologies, surface and material technologies, as well as lubricant and additive technologies on promises of continuing friction and wear reduction trends. The international accords on climate change require further gains in fuel efficiency and energy sustainability from all industry sectors including those in the automotive and the broader internal combustion engine industries, and the latter encompass off-highway, power generation, marine, and rail industries as well. This paper focsuses on friction reduction in mainly automotive engines, however. The paper starts with a clarification of the common descriptors of mechanical losses and friction in the engine, followed by the topic of lubrication fundamentals such as lubrication regimes. Then the lubrication of the contacting surfaces in each of the major engine subsystems is discussed in turn. These subsystems include the piston assembly: ring-pack/liner,

The torsional static and dynamic behaviors of circular nanosolids such as nanoshafts, nanorods and nanotubes are established based on a new nonlocal elastic stress field theory. Based on a new expression for strain energy with a nonlocal nanoscale parameter, new higher-order governing equations and the corresponding boundary conditions are first derived here via the variational principle because the classical equilibrium conditions and/or equations of motion can- not be directly applied to nonlocal nanostructures even if the stress and moment quantities are replaced by the corresponding nonlocal quantities. The static twist and torsional vibration of cir- cular, nonlocal nanosolids are solved and discussed in detail. A comparison of the conventional and new nonlocal models is also presented for a fully fixed nanosolid, where a lower-order governing equation and reduced stiffness are found in the conventional model while the new model reports opposite solutions. Analytical solutions and numerical examples based on the new nonlocal stress theory demonstrate that nonlocal stress enhances stiffness of nanosolids, i.e. the angular displace- ment decreases with the increasing nonlocal nanoscale while the natural frequency increases with the increasing nonlocal nanoscale.

AIM：To investigate the cytotoxic effects of spray-dried extracts of Phyllanthus niruri in combination with cis- platin on two cancer cell lines. METHODS： colorectal carcinoma （HT29） and human hepatocellular carcinoma （HepG2） cells were treated with spray-dried extracts of Phyllanthus niruri （SDEPN） either alone or in combination with cisplatin at differ- ent concentrations （0.5 mg/mL and 1 mg/mL） for 4 h and 24 h. To verify and quantify cancer cells treated with these products as well as identify the cell cycle stage and cell viability, we stained the cells with prop- idium iodide and assessed them by flow cytometry. The percentage of cells in different cell cycle phases was quantified and data were expressed as histo- grams. Significant differences between groups were determined using analysis of variance and Bonferroni＇s test, as indicated. A value of P 〈 0.05 was considered to be statistically significant. RESULTS： SDEPN had significantly different cyto- toxic effects on HT29 （2.81 4- 0.11 vs 3.51 4- 1.13, P 〉 0.05） and HepG2 （5.07± 0.3 vs 15.9 ± 1.04, P 〈 0.001） cells when compared to control cells for 4 h. SDEPN also had significantly different cytotoxic effects on HT29 （1.91 ± 0.57 vs 4.53± 1.22, P 〉 0.05） and HepG2 （14.56 ± 1.6 vs 35.67 ± 3.94, P 〈 0.001） cells when compared to control cells for 24 h. Both cell lines were killed by cisplatin in a dose-dependent manner compared to control cells （HepG2 cells for 4 h： 10.78 ± 1.58 vs 53.89 ± 1.53, P 〈 0.001; 24 h： 8.9 ± 1.43 vs 62.78 ± 1.87, P 〈 0.001 and HT29 cells for 4 h： 9.52 ±0.913 vs 49.86 ± 2.89, P 〈 0.001; 24 h： 11.78 ± 1.05 vs 53.34 ± 2.65, P 〈 0.001）. In HT29 cells, pretreat- ment with SDEPN and subsequent treatment with cis-platin resulted in a greater number of cells being killed （12.78 ± 1.01 vs 93.76 ± 1.6, P 〈 0.001）. The effects of acyclic liquid nitrogen(LN)treatment in a temperature range of-196℃to 50℃on the thermal and magnetic stability of Fe78Si9B13 and Fe73.5Si13.5B9Nb3cu1 glassy ribbons have been *** intrinsic heterogeneities of the metallic glasses can be activated through cryogenic thermal cycling,making irreversible structural changes after the treatment and inducing rejuvenation to the *** microstructural changes of both Fe-based metallic glass(MG)and nanocrystalline alloy induced by LN treatment were *** experimental results show that the LN treatment could effectively rejuvenate the Fe-Si-B MGs and change their thermomechanical and magnetic *** on the partially-crystallinity and well-known magnetic constants,the increase of the energy at the order of 10m J/g and magnetic domain wall movement and rotation at the order of 5-6μm and 0.5°-0.8°are found for FINEMET-type amorphous alloy after LN *** is also found that LN treatment can contribute a little stored energy to the magnetic domain wall movement and magnetic domain rotation.
