Purpose: To quantify retinal function longitudinally and crosssectionally in p atients with autosomal-recessive early-onset severe retinal dystrophy (EOSRD) associated with RPE65 mutations. Subjects and methods: The ocular phenotype was characterized in four children from three families up to the second decade of li fe, and in three siblings from one family aged 43-54 years carrying compound he terozygous or homozygous mutations in RPE65. Standard clinical examination inclu ded colour vision testing, fundus photography and Goldmann visual fields (GVF). Full-field ERGs (in all) and multifocal ERGs (in two patients) were also record ed. Visual performance and fundus appearance were compared to literature data. R esults: In childhood, visual acuity (va) ranged from 0.1 to 0.3, and GVF for tar get V4 was well preserved. va and GVF were measurable in only one of the three a dult siblings. Nystagmus was present in two of four children and two of three ad ults. Photophobia was absent in childhood and developed in adulthood. Funduscopi c changes were discrete during the first decade of life in three of four childre n; one patient had clear macular changes already at age 5 years. All three adult siblings had distinct retinal changes including the macula. Bone spicules were not a feature. Residual colour vision was present in all patients with measurabl e va. Rod ERGs were absent at any age; cone ERGs were detectable in early childh ood. To date, va data have been reported in 51 patients, visual fields in 29 pat ients, and a detailed fundus description in 34 patients. For all three parameters, data were compara ble to the results in our patient cohort. conclusion: In childhood, patients wit h RPE65 mutations have better visual functions than typically seen in Leber cong enital amaurosis. The phenotype shows a common progressive pattern with intrafam ilial and interfamilial variation. The data suggest a preserved retinal morpholo gy at young ages, arguing for vision-restoring

Purpose: To determine the prevalence of diabetic retinopathy and the causes of visual impairment in an unselected population of type 2 diabetes patients, and to describe the risk factors for developing diabetic retinopathy in this populat ion. methods: A total of 10851 type 2 diabetes patients were identified in the c ounty of úrhus. A representative sample of 378 patients underwent a routine ocu lar examination, including fundus photography. Blood pressure and serum haemoglo bin A1c, total cholesterol, high density lipoprotein cholesterol, triglyceride a nd apolipoprotein a were measured. Results: The prevalence of diabetic retinopat hy in the type 2 diabetes population was 31.5%. In all, 2.9%had proliferative diabetic retinopathy and 5.3%had clinically significant macular oedema. Of the latter, 8/20 (40%) were newly identified and had not yet been lasertreated. The re was a positive correlation between severity of retinopathy and duration of di abetes, HbA1c, systolic blood pressure and treatment with insulin. None of the p atients had social blindness (visual acuitydevelo ped visual impairment (va conclusion: The prevalence of diabetic retinop athy and visual impairment in this unselected type 2 diabetes population was low er than anticipated from the existing literature,and causes other than diabetic retinopathy contributed significantly to the occurrence of visual loss. A substa ntial number of the patients with vision-threatening diabetic maculopathy had n ot been referred for timely photocoagulation treatment.

Background: 18-Fluorodeoxyglucose positron emission tomography(PET) is used c linically to detect recurrent colon cancer after surgical resection, but the sen sitivity of PET for premalignant colon lesions and early stage colon cancer is n ot well defined. methods: In a prospective study, 45 patients with a total of 58 colonic neoplasms, including premalignant polyps, premalignant, flat lesions, a nd early stage cancers, were evaluated by PET. Results: The sensitivity of PET f or cancer was 62%(8/13). PET detected 100%(7/7) of cancers 2 cm or larger but only 17%(1/6) of cancers smaller than 2 *** detected 23%(3/13) of flat, pre malignant lesions; 70%(7/10) of protruded, premalignant lesions 3 cm or larger; 38%(3/8) of protruded, premalignant lesions between 2 and 2.9 cm;and 14%(2/14 ) of protruded, premalignant lesions between 1 and 1.9 cm. There was no false-p ositive PET reading. conclusions:PET has limited sensitivity for flat, premalign ant lesions;protruded, premalignant lesions smaller than 3 cm; and colon cancers smaller than 2 cm.

A substantial number of studies have been published since the Ninth International Workshop on Tropical cyclones(IWTc-9)in 2018,improving our understanding of the effect of climate change on tropical cyclones(Tcs)and associated hazards and *** studies have reinforced the robustness of increases in Tc intensity and associated Tc hazards and risks due to anthropogenic climate *** modeling and observational studies suggested the potential influence of anthropogenic climate forcings,including greenhouse gases and aerosols,on global and regional Tc activity at the decadal and century time ***,there are still substantial uncertainties owing to model uncertainty in simulating historical Tc decadal variability in the Atlantic,and the limitations of observed Tc *** projected future change in the global number of Tcs has become more uncertain since IWTc-9 due to projected increases in Tc frequency by a few climate models.A new paradigm,Tc seeds,has been proposed,and there is currently a debate on whether seeds can help explain the physical mechanism behind the projected changes in global Tc *** studies also highlighted the importance of large-scale environmental fields on Tc activity,such as snow cover and air-sea *** projections on Tc translation speed and medicanes are new additional focus topics in our *** and future research are proposed relevant to the remaining scientific questions and assisting policymakers.

To investigate chorionic villous vasculogenesis(maturation)and development of the vasculosyncytial membrane(margination)using cD34immunohistochemistry.c ase -control *** analysis of first trimester chori-onic *** patients with anembryonic pregnancies,12with embryonic death,and 12with t erminated normal *** analysis of chorionic villi blinded to group and gestational age using *** parameters(mean functional vascular area,vessels with a lumen,and heman giogenetic cords,peripherally or centrally located).Terminated normalpregnancies show significantly more vessels per chorionic villus(maturation)(mean ±SEm)in comparison with embryonic deaths and anembryonic pr egnancies(5.3±0.3vs.1.4±0.2and 0.7±0.1),located mainly peripherally(margination)(3.0±0.2vs.0.9±0.2and 0.2±0.0).Anembryonic pregnancies show sign ifi-cantly more centrally located cords in comparison with embryonic deaths and termination of pregnancies(3.3±0.2vs.2.7±0.2and 1.5±0.1).A defective chori-onic villous vascularization,demo nstrating inadequate vasculogenesis and abnormal develo pment of the vascu-losyncytial membrane,is seen in pre gnancies complicated by embryonic death and is even more pr onounced in anembryonic *** of placental vasculo-genesis is a basic feature in all types of pregnancy and is subsequently modulated directly or indirectly by embryonic signaling.

Hepatitis A virus (HAV) superinfection is associated with a high risk of liver failure and death in patients with underlying chronic liver disease. Although HAV vaccination is recommended for all patients with chronic hepatitis c virus (HcV) infection, little is known about adherence to these recommendations in clinical practice. The aims of this study were to determine the frequency of HAV testing and vaccination among patients with chronic HcV infection. We conducted a retrospective cohort study of 1,193 patients diagnosed with chronic HcV infection over a 1-year period. During 1,646 person-years of follow-up, patients were seen by their primary care provider a median of 10.0 times (interquartile range, 4.0- 20.0). HAV antibody testing was performed in 640 subjects (53.6% ), and 317 (49.5% ) of those tested were susceptible (HAV antibody negative). Only 94 of the 1,193 patients (7.9% ) received the HAV vaccine, including 26.8% of the 317 susceptible patients, 0.9% of the 323 patients who were already immune to HAV, and 1.1% of the 553 subjects who were never tested. Among the 94 vaccinated patients, 45 received only one dose of the vaccine. Three of the unvaccinated patients developed acute HAV infection during follow-up, and 1 of them died of acute liver failure. In conclusion, despite published recommendations to vaccinate against HAV in patients with chronic HcV infection, we found that HAV testing and vaccination rates were low in clinical practice. Public health programs to increase awareness about HAV vaccination in patients with chronic liver disease are needed.

The East Asian Very Long Baseline Interferometry(VLBI) Network(EAVN) is a rapidly evolving international VLBI array that is currently promoted under joint efforts among china,Japan and *** aims at forming a joint VLBI Network by combining a large number of radio telescopes distributed over East Asian *** the combination of the Korean VLBI Network(KVN) and the VLBI Exploration of Radio Astrometry(VERA) into Ka va,further expansion with the joint array in East Asia is actively *** we report the first imaging results(at 22 and 43 GHz) of bright radio sources obtained with Ka va connected to Tianma 65-m and Nanshan 26-m Radio Telescopes in *** test the EAVN imaging performance for different sources,we observed four active galactic nuclei(AGN) having different brightness and *** a result,we confirmed that the Tianma 65-m Radio Telescope(TmRT) significantly enhances the overall array sensitivity,a factor of 4 improvement in baseline sensitivity and 2 in image dynamic range compared to the case of Ka va *** addition of the Nanshan 26-m Radio Telescope(NSRT) further doubled the east-west angular *** the resulting high-dynamic-range,high-resolution images with EAVN(Ka va+TmRT+NSRT),various fine-scale structures in our targets,such as the counter-jet in m87,a kink-like morphology of the 3 c 273 jet and the weak emission in other sources are successfully *** demonstrates the powerful capability of EAVN to study AGN jets and to achieve other science goals in *** expansion of EAVN will further enhance the angular resolution,detection sensitivity and frequency coverage of the network.
