BAcKGROUND colorectal anastomotic leakage(cAL)is one of the most dreaded complications after colorectal surgery,with an incidence that can be as high as 27%.This event is associated with increased morbidity and mortality;therefore,its early diagnosis is crucial to reduce clinical consequences and costs.Some biomarkers have been suggested as laboratory tools for the diagnosis of cAL.AIM To assess the usefulness of plasma c-reactive protein(cRP)and calprotectin(cLP)as early predictors of cAL.METHODS A prospective monocentric observational study was conducted including patients who underwent colorectal resection with anastomosis,from March 2017 to August 2019.Patients were divided into three groups:G1–no complications;G2–complications not related to cAL;and G3–cAL.Five biomarkers were measured and analyzed in the first 5 postoperative days(PODs),namely white blood cell(WBc)count,eosinophil cell count(Ecc),cRP,cLP,and procalcitonin(PcT).clinical criteria,such as abdominal pain and clinical condition,were also assessed.The correlation between biomarkers and cAL was evaluated.Receiver operating characteristic(ROc)curve analysis was used to compare the accuracy of these biomarkers as predictors of cAL,and the area under the ROc curve(AUROc),specificity,sensitivity,positive predictive value,and negative predictive value(NPV)during this period were estimated.RESULTS In total,25 of 396 patients developed cAL(6.3%),and the mean time for this diagnosis was 9.0±6.8 d.Some operative characteristics,such as surgical approach,blood loss,intraoperative complications,and duration of the procedure,were notably related to the development of cAL.The length of hospital stay was markedly higher in the group that developed cAL compared with the group with complications other than cAL and the group with no complications(median of 21 d vs 13 d and 7 d respectively;Pctive performance was on POD4 and POD5,with the largest AUROc of 0.84 on POD4.Worsening of the clinical condition was associated with t

BAcKGROUND colorectal anastomotic leakage(cAL),a severe postoperative complication,is associated with high morbidity,hospital readmission,and overall healthcare costs.Early detection of cAL remains a challenge in clinical practice.However,some decision models have been developed to increase the diagnostic accuracy of this event.AIM To develop a score based on easily accessible variables to detect cAL early.METHODS Based on the least absolute shrinkage and selection operator method,a predictive classification system was developed[Early coloRectAL Leakage(E-cRALL)score]from a prospective observational,single center cohort,carried out in a colorectal division from a non-academic hospital.The score performance and cAL threshold from postoperative day(POD)3 to POD5 were estimated.Based on a precise analytical decision model,the standard clinical practice was compared with the E-cRALL adoption on POD3,POD4,or POD5.A cost-minimization analysis was conducted,on the assumption that all alternatives delivered similar health-related effects.RESULTS In this study,396 patients who underwent colorectal resection surgery with anastomosis,and 6.3%(n=25)developed cAL.Most of the patients who developed cAL(n=23;92%)were diagnosed during the first hospital admission,with a median time of diagnosis of 9.0±6.8 d.From POD3 to POD5,the area under the receiver operating characteristic curve of the E-cRALL score was 0.82,0.84,and 0.95,respectively.On POD5,if a threshold of 8.29 was chosen,87.4%of anastomotic failures were identified with E-cRALL adoption.Additionally,score usage could anticipate cAL diagnosis in an average of 5.2 d and 4.1 d,if used on POD3 and POD5,respectively.Regardless of score adoption,episode comprehensive costs were markedly greater(up to four times)in patients who developed cAL in comparison with patients who did not develop cAL.Nonetheless,the use of the E-cRALL warning score was associated with cost savings of€421442.20,with most(92.9%)of the savings from patients who did not develop cAL.cONcLUSION The E-cRALL

In the field of tissue engineering the choice of materials is of great importance given the possibility to use biocompatible polymers produced by means of biotechnology.A large number of synthetic and natural materials have been used to this purpose and processed into scaffolds using Electrospinning technique.Among materials that could be used for the fabrication of scaffold and degradable membranes,natural polymers such as collagen,elastin or fibroin offer the possibility to design structures strictly similar to the extracellular matrix(EcM).Biotechnology and genetic engineering made possible the advent of a new class of biopolymers called protein-based polymers.One example is represented by the silk-elastin-proteins that combine the elasticity and resilience of elastin with the high tensile strength of silk-fibroin and display engineered bioactive sequences.In this work,we use electrospinning technique to produce a fibrous scaffold made of the corecombinamer Silk-ELR.Obtained fibres have been characterized from the morphological point of view.Homogeneity and morphology have been explored using Scanning Electron Microscopy.A thorough study regarding the influence of Voltage,flow rate and distance have been carried out to determine the appropriate parameters to obtain the fibrous mats without defects and with a good distribution of diameters.cytocompatibility has also been in vitro tested.For the first time we use the co-recombinamer Silk-ELR for the fabrication of a 2.5 angioplasty balloon coating.This structure could be useful as a coated scaffold for the regeneration of intima layer of vessels.

BAcKGROUND Primary sclerosing cholangitis(PSc)manifests within a broad ethnic and racial spectrum,reflecting different levels of access to health care.AIM To evaluate the clinical profile,complications and survival rates of patients with PSc undergoing liver transplantation(LTx)at a Brazilian reference center.METHODS All patients diagnosed with PSc before or after LTx were included.The medical records were reviewed for demographic and clinical variables,including outcomes and survival.The level of statistical significance was set at Pcohort represented 1.6%(n=34)of the 2113 patients receiving liver grafts at our service over the past two decades.Most were male(n=19;56%).The average age(40±14 years)was similar for men and women(P=0.347).The mean follow-up time from diagnosis to LTx was 68 mo.Most patients had the classic form of PSc.Three women had PSc/autoimmune hepatitis overlap syndrome,and one patient had small-duct PSc.Alkaline phosphatase levels at diagnosis and pre-LTx model for end-stage liver disease.scores were significantly higher in males.Inflammatory bowel research(IBD)was investigated by colonoscopy in 26/34(76%)and was present in most cases(18/26;69%).IBD was less common in women than in men(44.4%vs.55.6%)(P=0.692).cholangiocarcinoma(ccA)was diagnosed in 2/34(5.9%)patients by histopathology of the explant(survival:3 years 6 mo,and 4 years 11 mo).Two patients had complications requiring a second LTx(one after 7 d due to hepatic artery thrombosis and one after 17 d due to primary graft dysfunction).Five patients(14.7%)developed biliary stricture.The overall median post-LTx survival was 66 mo.Most deaths occurred in the first year(infection n=2,primary liver graft dysfunction n=3,unknown cause n=1).The 1-year and 5-year survival rates of this cohort were 82.3%and 70.6%,respectively,matching the mean overall survival rates of LTx patients at our center(87.1%and 69.43%,respectively)(P=0.83).cONcLUSION Survival after 1 and 5 years was similar to that of other LTx indications.The observed

Laminated composite materials are widely implemented in several engineering constructions. For its relative light weight, these materials are suitable for aerospace, military, marine, and automotive structural applications. To obtain safe and economical structures, the modelling analysis accuracy is highly relevant. Since meshless methods in the recent years achieved a remarkable progress in computational mechanics, the present work uses one of the most flexible and stable interpolation meshless technique available in the literature—the Radial Point Interpolation Method(RPIM).Here, a 2 D approach is considered to numerically analyse composite laminated beams. Both the meshless formulation and the equilibrium equations ruling the studied physical phenomenon are presented with detail. Several benchmark beam examples are studied and the results are compared with exact solutions available in the literature and the results obtained from a commercial finite element software. The results show the efficiency and accuracy of the proposed numeric technique.

Novel Investigation on Ammonium Thiosulphate (ATS) as an Inhibitor of Soil Urease and Nitrification

Among the numerous products so far promoted as effective urease and/or nitrification inhibitors, it is possible to detect a renewed interest in environmentally friendly tools, such as ammonium thiosulphate (ATS, (NH4)2S2O3) which is currently used as fertilizer for N and S nutrition. Among contradictory results accounted in the current literature, there is reported that ammonium thiosulphate (ATS) exerts inhibitory activity at large but unlikely agronomic rates of 2500 - 5000 mg·kg-1 soil. We carried out a novel experiment aimed to: a) verify the inhibitory action of ATS, even when applied in soil at low rates (25 and 100 mg·kg-1 soil as S-ATS), towards urease activity and nitrification in urea treated and not treated soils;b) investigate on the influence of ATS on the soil microbial biomass as it is generally assumed that soil microorganisms are the main agents of urea hydrolysis. For these purposes we selected an arable sandy soil and a grassland sandy-loam soil which are treated with urea or/and ATS. Results obtained from this novel investigation showed that a) ATS significantly decreased urease activity in both soil types and that the exerted inhibitory effect was moderate and short-term;b) ATS retarded the hydrolysis of urea and lowered nitrate production in the urea treated soils;c) the available fraction of iron and manganese in the used soils might be involved in the mechanism of inhibition;d) ATS did not affect the size of soil microbial biomass pool;e) the efficiency of ATS as urease and nitrification inhibitor was more evident in the sandy soil. Because of its properties, ATS may be applicable on many crops without being harmful on the soil microbial pool.

Attalea speciosa Mart. ex Spreng (baba?u) is a palm tree of considerable economic importance in Brazil, especially in the northeast portion of the country. The myxobiota of baba?u was studied by examining living and dead trunks, leaves, inflorescences and fallen fruits on the ground during the wet season at the Teresina Park, Piauí State, Brazil. Taxonomic diversity, species diversity, frequency of occurrence, abundance and constancy of each species were determined in two areas of deciduous mixed forest. The taxonomic diversity of the myxobiota was similar, with values of 1.5 and 1.6 for the two areas. The highest frequency of occurrence value was recorded for the spathe (50% -54%), followed by the petiole (18% -19%) and leaf sheath (4% -19%). Myxomycetes were absent on fallen fruits and rare on the leaf blade (6%) and dead (9%) or living (2%) trunks. Hemitrichia serpula (Scop.) Rostaf. ex Lister, Perichaena depressa Lib., Arcyria cinerea (Bull.) Pers. and H. calyculata (Speg.) M. L. Farr were the species characterized by the highest levels of constancy, abundance and frequency. cribraria microcarpa (Schrad.) Pers. and Stemonitopsis typhina (F. H. Wigg.) Nann.-Bremek. were occasionally present. ceratiomyxa fruticulosa (Mull.) T. Macbr., clastoderma debaryanum A. Blytt, craterium aureum (Schumach.) Rostaf. and Physarum melleum (Berk. & Broome) Massee were infrequently encountered. Other species recorded were Arcyria denudata (L.) Wettst., cribraria cancellata (Batsch.) Nann.-Bremek., c. violacea Rex, Lycogala exiguum Morgan, Metatrichia vesparia (Batsch) Nann.-Bremek. ex G. W. Martin, Physarum bogoriense Racib., P. nucleatum Rex, P. pusillum (Berk. & M. A. curtis) G. Lister and Stemonitis axifera (Bull.) T. Macbr. The myxobiota of the shaded study area was characterized by a higher diversity (6.10) than the unshaded area (2.10), and the incidence of myxomycetes on the A. speciosa trees selected for study was appreciably higher (100% and 40%, respectively).
