Parietal Hernias in Adults at Bouaké University Hospital: Epidemiological and Diagnostic Aspects

Introduction: A hernia of the abdominal wall is defined as the externalization of a viscera of the abdominal cavity in relation to a deficiency of the components of the wall. Topographies are diverse and diagnosis is based on clinical practice. The objective of this study was to describe the epidemiological and diagnostic aspects of abdominal wall hernias at the University Hospital of Bouaké, côte d’Ivoire. Patients and Methods: This was a descriptive retrospective study that ran from January 2010 to December 2022. It concerned all patients admitted for abdominal wall hernias. Results: We collected 497 records of patients admitted for abdominal wall hernia. Abdominal wall hernias accounted for 7.6% of patients admitted to the ward (n = 6512) and were the fourth most common patient. These were males (79%;n = 392) and females (21%;n = 105). The mean age was 43 years [range: 17 and 70 years]. The occupation was dominated by forced labourers with 73% (n = 372) more specifically farmers (42%;n = 209). The average consultation time was 2 days (extreme 1 and 8 days). The main clinical signs were abdominal pain for strangulated hernias with or without occlusive signs (n = 397;79.8%) and reducible bothersome swelling with intermittent pain (n = 100;20.2%) for non-strangulated hernias. Hernias of the groin (inguinal and femoral hernia) were the most frequent with 85% (n = 422) followed by hernias of the white line (epigastric, umbilical and hypogastric) with 14% (n = 71) and lumbar hernias (Spiegel, Jean-Louis Petit and Grynfeltt) with 1% (n = 4), we did not note any cases of pelvic hernia (obturator and ischiatic). conclusion: Parietal hernias are a common surgical condition at the Ivory coast University Hospital, particularly in Bouaké. Groin hernia is the most common topographic variety. Strangulation is the main complication.

AIM: To assess the usefulness of urinary trypsinogen-2 test strip, urinary trypsinogen activation peptide (TAP),and serum and urine concentrations of the activation peptide of carboxypeptidase B (cAPAP) in the diagnosisof acute ***: Patients with acute abdominal pain and hospitalized within 24 h after the onset of symptoms were prospectively studied. Urinary trypsinogen-2 was considered positive when a clear blue line was observed (detection limit 50 μg/L). Urinary TAP was measured using a quantitative solid-phase ELISA, and serum and urinary cAPAP by a radioimmunoassay ***: Acute abdominal pain was due to acute pancreatitis in 50 patients and turned out to be extrapancreatic in origin in 22 patients. Patients with acute pancreatitis showed significantly higher median levels of serum and urinary cAPAP levels, as well as amylase and lipase than extrapancreatic controls. Median TAP levels were similar in both groups. The urinary trypsinogen-2 test strip was positive in 68% of patients with acute pancreatitis and 13.6% in extrapancreatic controls (PcAPAP was the most reliable test for the diagnosis of acute pancreatitis (sensitivity 66.7%, specificity 95.5%, positive and negative predictive values 96.6% and 56.7%, respectively), with a 14.6 positive likelihood ratio for a cut-off value of 2.32 nmol/***: In patients with acute abdominal pain,hospitalized within 24 h of symptom onset, cAPAP in serum and urine was a reliable diagnostic marker of acute pancreatitis. Urinary trypsinogen-2 test strip showed a clinical value similar to amylase and *** TAP was not a useful screening test for the diagnosis of acute pancreatitis.

Aspergillus parasiticus, a common fungal contaminant in food, produces aflatoxin B1, which is classified as human carcinogen. Kefir is an ancient fermented beverage obtained by the fermentation of different substrates with kefir grains. A very important waste produced by the dairy cheese industry is the whey permeate, which nowadays is a strong ambient contaminant. The aim of this work was to assess the effect of whey permeates fermented with kefir grains against A. parasiticus growth, aflatoxin B1 biosynthesis, and the kefir microorganisms protection against the cell damage produced by aflatoxin B1. It was observed that kefir-cell-free-supernatants (cFS) produced fungal inhibition. A fungicidal effect was observed with 65% v/v of cFS in the culture medium (final pH 4.55 and total undissociated lactic and acetic acid concentration 34.08 mM). Under these conditions, aflatoxin production was not detected. Finally, it was found that non-viable kefir microorganisms protected HepG2 cells from the damage produced by aflatoxin B1.

Invasion genetics： Lessons from a ubiquitous bird, the house sparrow Passer domesticus

Following an introduction, non-native species are exposed to environments that differ from those found in their native range; further, as these non-native species expand beyond the site of introduction, they must constantly adapt to novel environ- ments. Although introduced species are present across most ecosystems, few species have successfully established themselves on a truly global scale. One such species, the house sparrow Passer domesticus, is now one of the world＇s most broadly distributed vertebrate species and has been introduced to a great part of its current range. To date, work on four continents suggests both ge- netic and phenotypic variation exists between native and introduced ranges. As such, house sparrows represent an excellent op- portunity to study adaptations to novel environments and how these adaptations are derived. The global distribution of this spe- cies and the multiple independent introductions to geographically isolated sites allow researchers to ask questions regarding ge- netic variation and adaptation on a global scale. Here, we summarize the molecular studies of invasive house sparrows from the earliest work using allozymes through more recent work on epigenetics; using these studies, we discuss patterns of dispersal of this species. We then discuss future directions in techniques （e.g. next generation sequencing） and how they will provide new in- sight into questions that are fundamental to invasion biology. Finally, we discuss how continued research on the house sparrow in light of these genetic changes and adaptations will elucidate answers of adaptation, invasion biology, range expansion, and resi- lience in vertebrate systems generally

Dear Editor,Breast cancer(Bc)is the most commonly diagnosed cancer and the fifth cause of cancer-related death worldwide[1].Despite the advances in Bc targeted therapies,cytotoxic chemotherapy is still widely used[2].However,around 20%-30%of Bc patients develop metastasis after treatment as a consequence of drug resistance[3].In this context,microRNAs have emerged as potential therapeutic targets to overcome therapy resistance[4].Therefore,we aimed to elucidate the molecular mechanisms underlying resistance to doxorubicin,one of the most effective chemotherapeutic agents used in *** are detailed in Supplementary Materials.

Objective To investigate alterations in the microtubule-associated protein 2 (MAP-2) of neurons in Wistar rats and the effect of nimodipine (Nim), D-2-amino-5-phosphonovaleric acid (D-AP-5) and mild hypothermia on neuronal MAP-2 following fluid percussion injury (FPI).Methods Alterations of MAP-2 in Wistar rat neurons following FPI were measured by a confocal laserscanning microscope using MAP-2 immunofluorescence staining as a MAP-2 *** MAP-2 immunofluorescence staining was limited to the cell bodies and dendritic compartments of neurons and more intense in dendrites than in cell bodies. The loss of MAP-2 was marked at 3 h posttrauma ( P ＜ 0.01 ), and reached a maximum at 48 h post-trauma. Afterwards, fluorescence recovered partly at 72 h post-trauma. The application of Nim markedly reduced the loss of MAP-2 immunoreectivity within 1 h post-trauma ( P ＜ 0.01 ), and the application of D-AP-5 markedly reduced the loss of MAP-2immunoreactivity within 10 h post-injury ( P ＜ 0.01 ). The application of mild hypothermia decreased the loss of MAP-2 immunoreactivity within 1 h post-injury (P＜ 0.05).conclusions The partial recovery of fluorescence at 72 h post-trauma indicate that the partial structure of the neuronal microtubules can be repaired by itself. Nim, D-AP-5 and mild hypothermia reduce the degradation of MAP-2 by different mechanisms. The treatment of neuronal cytoskeleton degradation following FPI must employ multiple therapeutic approaches.

The adsorption capacity of hydrogels derived from modified xylan hemicellulose has been tested in order to develop new bio-based adsorbent materials useful for removing metal ions pollutants,such as cd(Ⅱ),cu(Ⅱ)and Pb(Ⅱ)from an aqueous *** was extracted from bleached kraft pulp of eucalyptus and subsequently modified with different proportions of functional sulfonic acid groups(HA3-HA7)and sulfonate groups(HS30-HS70)to generate *** results showed that all the synthesized hydrogels were capable of adsorbing metal ions,being the hydrogels with 30%and 50%xylan the ones that presented the highest adsorption *** capacity studies at different initial concentrations revealed that at an initial concentration of 300 mg·L-1,the HA3 hydrogel presented an adsorption capacity of 193mg Pb(Ⅱ),182 mg cd(Ⅱ),and 66 mg cu(Ⅱ)per g *** HA5 hydrogel presented a capacity of 185 mg Pb(Ⅱ),113 mg cd(Ⅱ),and 48 mg cu(Ⅱ)per g *** HS30 hydrogel exhibited an adsorption of 205 mg Pb(Ⅱ),174 mg cd(Ⅱ),and 71 mg cu(Ⅱ)per g hydrogel,and HS50 hydrogel exhibited an adsorption capacity of 273 mg Pb(Ⅱ),143 mg cd(Ⅱ),and 45 mg cu(Ⅱ)per g *** results show that modified Xylan hemicellulose is a promising adsorbent for removal cd(Ⅱ),cu(Ⅱ),and Pb(Ⅱ)ions from aqueous solutions.
