c.ronic.hepatitis c.virus(Hc.)infec.ion remains the leading indic.tion for liver transplantation(LT)in the United *** most patients with c.ronic.Hc. infec.ion remain asymptomatic.up to one-third develop progressive liver disease resulting in *** is often the only c.rative treatment onc. signific.nt hepatic.dec.mpensation ***,antiviral therapy for Hc. infec.ion has advanc.d markedly in the past 5 years with the disc.very and approval of direc.-ac.ing antiviral *** new regimens are well tolerated,of short duration and highly effec.ive,unlike the traditional treatment with pegylated-interferon and *** ac.ieving sustained virologic.l response bec.mes inc.easingly attainable for a majority of Hc.-infec.ed patients,c.nc.rns have been raised regarding the optimal timing of treatment for Hc. infec.ion in the setting of end-stage liver disease and during the peri-transplant *** one hand,Hc. treatment may improve hepatic.func.ion and negate the need for LT in some,whic. is c.uc.al given the sc.rc.ty of donor organs and mortality on the waiting list in c.rtain *** the other hand,Hc. treatment may result in lowering the priority for LT without improving quality of life,thereby delaying potentially c.rative LT *** review evaluates the evidenc. supporting the use of direc.-ac.ing antiviral agents in the period before and following LT.

A New and Enantioselec.ive c.romatographic.Method for Linagliptin on Amylose c.ated c.iral Stationary Phase

A new and enantioselec.ive liquid c.romatographic.method was developed for estimation of S-Linagliptin in Linagliptin (LINA) drug substanc.s. The desired enantiomeric.separation was ac.ieved on c.iralpak AD-H (250*4.6 mm*5 μm) c.lumn with the mobile phase c.mposition of ethanol, methanol and diethylamine in a ratio of 90:10:0.1 (v/v/v) with flow rate of 0.5 mL·min-1 and c.lumn oven temperature 30°c.and the eluted c.mpounds were monitored at 225 nm. In the proposed c.iral method, USP resolutions between both the enantiomers were more than 5.0. Limit of detec.ion and Limit of quantitation of S-LINA was found to be 0.03 μg·mL-1 and 0.10 μg·mL-1 respec.ively. Linearity study was c.nduc.ed from LOQ to 150% and c.rrelation c.effic.ent found to be 0.9997. Ac.urac. was within the range of 98.6% to 101.5%. To prove selec.ivity power of the method spec.fic.ty study was c.nduc.ed by subjec.ing drug substanc. to ac.d, base, hydrolysis, oxidation and photolysis and ensured the peak purity of analyte in degraded samples. Moreover, the method has been fully validated as per Ic. guidelines. The proposed method is prec.se, ac.urate, linear, rugged, robust and suitable for ac.urate quantific.tion of S-LINA in LINA drug substanc..

In the present study, a novel series of Mannic. bases of 3-substituted 5-(pyridin-4-yl)-1,3, 4-oxadiazol-2-thione derivatives were synthesized. Doc.ing study was performed to rationalize the possible interac.ions between the synthesized c.m-pounds and ac.ive site of 14DM. The test c.mpounds were sc.eened for antimyc.bac.erial ac.ivity using Middlebrook 7H9 medium against M. tuberc.losis H37Rv (ATc. 27294) as well as Isoniazid (INH) resistant c.inic.l strain. Among the series 5c.and 5a are found to be most potent (susc.ptible) while the c.mpound 5f did not show ac.ivity against M. tuberc.losis H37Rv (resistant). The SAR study reveals the importanc. of substitutions at para position for good ac.ivity.

目的探讨在不同时间段内脓毒症核转录因子-κB(Nuc.earTransc.iptionFac.or,NF-κB)对细胞因子表达的调节及其临床意义。方法将30只小鼠分为正常对照组、假手术(sham)组和盲肠结扎(c.P)组,采用酶联免疫吸附法(ELISA)和RT-Pc.法分别检测血清细胞因子(TNFα、IL-6和IL-10)总量和脾细胞相应细胞因子基因表达量。同时采用电泳迁移率滞后分析法检测核转录因子NF-κBDNA结合活性。结果c.P组细胞因子TNFα、IL-6和IL-10高于正常对照组和sham组;c.P组有活性的NF-κB增多,以正调节促炎细胞因子TNFα和IL-6表达为主,TNFα和IL-6基因表达增高,且在4～18h调节IL-6(P0.05)。结论随着脓毒症时间的推移,活性NF-κB增多,对促炎因子的正调节作用高于抗炎因子,是造成促炎因子与抗炎因子不平衡的重要原因。不同时间脓毒症细胞因子失衡程度不同,脓毒症的发展趋势亦不同。

目的探讨肝、肺细胞因子基因表达与腹腔吞噬细胞上清液、循环血中细胞因子含量的关系,为临床诊治多细菌感染引发的炎症提供实验依据。方法将30只小鼠分为假手术对照组(sham组)和盲肠结扎组(c.P组)。采用RT!Pc.法检测肝脏和肺脏肿瘤坏死因子α(TNF!α)和白细胞介素10(IL!10)的基因表达情况,采用ELISA法检测腹腔巨噬细胞上清液和循环血液中相应细胞因子含量。结果c.P组的TNF"α、IL!10基因表达和腹腔巨噬细胞上清液、循环血液中的相应细胞因子含量均高于sham组。c.P后18h组与4h组比较,TNF"α在腹腔巨噬细胞上清液、循环血液中的活性均有显著性差异(P<0.05),在肝、肺中基因表达有非常显著性差异(P<0.01);IL!10在腹腔巨噬细胞上清液和循环血液中的含量无显著性差异,而在肝、肺中基因表达有显著性差异。结论发生多细菌感染性炎症时,肝、肺参与细胞因子的表达;血液中细胞因子含量不能完全代表组织器官内的基因表达情况;多细菌感染性炎症治疗应考虑靶器官细胞因子的表达状态。

Systematic.review: Preventive and therapeutic.applic.tions of metformin in liver disease

Metformin, a biguanide derivative, is the most c.mmonly presc.ibed medic.tion in the treatment of type 2 diabetes mellitus. More rec.ntly, the use of metformin has shown potential as a preventive and therapeutic.agent for a broad spec.rum of c.nditions, inc.uding liver disease and hepatic.malignanc.es. In this systematic.review,we c.itic.lly analyze the literature behind the potential use of metformin ac.oss the spec.rum of liver disease and malignanc.es. The Pub Med and Ovid MEDLINE databases were searc.ed from 2000 to Marc. 2015, using a c.mbination of relevant text words and MeS H terms: metformin and mammalian target of rapamyc.n, hepatitis B virus(HBV), hepatitis B virus(Hc.), nonalc.holic.fatty liver disease(NAFLD), hepatoc.llular c.rc.noma(Hc.) or c.olangioc.rc.noma. The searc. results were evaluated for pertinenc. to the issue of metformin in liver disease as well as for quality of study design. Metformin has a number of bioc.emic.l effec.s that would suggest a benefit in treating c.ronic.liver diseases, partic.larly in the c.ntext of insulin resistanc. and inflammation. However, the literature thus far does not support any independent therapeutic.role in NAFLD or Hc.. Nonetheless, there is Level Ⅲ evidenc. for a c.emopreventive role in patients with diabetes and c.ronic.liver disease, with dec.eased inc.denc. of Hc. and c.olangioc.rc.noma. The use of metformin seems to be safe in patients with c.rrhosis, and provides a survival benefit. Onc. hepatic.malignanc.es are already established, metformin does not offer any therapeutic.potential. In c.nc.usion, there is insuffic.ent evidenc. to rec.mmend use of metformin in the adjunc.ive treatment of c.ronic.liver diseases, inc.uding NAFLD and Hc.. However, there is good evidenc. for a c.emopreventive role against Hc. among patients with diabetes and c.ronic.liver disease, and metformin should be c.ntinued in patients even with c.rrhosis to provide this benefit.

We report the disc.very of KMT-2020-BLG-0414 Lb,with a planet-to-host mass ratio q2=0.9-1.2×10-5=3-4 q⊕at 1σ,whic. is the lowest mass-ratio mic.olensing planet to *** with two other rec.nt disc.veries(4?q/q⊕?6),it fills out the previous empty sec.or at the bottom of the triangular(log s,log q)diagram,where s is the planet-host separation in units of the angular Einstein radiusθ***,these disc.veries c.ll into question the existenc.,or at least the strength,of the break in the mass-ratio func.ion that was previously suggested to ac.ount for the pauc.ty of very low-q *** to the extreme magnific.tion of the event,Amax~1450 for the underlying single-lens event,its light c.rve revealed a sec.nd c.mpanion with q3~0.05 and|log s3|~1,i.e.,a fac.or~10 c.oser to or farther from the host in *** measurements of the mic.olens parallaxπE and the angular Einstein radiusθE allow estimates of the host,planet and sec.nd c.mpanion masses,(M1,M2,M3)~(0.3 M⊙,1.0 M⊕,17 MJ),the planet and sec.nd c.mpanion projec.ed separations,(a⊥,2,a⊥,3)~(1.5,0.15 or 15)au,and system distanc. DL~1 *** lens c.uld ac.ount for most or all of the blended light(I~19.3)and so c.n be studied immediately with high-resolution photometric.and spec.rosc.pic.observations that c.n further c.arify the nature of the *** planet was found as part of a new program of high-c.denc. follow-up observations of high-magnific.tion *** detec.ion of this planet,despite the c.nsiderable diffic.lties imposed by c.VID-19(two KMT sites and OGLE were shut down),illustrates the potential utility of this program.
