Laparoscopic approach in gastrointestinal emergencies

This review focuses on the laparoscopic approach to gastrointestinal emergencies and its more recent indications. Laparoscopic surgery has a specific place in elective procedures, but that does not apply in emergency situations. In specific emergencies, there is a huge range of indications and different techniques to apply, and not all of them are equally settle. We consider that the most controversial points in minimally invasive procedures are indications in emergency situations due to technical difficulties. Some pathologies, such as oesophageal emergencies, obstruction due to colon cancer, abdominal hernias or incarcerated postsurgical hernias, are nearly always resolved by conventional surgery, that is, an open approach due to limited intraabdominal cavity space or due to the vulnerability of the bowel. These technical problems have been solved in many diseases, such as for perforated peptic ulcer or acute appendectomy for which a laparoscopic approach has become a wellknown and globally supported procedure. On the other hand, endoscopic procedures have acquired further indications, relegating surgical solutions to a second place; this happens in cholangitis or pancreatic abscess drainage. This endoluminal approach avoids the need for laparoscopic development in these diseases. Nevertheless, new instruments and new technologies could extend the laparoscopic approach to a broader array of potentials procedures. There remains, however, a long way to go.

Role of Angiotensin-(1-7) on Renal Hypertrophy in Streptozotocin-Induced Diabetes Mellitus

Participation of angiotensin II in chronic kidney diseases including diabetic nephropathy (DN) has been extensively described. Similarly, several studies support a protective role for angiotensin-(1-7). However, other studies suggest that some of the cellular effects of angiotensin-(1-7) may be deleterious. The objective of this study was to determine the role of exogenous angiotensin-(1-7) on renal hypertrophy development in rats with streptozotocin-induced diabetes. A control group and three groups of rats with streptozotocin-induced diabetes: untreated diabetic rats, diabetic rats treated with captopril, and diabetic rats treated with angiotensin-(1-7), were studied. After two weeks of treatment, the kidneys were removed under anesthesia with pentobarbital. The kidneys were weighed and the renal cortex was separated for analysis of AT1R, TGF-β1, MASR, and AcE2 expression by western blot. Rats in the three groups with diabetes had hyperglycemia, increased food and water consumption, and higher urinary volume than control rats. Treatment with captopril or angiotensin-(1-7) reversed streptozotocin-induced renal hypertrophy, measured by kidney weight, protein/DNA ratio in renal cortex, glomerular area, or proximal tubular cells area, proteinuria, and creatinine clearance reduction. Patient Satisfaction Index and Quality of Life Measurement with Breast-Q after Breast Reconstruction in a Plastic Surgery center in Mexico

Introduction: cancer is one of the most devastating pathologies to affect the breast. Mastectomy stigmas are associated with depression, body image dysmorphia, and decreasing quality of life. BREAST-Q is a PROM (Patient Reported Outcome Measures) that has proven useful in measuring satisfaction with breast reconstruction results from the patient’s point of view. Objectives: To measure the satisfaction index and improvement in quality of life after breast reconstruction for breast cancer sequelae in our hospital in the last 5 years. Materials and Methods: Descriptive Study that includes patients with Breast cancer diagnosis that underwent mastectomy and breast reconstruction (prosthesis or autologous tissue) in “Hospital central Sur de Petroleos Mexicanos” (January 2015 to January 2020), whose satisfaction index was measured with BREAST-Q one year after reconstruction. Results: 153 patients were included in the analysis. Mean global satisfaction was 74 points. We observed a tendency towards higher psychosocial, sexual and appearance satisfaction in patients who underwent reconstruction with autologous tissue. The mean satisfaction with provided information was 64 points and with the medical team > 90 points. conclusion: Breast reconstruction is associated to a high satisfaction index and quality of life improvement regardless of the technique. BREAST-Q proved to be useful in evaluating patient experience and it helped us identify areas of opportunity to improve our care.

Terahertz Modulation of Hexagonal-Shaped Metamaterials Employing ZnO Quantum Dots

The utilization of Uv excitation to verify the terahertz (THz) wave modulation of hexagonal-shaped metamaterial (MM) arrays coated with synthesized photoluminescent, down-shifting ZnO quantum dots (QDs) of two different radius sizes, namely, 3.00 nm (pH 10) and 2.12 nm (pH 12), respectively is reported. In order to characterize the behavior of the MM before and after deployment of the ZnO QDs, THz time domain spectroscopy in transmission mode was employed. Upon exposure to Uv excitation, the collected amplitude modulation values were 9.21% for the pH 12 and 4.55% for the pH 10 ZnO QDs, respectively. It is anticipated that the ability to actively tune the performance of otherwise passive structures will promote the proliferation of THz signal modulation devices in the near future.

Duodenal tuberculosis presenting as gastric outlet obstruction:A case report

Gastric outlet obstruction is commonly associated with malignancies and peptic ulcer disease. However, when no malignancy is seen and the patient is nonresponsive to conventional peptic ulcer treatment, other etiologies need to be explored. We report a case of gastric outlet obstruction due to duodenal tuberculosis. The patient is a 31 year old male who presented with 1 year history of recurrent epigastric pain and an a cute episode of vom iting. Endoscopy revealed duodenal stricture. computed tomography scan showed pyloro antral thickening. The patient was referred to the surgery service and underwent an exp lor atory laparotomy and gastrojejunostomy. A duodenal mass and calcified lymph nodes were noted on exploration and biopsy revealed a tuberculous origin. The patient was started on anti-tuberculosis medications and had impro ved on discharge. Gastroduodenal tuber culosis is rare and pyloric stenosis resulting from tuber culosis is even rarer. This, however, should be consid ered in patien ts who come from areas where the dise ase is endemic.

Pig Reproductive and Respiratory Syndrome virus (PRRSv) Does Not Attach to Boar Sperm;It Affects Only the velocity Pattern and the Mobility Pattern

The purpose of the study was to evaluate the sperm viability of semen infected with PRRSv viral particles, observing the effect of the virus on the motility of boar sperm. The work was carried out at the FMvZ-BUAP Genetics and Reproduction Laboratory. 5 stallions were used. Each sample contained 1 × 106 sperm, the PRRS virus strain was ATcc-vR-2332 (0, 102, 104 and 106 copies of RNA/mL in triplicate), it was observed daily at the cASA;Hamilton Thorne®. cells with MT (P valuation with 201 ± 7.3, 167 ± 10.1, 165 ± 14.6, 134 ± 8.2 and 120 ± 8.8, respectively. The % MP between control and virus concentrations (P ≥ 0.05). The Lcv on day 1 and 7 PI at 10X2 and 10X6 (P vs control. In the correlation Matrix, where it is observed that there is a correlation between vSL and vAP, vSL and vcL, vcL and ALH, vAP with ALH. There is a correlation of vSL and ALH, STR and ALH. In this study there were (P ≤ 0.01) in the vcL, in the concentrations (102) 162.81 ± 10.65 and (106) 177.12 ± 5.77 vs 193.04 ± 4.62 of control. This indicates that altering these parameters would be related to fertility and the PRRS virus affects the Lcv. Regarding the vSL, it was observed that the sperm infected with viruses 102, 104 and 106 of 48.00 ± 3.38, 49.88 ± 1.83 and 50.55 ± 2.24 vs. 56.66 ± 1.68 of control respectively, the control would have greater possibilities of fertilizing the oocyte. In this study, it was found (P ≤ 0.01) in the vAP with 102 of 77.26 ± 5.16, 104 with 83.35 ± 2.41 and 106 with 81.29 ± 3.14 vs the control with 90.56 ± 2.07. Soil Organic Matter and Grain Yield of Rainfed Maize in Luvisols of campeche, México

The organic matter is an important soil component, due its favorable effects on soil physical and chemical properties and, by consequence, on crop yields. So, the objective of this work was quantify the changes through the time of soil organic matter in rainfed maize (Zea mays L.) system, in a period of 30 years of continuous crop and in relation to system of natural vegetation system, and also the effects of these changes on the crop yield, in Luvisols soils of State of campeche, México. In production cycle of 2004, 53 farmer plots in rainfed maize system of different land use time and five sites of forest natural vegetation system were sampled for soil, to quantify physical and chemical properties of soil, and it was taken data about climate, system management and crop yield. The data were analiced by regression analysis, considered the organic matter as function of land use time and factors of soil and management of system, and the crop yield as function of soil organic matter and factors of soil, climate and management of system. The soil organic matter diminished from 5.68% in forest natural vegetation system to 3.59% after 16 - 30 years of cultivation, and the soil annual incorporation of vegetative mulch (weeds and stubble of maize) increased the organic matter from 0.14% after 1 - 5 years to 0.46% after 16 - 30 years of cultivation. Without fertilization, the diminution of soil organic matter caused 907 kg⋅ha−1 less of maize yield, and the fertilization with 200 kg⋅ha−1 of diamonic phosphate (36 kg⋅N⋅ha−1 and 92 kg P2O5 ha−1) increased the maize yield 1224 kg⋅ha−1 after 1 - 5 years and 1421 kg⋅ha−1 after 16 - 30 years of cultivation, but not compensed less maize yield of 711 kg⋅ha−1 due the diminution of soil organic matter.

BAcKGROUND Hepatitis c virus(Hcv)infection is a public health concern *** factors,including genetic polymorphisms,may be evolved in the progression of Hcv infection to liver *** lambdas(IFNLs)modulate the immune response during viral *** induce antiviral activity,interfering in the viral replication by promoting the expression of several genes that regulate immunological *** interferon lambda-4(IFNL4)rs12979860 polymorphism,which is characterized by a c to T transition in intron 1,is associated with spontaneous and treatment-induced clearance of Hcv infection and may play a role in the development of Hcv-associated liver diseases,including hepatocellular carcinoma(Hcc).AIM To investigate the association of IFNL4 rs12979860 polymorphism with fibrosis,cirrhosis,and Hcc in patients with chronic Hcv *** This study was comprised of 305 chronic Hcv-infected patients(53 fibrosis,154 cirrhosis,and 98 Hcc cases).The control group was comprised of 260 Hcvnegative healthy *** IFNL4 rs12979860 polymorphism was genotyped using the TaqMan *** was diagnosed based on liver biopsy findings,while cirrhosis was diagnosed through clinical,laboratory,anatomopathological,and/or imaging *** was diagnosed through imaging tests,tumor,and/or anatomopathological *** The T allele was observed in the three groups of patients(fibrosis,cirrhosis,and Hcc)at a significantly higher frequency when compared with the control group(P=0.047,Pctively).Also,genotype frequencies presented significant differences between the control group and cirrhosis patients(Pcc patients(P=0.002).The risk analysis was performed using the codominant and dominant T allele *** the codominant model,it was observed that the cT genotype showed an increased risk of developing cirrhosis in comparison with the cc genotype[odds ratio(OR)=2.53;95%confidence interval(cI):1.55-4.15;

The Monte Desert is characterized by a great diversity of landforms created with fluvial， alluvial which the vegetation patterns are related to. The present work has the following objectives: 1: determine whether topographical attributes， surface characteristics， soil properties and vegetation patterns vary between alluvial landforms， and 2: define whether morphometric， soil and surface properties influence vegetation patterns along alluvial landscape. Morphometric data were obtained by processing a 5 m digital elevation model. The coverage of rock fragments， fine sediments and mulch was quantified. Observations and descriptions of the soil profiles were restricted to the uppermost 50 cm. vegetation properties were calculated using a Point Quadrat Method. The relationship between variables was evaluated through multivariate statistical analysis. The main results show the presence of 45 plant species distributed in 19 families， where shrubs are dominant. The wind effect， topographic wetness and dissection of the landscape are limiting factors of diversity. The coverage of superficial rock fragments influence vegetation coverage through the distribution and availability of rainwater. Furthermore， the different soil textures reveal that the silt content favors an increase in vegetation coverage. The presence of v horizon could condition the installation and development of vegetation in the early stages of growth.
