低频率抗体抗-Mur引起的溶血性输血反应

目的 :研究抗Mur抗体的血清学特异性及其在输血中的临床意义。方法 :患者的血清 ,与 3组试剂红细胞 (其中二组为Miltenberger抗原试剂红细胞组 )和 2 8个低频率血型抗原 ,在盐水介质、低离子强度介质和低离子间接抗球蛋白介质中反应 ,分析鉴定其抗体的特异性。结果 :患者血清与三组试剂红细胞在多种反应介质中的反应的结果显示患者血清中含有抗Mur抗体 ,患者血清与 2 8个低频率血型抗原的反应证实该例抗体只与Mur抗原反应。结论 :该例同种抗体为特异性抗Mur抗体 ,且引起溶血性输血反应。在亚洲人中Miltenberger血型系抗体筛选意义值得探讨。

【摘要】背景心血管疾病一级预防临床指南的修订扩大了接受他汀类药物治疗的患者数量。目的分析临床指南修订的潜在影响。设计与场所基于爱尔兰老龄问题纵向研究中年龄≥ 50岁的人群,对7种欧洲连续性指南的影响进行建模。方法根据不断变化的指南建议和模拟的相关潜在成本,分析无心血管疾病史的患者他汀类药物治疗资格。为预防7 种指南中推荐的治疗最低基线风险,以及2016 年指南包含的低、中、高和极高风险患者的1种主要心血管事件,作者计算了所需治疗的患者数量。比较计算所得数据与患者报告的治疗所需数量,以证明服用日常药物的合理性。结果符合他汀类药物治疗的患者比例从1987 年的8%增加到2016年的61%;相关成本从每年1 390万欧元增加到1.071亿欧元。每种指南推荐的最低风险的需要治疗的患者数量从40增加到400。到2016年,需要治疗的最低风险的患者数量为400,而极高风险的患者数量为≤25。达到治疗所需数量的他汀类药物作为日常药物服用,符合该资格的患者比例随着指导方针的修订而下降。结论他汀类药物治疗资格的增加会影响目前大部分人口和医疗保健预算。采取和报销他汀类药物应考虑预期的成本效益和患者的可接受性。

A Dialectal Chinese Speech Recognition Framework

A framework for dialectal Chinese speech recognition is proposed and studied, in which a relatively small dialectal Chinese （or in other words Chinese influenced by the native dialect） speech corpus and dialect-related knowledge are adopted to transform a standard Chinese （or putonghua, abbreviated as pTH） speech recognizer into a dialectal Chinese speech recognizer. Two kinds of knowledge sources are explored： one is expert knowledge and the other is a small dialectal Chinese corpus. These knowledge sources provide information at four levels： phonetic level, lexicon level, language level, and acoustic decoder level. This paper takes Wu dialectal Chinese （WDC） as an example target language. The goal is to establish a WDC speech recognizer from an existing pTH speech recognizer based on the Initial-Final structure of the Chinese language and a study of how dialectal Chinese speakers speak putonghua. The authors propose to use contextindependent pTH-IF mappings （where IF means either a Chinese Initial or a Chinese Final）, context-independent WDC-IF mappings, and syllable-dependent WDC-IF mappings （obtained from either experts or data）, and combine them with the supervised maximum likelihood linear regression （MLLR） acoustic model adaptation method. To reduce the size of the multipronunciation lexicon introduced by the IF mappings, which might also enlarge the lexicon confusion and hence lead to the performance degradation, a Multi-pronunciation Expansion （MpE） method based on the accumulated uni-gram probability （AUp） is proposed. In addition, some commonly used WDC words are selected and added to the lexicon. Compared with the original pTH speech recognizer, the resulting WDC speech recognizer achieves 10-18% absolute Character Error Rate （CER） reduction when recognizing WDC, with only a 0.62% CER increase when recognizing pTH. Background The aim of this study was to identify similarities and diferences in the unmet supportive care needs(USCN)of families of children with major chronic health conditions(CHCs)using a universal need assessment *** A cross-sectional online survey involving parents of children with congenital heart disease(CHD),type 1 diabetes mellitus(T1D),cancer,and asthma diagnosed within the last 5 years recruited via social media and support ***-four items assessing the USCN across six domains(care needs,physical and social needs,informational needs,support needs,fnancial needs,child-related emotional needs)were responded to on a 4-point Likert scale[no need(1)to high need(4)].Descriptive statistics identifed the level of need,and linear regressions identifed factors associated with higher need domain *** to small numbers,the asthma group was excluded from comparisons across *** One hundred and ninety-four parents completed the survey(CHD:n=97,T1D:n=50,cancer:n=39,and asthma:n=8).parents of children with cancer were most likely to report at least one USCN(92%),followed by parents of children with T1D(62%).The fve most commonly reported USCN across CHCs were drawn from four domains:child-related emotional,support,care,and *** need items were included in the top fve needs for all conditions.A higher USCN was associated with a greater frequency of hospital visits and the absence of parental *** Using a universal need assessment tool,this is one of the frst studies to characterize USCN in families of children diagnosed with common *** proportions endorsing diferent needs varied across conditions,the most endorsed needs were similar across the illness *** suggests that support programs or services could be shared across diferent CHCs.

Hepatocellular carcinoma(HCC)is the most common liver-related complication seen in patients with non-alcoholic fatty liver disease(NAFLD)and non-alcoholic steatohepatitis(NASH)(1).NASH is strongly associated with type 2 diabetes mellitus(T2DM)but it has also been apparent for a number of years that T2DM is associated with an increased risk of HCC independent of the presence of NASH(2).A 2006 meta-analysis of retrospective cohort studies and case-control studies demonstrated an association between T2DM and an increased risk of HCC(3).International guidelines advocate surveillance for HCC in patients with NASH who have progressed to cirrhosis(4).An important clinical question is whether T2DM confers an increased risk of HCC once patients with NASH have progressed to *** a recent study by Yang and colleagues(5),the authors reported strong evidence that T2DM is an independent risk factor for HCC in patients with NASH-related *** and colleagues performed a retrospective analysis of a prospectively registered research dataset of patients with NASH-related cirrhosis.

This study evaluates the mediation role of taxpayer egoism in the nexus between tax evasion intention(TEI)and its *** is measured as a composite variable(for mediation in the pROCESS macro)and then as a latent variable(for mediation in Analysis of Moment Structure Software AMOS).Further,the stringent assumptions of mediation analysis by Baron and Kenny(Strategic,and Statistical Considerations 51:1173-1182,1986)are challenged via assertions by Hayes(Introduction to Mediation,Moderation and Conditional process Analysis:A Regression-Based Approach,The Guilford press,2018).The proxies for TEI are the respondents'ethical attitudes toward taxation Determinants of tax evasion intention are grouped into perceived behavioral control attitude and subjective norms as per theory of planned behavior(TpB)and extant *** detailed variables adopted from the literature include tax awareness,tax information,tax complexity,perceived power of authority,tax morals,tax fairness,and trust in government.A totalof 151 responses were collected Mediation analysis was done via bootstrapping in the pROCESS macro and through AMOS.A plethora of studies focus on determinants of tax evasion,tax compliance and their intentions However,none combine analysis via the two tools ***,few challenge the old way of conducting mediation analysis The data provide statistical support for egoism's mediation in the effect of four determinants on tax evasion *** further confirms that many of the old preconditions for mediation are unnecessary given modern-day bootstrapping *** into determinants of TEI and egoism have been provided However,this study uses cross-sectional data Future studies must use longitudinal data or experimental manipulation to infer more specific effects.

BACKGROUND:Endoscopic therapy has been successful in the management of biliary complications after both deceased donor liver transplantation(DDLT) and living donor liver transplantation(LDLT).LDLT is thought to be associated with higher rates of biliary complications,but there are few studies comparing the success of endoscopic management of anastomotic strictures between the two *** study aims to compare our experience in the endoscopic management of anastomotic strictures in DDLT versus ***:This is a retrospective database review of all liver transplant patients undergoing endoscopic retrograde cholangiopancreatography(ERCp) after liver *** frequency of anastomotic stricture and the time to develop and to resolve anastomotic stricture were compared between DDLT and *** response of anastomotic stricture to endoscopic therapy was also ***:A total of 362 patients underwent liver transplantation between 2003 and 2011,with 125 requiring ERCp to manage biliary ***-three(9.9%) cases of DDLT and 8(27.6%) of LDLT(p=0.01) were found to have anastomotic *** comparing DDLT and LDLT,there was no difference in the mean time to the development of anastomotic strictures(98±17 vs 172±65 days,p=0.11),likelihood of response to ERCp [22(66.7%) vs 6(75.0%),p=0.69],mean time to the resolution of anastomotic strictures(268±77 vs 125±37 days,p=0.34),and the number of ERCps required to achieve resolution(3.9±0.4 vs 4.7±0.9,p=0.38).CONCLUSIONS:Endoscopic therapy is effective in the majority of biliary complications relating to liver *** strictures occur more frequently in LDLT compared with DDLT,with equivalent endoscopic treatment response and outcomes for both groups.
