Outpatient prevention counseling for malaria in northwest Nigeria: A single-centre descriptive cross-sectional study

Objective:To assess the proportion of caregivers of children and patients who received malaria prevention counseling from their healthcare provider at the time of outpatient malaria ***:This was a descriptive cross-sectional study involving 353 randomly selected adult patients and caregivers of children attending the general and paediatric outpatient clinics of a Nigerian *** interviewer-administered questionnaire was used to collect data on participants'sociodemographics and last malaria-episode *** square test and logistic regression analysis were used to determine factors associated with and predictors of receiving malaria-prevention counseling,***:The mean age of the respondents was(33.6±9.6)years;they were predominantly females(257,72.8%).Most had at least secondary-level education(304,86.1%);the family size was mostly 1-6 persons in 202(57.2%)*** were treated mostly by doctors 256(72.5%),but only 132(37.4%)received malaria prevention ***-level(x2=5.42,P=0.02),family size(Fisher's exact,P=0.01),place of treatment(Fisher's exact,P=0.0002),pretreatment malaria test(x2=9.48,P=0.002),who prescribed treatment(Fisher's exact P=0.001),consultation time(x2=22.09,P<0.001),and previous demand for malaria prevention counseling(x2=36.59,P<0.001)were associated with receiving malaria prevention ***,consultation time[Odd ratio(OR)=2.38,95%confidence interval(CI):1.28,4.40,P=0.006]and previous demand for malaria prevention counseling(OR=3.62,95%CI:2.15,6.10,P<0.001)were independent predictors of receiving ***:The proportion of respondents that received malaria prevention counseling was surprisingly *** offers opportunity for policy review towards improving the quality of outpatient malaria care through alternative strategies for giving malaria prevention information,especially in busy clinics where consultation time is constrained.

Utilization of accident and emergency department at a semi-urban Nigerian hospital: a preliminary prospective study

Objective: To assess emergency department utilization at a semi-urban (resource-limited) Nigerian hospital. Methods: A systematic random sampling technique was adopted. A proforma was used to obtain general information such as age, gender, mode of transfer, time of presentation, symptom duration, diagnoses, treatment duration, treatment outcome (transfer to the ward, referral to another hospital, discharge or death) and date and time of discharge. Chi-square test and logistic regression analysis were used to determine the association of variables with mortality and predictors, respectively. Results: Patients were predominantly male (62.2%) with a mean age of (36.0±19.0) years. Most visits occurred in September (49.1%). The median symptom duration was 24 h (interquartile range: 4.0, 72.0 ). More incidences were caused by non-surgical (61.9%) than surgical reasons. Infectious diseases (predominantly malaria, 34.5%) and injuries from road traffic accidents (mostly head injuries, 9.4%) were the commonest non-surgical and surgical cause, respectively. The mortality rate was 9.2%. Typhoid-intestinal-perforation and sepsis contributed 45.2% of overall mortality. Age (x2=16.44, P<0.001), symptom duration (x2=22.57, P<0.001), and visiting month (Fishers exact, P=0.002) were associated with mortality. Moreover, age ( 37 years) (OR=4.60, 95%CI=1.96-10.82, P<0.001) and visiting in September/October (OR=4.01, 95%CI=1.47-10.93, P=0.007) were the predictors of mortality. Conclusions: Though most patients in emergency department survive, the mortality is still high. Appropriate hospital and community interventions should be implemented to reduce mortality.

Development and Evaluation of the Mechanical Properties of Coconut Fibre Reinforced Low Density Polyethylene Composite

This research work developed and evaluated the mechanical properties of coconut fibre reinforced low density polyethylene (LPDE) composite material. The effect of fibre loading on the mechanical properties: tensile, flexural, and impact of the developed composite material have been investigated. Also carried out was the effect of fibre loading on the water absorptivity of the developed material. Sample categories of the developed composite were prepared by varying the fibre contents by weight at 0%, 10%, 20%, and 30%. The aim is to reduce the excessive waste disposal of LDPE materials that are largely found in the form of disposed water package materials (or pure water sachets) that usually affects the environment in the form of pollution. The water retting process was applied in extracting and cleaning fibre (or coir), while the mixed coir-LDPE (or developed composite material) was prepared by Compression Moulding Technique (CMT). The tensile and flexural properties were tested using Hounsfield Monsanto Tensometer (type w) while the impact properties were tested using the Charpy Impact testing machine. The microstructure of the composite was investigated using Scanning Electron Microscopy (SEM). The fractured surface morphology of the composite samples indicated a homogeneous mixture of the coir fibre and LDPE matrix. However, weak interfacial bonding between the coir fibre and LDPE matrix was also observed. The analysis of the water absorptivity showed that the developed composite materials have low water absorptivity at low fibre loading. However, at higher fibre loading, the water absorptivity increases significantly.

Conjugate Heat Transfer Predictions of Gas Turbine Hot Walls Jets Cooling: Influence of Short Hole Grid Resolutions Using Computational Fluid Dynamics*

Short hole investigations relevant to gas turbine (GT) hot walls cooling heat transfer techniques, were carried out using computational fluid dynamics (CFD) combined with conjugate heat transfer (CHT) code. The CFD software are commercial ones: ICEM for grid modelling and ANSYS Fluent for the numerical calculation, where symmetrical application prevails. The CFD CHT predictions were undertaken for Nimonic-75 metal walls with square (152.4 mm) arrays of 10 holes, whereby the lumped heat capacitance method was applied in order to determine the surface average heat transfer coefficient (HTC), h (W/m2 K) and the dimensionless Nusselt number, Nu. The major parameters considered for the short hole geometries are the pitch to diameter, X/D and length to diameter, L/D ratios and both were varied with range of D values, but X of 15.24 mm and L of 6.35 mm kept constant. Also applied, are variable mass flux, G (kg/s∙m2) and were used in predicting the flow aerodynamics in the short holes. The predictions were for classic thermal entry length into a round hole, as vena contracta, flow separation and reattachment dominates the holes, hence the development of thermal profile through the depth of the GT hot walls. Additionally, the acceleration of the flow along the wall surfaces as it approaches the holes, was a significant part of the overall heat transfer. This was shown to be independent of the hole length, even though the L/D parameter is a critical component to enhanced heat transfer. The CFD CHT predictions showed that validation of the HTC h, Nu and pressure loss, ∆P are in better agreement with measured data and within reasonable acceptance. The ∆P agreement signifies that the aerodynamics were predicted correctly, which is also the reason why the HTC expressed per wall hole approach surface area and Nu were better predicted. This illustrates how effective and efficient the wall internal heat transfer cooling is for gas turbine hot wall heat transfer
