Efficacy and safety of endoscopic prophylactic treatment with undiluted cyanoacrylate for gastric varices

AIM:To evaluate the efficacy and safety of undiluted n-butyl-2 cyanoacrylate plus methacryloxysulfolane(nBCM)as a prophylactic treatment for gastric varices(gV)***:This prospective study was conducted at a single tertiary-care teaching hospital between October 2009 and March *** with portal hypertension(PH)and gV,with no active gastrointestinal bleeding,were enrolled in primary prophylactic treatment with nBCM injection without lipiodol *** diagnosis of gV was based on endoscopy and confirmed with endosonography(EUS); the same procedure was used after treatment to confirm eradication of *** puncturing the gV with a regular injection needle,1 mL of undiluted nBCM was injected intranasally into *** injection was repeated as necessary to achieve eradication or until a maximum total volume of 3 mL of nBCM had been *** were followed clinically and evaluated with endoscopy at 3,6 and 12 *** follow-ups were performed *** main outcome measures were efficacy(gV eradication),safety(adverse events related to cyanoacrylate injection),recurrence,bleeding from gV and mortality related to gV ***:A total of 20 patients(15 male)with PH and gV were enrolled in the study and treated with undiluted nBCM *** 2(10%)patients had no esophageal varices(EV); 18(90%)patients were treated with endoscopic band ligation to eradicate EV before inclusion in the *** patients were followed clinically and endoscopically for a median of 31 mo(range:6-40 mo).Eradication of gV was observed in all patients(13 patients were treated with 1 session and 7 patients with 2 sessions),with a maximum injected volume of 2 mL *** patient had gV recurrence,confirmed by EUS,at 6-mo follow-up,and another had late recurrence with gV bleeding after 35 mo of follow-up; overall,gV recurrence was observed in 2 patients(10%),after 6 and 35 mo of follow-up,and gV bleeding rate was 5%(1 patient).Mild epigastric pain was r

Background: Endoscopic sclerotherapy (ES) and band ligation are standard treatments for esophageal varices. Unfortunately, recurrence is common and seems to be related to esophageal collateral vessels, easily identified by EUS. Eradication of these vessels might lead to a more durable therapeutic effect. Objective: To compare ES with EUS-guided sclerotherapy of collateral vessels (EUS-ES). Design: Randomized controlled trial. Setting: Endoscopy Unit, Division of gastroenterology. Universidade Federal de S o Paulo, S o Paulo, Brazil. Patients and Interventions: Fifty cirrhotic patientswith esophageal varices were randomized into 2 groups: ES (n = 25) and EUS-ES (n = 25). EUS-ES was targeted at collateral veins. Patients were followed-up for at least 6 months after eradication. Main Outcome Measurements: Efficacy in eradication, complications, and recurrence of varices. Results: Varices were eradicated in 48 patients who adhered to the study protocol. The mean (SD) number of sessions until eradication was 4.3 (1.5) for the ES group and 4.1 (1.2) for the EUS-ES group. In ES group, 4 patients had mild bleeding. In EUS-ES group, 4 patients had pain. The mean (SD) length of the follow-up period was 22.6 (6.9) months for the ES group and 24.9 (8.1) months for the EUS-ES group. Recurrence was seen in 4 patients after ES and in 2 after EUS-ES (P = .32). The presence of collateral vessels was associated with recurrence (P = .003). Conclusion: EUS-ES is as safe and effective as ES in variceal eradication. Recurrence tends to be less frequent and occurs later. Persistence of esophageal collateral vessels after sclerotherapy is a risk factor for recurrence.

ng>Introduction:ng> To perform a Latin-American multicentric study for the prediction of benign and malignant thyroid nodules using Alpha Score, and to compare it with ACR TIRADS® and Bethesda®. ng>Materials and Methods:ng> A prospective multicentric study in 10 radiological hospitals and institutions of Latin America was performed and 818 thyroid nodules were analyzed by ultrasound and classified by using both ACR TIRADS® and Alpha Score;fine-needle aspiration biopsy was performed when needed and classified with Bethesda. The relationships between predictors were analyzed by using binary logistic regression, statistical significance was defined by a p-value of 0.05, with an error margin of 4% and 95% confidence intervals. ng>Results:ng> Alpha Score 2.0 establishes five types of malignant predictors: microcalcifications, irregular borders, taller-than-wide shape, predominant solid texture and hypoechogenicity;a diameter equal to or greater than 1.5 cm adds an extra point to the final score. Resulting classification divides Tns into 4 categories: benign (1.9%), low suspicion (8.7%), mild suspicion (13.6%) and high suspicion (75.7%) of malignancy probability;sensitivity of 82%, specificity of 74%, the positive predictive value of 94%, the negative predictive value of 51%, the statistical accuracy of 81%, odds ratio of 108.89 and correlation with ACR TIRADS of 0.77 and Bethesda of 0.91.ng> Conclusions: ng>Alpha Score 2.0 has superior diagnostic accuracy and performance compared to the previously published Alpha Score and is able to classify a benign Tn in a precise, safe and accurate way, avoiding unnecessary FnABs or determining the necessity of FnAB in cases of moderate to high suspicion of malignancy.

Punica granatum *** known antimicrobial and anti-inflammatory *** of different parts of this plant have been commonly used in the folk medicine to prevent and treat infections;and studies have supported the use of *** extracts for this ***,little is known of the effects of ***-derived fractions in severe infectious diseases such as ***,we investigated the actions of the hexane fraction(HF),rich in lipophilic compounds,obtained from the leaves of this plant,in a murine model of polymicrobial *** pre-treatment with HF increased mortality in septic *** same animals presented with lower levels of interleukin-6,nitric oxide and hydrogen peroxide in their peritoneal lavage samples than vehicle *** also increased polymorphonuclear cell accumulation in to the peritoneum of mice with ***-derived nitric oxide upon lipopolysaccharide stimuli was markedly reduced by incubation with ***,triterpenes,phytosterols,vitamin E and ascorbyl palmitate were detected in the *** results indicate that *** leaf lipophilic fractions may worsen sepsis *** effect may be associated with its bioactive compounds which act synergistically or not,increasing sepsis mortality in vivo.
