Introduction-Objective: COVId-19 is a highly transmissible but often mild viral infection. However, some patients can present severe COVId-19 and subsequently die. The aim of the present study was to assess the risk factors for COVId-19 related death during the first three waves of the disease at the Epidemic Treatment Center (ETC) of dakar Principal Hospital (dPH). Method: We conducted a descriptive and analytical perspective survival study from April 4, 2020 to September 25, 2021, including adult patients with COVId-19, hospitalized at the ETC of dPH. Log Rank test and multivariate Cox model were performed to identify risk factors for death. Results: We included 556 COVId-19 patients with mean age of 57 ± 17 years and a male-to-female ratio of 1.26. The number of deaths during one month of follow-up was 41, representing a cumulative risk of 7.4%. The log Rank test showed that being from the third wave (p = 0.0056), advanced age (p = 0.00098), presence of at least one comorbidity (p = 0.034), High blood pressure (p = 0.024), d-dimer level ≥ 1000 IU/L (p Conclusion: Our study showed that elderly and third-wave of COVId-19 patients were more at risk to die. Knowledge of risk factors for COVId-19 related death could improve the prognosis of these patients.

AIM To evaluate the efficacy of Nexobrid~? in the initial management of burns and lessons learned with the *** From January 27^(th) 2015 until January 25^(th) 2016, 25 patients aged between 18-94 years old with deep partial and full thickness burns were treated with Nexobrid~? covering 1%-30% of their total body surface area(TBSA). The debridement was applied in the first 96 h post-injury following the protocol suggested for Nexobrid~?. In patients with burns of more than 15% TBSA a second application of Nexobrid~? was performed. After the removal of the product-4 h post application and after a 2 h period of wet dressing-we used several products to cover the wound like Suprathel~?, Biobrane~?, Mepitel~? with wet dressing, silver sulphadiazine 1% cream, and in some cases even autografts. We treated patients with inhalation injury as well. All the procedures were done under deep sedation, regional blocks in extremities or general anaesthesia in the intensive care unit room or in the operating *** After these first 25 cases, we have observed that patients with partial thickness burns treated with Nexobrid~?, experienced great benefits in the reduction of the need for autografting compared with the standard of care. This isbecause after selective enzymatic debriding of the burn scar we can distinguish different wound beds, which can coexist in the same patient, and we also managed to associate each one to its ability to epithelize. In major burns, besides the improvement in wound healing, we observed an important improvement in their general state. This may be because SIRS significantly improved through a bloodless debridement of necrotic tissue, decreasing the requirements of vasoactive drugs and fluid resuscitation. Circumferential burns also benefited from enzymatic debridement, observing a decrease in the number of compartment syndromes and the need for escharotomies. Background: Low serum testosterone is associated with several cardiovas cular r isk factors including dyslipidaemia, adverse clotting profiles, obesity, and ins ulin resistance. Testosterone has been reported to improve symptoms of angina an d delay time to ischaemic threshold in unselected men with coronary disease. Obj ective: This randomised single blind placebo controlled crossover study compar ed testosterone replacement therapy (Sustanon 100) with placebo in 10 men with isc haemic heart disease and hypogonadism. Results: Baseline total testosterone and bioavailable testosterone were respectively 4.2 (0.5) nmol/l and 1.7 (0.4) nmol/ l. After a month of testosterone, delta value analysis between testosterone and placebo phase showed that mean (Sd) trough testosterone concentrations increased significantly by 4.8 (6.6) nmol/l (total testosterone) (p=0.05) and 3.8 (4.5) n mol/l (bioavailable testosterone) (p=0.025), time to 1 mm ST segment depression assessed by bruce protocol exercise treadmill testing increased by 74 (54) secon ds (p=0.002), and mood scores assessed with validated questionnaires all improve d. Compared with placebo, testosterone therapy was also associated with a signif icant reduction of total cholesterol and serum tumour necrosis factor αwith del ta values of -0.41 (0.54) mmol/l (p=0.04) and -1.8 (2.4) pg/ml (p=0.05) respec tively. Conclusion: Testosterone replacement therapy in hypogonadal men delays t ime to ischaemia, improves mood, and is associated with potentially beneficial r eductions of total cholesterol and serum tumour necrosis factor α.

In this paper we describe the potential of the enhanced X-ray Timing and Polarimetry(eXTP) mission for studies related to accretion flows in the strong field gravity regime around both stellar-mass and supermassive black-holes. eXTP has the unique capability of using advanced "spectral-timing-polarimetry" techniques to analyze the rapid variations with three orthogonal diagnostics of the flow and its geometry, yielding unprecedented insight into the inner accreting regions, the effects of strong field gravity on the material within them and the powerful outflows which are driven by the accretion process.
