Non-compliance in surveillance for patients with previous resection of large (≥ 1 cm) colorectal adenomas

AIM: To assess the extent and reasons of non- compliance in surveillance for patients undergoing polypectomy of large (≥ 1 cm) colorectal adenomas. METHODS: Between 1995 and 2002, colorectal adenomas ≥ 1 cm were diagnosed in 210 patients and subsequently documented at the Erlangen Registry of Colorectal Polyps. One hundred and fi fty-eight patients (75.2%) could be contacted by telephone and agreed to be interviewed. Additionally, records were obtained from the treating physicians. RESULTS: Fifty-four out of 158 patients (34.2%) neglected any surveillance. Reasons for non-compliance included lack of knowledge concerning surveillance intervals (45.8%), no symptoms (29.2%), fear of examination (18.8%) or old age/severe illness (6.3%). In a multivariate analysis, the factors including female gender (P = 0.036) and age > 62 years (P = 0.016) proved to be signif icantly associated with non-compliance in surveillance. CONCLUSION: Efforts to increase compliance in surveillance are of utmost importance. This applies particularly to women’s compliance. Effective strategies for avoiding metachronous colorectal adenoma and cancer should focus on both the improvement in awareness and knowledge of patients and information about physicians for surveillance.

The aim of the present study was to assess if semen quality declines during in vitro fertilization （IVF） and whether or not this phenomenon is triggered by chronic male stress. In order to test this hypothesis, we first investigated a retrospective cohort of 155 male IVF patients （testing cohort）. Subsequently, we started a prospective cohort study in men undergoing their first IVF and assessed semen quality and subjective male chronic stress using a validated tool, i.e. the Fertility Problem Inventory （FPI） questionnaire. The association between stress and sperm quality decline measured 4-6weeks before the start of IVF （T1） and at the day of oocyte retrieval （T2） was the primary outcome. Live birth rate, first trimester abortion and rate of poor responders were secondary outcomes. In the testing cohort, mean progressive motility, but not mean sperm density significantly declined. There were 78/154 （51%） men who showed a decline in semen density and 50/154 （32%） men who showed a decline in progressive motility. In the validation cohort, progressive motility declined, whereas, sperm density increased from T1 to T2. Of 78 men, 27 men had increased stress （FPI-score 〉 146）. Sperm density and progressive motility were not significantly different in men with and without stress. However, in the presence of male stress, couples had a higher rate of poor responders, miscarriages and a lower rate of live births. Subjective stress is not associated with a decline in semen quality observed during IVF but may be associated with adverse ore^nancv outcome.

Ferritins are major players in plant iron homeostasis. Surprisingly, their overexpression in transgenic plants led only to a moderate increase in seed iron content, suggesting the existence of control checkpoints for iron loading and storage in seeds. This work reports the identification of two of these checkpoints. First, measurement of seed metal content during fruit development in Arabidopsis thaliana reveals a similar dynamic of loading for Fe, Mn, Cu, and Zn. The step controlling metal loading into the seed occurs by the regulation of transport from the hull to the seed. Second, metal loading and ferritin abundance were monitored in different genetic backgrounds affected in vacuolar iron transport （AtVIT1, AtNRAMP3, AtNRAMP4） or plastid iron storage （AtFER1 to 4）. This approach revealed （1） a post-translational reg- ulation of ferritin accumulation in seeds, and （2） that ferritin stability depends on the balance of iron allocation between vacuoles and plastids. Thus, the success of ferritin overexpression strategies for iron biofortification, a promising approach to reduce iron-deficiency anemia in developing countries, would strongly benefit from the identification and engineering of mechanisms enabling the translocation of high amounts of iron into seed plastids.

Introduction: Child abuse is a serious health problem with compelling evidence that the phenomenon is common throughout the world. The Center “SOS enfants ULB” is a specialized team, established in a pediatric ward, which had mission to prevent and to treat the situations of children victims of physical, sexual, psychological abuse or of neglect. Our objectives were to describe the characteristics of 439 children hospitalized for (suspicion of) maltreatment and to investigate different factors potentially associated with the children at risk of neglect or maltreatment, the children victims of neglect and the physically maltreated children. Methods: Chi square test and multinomial logistic regression models with clustered robust standard error were applied to assess the relationship between the three types of mistreatment and the potential associated factors. Results: Maltreatment was observed for almost one in two children (48.7%) and approximately four on ten (41.5%) were considered at risk. Physical abuse was the most prevalent (57.9%) among the maltreated children and neglect was the second most prevalent (37.4%) form of maltreatment. Regarding the criteria leading to hospitalisation, at least one protective criterion was observed for a little more than eight children on ten. The parental criteria have shown that social problems and conjugal conflicts were the most prevalent for this category of hospitalisation criteria. Conclusion: These data, concerning the hospitalized children for which there is (suspicion of) mistreatment, collected by this team make important contribution to describing child maltreatment and its associated factors. Despite the fact that these hospitalized cases may be probably more serious than those who were not reported, every event, however small it may be, should be known for better organizing each level of prevention.

AIM:To analyze the pathogenetic role and potential clinical usefulness of the epidermal growth factor receptor(EGFR)and the human epidermal growth factor receptor 2(HER2)in patients with advanced biliary tract cancer(BTC). METHODS:EGFR and HER2 expression was studied in biopsy samples from 124 patients(51%women; median age 64.8 years),with advanced BTC diagnosed between 1997 and *** micrometers sections of paraffin embedded tissue were examined by standard, FDA approved *** with scores of 2+or 3+for HER2 expression on immunochemistry were additionally tested for HER2 gene amplification by fluorescence in situ hybridisation(FISH).RESULTS:34/124 patients(27.4%)had gallbladder cancer,47(37.9%)had intrahepatic BTC and 43(34.7%)had extrahepatic or perihilar *** expression was examined in a subset of 56 samples. EGFR expression was absent in 22/56 tumors(39.3%). Of the remaining samples expression was scored as 1+in 12(21.5%),2+in 13(23.2%)and 3+in 9(16%), ***2 expression was as follows:score 0 73/124(58.8%),score 1+27/124(21.8%),score 2+ 21/124(17%)and score 3+4/124(3.2%).HER2 gene amplification was present in 6/124,resulting in an overall amplification rate of 5%. CONCLUSION:Our data suggest that routine testing and therapeutic targeting of HER2 does not seem to be useful in patients with BTC,while targeting EGFR may be promising.

Towards Sustainable Land Uses within the Elbe River Biosphere Reserve in Lower Saxony, Germany by Means of TerraSAR-X Images

Floods are one of the major hazards worldwide. They are the source of huge risks in rural and urban areas, resulting in severe impacts on the civil society, industry and the economy. The Elbe River has suffered from many severe floods during recent decades. In this study, the zones flooded during 2011 were analyzed using TerraSAR-X images and a digital elevation model for the area in order to identify possible ways to mitigate flood hazards in the future, regarding sustainable land-use. Two study areas are investigated, around the Walmsburg oxbow and the Wehningen oxbow. These are located between Elbe-Kilometer (505-520) and (533-543), respectively, within the Lower Saxonian Elbe River Biosphere Reserve. Those areas are characterized by several types of land use, with agricultural land use being predominant. The study investigated the possibility of using a Decision-Tree object-based classifier for determining the major land uses and the extent of the inundation areas. The inundation areas identify for 2011 submerged some agricultural fields that must be added to existing flood risk maps, and future cultivation activities there prevented to avoid the possible economic losses. Furthermore, part of the residential area is located within the high flood zone, and must be included in risk maps to avoid the possible human and economic losses, to achieve sustainable land use for the areas studied.
