In this present study, we analyzed the effects of Prandtl and Jacob numbers and dimensionless thermal conductivity on the velocity profiles in media (porous and liquid). The transfers in the porous medium and the liquid film are described respectively by the improved Wooding model and the classical boundary layer equations. The mesh of the digital domain is considered uniform in the transverse and longitudinal directions. The advection and diffusion terms are discretized with a back-centered and centered scheme respectively. The coupled systems of algebraic equations thus obtained are solved numerically using an iterative line-by-line relaxation method of the Gauss-Seidel type. The results show that the parameters relating to the thermal problem (the dimensionless thermal conductivity, the Prandtl (Pr) and Jacob (Ja) numbers) have no influence on the dimensionless speed, although the thermal and hydrodynamic problems are coupled. Via the heat balance equation. So, at first approximation with the chosen constants, we can solve the hydrodynamic problem independently of the thermal problem.

Flow records for stations in the Casamance basin are incomplete. Several gaps were noted over the 1980-2021 study period, making this study tedious. The aim of this study is to assess the potential impact of climate change on the flow of the Casamance watershed at Kolda. To this end, hydrological series are simulated and then extended using the GR2M rainfall-runoff model, with a monthly time step. Projected climate data are derived from a multi-model ensemble under scenarios SSP2-4.5 (scenario with additional radiative forcing of 4.5 W/m2 by 2099) and SSP5-8.5 (scenario with additional radiative forcing of 8.5 W/m2 by 2099). An analysis of the homogeneity of the rainfall data series from the Kolda station was carried out using KhronoStat software. The Casamance watershed was then delimited using ArcGIS to determine the morphometric parameters of the basin, which will be decisive for the rest of the work. Next, monthly evapotranspiration was calculated using the formula proposed by Oudin et al. This, together with rainfall and runoff, forms the input data for the model. The GR2M model was then calibrated and cross-validated using various simulations to assess its performance and robustness in the Casamance watershed. The version of the model with the calibrated parameters will make it possible to extend Casamance river flows to 2099. This simulation of future flows with GR2M shows a decrease in the flow of the Casamance at Kolda with the two scenarios SSP2-4.5 and SSP5-8.5 during the rainy period, and almost zero flows during the dry season from the period 2040-2059.

Obstructive Tumor of the Palate in the Old Person: A Case Report

Introduction: Pleomorphic adenoma or mixed salivary gland tumor is a heterogeneous benign tumor of the salivary glands. The most common site is the parotid gland. Its extra-parotid locations, particularly in the accessory salivary glands, are rarer. We report a case of a pleomorphic adenoma of the posterior palate which posed management problems. Observation: This is an 85-year-old patient who consulted for a tumor of the posterior palate that had been evolving for 3 years. The examination revealed a globular tumor of the posterior palate extending beyond the midline by approximately 7 cm in long axis, shooting towards the oropharynx and hindering breathing, speech and eating, indicating a life-saving tracheotomy. A CT scan of the facial area revealed a well-circumscribed tumor at the expense of the soft palate, with multiple sites of bone lysis. The biopsy performed was in favor of a pleomorphic adenoma. The patient underwent total surgical excision of a huge tumor on the palate. The aftermath of the operation was marked by a loosening of the sutures with an oronasal fistula requiring the creation of an obturator plate due to the patient’s refusal to have another operation. Conclusion: Large pleomorphic adenoma of the posterior palate is a rare entity that can cause respiratory problems and surgical difficulties. His prognosis is generally good.

Introduction: Sinonasal polyposis (SNP) is a chronic inflammatory disease of the mucosa of the nasal cavities and facial sinuses. It is characterized by an oedematous, multifocal and bilateral degeneration of the nasosinus mucosa, which originates in the lateral masses of the ethmoid, where it causes the formation of smooth, gelatinous, translucent and pyriform polyp lesions. The objective of this study was to review epidemiological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatment. Patients and Methods: This is a retrospective study on 60 patients followed at the ENT department of the Fann National University Hospital Center, from January 2010 to December 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (M/F) was 0.8. In the patients’ histories, we found 18% asthma and 10% Widal’s disease. The average consultation time was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan performed in 58% of cases made it possible to specify the extent of the lesions;the involvement of the ethmoidal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received medical treatment with local corticosteroids. Endoscopic surgical treatment was initiated in 43% of cases after failure of corticosteroid-based medical treatment. The evolution under treatment marked by the reappearance of symptoms that increased each month. At one month postoperatively, all clinical symptomatology had improved with the exception of olfactory disorders, which persisted in 3 patients. At 12 months we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is almost always clinical. The study conducted at Ndiebene Gandiol 1 school in Senegal has unveiled serious environmental and public health challenges. The wastewater analysis revealed high levels of Biochemical Oxygen Demand (BOD5), Chemical Oxygen Demand (COD), and fecal coliforms, signaling potential risks to the well-being of students and staff. This situation mirrors a wider issue in rural educational settings, where inadequate sanitation persists. Intensive wastewater treatment options are known for their effectiveness against high pollutant loads but are resource-intensive in both energy and cost. Conversely, extensive treatment systems, while requiring more land, provide a sustainable alternative by harnessing natural processes for pollutant removal. The research suggests a hybrid treatment approach could serve the school’s needs, balancing the robust capabilities of intensive methods with the ecological benefits of extensive systems. Such a solution would need to be tailored to the specific environmental, financial, and logistical context of the school, based on comprehensive feasibility studies and stakeholder engagement. This study’s findings underscore the urgency of addressing sanitation in schools, as it is intrinsically linked to the health and academic success of students. Quick, effective, and long-term strategies are vital to secure a healthier and more prosperous future for the youth. With proper implementation, the school can transform its sanitation facilities, setting a precedent for rural educational institutions in Senegal and similar contexts globally.

Cancers of the lip are the leading cause of labial defects, and treatment is essentially surgical. The success of a repair is assessed by two essential criteria: The functional character of the lip (restoration of continence allowing feeding) and the aesthetic quality of the repair. However, for many patients from countries whose medical infrastructure does not allow them to undertake complex reconstructions locally, medical transfers represent their only chance of treatment. The aim of this article is to share with you an extreme clinical case of labial reconstruction, the management of which was a real challenge, using two major flaps: the free ante-brachial flap and the DUFOURMENTEL-type bi-pediculated scalp flap.

Management of Square Carcinoma of the Lower Lip Using Camille-Bernard Flap under Local Anesthesia: When Constraints Dictate Our Attitude

Introduction: Squamous cell carcinomas of the lip essentially pose a therapeutic management problem. In the West, their treatment is done under general anesthesia and follows the classic rules of oncology. In our context of exercise, we sometimes have a different attitude dictated by various constraints. Observation: 55-year-old patient with no notable history, seen in consultation for a very large ulcerative-budding lesion on the right 2/3 of the lower lip with mucocutaneous involvement without lymph node findings found on examination. Faced with this suspicious lesion and the patient’s financial difficulties, we decided to operate under local anesthesia to remove this tumor. The oncological excision was followed by immediate plasty using a Camille-Bernard flap with a good immediate aesthetic and functional result and postoperative day 7. The patient was unable to take his surgical specimen to anatomy-pathology and was lost to follow-up due to lack of financial resources. On anatomo-pathological analysis of the surgical specimen, the margins were healthy with confirmation of squamous cell carcinoma (anapathological reading made for scientific interest). Discussion: The particular mentality of certain patients and their difficult financial conditions push us to have an unconventional therapeutic attitude in oncology, but which nevertheless makes it possible to resolve certain problems that we often face. Local anesthesia is possible and realistic in cases like ours and the results can be acceptable and life-saving.

Ecological Wastewater Treatment System Using a Horizontal Flow Biological Reactor: The Case of Vetiver

Confronted with the challenge of wastewater management, particularly in the school environment of Senegal, our study set out to achieve multiple objectives. Following field surveys, laboratory analyses of wastewater samples were carried out, revealing a significant pollutant load. In the community of Gandiol, near Saint-Louis (Senegal), the school of Ndiebene Gandiol 1 faces significant sanitation challenges. Our study aimed to address this issue by using a constructed filter composed of two filtering bed cells measuring 12 × 8.5 m, preceded by a septic tank. We particularly focused on the influence of Vetiver;a plant chosen for its purification potential. Our analyses showed remarkable efficiency of the filter. Elimination rates reached 95% for 5-Day Biochemical Oxygen Demand (BOD5), 91% for Chemical Oxygen Demand (COD), and 92% for SS, far exceeding the Senegalese standards set at 50 mg/L, 200 mg/L, and 40 mg/L, respectively. Furthermore, the concentration of fecal coliforms was reduced to 176 FCU/100mL, well below the Senegalese threshold of 2000 FCU/100mL and close to the World Health Organization’s (WHO) recommendation of 1000 FCU/100mL. However, despite these promising results, some parameters, particularly the concentration of certain pollutants, approached the thresholds defined by European legislation. For example, for Suspended Solids (SS), the post-treatment level of 3 mg/L was well below the Senegalese standard but edged close to the European minimum of 10 mg/L. In conclusion, the Vetiver filter demonstrated a remarkable ability to treat school wastewater, offering high pollutant elimination percentages. These results suggest significant opportunities for the reuse of treated water, potentially in areas such as irrigation, though some adjustments may be necessary to meet the strictest standards such as those of the European union (EU).

The overarching goal of this study is to offer an effective and sustainable solution to the challenges of sanitation in rural and school settings in the northern region of Senegal. The study explores a wastewater treatment approach based on phytoremediation, with a particular focus on the use of horizontally-flowing reed bed filters. Furthermore, it aims to adapt and optimize these systems for the specific needs of Senegal, focusing on wastewater in school environments. Thus, we constructed a horizontally-flowing reed bed filter, planted with Typha, at the Ndiébène Gandiol school in Senegal. We will investigate the efficiency of wastewater treatment by this horizontally-flowing reed bed filter, emphasizing the role of the plant used: Typha. The filter is described in detail, specifying its dimensions, its composition of flint gravel, and the choice of plants, namely Typha. The experimental protocol is detailed, describing the sampling at the entrance and exit of the filter to evaluate water quality. The parameters analyzed include Chemical Oxygen Demand (COD), Biochemical Oxygen Demand over 5 days (BOD5), suspended solids, ammonium, nitrates, phosphates, pH, conductivity, and fecal coliforms. The results indicate a significant improvement in water quality after treatment. COD, BOD5, suspended solids, and fecal coliforms are greatly reduced, thus demonstrating the efficacy of the Typha filter. However, nitrate concentrations remain relatively stable, suggesting room for improvement in their elimination. A perspective of reuse of the treated water is considered, showing that the effluents from the planted filter meet Senegalese and international standards for irrigation. The findings suggest that these waters could be used for a variety of crops, thereby reducing the pressure on freshwater resources. Correlation between Total PSA Level and the Histoprognostic Gleason Score

Prostate cancer is a public health problem in Senegal. It is one of the most common cancers in men and can be detected early by PSA measurement and confirmed by the pathological study which specifies the histological type and evaluates the histoprognostic scores (Gleason and pTNM). We did not find any anatomopathological study highlighting a link between the PSA t rate and histoprognostic Gleason scores in Senegal. Objective: We carried out this work aimed at determining whether there is a correlation between the PSA t rate and the histoprognostic Gleason score, in our context. Methodology: This is a retrospective, descriptive and analytical study conducted from January 2013 to October 2021, based on histologically proven prostate cancer cases with a specified PSA level, diagnosed in the pathological anatomy and cytology laboratory of Aristide Le Dantec hospital. Results: We identified 654 cases of prostate cancer. The average age was 68.59 ± 5.8 years with extremes of 40 years to 92 years. More than half of our cohort presented a clinical stage T2c, i.e. 56.94%. The average prostate volume on ultrasound was 81.81 ± 66 cc. The median PSA t was 110.5 ng/ml, with extremes ranging from 2 ng/ml to 74,770 ng/ml. Prostatic adenocarcinoma was the only histological type found in our patients. There was a predominance of Gleason score 6 (PICU grade group 1) observed in 35.17% of patients. Well-differentiated cancers were predominant and represented 35.17%. There was a statistically significant positive correlation between PSA t level and Gleason score (Spearman’s Rho = 0.305, p = 0.000). Conclusion: This study shows that in the Senegalese population, in patients with prostate cancer, the higher the PSA t level rises above normal, the higher the Gleason score tends to be.
