Background:Hepatocellular carcinoma (HCC) is the most common cause of cancer-related death in Saudi Arabia. Our study aimed to investigate the patterns of HCC and the effect of tNm staging, Alfa-fetoprotein (AFP), and Child-turcotte Pugh (CtP) on patients' overall survival (OS).methods:A retrospective analysis was conducted on 43 HCC patients at a single oncology center in Saudi Arabia from 2015 to 2020. All patients had to fulfill one of the following criteria: (a) a liver lesion reported as definitive HCC on dynamic imaging and/or (b) a biopsy-confirmed diagnosis.Results:the mean patient age of all HCC cases was 66.8 with a male-to-female ratio of 3.3:1. All patients were stratified into two groups: viral HCC (n = 22, 51%) and non-viral HCC (n = 21, 49%). Among viral-HCC patients, 55% were due to HBV and 45% due to HCV. Cirrhosis was diagnosed in 79% of cases. Age and sex did not significantly statistically differ in OS among viral and non-viral HCC patients (-value > 0.05). About 65% of patients had tumor size >5 cm during the diagnosis, with a significant statistical difference in OS (-value = 0.027). AFP was >400 ng/ml in 45% of the patients. there was a statistically significant difference in the OS in terms of AFP levels (-value = 0.021). A statistically significant difference was also observed between the CtP score and OS (-value = 0.02). CtP class B had the longest survival. BSC was the most common treatment provided to HCC patients followed by sorafenib therapy. there was a significant statistical difference in OS among viral and non-viral HCC patients (-value = 0.008).Conclusions:the most common predictors for OS were the underlying cause of HCC, AFP, and tumor size. Being having non-viral etiology, a tumor size >5 cm, an AFP > 400 ng/mL, and a CtP score class C were all negatively associated with OS.

A bus network design problem in a suburban area of Hong Kong is *** objective is to minimize the weighted sum of the number of transfers and the total travel time of passengers by restructuring bus routes and determining new frequencies.A mixed integer optimization model is developed and was solved by a Hybrid Enhanced Artificial Bee Colony algorithm(HEABC).A case study was conducted to investigate the effects of different design parameters,including the total number of bus routes available,the maximum route duration within the study area and the maximum allowable number of bus routes that originated from each *** model and results are useful for improving bus service policies.

AIm to assess nutritional recovery,particularly regarding feeding jejunostomy tube(FJt)utilization,following upper gastrointestinal resection for malignancy. mEtHODS A retrospective review was performed of a prospectively-maintained database of adult patients who underwent esophagectomy or gastrectomy(subtotal or total)for cancer with curative intent,from January 2001 to June 2014. Patient demographics,the approach to esophagectomy,the extent of gastrectomy,FJt placement and utilization at discharge,administration of parenteral nutrition(PN),and complications were evaluated. All patients were followed for at least ninety days or until *** the 287 patients underwent upper GI resection,comprised of 182 esophagectomy(n=107 transhiatal,58.7%; n=56 Ivor-Lewis,30.7%)and 105 gastrectomy [n=63 subtotal(SG),60.0%; n=42 total(tG),40.0%]. 181 of 182 esophagectomy patients underwent FJt,compared with 47 of 105 gastrectomy patients(99.5% vs 44.8%,P m most had undergone tG(n=39,92.9% vs n=8 SG,12.9%,P median length of stay was similar between esophagectomy and gastrectomy groups(14.7 d vs 17.1 d,P=0.076). Upon discharge,87 esophagectomy patients(48.1%)were taking enteral feeds,with 53(29.3%)fully and 34(18.8%)partially dependent. meanwhile,20 of 39 tG patients(51.3%)were either fully(n=3,7.7%)or partially(n=17,43.6%)dependent on tube feeds,compared with 5 of 8 SG patients(10.6%),all of whom were partially dependent. Gastrectomy patients were significantly less likely to be fully dependent on tube feeds at discharge compared to esophagectomy patients(6.4% vs 29.3%,P=0.0006). PN was administered despite FJt placement more often following gastrectomy than esophagectomy(n=11,23.4% vs n=7,3.9%,P=0.0001). FJt-specific complications requiring reoperation within 30 d of resection occurred more commonly in the gastrectomy group(n=6),all after tG,compared to 1 esophagectomy patient(12.8% vs 0.6%,P=0.0003). In this paper, the effects of refractive index (RI) of surrounding medium and ambient temperature on the transmission characteristics of a long period grating (LPG) are experimentally analyzed. the spectral behavior of LPG is investigated when the ambient index is higher or lower than that of the cladding material. the results show that the refractive index sensitivity of lower order attenuation bands is very low compared with that of the highest order attenuation band. But in the case of temperature, the lower order attenuation bands of the LPG can also exhibit good sensitivity like the higher-order bands.

Optical pulses are fundamentally defined by their temporal and spectral *** ability to control pulse properties allows practitioners to efficiently leverage them for advanced metrology,high speed optical communications and attosecond ***,we report 11×temporal compression of 5.8 ps pulses to 0.55 ps using a low power of 13.3 *** result is accompanied by a significant increase in the pulse peak power by 9.4×.these results represent the strongest temporal compression demonstrated to date on a complementary metal–oxide–semiconductor(CmOS)*** addition,we report the first demonstration of on-chip spectral compression,3.0×spectral compression of 480 fs pulses,importantly while preserving the pulse *** strong compression achieved at low powers harnesses advanced on-chip device design,and the strong nonlinear properties of backend-CmOS compatible ultra-silicon-rich nitride,which possesses absence of two-photon absorption and 500×larger nonlinear parameter than in stoichiometric silicon nitride *** demonstrated work introduces an important new paradigm for spectro-temporal compression of optical pulses toward turn-key,on-chip integrated systems for all-optical pulse control.

Background: Pediatric supracondylar humerus fractures are the most common elbow fractures in children. Operative management includes closed reduction and placement of 2 to 3 laterally based pins. Occasionally, a medial pin is used to create a crossed fixation pattern, despite risk of nearly 10% iatrogenic ulnar nerve injury. the objective of this study was to assess the trends and outcomes in the operative management of pediatric supracondylar humerus fractures at a level one academic trauma center. materials & methods: A retrospective review was performed on all children sustaining a Gartland type II or III supracondylar humerus fractures treated by closed or open reduction and percutaneous pinning in 2006-2008 and 2009-2011 at a level one academic trauma center by two of the authors (JtR, Lmt). Pin placement patterns were evaluated and compared based on year performed. Outcomes measured were rates of ulnar nerve symptoms, non-union, re-operation, and varus malalignment. Data analysis was performed using a Fisher exact test on StAtA software. Results: A total of 49 patients met inclusion criteria. Of 22 patients treated in 2006-2008, 5 (23%) were type II and 17 (77%) were type III. From 2009-2011, 16 (59%) were type II and 11 (41%) were type III. Comparison of pinning pattern in type II fractures between 2006-2008 and 2009-2011 did not indicate statistical significance (p = 0.429). Comparison of pinning pattern in type III fractures during the same time period did show that there was a statistically significant decrease (p = 0.010) in the number of cross pin fixations. there were no ulnar nerve injuries, non-unions, re-operations, or varus malalignment in any patient on final follow-up. Conclusion: this study shows that there has been a significant decrease in cross pin fixation for pediatric type III supracondylar humerus fractures with equivalent clinical outcomes at a Level I trauma center. Objective to review the research carried out in Hong Kong on nasopharyngeal carcinoma (NPC) over the past 10 years. methods the literature published in Hong Kong over the past 10 years on nasopharyngeal carcinoma were reviewed in the light of other important international publications on the same subject. Results the research carried out in Hong Kong covers etiology, natural behavior, treatment and prognosis of NPC. these studies not only elucidated the different aspects of NPC, many of these new findings will also guide future directions of research. Conclusions major efforts have been made to research into the various aspects of NPC with important findings which will translate into better treatment results. As this tumor is uncommon in other parts of the world, continued efforts to improve the prevention, early diagnosis, treatment and care of this tumor are required in China.

the impact of Ga^(3+)and Gd^(3+)co-substitution on different types of behavior of Ni-Co nanospinel ferrites(NSFs),(Ni_(0.5)Co_(0.5))[Ga_(x)Gd_(x)Fe_(2-2x)]O_(4)(0.000≤x≤0.025),was *** were fabricated through ultrasound *** structure,composition,and spherical morphology of all products were verified by numerous characterization techniques such as X-ray diffraction(XRD),scanning and transmission electron microscopes(SEm and tEm),and selected area electron diffraction(SAED).mossbauer spectra are composed of two ferromagnetic sextets and one paramagnetic *** spectra were fitted to extract hyperfine parameters of the doped *** sublattices'hyperfine magnetic field decreases with *** isomer shift values show that the mossbauer spectra are composed of magnetic Fe^(3+)*** magnetization measurements at variable magnetic field(m-H)were investigated via a vibrating sample magnetometer(VSm)at temperatures(t)from 300 to 10 *** NSFs disclose ferrimagnetic behavior at both 300 and 10 K at which they are soft and hard,*** determined that the remanence magnetization(m_(r)),saturation magnetization(m_(s)),and magneton number(n_(B))decrease with increasing Ga^(3+)and Gd^(3+)ions *** reduction in these values is predominantly recognized to be the impact of cation redistribution and surface spins on tetrahedral(t_(d))and octahedral(O_(h))*** the Ga^(3+)and Gd^(3+)contents increase,the coercivity(H_(c))is also found to decrease.
