BACKGROUND About 10%-30%of acute pancreatitis remain idiopathic(IAP)even after clinical and imaging tests,including abdominal ultrasound(US),contrast-enhanced computed tomography(CECT)and magnetic resonance cholangiopancreatography(MRCP).This is a relevant issue,as up to 20%of patients with IAP have recurrent episodes and 26%of them develop chronic *** data are available on the role of EUS in clarifying the etiology of IAP after failure of one or more cross-sectional *** To evaluate the diagnostic gain after failure of one or more previous crosssectional *** We retrospectively collected data about consecutive patients with AP and at least one negative test between US,CECT and MRCP,who underwent linear EUS between January 2017 and December *** investigated the EUS diagnostic yield and the EUS diagnostic gain over different combinations of these crosssectional imaging techniques for the etiologic diagnosis of *** and frequency of EUS diagnosis were also analyzed,and EUS diagnosis was compared with the clinical *** EUS,patients were followed-up for a median of 31.5 mo to detect cases of pancreatitis *** We enrolled 81 patients(63%males,mean age 61±18,23%with previous cholecystectomy,17%with recurrent pancreatitis).Overall EUS diagnostic yield for AP etiological diagnosis was 79%(20%lithiasis,31%acute on chronic pancreatitis,14%pancreatic solid or cystic lesions,5%pancreas divisum,5%autoimmune pancreatitis,5%ductal abnormalities),while 21%remained ***,CECT and MRCP,taken alone or in combination,led to AP etiological diagnosis in 16(20%)patients;among the remaining 65 patients,49(75%)obtained a diagnosis at EUS,with an overall EUS diagnostic gain of 61%.Sixty-eight patients had negative US;among them,EUS allowed etiological diagnosis in 59(87%).Sixty-three patients had a negative CECT;among them,47(74%)obtained diagnosis with ***-four had a negative MRCP;among them,20(83%)had EUS diag

Obsessive-compulsive disorder(OCD) is one of the most difficult additional diagnoses to manage in patients with bipolar disorder(BD) since the gold standard treatment for one disease(antidepressants for OCD) can worsen the other. This case report describes the efficacy of aripiprazole augmentation as maintenance therapy in a young patient with comorbid BD-OCD. Our patient presented complete remission of affective and obsessivecompulsive symptoms with remarkable improvement in social and occupational functioning for 24 *** drug reactions were not severe enough to result in drug discontinuation. In consideration of the important nosological, clinical and therapeutic implications, future research efforts may lead to more grounded guidelines,which are greatly needed in patients with comorbid BDOCD.

Intense exercise may cause heart remodeling to compensate increases in blood pressure or volume by increasing muscle mass. Cardiac changes do not involve only the left ventricle, but all heart chambers. Physiological cardiac modeling in athletes is associated with normal or enhanced cardiac function, but recent studies have documented decrements in left ventricular function during intense exercise and the release of cardiac markers of necrosis in athlete's blood of uncertain significance. Furthermore, cardiac remodeling may predispose athletes to heart disease and result in electrical remodeling, responsible for arrhythmias. Athlete's heart is a physiological condition and does not require a specific treatment. In some conditions, it is important to differentiate the physiological adaptations from pathological conditions, such as hypertrophic cardiomyopathy, arrhythmogenic dysplasia of the right ventricle, and non-compaction myocardium, for the greater risk of sudden cardiac death of these conditions. Moreover, some drugs and performance-enhancing drugs can cause structural alterations and arrhythmias, therefore, their use should be excluded.

Background: In patients undergoing pneumonectomy, intraoperative pulmonary and cardiac complications are the major cause of morbidity and mortality. Protective lung ventilation strategies may decrease the overall lung injury. Right, ventricular dysfunction may occur during the surgery and after the pneumonectomy, in the early postoperative period, with reduced RV ejection fraction and increased RV end-diastolic volume index, caused by increased RV afterload. Case report: We describe the case of a 28-year-old non-smoker female who underwent to a right pneumonectomy. The patient presented intraoperative hemodynamic instability and signs of RV dysfunction, requiring vasoactive amines and nitric oxide. Discussion: This article is intended to provide an overview of the anesthetic management for pneumonectomy including the hemodynamic management and considerations of the causes and management of right ventricular dysfunction.

Sustainability Assessment of Alternatives for Heat Generation and Transfer in Saunas

In the search for renewable energy sources, solar energy appears, among others, as a powerful alternative;mainly for those countries located at Earth’s equatorial region. This is the case of a Fresnel device that uses solar radiation to warm-up the air;for instance, within saunas. Authors coined the term sustainable technological innovation to refer to a technological innovation in accordance to the principles of sustainability. The object of this research was comparing the sustainability of a Fresnel Lens Solar Concentration (FLSC) against the sustainability of other twelve alternatives to generate heat. This article assesses the sustainability of fourteen technological options for heat generation and transfer to saunas in the northwest region of Mexico where temperatures often reach above 45 C and where there is a greater potential for exploiting solar energy. Emergy accounting, economic analysis, and global warming potential are the three indicators considered in quantifying sustainability. Results show that infrared ceramic photovoltaic, infrared carbon photovoltaic and infrared metal photovoltaic are the most sustainable systems. The Fresnel device reached the fourth and tenth position, using electricity from photovoltaic and from grid respectively, among all fourteen assessed devices, respectively. Although Fresnel equipment does not release any global warming gases into the atmosphere, or even it demands low amount of non-renewable resources, 1.40 and 4.47 E17 seJF/yr. it requires high economic investment, 20,800 and 79,600 USD/yr, for the implementation and operation phases, which decrease its sustainability performance compared to twelve other evaluated devices.

The administration of statins in patients with liver disease is not an absolute contraindication. Hepatotoxicity is a rare and often dose-related event and in the literature there are only a few described cases of fatal rhab-domyolysis in patients with chronic liver disease after statin administration. During treatment with 3-hydroxy-3-methylglutaryl-coenzyme A reductase inhibitors,the factors responsible for myopathy may either be related to the patient,or due to interactions with other medications that are metabolic substrates of the same isozymes and therefore able to increase blood statin concentration. The most important side effects consist of increased transaminase levels,abdominal pain or muscle weakness,increased serum levels of creatine kinase and rhabdomyolysis. In this article we report a case of fatal rhabdomyolysis with acute renal failure after gastric endoscopy,where midazolam was used as a sedation agent in a patient with chronic liver disease treated with a high dose of atorvastatin. Therefore,we suggest paying particular attention to the potential risks of associating atorvastatin and midazolam in patients with chronic liver disease who need to undergo gastric endoscopy.

报道了实验室规模利用未经改性的虾壳做为吸附材料去除鞣制废水中的三价铬。实验发现对铬的去除率超过90%,证明该材料有能力去除三价铬。其对铬的吸附过程可以用Brauner-Emmett-Telller吸附等温线很好地描述,表明虾壳材料同时存在强、弱两种吸附位点。动力学实验表明吸附过程在最初的2 h内迅速完成,之后形成一个平台,可以用拟二级方程很好地模拟。论文还分析比较了虾壳与其他经常用于脱铬的化学品的效果差异,结合扫描电子显微镜形貌分析结果,表明虾壳的处理效率与碳酸氢钠和氢氧化钠的处理效率相当,但是去除机理不同,铬在虾壳多糖基质上的吸附占主导,少部分是铬盐的沉淀。用回收的三价铬直接鞣制的皮革性能差,而将吸附产生的铬泥调节p H值后作为鞣剂,能够获得令人满意的鞣制效果。
