AIM： To compare the cleansing quality of polyethylene glycol electrolyte solution and sodium phosphate with different schedules of administration, and to evaluate whether the timing of the administration of bowel preparation affects the detection of polyps. METHODS： One hundred and seventy-seven consecutive outpatients scheduled for colonoscopy were randomized in one of four groups to receive polyethylene glycol electrolyte solution or oral sodium phosphate with two different timing schedules. Quality of cleansing, polyp detection, and tolerance were evaluated. RESULTS： Patients receiving polyethylene glycol or sodium phosphate on the same day as the colonoscopy, obtained good to excellent global cleansing scores more frequently than patients who received polyethylene glycol or sodium phosphate on the day prior to the procedure （P 〈 0.001）. Flat lesions, but not fiat adenomas, were more frequent in patients prepared on the same day （P = 0.02）. CONCLUSION： The quality of colonic cleansing and the detection of fiat lesions are significantly improved when the preparation is taken on the day of the colonoscopy.

The complete analytical solution of the Riemann problem for the homo-geneous Dispersive Nonlinear Shallow Water Equations [Antuono, Liapidevskii andBrocchini, Stud. Appl. Math., 122 (2009), pp. 1-28] is presented, for both wet-bed anddry-bed conditions. Moreover, such a set of hyperbolic and dispersive depth-averagedequations shows an interesting resonance phenomenon in the wave pattern of the solu-tion and we define conditions for the occurrence of resonance and present an algorithmto capture it. As an indirect check on the analytical solution we have carried out a de-tailed comparison with the numerical solution of the government equations obtainedfrom a dissipative method that does not make explicit use of the solution of the localRiemann problem.

Synucleinopathies are a group of progressive neurodegenerative disorders characterized by the accumulation of α-synuclein(α-Syn) aggregates in Lewy bodies(LBs) and Lewy neurites(LNs) in Parkinson's disease(PD) and dementia with Lewy bodies(DLB), and in glial cytoplasmic inclusions in multiple system atrophy(MSA). α-Syn is a 140 amino acid intrinsically disordered protein, which tends to self-aggregate and form fibrils in these neuropathological hallmark inclusions. Several lines of evidence sug gest that misfolding and aggregation of α-Syn is a critical step leading to neuronal dysfunction and death.

AIM: To analyze the epidemiology, clinical characteristics, treatment patterns and outcome in hepatocellular carcinoma (HCC) patients. METHODS: We analyzed clinical, pathological and therapeutic data from 256 consecutive patients, examined at S. Croce Hospital in Cuneo-Piedmont, with a diagnosis of HCC between 30th June 2000 and 1st July 2010. We analyzed the hospital imaging database and examined all medical records, including the diagnosis code for HCC (155.0 according to the ICD-9M classification system), both for inpatients and outpatients, and discovered 576 relevant clinical records. After the exclusion of reports relating to multiple admissions for the same patient, we identified 282 HCC patients. Moreover, from this HCC series, we excluded 26 patients: 1 patient because of an alternative final diagnosis, 8 patients because of a lack of complete clinical data in the medical record and 17 patients because they were admitted to different health care facilities, leaving 256 HCC patients. To highlight possible changes in HCC patterns over the ten-year period, we split the population into two five-year groups, according to the diagnosis period: 30 th June 2000-30th June 2005 and 1st July 2005-1st July 2010. Patients underwent a 6-mo follow up. RESULTS: Two hundred and fifty-six HCC patients were included (male/female 182/74; mean age 70 years), 133 in the first period and 123 in the second. Hepatitis C virus (HCV) infection was the most common HCC risk factor (54.1% in the first period, 50.4% in the second;P = 0.63); in the first period, 21.8% of patients were alcoholics and 15.5% were alcoholics in the second period (P > 0.05); the non-viral/non-alcoholic etiology rate was 3.7% in the first period and 20.3% in the second period (P < 0.001). Child class A patients increased significantly in the second period (P < 0.001). To perform a meta-analysis of observational studies on inflammatory markers levels and occurrence of colorectal *** and EMBASE databases were searched until March 2016 for the articles reporting on the circulating levels of inflammatory markers, including: C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α) and risk of colorectal adenoma. Random-effects models were used to calculate summary odds ratios (ORs) with 95%CIs for the highest vs lowest category of exposure. Heterogeneity was assessed by using the Q test and I2 statistic. Subgroup analyses were also performed to test for potential source of *** total of 14 case-control studies were included. Ten studies on CRP including a total of 3350 cases and 4168 controls showed non-significant summary (OR = 1.23, 95%CI: 0.98-1.54; I2 = 54%, Pheterogeneity = 0.01) in the general analysis, but significant increased odds when considering only advanced adenoma (OR = 1.59, 95%CI: 1.09-2.32; I2 = 44%, Pheterogeneity = 0.15). Subgroup and stratified analyses revealed a potential influence of smoking status and aspirin use on the association between CRP levels and colorectal adenoma. Five studies examined the association between circulating levels of TNF-α and colorectal adenoma risk, including a total of 1,568 cases and 2,832 controls. The summary OR for the highest vs the lowest category of exposure was 1.00 (95%CI: 0.77-1.29). The relationship between circulating IL-6 levels and colorectal adenoma risk was investigated in 7 studies including a total of 1936 cases and 3611 controls. The summary OR for the highest vs the lowest category of exposure was 1.19 (95%CI: 0.92-1.55).CONCLUSIONSummary of current evidence suggests a positive association of CRP levels and advanced colorectal adenoma risk. The role of potential confounding factors should be further evaluated.

Nitrous oxide(N_(2)O)is a potent greenhouse gas(GHG)contributing to global warming,with the agriculture sector as the major source of anthropogenic N_(2)O emissions due to excessive fertilizer *** is an urgent need to enhance regional-/watershed-scale models,such as Soil and Water Assessment Tool(SWAT),to credibly simulate N_(2)O emissions to improve assessment of environmental impacts of cropping ***,we integrated the DayCent model’s N_(2)O emission algorithms with the existing widely tested crop growth,hydrology,and nitrogen cycling algorithms in SWAT and evaluated this new tool for simulating N_(2)O emissions in three agricultural systems(i.e.,a continuous corn site,a switchgrass site,and a smooth brome grass site which was used as a reference site)located at the Great Lakes Bioenergy Research Center(GLBRC)scale-up fields in southwestern *** three systems represent different levels of management intensity,with corn,switchgrass,and smooth brome grass(reference site)receiving high,medium,and zero fertilizer application,*** indicate that the enhanced SWAT model with default parameterization reproduced well the relative magnitudes of N_(2)O emissions across the three sites,indicating the usefulness of the new tool(SWAT-N_(2)O)to estimate long-term N_(2)O emissions of diverse cropping ***,parameter calibration can significantly improve model simulations of seasonality of N_(2)O fluxes,and explained up to 22.5%-49.7%of the variability in field *** sensitivity analysis indicates that climate change(e.g.,changes in precipitation and temperature)influences N_(2)O emissions,highlighting the importance of optimizing crop management under a changing climate in order to achieve agricultural sustainability goals.
