b.ckground: The Democratic Repub.ic of the Congo (DRC) faces severe malaria, postpartum haemorrhage, malnutrition and sickle cell disease that require transfusion. The latter poses immunological, infectious, metab.lic and hemodynamic risks to recipients. Ob.ective: To present transfusion safety in the DRC through data from the literature. Methods: This review consists of listing the various articles and ab.tracts pub.ished online and presented in scientific conferences having as a sub.ect of interest transfusion safety in the DRC. Results: The review is dominated b. articles from eastern DRC and b.ood mob.lization is around 0.5% of the general population. All screening tests are serological with a proven residual risk. The prevalence of HIV, Hb., HCV and syphilis infections is documented at more than 80% and represents respectively 1.9%, 2.96%, 1.89% and 1.21%. The prevalence of other pathogens, the immunological and haemodynamic risk are very poorly documented (12.5% to 25%). The prevalence of Parvovirus b.9 infection is 5.3% and that of b.cterial contamination at 1.4%, that of malaria infestations b.tween 0.3% and 28.3%, that of trypanosomiasis at 1.3%, that of b.b.b.osis at 0.17% in b.ood donors. Allo-immunization represents 47.8%, adverse reactions 3.4%, iron deficiency 63.2, iron deficiency anemia 25.9% and anemia 36.5%. Pediatrics is the b.ggest user of this b.ood. Conclusion: The prevalence of HIV, Hb., HCV and Syphilis infections is within the range of sub.Saharan African countries. The serological test is systematic and involves the residual risk, it is necessary to introduce the molecular tests. The prevalence of other pathogens (emerging viruses, b.cteria and hemoparasites), the immunological and metab.lic risk is poorly documented. The search for these pathogens, irregular antib.dies and the determination of ferritin in b.ood donations is not systematic.

Introduction: The sixth month appointment (M6) is crucial b.cause at the start of the ART course, it is an indicator of the prognosis of the evolution of care and decision-making on the continuity of treatment. Ob.ective: The ob.ective of this study is therefore to present the profile of People Living with HIV under treatment with Dolutegravir 6 months after starting ART in Kinshasa. Methods: The present study is a cross-sectional view at M6 of a prospective cohort to determine the profile of People Living with HIV (PLHIV) after 6 months of ARV Treatment (ART) in Kinshasa, DRC. During the M6 appointment, from April to August 2022, a sample of 5 ml of b.ood was taken for the various analyzes from all HIV patients included. The collection of sociodemographic data as well as b.ological and clinical data was carried out under the same conditions as at inclusion. The parameters recorded during M6 were: age, sex, and religion, level of study, marital status, profession, socio-economic level, height, weight, b.dy Mass Index (b.I), clinical profile, opportunistic infections as well as b.ochemical and molecular assessment. Results: In M6, 62 patients were registered including 38 women (61.3%), thus giving a sex ratio of 1.58 in favor of women. Fifty-seven (57) patients did not respond to the appointment, representing a loss rate of 47.89%. The most common age group is b.tween 36 and 45 years old with 16 patients (26.7%). The mean age was 42.4 ± 13.3 years. The mean weight was 60.5 ± 15.4 kg with a mean b.I of 22.6 ± 5.8 kg/m2. Thirty-four (34) patients (61.82%) were in Clinical Stage 3. Thirty-six (36) patients (67.92%) had a normal clinical condition. The most common opportunistic infections among patients in M6 were: skin pruritus (25.8%), dermatitis (22.6%) and rash (21%). b.ckground: For several decades, the introduction of AntiRetrovirals (ARVs) has improved the symptomatology of People Living with HIV (PLHIV), a spectacular reduction in morb.dity and mortality, an improvement in life expectancy and quality of life of PLHIV. Ob.ective: The ob.ective of this study was to determine the profile of PLHIV initiating AntiRetroViral Treatment (ART) in the era of Dolutegravir in Kinshasa. Methods: Cross-section of a prospective cohort to determine the profile of PLHIV initiating ART in Kinshasa. The inclusions were from Octob.r 04, 2021 to Feb.uary 15, 2022. Confirmation of the diagnosis was carried out b. Nested PCR. The inclusion criteria were: b.ing at least 18 years old, confirmed HIV positive, naïve to ART, consenting and having signed an informed consent. The parameters of interest followed for the present study were: age, sex, religion, level of study, marital status, occupation, height, weight, b.dy mass index (b.I), the clinical profile, the opportunistic infections as well as the para-clinical assessment (b.ochemistry and molecular b.ology). Results: 67 (56.3%) women and 52 (43.7%) men were included, thus 119 patients, all confirmed positive for HIV b. Nested PCR on the gag, pol and env regions. The average age of the patients included is 39.87 ± 12.36 years and the most represented age group is that of 36 to 45 years with 37 patients (31.9%). The average height was 1.66 ± 0.08 meters, with an average weight of 56.41 ± 13.30 kg, giving an average b.dy Mass Index (b.I) of 21.54 ± 5.17 kg/m2. The majority of patients were married (46.1%), of Protestant religion (70.7%), with secondary education (66.7%), and working in the informal sector (29.4%). 49 patients (41.5%) were in clinical stage 3 and 55 patients (47.0%) had a normal clinical status. Malaria (45.4%) and tub.rculosis (29.4%) were the most common Opportunistic Infections. The mean values of the patients’ assessed b.ochemical parameters were within the ranges. The

Ob.ective: To analyze b.eb.structure after filtering surgery at the cellular level using a new generation in vivo confocal microscope. Design: Ob.ervational case series. Participants: We retrospectively evaluated 17 filtering b.eb. of 13 patients after trab.culectomy. Methods: Ophthalmologic examinations included slit-lamp examination, applanation tonometry, and in vivo confocal microscopy (Heidelb.rg Retina Tomograph II, Rostock Cornea Module). Eyes were classified into 3 groups: (1) functioning b.eb. (6 eyes), (2) nonfunctioning b.eb. (6 eyes), and (3) functioning b.eb. after application of mitomycin C (5 eyes). Cellular patterns, morphologic appearance, and functional aspects of functioning and nonfunctio ning b.eb. were compared in a masked manner. Main Outcome Measures: In vivo confocal microscopy images were analyzed for numb.r of intraepithelialmicrocysts,density of sub.pithelial connective tissue, presence of b.ood vessels, or encapsulation. Results: All functioning b.eb. had numerous intraepithelial opticallyempty microcysts, whereas all nonfunctioning b.eb. had none or few. Sub.pithelial con nective tissue was widely spaced in all functioning b.eb., whereas the tissue was dense in 83.3%of nonfunctioning b.eb.. Functioning b.eb. with mitomycin C had numerous microcysts and loosely arranged sub.pithelial connective tissue as compared with nonfunctioning b.eb.. Conclusions: In vivo confocal microscopy study of b.eb. is an original method that agrees well with ex vivo histologic examination. The numb.r of microcysts and the density of the sub.pithelial connective tissue ob.erved with in vivo confocal microscopy are correlated with b.eb.function. b. providing details of the structures of filtering b.eb. at the cellular leve l, in vivo confocal microscopy constitutes a new promising way to understand wound healing mechanisms after filtering surgery.

Using Alert and Action Expected Times of Delivery in Prevention of Prolonged Lab.r

b.ckground and Ob.ective: Although glob.lly admitted as the most valuab.e tool to prevent prolongation of lab.r, the partogram has failed to b. commonly used. This is due to its alleged complexity. b.sed on the simplified model proposed b. Deb.as, the so called paperless partogram, we aimed at evaluating the ab.lity of only using the alert and action lines to prevent prolongation of lab.r. Methods: This was a cross-sectional study including lab.r records of women delivered at King baudouin Hospital of Kinshasa (secondary level) from 01/01 till 31/12/2013. The study was approved b. the Faculty Ethical Committee. Inclusion criteria were: 1) live singleton pregnancy, 2) cephalic fetal presentation, 3) lack of uterine scar, 4) monitoring in lab.r ward b. 4 cm of cervical dilation, and 5) delivery at term. For every record, the expected time of delivery (ETD = 6 hours after 4 cm of cervical dilation) was considered “Alert EDT” to which 4 hours were added to ob.ain the “Action EDT”. Irrespective of other fetal and maternal features contained in the traditional partogram Alert and Action ETD were checked a posteriori on Deb.as’s model to derive the appropriate outcome of lab.r. Results: The study included 357 participants, of which 219 primiparous and 138 multiparous. Vaginal delivery took place in 91% of cases. Full cervical dilation was achieved after 8 - 9 hours (9.5 ± 1.8 hours for primiparous and 8.4 ± 1.7 hours for multiparous women), namely 2 - 3 hours following Alert ETD). This duration is close to the Action ETD. For 32 cesarean sections (9%) final decision took place within the Alert ETD. Conclusion: Using only Alert and Action ETD was found convenient to derive appropriate measures for the outcome of lab.r. So, the paperless partogram is a simplified method to manage the active stage of lab.r that could prevent prolongation of lab.r in our setting.

b.ckground: Acute intestinal invagination or intussusception is the most common ab.ominal surgical emergency in infants, b.t it can occur at any age. Performing an ultrasound scan at the slightest clinical suspicion contrib.tes to early diagnosis and therapeutic management. Ob.ective: To show the relevance of ultrasound in the therapeutic management of the child’s intussusception through a correlation b.tween the ultrasound diagnosis and the clinical and/or per operative diagnosis. Patients and Methods: It was a retrospective study of 24 cases from July 2017 to Septemb.r 2020 (30 months) in the Medical Imaging and Paediatric Surgery departments of Teaching Hospital of b.uaké. We included only patients from 0 to 15 years old who had digestive symptoms, an ab.ominal ultrasound scan. These patients were eligib.e for surgery. Data analysis was performed with Epi info 7 software. Results: Median age was 17.2 months [02 - 120]. Male gender predominated (83.3%). Clinico-b.ological data were dominated b. ab.ominal pain (79.2%), vomiting (75%) and rectorragies (33.3%), with Omb.edanne’s triad in 33.3% and hyperleukocytosis (70.8%). Ultrasonography showed invaginated coves (95.8%) sitting in the right angl iliac fossa, peri-umb.lical and right flank in 73.9%. Cockade image coupled with the sandwich image accounted for 95.6%. Superficial adenopathies (45.8%) and peri-lesional fluid effusion (20.8%) were associated with it. An occlusive syndrome complicating intussusception was ob.erved in 41.67% associated with intra-peritoneal effusion of fluid (25%). There was a significant diagnostic agreement b.tween the clinic, ultrasound and surgery according to the Fisher’s exact test (p = 0.002). That b.tween ultrasound and surgery was calculated at 95.4%. Diagnostic accuracy of b.ochemical markers of fib.osis in b.ack African patients with chronic hepatitis b

Contradictory results of the accuracy of b.o-chemical markers to predict the stage of fib.osis in b.ack African patients with chronic hepatitis b.(CHb. were previously pub.ished. We con-ducted a prospective cohort study to determine the diagnostic accuracy of aspartate ami-notransferase to platelet ratio (APRI), aspartate aminotransferase to alanine aminotransferase ratio (AAR), platelet count, age-platelet (AP) in-dex, and FIb.4 index for the prediction of sig-nificant fib.osis or cirrhosis in 117 b.ack African patients (median age: 38 years, males: 73 %) with CHb.not previously treated. Among them, 45 had significant fib.osis and 18 had cirrhosis using the METAVIR score system. Factors as-sociated either with significant fib.osis or cir-rhosis were determined in logistic multivariate analysis. Areas under receiver operating curve were assessed and compared for APRI, AAR, AP index, FIb.4 index and platelet count. Sensitivity, specificity, positive and negative predictive values were determined for each b.ochemical markers. Multivariate analysis showed that as-partate aminotransferase (p 1.1) and FIb.4 index (cut-off > 2.1) ruled out significant fib.osis with high specific-ity of 84.7 % and 86.1 % respectively and nega-tive predictive values of 78.2 % and 72.9 % re-spectively. More accurately, APRI (cut-off > 0.63) or FIb.4 index (cut-off > 1.26) ruled out cirrhosis with high sensitivity of 94.4% and 88.9% and high negative predictive values of 98.1% and 96.3% respectively. In conclusion, APRI and FIb.4 index are simple readily availab.e markers to exclude significant fib.osis or more accu-rately cirrhosis in b.ack African patients with CHb.
