Role of angiotensin converting enzyme and angiotensinogen gene polymorphisms in angiotensin converting enzyme inhibitor-mediated antiproteinuric action in type 2 diabetic nephropathy patients

AIM To investigate the role of genetic variants of angiotensin converting enzyme(ACE) and angiotensinogen(AGT) genes in the antiproteinuric efficacy of ACE inhibitor therapy in diabetic nephropathy(DN) *** In the present study, 270 type 2 diabetes mellitus patients with nephropathy were enrolled and treated with ACE inhibitor(ramipril) and followed at 6 mo for renal function and albumin excretion by estimating serum creatinine, end stage renal disease, and albumin/creatinine ratio(ACR) in urine. Genotyping of ACE I/D and AGT M235 T polymorphisms were performed by using primer specific polymerase chain reaction(PCR) and PCR-RFLP techniques, respectively. RESULTS Forty-eight percent of DN patients(responders) benefited with respect to proteinuria from ACE inhibitor therapy at 6 mo follow-up. A significant reduction in ACR was observed after 6 mo treatment with ACE inhibitor irrespective of whether DN patients were micro-albuminuric(≥ 30 and < 300 mg/g creatinine) or macro-albuminuric(≥ 300 mg/g creatinine) at the time of enrollment. However, macro-albuminuric patients(55%) showed better response to therapy. A reduction in urinary ACR was found independent of genotypes of ACE I/D and AGT M235 T polymorphisms although macro-albuminuric patients having TT genotype showed statistically insignificant increased response(72%). CONCLUSION ACE inhibitor therapy reduced urinary ACR by ≥ 30% in 50% of DN patients and the response is independent of ACE I/D and AGT M235 T polymorphisms.

Pregnancy Outcomes in Smokeless Tobacco Users Cohort Study in a Tertiary Care Hospital

There is a dearth of literature regarding the pregnancy outcomes in antenatal women using smokeless tobacco products (STP). Objective: To compare maternal and neonatal pregnancy outcomes in antenatal women using smokeless tobacco products with those not using tobacco products. Method: Pregnant women attending antenatal clinic at Guru Teg Bahadur hospital, Delhi received brief information about smokeless tobacco products. Antenatal women using STP and matched controls, 92 in each group (total 184) were recruited for the study. After obtaining consent, the urinary cotinine level was measured in both groups at first contact. Antenatal, labour and postpartum events were recorded in both groups. Urinary cotinine levels were again measured at 6 months in the user group. Results: Average age of antenatal women under study was 24.8 yrs and 25.21 yrs in user and non-user groups respectively. More women in the user group were illiterate (P ? 20.39 μg/ml and 24.37 +/? 20.14 μg/ml respectively (p < 0.0001). Women using STPs for more than 5 yrs recorded higher urinary cotinine levels. There was no significant difference in urinary cotinine levels with the type of tobacco product consumed. Pregnancy outcomes of 39 women are known. There was no significant difference in antenatal, intra-natal and postpartum complications in the two groups. Neonatal weight difference was 20 ***: Smokeless tobacco products are the most common forms used by women in low and middle income countries. Larger studies are required to understand their effects on pregnancy outcomes.

Effect of Low Level Subchronic Microwave Radiation on Rat Brain

Objective The present study was designed to investigate the effects of subchronic low level microwave radiation （MWR） on cognitive function, heat shock protein 70 （HSP70） level and DNA damage in brain of Fischer rats. Methods Experiments were performed on male Fischer rats exposed to microwave radiation for 90 days at three different frequencies： 900, 2800, and 2450 MHz. Animals were divided into 4 groups： Group I： Sham exposed, Group I1： animals exposed to microwave radiation at 900 MHz and specific absorption rate （SAR） 5.953 x 10-4 W/kg, Group II1： animals exposed to 1800 MHz at SAR 5.835 x 20-4 W/kg and Group IV： animals exposed to 2450 MHz at SAR 6.672 x 10-4 W/kg. All the animals were tested for cognitive function using elevated plus maze and Morris water maze at the end of the exposure period and subsequently sacrificed to collect brain tissues. HSP70 levels were estimated by ELISA and DNA damage was assessed using alkaline comet assay. Results Microwave exposure at 900-2450 MHz with SAR values as mentioned above lead to decline in cognitive function, increase in HSP70 level and DNA damage in brain. Conclusion The results of the present study suggest that low level microwave exposure at frequencies 900, 2800, and 2450 MHz may lead to hazardous effects on brain.

Diversity of Orchids in Terms of Their Distribution, Uses and Conservation in Annapurna Conservation Area of Nepal

The Annapurna Conservation Area (ACA), the first conservation area and the largest protected area (PA) in Nepal, is incredibly rich in biodiversity. Notwithstanding this, orchids in the ACA have not been explored enough yet thus making the need for ambitious research to be carried out. Previous study only included 81 species of orchids within ACA. This study aims to update the record of species and genera richness in the ACA. In total 198 species of orchids, belonging to 67 genera (40% and 62% of the total recorded orchid species and genera in Nepal) has been recorded in ACA. This represents an increase of 144% in species and 56% in genera over the previous data. Out of the 198 species, 99 were epiphytes, 6 were holomycotrophic and 93 were terrestrial. Among the 67 genera, Bulbophyllum (17) species were dominant, followed by Dendrobium (16), Herminium (10), Coelogyne, Plantanthera (9 each), Eria, Habenaria, Oberonia (8 each), Calanthe (7), and Liparis (6). Fifty-six species were found to be ornamentally significant and 85 species medicinally significant.

Early diagnosis of cerebral fat embolism in a patient with contradiction to MRI is *** we report an interesting case,where the raised optic nerve sheath diameter helped us to predict the early cerebral involvement with fat emboli in a left femoral shaft fracture *** scan could not be performed due to the presence of a metallic implant in the patient from a previous *** was later diagnosed as an atypical presentation of fat embolism *** nerve sheath monitoring also helped us to guide further management of the patient.

Background Nepal has achieved and sustained the elimination of leprosy as a public health problem since 2009,but 17 districts and 3 provinces with 41%(10,907,128)of Nepal’s population have yet to eliminate the *** cases and grade-2 disabilities(G2D)indicate recent transmission and late diagnosis,respectively,which necessitate active and early case *** operational research was performed to identify approaches best suited for early case detection,determine community-based leprosy epidemiology,and identify hidden leprosy cases early and respond with prompt *** Active case detection was undertaken in two Nepali provinces with the greatest burden of leprosy,Madhesh Province(40%national cases)and Lumbini Province(18%)and at-risk prison populations in Madhesh,Lumbini and Bagmati *** detection was performed by(1)house-to-house visits among vulnerable populations(n=26,469);(2)contact examination and tracing(n=7608);in Madhesh and Lumbini Provinces and,(3)screening prison populations(n=4428)in Madhesh,Lumbini and Bagmati Provinces of *** case direct medical and nonmedical costs for each approach were *** New case detection rates were highest for contact tracing(250),followed by house-to-house visits(102)and prison screening(45)per 100,000 population ***,the cost per case identifed was cheapest for house-to-house visits[Nepalese rupee(NPR)76,500/case],followed by contact tracing(NPR 90,286/case)and prison screening(NPR 298,300/case).House-to-house and contact tracing case paucibacillary/multibacillary(PB:MB)ratios were 59:41 and 68:32;female/male ratios 63:37 and 57:43;pediatric cases 11%in both approaches;and grade-2 disabilities(G2D)11%and 5%,*** leprosy was not signifcantly diferent among household and neighbor contacts[odds ratios(OR)=1.4,95%confdence interval(CI):0.24-5.85]and for contacts of MB versus PB cases(OR=0.7,95%CI 0.26-2.0).Attack rates were not signifcantly d

Clinical characteristics and management of patients with fat embolism syndrome in level I Apex Trauma Centre

Purpose: Fat embolism syndrome (FES) is systemic manifestation of fat emboli in the circulation seen mostly after long bone fractures. FES is considered a lethal complication of trauma. There are various case reports and series describing FES. Here we describe the clinical characteristics, management in ICU and outcome of these patients in level I trauma center in a span of 6 months. Methods: In this prospective study, analysis of all the patients with FES admitted in our polytrauma intensive care unit (ICU) of level I trauma center over a period of 6 months (from August 2017 to January 2018) was done. Demographic data, clinical features, management in ICU and outcome were analyzed. Results: We admitted 10 cases of FES. The mean age of patients was 31.2 years. The mean duration from time of injury to onset of symptoms was 56 h. All patients presented with hypoxemia and petechiae but central nervous system symptoms were present in 70% of patients. The mean duration of mechanical ventilation was 11.7 days and the mean length of ICU stay was 14.7 days. There was excellent recovery among patients with no neurological deficit. Conclusion: FES is considered a lethal complication of trauma but timely management can result in favorable outcome. FES can occur even after fixation of the fracture. Hypoxia is the most common and earliest feature of FES followed by CNS manifestations. Any patient presenting with such symptoms should raise the suspicion of FES and mandate early ICU referral.

VLF/LF Amplitude Perturbations before Tuscany Earthquakes, 2013

The US Navy VLF/LF Transmitter’s NSY signal (45.9 kHz) transmitted from Niscemi, Sicily, Italy, and received at the Kiel Long Wave Monitor, Germany, was analyzed for the period of two months, May and June (EQ-month) of 2013. There were 12 earthquakes of magnitude greater than 4 that hit Italy in these two months,?of which the earthquake of 21st June having magnitude of 5.2?and a shallow focal depth of 5 km was the major one. We studied the earthquake of 21st of June 2013, which struck Tuscany,?Central Italy, (44.1713°N and?10.2082°E) at 10:33 UT, and also analyzed the effects of this earthquake on the?sub-ionos-?pheric VLF/LF signals. In addition, we also studied another earthquake, of magnitude 4.9, which hit the same place at 14:40 UT on 30th of June and had shallow focal depth of 10 km. We assessed the data using terminator time (TT) method and night time fluctuation method and found unusual changes in VLF/LF amplitudes/phases. Analysis of trend, night time dispersion, and night time fluctuation was also carried and several anomalies were detected. Most ionospheric perturbations in these parameters were found in the month?of June, from few days to few weeks prior to the earthquakes. Moreover, we filtered the possible effects due to geomagnetic storms, auroras, and solar activities?using parameters like?Dst index, AE index, and Kp index for analyzing the geomagnetic effects,?and Bz (sigma) index, sunspot numbers, and solar index F10.7 for analyzing the solar activities for the confirmation of anomalies as precursors.

Knowledge and prevention measures against dengue in southern Nepal

Objective:To ascertain the knowledge and prevention measures against dengue among selected adult population in southern ***:A cross sectional study was conducted in Rapti Zonal Hospital,Dang and Bharatpur Hospital,Chitwan during August-November,2011.A total of 212 patients were interviewed regarding their knowledge and practices about dengue *** samples were collected and investigated for the presence of anti-dengue IgM antibodies by ***:Data from 156 patients greater than 15 years were *** them,21 samples were found to possess anti-dengue IgM antibodies by ***,economically active people,farmers,unemployed and housewives were more frequently affected by *** the total,118(76%)were reported to know the disease ***,headache and myalgia were the most known symptoms and house drains and kitchen garden were known as the most common breeding sites of *** disposal,use of mosquito nets and covering water containers were the best known preventive ***:The overall knowledge of dengue was high,but females,old persons,unemployed and housewives had relatively little knowledge of ***,these groups may need special attention in future dengue education *** with knowledge of the disease more frequently reported the use of preventive measures,indicating the value of education programs as a tool in dengue prevention.
