The Frequency of K-ras Mutation in Colorectal Adenocarcinomas with Absence of Distant Metastasis at Diagnosis

Purpose: K-ras mutations were reported to be in 40% - 60% of patients with sporadic colorectal cancers (CRCs). HER-2/neu oncogene that is important in breast carcinoma was reported in CRCs in several studies. The aims of our study are to determine;the frequency of K-ras mutation in CRC and positivity of HER-2/neu, the relation between K-ras mutation and HER-2/neu positivity, and also the relation between clinicopathological findings with K-ras mutation and HER-2/neu expression. Methods: Total of 35 colon resection specimen from patients without distant metastasis who were operated due to colorectal adenocarcinoma were included in the study. The sections were examined with light microscopy. Vascular and perineural invasions and pericolonic tumor deposits (PCTD) were investigated. HER-2/neu was applied immunohistochemically. DNA sample obtained from tumor paraffin block was screened for presence of 20 mutations in K-ras gene-codon 12, 13, 61 by AutoGenomics and Infinity Analyzer. Results: K-ras gene mutation was detected in 14 of 35 patients (40%). HER-2/neu positivity was detected in 7 cases (28.57%). There was not any significant correlation between HER-2/neu positivity, K-ras gene mutation and clinicopathological findings. There was direct correlation between PCTD and vascular invasion. Conclusions: There was not correlation between K-ras mutation and clinicopathological findings similar to several other studies. The relation between HER-2/neu expression and clinicopathological findings was not found.

Oxidized low density lipoprotein （ox-LDL） molecule is one of the most important modified lipoproteins produced during the oxidative stress. Modified lipoproteins have been defined as being part of the immune inflammatory mechanisms in association with oxidant stress. We have reported the accumulation of ox-LDL in Balb/c mice liver after bile duct ligation previously. Here, we investigated this finding in human beings with obstructive jaundice. Our study demonstrates that obstructive jaundice results in tremendous accumulation of ox-LDL in the liver tissue of patients.

A geriatric patient with diffuse idiopathic skeletal hyperostosis

The most frequent health problems seen in senility are chronic and degenerative diseases. A 75-year-old male patient with the complaints of weight loss and difficulty in swallowing was admitted to our hospital from a nursing home. Upper system fiber-optic gastrointestinal endoscopy was performed and a mass at the junction of the hypopharynx and esophagus just below recessus piriformis obstructing almost the whole of the lumen and blocking the distal passage was detected. Computed tomography revealed marked narrowing secondary to osseous hypertrophy in the air column of the hypopharynx and proximal esophagus. Diffuse idiopathic skeletal hyperostosis or Forestier’s disease is an idiopathic disease characterized by the ossification of the anterior longitudinal ligament of vertebra and some of the extraspinal ligaments. In the present case we aim to discuss an elderly patient who suffered from dysphagia and weight loss and the diagnostic stages.

Degradation of Diclofenac in Molecularly Imprinted Polymer Submicron Particles by UV Light Irradiation and HCl Acid Treatment

A new molecularly imprinted polymer (MIP) was synthesized by precipitation polymerization using diclofenac (DFC) as a template. Binding characteristics of the MIP particles were evaluated by equilibrium binding experiments. DFC-MIP aqueous suspension and non-imprinted polymer (NIP) suspension were exposed to monochromatic UV light (253.7 nm) from low-pressure mercury lamps. UV-visible spectrophotometry (especially absorbance at 276 nm) showed that the DFC inside MIP particles degraded completely. After DFC-MIP suspension exposure to UV light the particles were completely regenerated after washing with water at least six times. The regenerated MIP particles rebounded considerable amount of DFC (approximately 88% removal of 44 ppm DFC). The stability of DFC was examined in the presence of various concentrations of hydrochloric acid (0.025 to 125 mM). Experimental results showed that degradation of DFC was efficient, depending on the acid concentration as well as the treatment time. However, there was no re-binding of DFC by the MIP particles after HCl treatment (and DDW washing) when exposed to DFC for 24 hours.

Risk Assessment Framework of mHealth System Vulnerabilities: A Multilayer Analysis of the Patient Hub

Although there have been remarkable technological developments in healthcare, the privacy and security of mobile health systems (mHealth) still raise many concerns with considerable consequences for patients using these technologies. For instance, potential security and privacy threats in wireless devices, such as Wi-Fi and Bluetooth connected to a patient hub at the application, middleware and sensory layers, may result in the disclosure of private and sensitive data. This paper explores the security and privacy of the patient hub, including patient applications and their connections to sensors and cloud technology. Addressing the privacy and security concerns of the patient hub called for a comprehensive risk assessment by using the OCTAVE risk assessment framework. Findings reveal that the highest risk concerned data exposure at the sensory layer. In spite of the countermeasures presented in this paper, most served as a means to identify risk early as opposed to mitigating them. The findings can serve to inform users of the potential vulnerabilities in the patient hub before they arise.

Contamination and Ecological Risk Assessment of Heavy Metal in the Sediment of Turag River, Bangladesh: An Index Analysis Approach

Encroachment, disposal of untreated domestic and industrial wastewater and dumping of solid wastes have degraded the overall quality of the river Turag, which is located in Dhaka—the Capital City of Bangladesh. The present study investigated the extent of pollution of sediments of this river and analyzed the regional variability for the concentrations of Cr, Pb, Zn, Cu and Cd—all of concern because of their potential toxicity, using atomic absorption spectrophotometer. As per US EPA sediment quality guideline, metal concentrations ranged between Cd: 0.00 - 0.80, Cr: 32.00 - 75.50, Cu: 46.30 - 60.00, Pb: 28.30 - 36.40, and Zn: 94.60 - 190.10 mg/kg in the Turag river sediments. Cr, Cu, Zn belongs to moderately to highly polluted, Pb and Cd belongs to not polluted for Turag river. The heavy metals contamination in the sediments were also evaluated by applying Index of geo-accumulation (Igeo), contamination factor (Cf), pollution load index (PLI) etc. Toxicity characteristics leaching procedure (TCLP) test (US EPA 1311) for sediment samples have been performed also for metals (Pb, Cd, Cr, Cu and Zn) to determine the readily toxicity level of heavy metals. Sieve analysis of sediment performed in this study to determine the physical characteristic of sediment samples. The metal concentrations are well below the regulated level as per US EPA. This index can be complemented with the contamination index, which allows more site-specific and accurate information on contaminant levels. If the aim of work on contamination evaluation is to assess the overall contamination of a study area, the indices are highly appropriate.

Uncommon Ethiology in Abdominal Pain: Celiac Axis Compression Syndrome

Celiac axis compression syndrome, one of the reasons of mesenteric ischemia, is an extremely rare etiology of abdominal pain. Primary pathological mechanism is the external compression of the celiac trunk by median arcuate ligament. The diagnosis of this condition is usually difficult and depends on angiographic findings and computerized tomography (CT) evaluations. Here we report a celiac axis compression syndrome case, presenting with chronic abdominal pain and weight loss, which was successfully treated by surgery.

AIM:To compare the effectiveness of argon plasma coagulation (APC) and heater probe coagulation (HPC) in non-variceal upper gastrointestinal ***:Eighty-five (18 female,67 male) patients admitted for acute gastrointestinal bleeding due to gastric or duodenal ulcer were included in the *** endoscopy was performed and HPC or APC were chosen randomly to stop the *** hemostasis and rebleeding rates were primary and secondary end-points of the ***:Initial hemostasis was achieved in 97.7% (42/43) and 81% (36/42) of the APC and HPC groups,respectively (P 0.05).CONCLUSION:APC is an effective hemostatic method in bleeding peptic *** multicenter trials are necessary to confirm these results.
