strong>Introduction:strong> Breast cancer is the most common female cancer in India and accounting for almost 1 in 4 cancer cases in women worldwide. According to GLOBOCAN 2018: breast cancer incidence is increased to 162,468 in 2018 compared to 144,937 in 2012. Biosimilar drugs allow expanding access to the therapies in the form of cost savings and leading to better overall health outcomes. Our study evaluates the efficacy and safety of Trastuzumab biosimilars and assesses overall survival in the study population. Materialsspan> & Methods: This prospective study was conducted in Healthcare Global Enterprises Ltd., Bengaluru, India, and all female patients diagnosed with Her2 positive, metastatic (mBC) and Locally advanced breast cancer (LABC), between March 2013 and November 2014, with at least 4 years of post-treatment follow up. Results:span style="font-family:Verdana;"> A total of 65 patients diagnosed with Her2 positive breast cancer and satisfied the selection criteria were included for the study. Partial Response (PR) was observed in 42 (64.6%) patients, stable Disease (sD) in 11 (16.9%) patients and Progressive Disease (PD) in 12 (18.5%) patients. The overall response rates were 46.1% PR, 30% sD, 23.8% PD in metastatic population and 76% PR, 7.2% sD, 15% PD observed in locally advanced disease. The mean overall survival of the study population was 20.75 ± 15.20 months

Prospective Analysis of Patients Treated with Chemotherapy in Metastatic Adenocarcinoma of Lung—single Centre Experience

Background: Lung cancer is the leading cause of cancer-related death worldwide with an estimated 1.6 million new cases diagnosed each year. According to standard guidelines in NsCLC, the effectiveness of combined chemotherapy regimen comprises of pemetrexed and platinum, and very few reports are available in Indian population. This study shows the experience with pemetrexed and platinum combination chemotherapy in metastatic NsCLC in first line setting. Methods: Total 61 patients with following inclusion criteria were recruited such as: 1) Ps of 1 and 2;2) NsCLC of non-squamous histology;3) locally advanced or metastatic disease at diagnosis;4) should have received minimum 3 cycles of pemetrexed and platinum as first line treatment and or continued for 6 cycles;5) EGFR/ALK mutated or EGFR/ALK unknown patients but must have received upfront chemo. Primary endpoint of the present study is to assess Disease Control Rate (DCR) (CR + PR + sD). Progression Free survival, Os and Toxicity assessment were secondary endpoints. Results: The mean number of average chemotherapy cycles was found to be 4.38. The range of chemotherapy was 2 - 6 cycles. Disease control rate, defined as (CR + PR + sD), was seen in 44 (72.1%) patients. (PR in 26 (42.62%), sD 18 (29.5%)), Progressive disease (PD) was observed in 17 (27.9%) patients. Overall study showed that patients had PFs of 9.414 months (95% CI 6.709 - 12.120) and Os of 13.437 months (95% CI 10.721 - 16.153). Conclusion: Pemetrexed and platinum combination is effective and well-tolerated chemotherapy regimen in patients with metastatic adenocarcinoma lung cancer patients. Pemetrexed and cisplatin may be more effective in particular. Early switch over to TKIs is preferred immediately after obtaining molecular subtype result.

Background: Basidiobolomycosis is a rare chronic subcutaneous infection caused by Basidiobolus ranarum, which is usually treated with potassium iodide. Extensive deforming lesions in children can occur owing to lack of early diagnosis and/or inappropriate treatment. Case report: An 8- year- old girl child presented to us with extensive deforming plaque- like lesions over the left thigh and leg with multiple ulcerations of 1 year’ s duration. Histopathology was suggestive of subcutaneous zygomycosis with splendore Hoeppli phenomenon. Microscopic examination of the tissue showed branching, sparsely septate fungal hyphae on a 10% KOH mount, and culture yielded Basidiobolus ranarum. The patient was initially treated with potassium iodide for 6 weeks to which only a poor response was observed. Itraconazole therapy resulted in rapid regression of the lesions and complete resolution after 15 weeks of therapy. she continues to be disease- free at 1 year of follow up. Conclusions: This case report highlights that in an older child even longstanding extensive basidiobolomycosis can be safely treated with itraconazole. surgery is not usually necessary in these patients.

目的:研究一种草药复方制剂Bresol对于肥大细胞脱颗粒以及组胺释放的保护作用。方法:使用大鼠腹膜内肥大细胞,在体外经化合物48/80诱导肥大细胞脱颗粒及组胺释放,评估Bresol稳定肥大细胞的作用。结果:显微镜下正常对照组涂片显示较多完整的肥大细胞,有极少量的脱颗粒肥大细胞和微量的组胺释放。阳性对照组中用化合物48/80培养的肥大细胞出现了显著的肥大细胞脱颗粒现象以及高浓度的组胺释放。而100mg/L浓度的Bresol明显抑制了化合物48/80诱导的肥大细胞脱颗粒。此外,Bresol可有效抑制化合物48/80诱导的组胺释放,且抑制效果与剂量有关。结论:Bresol能够在体外抑制化合物48/80诱导的肥大细胞脱颗粒和组胺释放。本研究的发现可解释Bresol对多种过敏疾病有效可能是通过一种非免疫机制。

背景过度通气作为神经外科手术麻醉中的重要方法，其有效性还缺乏验证。我们在由手术医师评估脑肿胀分级和测定颅内压（ICP）的条件下，选择中度低碳酸血症和正常血液碳酸数值两种情况对接受开颅手术的幕上脑肿瘤患者进行一项双周期随机交叉试验研究。方法275例幕上脑肿瘤患者随机接受以下治疗程序：一是过度通气（动脉二氧化碳分压，PaCO2=25±2mmHg）继以正常通气（动脉二氧化碳分压，PaCO2=37±2mmHg）；二是正常通气继以过度通气。通气和呼气末CO2分压保持稳定20分钟。患者随机接受丙泊酚输注或异氟烷吸入麻醉。在每一试验阶段末由不了解治疗程序的手术医师测定ICP，并以4级评定方法评估脑肿胀分级。结果以通用估计方程模型分析，我们发现过度通气降低了45％脑肿胀加重的风险，P=0．004，95％可信区间为22％-61％，需要治疗患者数为8。过度通气期间ICP均数±标准差为12．3±8．1mmHg，低于正常通气时的16．2±9．6mmHg，P〈0．001。麻醉方式对脑肿胀评估和ICP无影响。结论对幕上脑肿瘤患者而言，手术中过度通气可改善与ICP降低相关的由手术医师评估的脑肿胀程度。
