The main aims of the current study are to determine the morphological features of the periglacial landforms(non-sorted step,mud circle,stony earth circle,thufur,and congeliturbation)located on the Ilgaz Mountains,examine the physicochemical and mineralogical properties with pedological processes of the soils,and assess of the effects of climatic conditions controlling the development of *** Ilgaz Mountains(2587 m a.s.l.),located in the Western Black Sea Region,within the Anatolian Mountains,are important in terms of periglacial landforms(mud circles,stony earth circles,thufurs,non-sorted steps,non-sorted stripes,congeliturbation deposits,and block currents).The descriptive statistics of 123 periglacial landforms measured by fieldworks were *** distribution of freezing and thawing in the Ilgaz Mountains throughout the year was evaluated,and it was found that freezing takes place between December and March,freezing-thawing takes place in April,May,October and November,and thawing takes place between June and *** to soil properties,organic matter content changes from 1.88%to 12.72%in non-sorted step soils,while it is between 2.03%and 12.24%in stony earth circle *** organic matter is observed to be close to congeliturbation deposits at lower ratios compared to non-sorted steps,stony earth circles and mud *** soil reactions on stony earth circles and non-sorted steps vary between slightly acidic and slightly *** the other hand,soil samples taken from the mud circles are different from those taken from the non-sorted steps and stony earth *** soil reaction is acidic,and pH changes between 4.86 and *** lime content also varies between 2.81%and 32.08%,with an average of 12.02%.The texture properties of soils are dominantly loam and clay loam,as in the non-sorted steps,stony earth circles,and mud *** their mineralogical properties,the XRD study was carried out to determine the primer mineral

Aims: To evaluate the inflammatory response of the exposed pulp of incisor teeth in rats after direct pulp capping, using platelet rich plasma (PRP), enamel matrix derivate (EMD), mineral trioxide aggregate (MTA) and calcium hydroxide (Ca(OH)2). Methods: The study was conducted on 36 Wistar albino rats with a total of 144 incisor teeth. The pulps of 96 teeth of the rats were perforated and capped with different agents. Serving as the positive control group, the pulps of 24 teeth were only perforated and capped without capping agents, whereas the pulps of 24 teeth were used as the negative control group without being perforated (without any process). The research was ended with the extracting of the teeth on the 7th day-28th day. The teeth were taken to the routine and histological follows;cross sections were prepared and painted with hematoxylen & eosin. All of the sections were evaluated in terms of inflammatory reaction by histologic analysis taken by light microscope. Statistical analysis was used. The normal distribution of all data was tested with the Mann Whitney U and the differences between the groups were analyze dusing Kruskal Wallis test at 0.05 level. Results: There are no statistically significant differences in terms of inflammation type and necrosis among the treatment groups on 7 days’ post capping. However, improved inflammatory cell accumulation, hyperemia and lowest necrosis were observed from the samples treated with PRP (p < 0.05). Conversely, the EMD group indicated that the criteria of inflammation scores and hyper- emia were higher in the 28th day (p < 0.05). Conclusions: Most of cells accumulating in the PRP group and most necrosis were seen in the EMD group. These new PRP materials might serve as pulp capping biomaterials to induce initial healing response in the future.

Chronic renal failure is strongly related to poor outcomes, in cardiovascular system, such as: cardiac dilatation, left ventricular dysfunction and mitral valve regurgitation. Mitral calcification and regurgitation play an important role in prediction of morbidity and mortality in patients on dialysis. Percutaneous mitral valve repair (MVR) with the MitraClip system, which has begun to be used in severe mitral insufficiency patients with multiple comorbid medical conditions, including renal disease, is a new, alternative method. This intervention is not associated with cardiopulmonary bypass, thus;it has been proven to be safe and effective in risky patients. In this report, we would like to share our anesthetic experience in successful mitral valve repair by MitraClip system in a patient who had undergone renal transplantation twice.

The Association between Preoperative Parental Anxiety and Emergence Agitation in Preschoolers

Introduction: The preoperative distress and anxiety experienced by preschoolers are associated with an increased incidence of troubled recovery from anesthesia. However, influences of parental anxiety on children at different stages of the surgical processes are not clear. The aim of this study was to evaluate any existing association between preoperative parental anxiety and emergence agitation in a pediatric surgery population. Materials and Methods: A total of 60 children ASA class I or II, aged 3 - 12 years old, undergoing adenotonsillectomy with sevoflurane, were included in the study. Before surgery, we used State-Trait Anxiety Inventory S-T (STAI S-T) to assess parental anxiety. Emergence agitation of the children was assessed with Watcha scale and recorded every 10 min of first 30 min of the postoperative period. Parents were contacted 24 hours after the surgery to evaluate their satisfaction, post operative pain and any side effect observed in the children. Results: Preoperatively, the mean STAI-S scores of mothers were significantly higher than those of fathers (p 0.05). The children had the highest agitation scores at 10th postoperative minutes with an incidence of 73.9%. There was no correlation between parental anxiety and emergence agitation. Logistic regression analysis showed that low educational level of the parent was the only independent factor for state anxiety (OR: 8.96, 95% CI: 1.50 - 40.35, p = 0.030). Discussion: In this study, we observed that education level of the parents might affect their preoperative anxiety. The factors influential in parental anxiety were not correlated with pediatric emergence agitation in this study.

Objectives: Total leukocyte count, a marker of inflammation has long been reported to be related with coronary artery disease (CAD). Recently, differenttial leukocyte count and elevated neutrophil to lymphocyte ratio (N/L) has been shown to indicate an increased long-term risk of mortality in patients with stable CAD, myocardial infarction and undergoing cardiovascular surgery. Thses studies offer incremental prognostic value to total leukocyte count. We sought to determine the relationship between the leukocyte subtypes and N/L ratio and the presence and extent of CAD. Study design: The study consisted of 107 patients [70 (65.4%) men, 37 (34.6%) women, mean age 59.5 ± 10.6 years] referred to coronary angiography with suspected CAD. The subjects were classified into two groups according to the presence of significant CAD and the extent of CAD was determined by the Gensini method. Results: The lymphocyte count was found to be lower (2031 ± 741 cells/μL vs 2392 ± 611 cells/μL, p: 0.010) and the N/L ratio was found to be higher in patients with CAD (2.86 ± 1.57 vs 2.04 ± 1.01, p p: 0.005]. Conclusion: Our findings reveal that N/L ratio, a simple marker which can be derived from a routine complete blood count test was significantly and independently related to presence and severity of coronary atherosclerosis.

Electron Microscopic Investigation of Anterior Lens Capsule and Epithelium in Patients with Diabetes Mellitus

Purpose: To evaluate the histopathologic alterations of organelles in the epithelium of the anterior lens capsule. Methods: The interventional study included 26 eyes;of which 11 had non-proliferative diabetic retinopathy (diabetic group) and cataract, and 15 had age-related cataract (control group). We investigated the anterior lens capsule in patients with diabetes mellitus by using electron microscopy and to compare it between diabetic eyes and healthy eyes. Anterior capsule samples were obtained by circular continuous capsulorhexis during phacoemulsification procedures. All the samples were fixed and conventionally processed for electron microscopy analysis. Results: Demographic characteristics of the diabetic group and the control group were similar (p > 0.05). In the diabetic group, electron dense cells with an apoptotic appearance were seen and these cells had an apoptotic nucleus and prominent mitochondrial crystalysis. In addition, there was dilatation of the endoplasmic reticulum cistern. In the control group, lens epithelial cells and all their elements had a normal pattern. Neither cells with an apoptotic appearance nor mitochondrial crystalysis was seen, but there was dilatation of the endoplasmic reticulum cistern. Conclusions: Diabetes mellitus can engender structural abnormalities of organelles in the epithelium of the anterior lens capsule including mitochondrial crystalysis, dilatation of the endoplasmic reticulum cistern and apoptotic dense nucleus. It can be suggested that diabetes mellitus affects organelles of anterior lens epithelium in eyes with cataracts, while it causes non-proliferative changes in the retina.

Comparison of the Five Different Methods in Arterial Diameter Measurement

Background/Aims: In this study, the different types of arterial diameter measurement methods were com-pared with each other on rat model. Methods: The study was planned in three phases. In phase 1, all subjects (n = 30) underwent high resolution B-mode ultrasound examination and external diameter of the right com-mon carotid artery (RCCA) was measured. In phase 2, RCCA was explored. In phase 3, rats were put into three groups. Group 1 was kept at -20oC, group 2 was embedded in 4% formaldehyde solution. In group 3, circulatory system was filled with %20 latex solution. In all groups, digital image of the RCCA was taken after the processes mentioned above. Images were assessed by Image – Pro Plus software. Results: The greatest average diameter was observed in the high resolution B-mode ultrasound (phase 1). In group 1 and 2, direct measurement of the RCCA on living animal (phase 2) gave significantly wider diameter than those obtained at phase 3 in the same groups. Direct (phase 2) and latex (phase 3) measurements were equal in group 3. Conclusion: High resolution B-mode ultrasound gives larger, probably actual diameter of the artery. Latex injection results might correspond those encountered during operation.

Dynamic Thiol-Disulphide Homeostasis in Patients with Multiple Sclerosis

Objective: The aim of this study was to evaluate thiol-disulphide homeostasis in patients with Multiple Sclerosis (MS) during attack and remission. Design and Methods: A total of 82 patients admitted to our clinic were divided into 2 groups according to whether they are having an attack or not. Peripheral blood samples were taken from all patients and their native thiol, disulphide and total thiol concentrations were measured by a newly developed automated method. In addition, their disulphide/native thiol, disulphide/total thiol and native thiol/total thiol ratios were calculated. Results: The average disulphide values, disulphide/native thiol and disulphide/total thiol ratios of the patients with MS who were having an attack were found to be significantly higher compared to those in remission (p = 0.001, p = 0.001, p = 0.001, respectively). Conclusion: Our results have shown that thiol-disulphide balance was shifted to the oxidative area during the relapse. These homeostasis parameters, which can be easily measured by this newly developed method at low costs, may be important in terms of showing the disease activity and presenting the underlying mechanisms of the disease.

Evaluation of the Effects of Venous Cannulation Sites on Postoperative Delirium in Children: Hand or Foot?

Introduction: Choosing an appropriate cannulation site is important for doctors and patients. In our clinical practice, we have observed that agitation and pain were less in the postoperative period when the cannula was inserted site. Cannulation anxiety in adults and especially in children may increase the failure of interventions by activating the sympathetic system in varying degrees we aimed to investigate whether the site of the cannula inserted following the induction of inhalation is associated with postoperative agitation and pain in preschool children who would undergo an otolaryngology operation. Methods: Pediatric patients who would undergo adenoidectomy-tonsillectomy surgery between the ages of 3 - 7 were included in our study regardless of their genders. The patients have been randomly distributed into groups (Group E—hand, Group A—foot). The evaluation was performed 0 and 30 minutes after extubation by FLAAC Pain Scala and PAED. Results: When the hand and foot groups were compared in terms of P0 (PAED 0 min. rating) values, hand group results were statistically significantly higher. Similarly, at the time of P30 (PAED 30. min rating), The Hand group was determined to be high. Conclusion: As a result of the study, we believe that choosing the feet as the cannulation site may be useful in appropriate cases in pediatric patients.
