background: the purpose of this study is to prospectively evaluate the performance characteristics of endoscopy and EUS in the diagnosis of GI subepithelial masses. Methods: A total of 100 consecutive patients referred for the evaluation of a suspected GI subepithelial lesion were prospectively studied with endoscopy followed by EUS. Size, color, mobility, location (intramural or extramural), consistency (solid, cystic, or vascular), and presumptive diagnosis were recorded at the time of endoscopy. EUS then was performed, and size, echogenicity, location, and presumptive diagnosis were determined. Results: A total of 100 subepithelial lesions were evaluated. Endoscopy had 98% sensitivity and 64% specificity in identifying intramural lesions. Size measurement by endoscopy correlated with size measurement by EUS (r = 0.88). Histology was obt-ained in 23 cases, with the presumptive EUS diagnosis correct in only 48% of cases. Most incorrect EUS diagnoses occurred with hypoechoic 3rd and 4th layer masses. Conclusions: Endoscopy has high sensitivity but low specificity in identifying the location (intramural or extramural) of subepithelial lesions. In addition, EUS imaging alone is insufficient to accurately diagnose 3rd and 4th layer hypoechoic masses, and histologic confirmation should be obtained whenever possible.

Context: Weight loss elicits physiological adaptations relating to energy inta ke and expenditure that antagonize ongoing weight loss. Objective: To test wheth er dietary composition affects the physiological adaptations to weight loss, as assessed by resting energy expenditure. Design, Study, and Participants: A rando mized paralleldesign study of 39 over-weight or obese young adults aged 18 to 4 0 years who received an energy-restricted diet, either low-glycemic load or lo w-fat. Participants were studied in the General Clinical Research Centers of th e brigham and Women’s Hospital and the Children’s Hospital, boston, Mass, befo re and after 10%weightless. the study was conducted from January 4,2001, to May 6, 2003. Main Outcome Measures: Resting energy expenditure measured in the fast ing state by indirect calorimetry, body composition by dual-energy X-ray absor ptiometry, cardiovascular disease risk factors, and self-reported hunger. Resul ts: Resting energy expenditure decreased less with the low-glycemic load diet t han with the low-fat diet, expressed in absolute terms(mean[SE], 96 [24] vs 176 [27] kcal/d; P=.04) or as a proportion(5.9%[1.5%] vs 10.6%[1.7%]; P=.05). Pa rticipants receiving the low-glycemic load diet reported less hunger than those receiving the low-fat diet(P=.04). Insulin resistance(P=.01), serum triglyceri des(P=.01), C-reac-tive protein(P=.03), and blood pressure(P=.07 for b.th syst olic and diastolic) improved more with the low-gly-cemic load diet. Changes in body composition (fat and lean mass) in b.th groups were very similar(P=.85 and P=.45, respectively). Conclusions: Changes in dietary composition within prevai ling norms can affect physiological adaptations that defend body weight. Reducti on in glycemic load may aid in the prevention or treatment of obesity, cardiovas cular disease, and diabetes mellitus.

Prolactin (PRL) is a pleiotropic hormone associated with the progression of various cancers, including colorectal cancer (CRC). Here we investigate whether the association of serum PRL concentration and survival is affected by tumor location and preoperative radiotherapy (PRERT) in patients with CRC cancer. Serum PRL was determined in 82 CRC patients without previous treatment. Patients with PRL concentrations at and above the 75th percentile (high PRL) or below this level (low PRL), had a significant correlation with overall survival determined using the Kaplan-Meier method. In colon cancer, there was an increased risk of mortality when PRL values were at and above the highest quartile (22%vs. 73%; P = 0.01). In contrast, in rectal cancer, high PRL values were associated with a significant overall survival advantage (88%vs. 44%; P = 0.05), which became more significant (100%vs. 34%; P = 0.005) when only rectal cancer patients receiving PRERT were compared. these findings suggest that tumor location and adjuvant radiotherapy influence the association between circulating PRL and survival in CRC.

Aims: In children with convulsive status epilepticus (CSE)with fever, to deter mine the likelihood of acute bacterial meningitis (AbM), the proportion that are treated with antibiotics, and the proportion that have diognostic CSF sampling. Methods: Patients with an incident episode of CSE with fever were identified as part of an ongoing prospective population based study of CSE in childhood. Resu lts: there were 49 incident cases of CSE in the first six months. Ascertainment was 96%. Twenty four had CSE with fever, 16 had early parenteral antibiotics, n ine had diagnostic CSF sampling, and four had AbM. the population risk of AbM in CSE with fever was significantly higher than that of short seizures with fever (17%v 1.2%). Conclusions: the classical symptoms and signs of AbM may be abse nt in CSE with fever. A high index of suspicion for AbM in the child with CSE wi th fever is paramount. the most appropriate management is suggested to be early parenteral antibiotics and a lumbar puncture when there are no contraindications .

Objective: To determine if exposure to more than one course of antenatal gluco cortlcoids is associated with changes in infant blood pressure and myocardial wa ll thickness in the first month after birth. Design: Prospective cohort study. S etting: Tertiary neonatal intensive care unit. Participants: Mothers who were el igible for but declined to enter a randomised trial of repeated doses of antenat al glucocorticoids (ACTORDS)-that is, who had a singleton, twin, or triplet pre gnancy at be at continued risk of preterm birth. Main outcome measures: blood pressure daily for the first week th en weekly until 4 weeks of age. End diastolic interventricular septal and left v entricular posterior wall (EDIVS and EDLVPW) thickness at 48-72 hours after bir th. Results: thirty seven women were enrolled and delivered 50 infants. thirty m others (39 infants) were exposed to one course of glucocorticoids, and seven mot hers (11 infants) to more than one course. blood pressures were higher in the fi rst week after birth in infants exposed to multiple courses of glucocorticoids, and in infants with a latency between last exposure and delivery of less than se ven days. Systolic blood pressure on day 1 was > 2SD above published normal rang es in 67%of babies exposed to multiple courses and 24%of babies exposed to a s ingle course of glucocorticoids (p = 0.04). there was no difference between grou ps in thickness of the EDIVS or EDLVPW. However, 44/50 (88%) babies had EDIVS a nd 49/50 (98%) babies had EDLVPW thickness > 2 SD above the expected mean for b irth weight and gestation. EDIVS but not EDLVPW thickness increased with increas ing latency (mean 0.02 mm/day, P=0.03). Conclusion: Future randomised trials sho uld assess the long term effects of exposure to antenatal glucocorticoids, particularly multiple courses, on the cardiovascular status of the infan
