AIM： To study the expression of endothelial and inducible nitric oxide synthases （enOS and inOS） and their role in inflammatory bowel disease （IbD）. METHODS： We examined the effect of sera obtained from patients with active Crohn＇s disease （CD） and ulcerative colitis （UC） on the function and viability of human umbilical vein endothelial cells （HUVEC）. HUVECs were cultured for 0-48 h in the presence of a medium containing pooled serum of healthy controls, or serum from patients with active CD or UC. Expression of enOS and inOS was visualized by immunofluorescence, and quantified by the densitometry of Western blots. Proliferation activity was assessed by computerized image analyses of Ki-67 immunoreactive cells, and also tested in the presence of the nOS inhibitor, 10^-4 mol/L L-nAME. Apoptosis and necrosis was examined by the annexin-V-biotin method and by propidium iodide staining, respectively. RESULTS： In HUVEC immediately after exposure to UC, serum enOS was markedly induced, reaching a peak at 12 h. In contrast, a decrease in enOS was observed after incubation with CD sera and the enOS level was minimal at 20 h compared to control （18%±16% vs 23%± 15% P〈0.01）. UC or CD serum caused a significant increase in inOS compared to control （UC： 300%±21%; CD： 275% ± 27% vs 108% ± 14%, P〈0.01）. Apoptosis/necrosis characteristics did not differ significantly in either experiment. Increased proliferation activity was detected in the presence of CD serum or after treatment with L-nAME. Cultures showed tube-like formations after 24 h treatment with CD serum. COnCLUSIOn： IbD sera evoked changes in the ratio of enOS/inOS, whereas did not influence the viability of HUVEC. These involved down-regulation of enOS and up-regulation of inOS simultaneously, leading to increased proliferation activity and possibly a reduced antiinflammatory protection of endothelial cells.

Shear bands in metallic materials have been reported to be catastrophic because they normally lead to non-uniform plastic deformation. Ductility of laminated metallic composites deteriorates with increasing processing strain, particularly for those having hexagonal-close-packed(hcp) constituents due to inadequate slip systems and consequently prominent shear banding. Here, we propose a design strategy that counterintuitively tolerates the bands with localized strains, i.e. the shear banded laminar(SbL) structure, which promotes dislocation activation in hcp metals and renders unprecedented strengthductility combination in hcp-metal-based composites fabricated by accumulative roll bonding(ARb). The SbL structure is characterized with one soft hcp metal constrained by adjacent hard metal in which dislocations have been accumulated near the bimetal interfaces. High-energy X-ray diffraction astonishingly reveals that more than 90% of dislocations are non-basal in Ti layers of the SbL Ti/nb composite processed by eight ARb cycles. Moreover, dislocations occupy a high fraction of ~30%, promoting further cross slip. The unique stress field tailored by both shear banding and heterophase interface-mediated deformation accommodation triggers important slip. This SbL design is of significance for developing hcp-based laminates and other heterostructured materials with high performances.

Prophylactic transcatheter arterial embolization reduces rebleeding in non-variceal upper gastrointestinal bleeding: A meta-analysis

bACKGROUnD Despite the improvement in the endoscopic hemostasis of non-variceal upper gastrointestinal bleeding(nVUGIb),rebleeding remains a major *** To assess the role of prophylactic transcatheter arterial embolization(PTAE)added to successful hemostatic treatment among nVUGIb *** We searched three databases from inception through October 19th,*** controlled trials(RCTs)and observational cohort studies were *** compared patients with nVUGIb receiving PTAE to those who did not get *** outcomes were rebleeding,mortality,reintervention,need for surgery and transfusion,length of hospital(LOH),and intensive care unit(ICU)*** the quantitative synthesis,odds ratios(ORs)and weighted mean differences(WMDs)were calculated with the random-effects model and interpreted with 95%confidence intervals(CIs).RESULTS We included a total of 3 RCTs and 9 observational studies with a total of 1329 patients,with 486 in the intervention *** was associated with lower odds of rebleeding(OR=0.48,95%CI:0.29–0.78).There was no difference in the 30-d mortality rates(OR=0.82,95%CI:0.39–1.72)between the PTAE and control *** who underwent PTAE treatment had a lower chance for reintervention(OR=0.48,95%CI:0.31–0.76)or rescue surgery(OR=0.35,95%CI:0.14–0.92).The LOH and ICU stay was shorter in the PTAE group,but the difference was non-significant[WMD=-3.77,95%CI:(-8.00)–0.45;WMD=-1.33,95%CI:(-2.84)–0.18,respectively].COnCLUSIOn PTAE is associated with lower odds of rebleeding and any reintervention in ***,further RCTs are needed to have a higher level of evidence.

Increased duodenal expression of mi R-146a and-155 in pediatric Crohn's disease

AIM: To evaluate the role of microRnA (miR)-146a, -155 and -122 in the duodenal mucosa of pediatric patients with Crohn’s disease (CD) and the effect of transforming growth factor-β (TGF-β) on these miRs in duodenal epithelial and fibroblast ***: Formalin-fixed, paraffin-embedded biopsies derived from the macroscopically inflamed (CD inflamed: n = 10) and intact (CD intact: n = 10) duodenal mucosa of pediatric CD patients and control children (C: n = 10) were examined. Expression of miR-146a, -155 and -122 was determined by real-time polymerase-chain reaction (PCR). The expression of the above miRs was investigated in recombinant human TGF-β (1 nmol/L, 24 h) or vehicle treated small intestinal epithelial cells (CCL-241) and primary duodenal fibroblast cells derived from healthy children as ***: Expression of miR-146a was significantly higher in the inflamed duodenal mucosa compared to the intact duodenal mucosa of children with CD (CD inflamed: 3.21 ± 0.50 vs CD intact: 0.62 ± 0.26, P ≤ 0.01) and to the control group (CD inflamed: 3.21 ± 0.50 vs C: 1.00 ± 0.33, P ≤ 0.05). The expression of miR-155 was significantly increased in the inflamed region of the duodenum compared to the control group (CD inflamed: 4.87 ± 1.02 vs Control: 1.00 ± 0.40, P ≤ 0.001). The expression of miR-122 was unchanged in the inflamed or intact mucosa of CD patients compared to controls. Chronic kidney disease severely deteriorates the outcome of gastrointestinal bleeding: A meta-analysis

AIM To understand the influence of chronic kidney disease(CKD) on mortality, need for transfusion and rebleeding in gastrointestinal(GI) bleeding *** A systematic search was conducted in three databases for studies on GI bleeding patients with CKD or endstage renal disease(ESRD) with data on outcomes of mortality, transfusion requirement, rebleeding rate and length of hospitalization(LOH). Calculations were performed with Comprehensive Meta-Analysis software using the random effects model. Heterogeneity was tested by using Cochrane's Q and I2 statistics. Mean difference(MD) and OR(odds ratio) were ***1063 articles(EMbASE: 589; PubM ed: 459; Cochrane: 15) were found in total. 5 retrospective articles and 1 prospective study were available for analysis. These 6 articles contained data on 406035 patients, of whom 51315 had impaired renal function. The analysis showed a higher mortality in the CKD group(OR = 1.786, 95%CI: 1.689-1.888, P nd the ESRD group(OR = 2.530, 95%CI: 1.386-4.616, P = 0.002), and a rebleeding rate(OR = 2.510, 95%CI: 1.521-4.144, P n patients with impaired renal function. CKD patients required more unit red blood cell transfusion(MD = 1.863, 95%CI: 0.812-2.915, P nd spent more time in hospital(MD = 13.245, 95%CI: 6.886-19.623, P n the *** ESRD increases mortality, need for transfusion, rebleeding rate and LOH among GI bleeding patients. Prospective patient registries and observational clinical trials are crucially needed.
