The role of psychological factors or symptom pattern for the response to treatment in patients with unexplained (functional)dyspepsia is unknown. We hypothesized that patients with reflux-and ulcerlike symptoms would be more likely to respond to acid-lowering therapy, while psychological disturbances would be associated with a less favorable response to treatment. Seventy-eight patients with a diagnosis of functional dyspepsia were recruited and 75 completed the trial. Patients were treated for 4 weeks in a double-blind, placebo-controlled crossover trial starting in random order with either active drug(ranitidine, 150 mg b.d.) or placebo. Every 7 days, medication was switched from active drug to placebo, or vice versa. At entry, patient characteristics were assessed utilizing a semistructured standardized interview and standardized questionnaires,and weekly intensity of symptoms was assessed utilizing a visual analogue scale. Patients with a greater reduction of the symptom score during active treatment and an overall reduction of the global symptom score by more than 50% at the end of the study period were categorized as responders. Logistic regression analysis was utilized to assess the influence of symptom type and presence of psychological disturbances on the treatment response. During treatment the symptom score decreased significantly, from 32.1 ± 1.44 (SD) to 21.3 ± 1.9 at the end of the trial (P ble response (OR, 0.3; 95% Cl,0.1-0.9). Symptom pattern and psychological disturbances are independent predictors of treatment response. Patients with reflux-like symptoms and greater psychological disturbances are more likely to respond to an acid-lowering compound.

Purpose:High-resolution biometry of the anterior ocular segment is now becoming more and more important against a background of refractive surgery and the evaluation of potentially accommodative lens replacement *** aim of this study was a systematic investigation of the currently available non-contact methods for measuring the anterior chamber depth(ACD).Methods:The ACDs of 50 phakic eyes of 27 patients aged between 19 and 59 years were measured with the IOL-Master(Zeiss),the AC-Master(Zeiss),the Pentacam(Oculus)and slit-lamp pachymetry by Jaeger(haag-Streit).Results:The median anterior chamber depth in the investigated eyes was 3.63 mm for the IOL-Master(minimum 2.88 mm,maximum 4.22 mm),3.802 mm for the AC-Master(2.816 mm-4.373 mm),3.915 mm for the Pentacam(minimum 2.994 mm,maximum 4.614 mm)and 3.75 mm for Jaeger(2.887 mm-4.29 mm).With a probability of error of α =0.05 there were no significant differences concerning the ACD between the methods of Jaeger and AC-Master,Jaeger and IOL-Master,or Pentacam and AC-Master(Wilcoxon and Wilcox).The intra-individual variability was ± 5.4 μ m for AC-Master,± 12.7 μ m for Pentacam,± 24.5μ m for IOL-Master and± 41.2 μ m for *** maximum method-dependent difference in ACD determination was 285 μ ***:All the methods allow non-contact biometry,but the results might differ due to measuring principles inherent to the system,experience of the examiner and compliance of the *** coherence interferometry with the AC-Master offers the advantage of measurement exactly along the optical axis with the highest reproducibility and patient compliance.

治疗药物监测(Therapeutic Drug Monitoring,TDM)通过定量测定和解释血药浓度以优化药物治疗。TDM着眼于药代动力学的个体差异,使个体化药物治疗成为可能。在精神病学和神经病学领域中,有可能明显获益于TDM的主要患者群体包括少年儿童、孕妇、老年患者、智障患者、药物滥用者、涉法精神病患者、已知或怀疑药代动力学异常的患者以及合并躯体疾病影响药代动力学的患者。常规剂量下治疗无效,用药依从性难以判断,药物耐受性不佳,以及可能存在药代动力学方面的药物-药物相互作用等情况都是治疗药物监测的典型指征。然而,只有将TDM充分整合到临床治疗过程中,才能发挥其优化药物治疗的潜在优势。为了向临床医生和实验室提供有效的TDM信息,神经精神药理学与药物精神病学协会(Arbeitsgemeinschaft für Neuropsychopharmakologie und Pharmakopsychiatrie,AGNP)的TDM专家组在2004年发表了第一版《精神科治疗药物监测指南》。2011年进行了更新之后,现在再次更新。遵循新版指南,可能会改善神经精神药物治疗的效果,加快很多患者的康复,并降低医疗费用。

2.2剂量相关参考浓度范围对于TDM结果的解释,除了治疗参考浓度范围外还有另一个浓度范围,即所谓的剂量相关参考浓度范围。治疗参考浓度范围的应用是药效动力学问题,而剂量相关参考浓度范围的运用是药代动力学问题。后者比较的是测得的药物浓度与理论预期药物浓度范围。参照药代动力学研究,优先考虑没有合并用药或药物基因组异常的患者群(“正常”患者),在日维持剂量(Daily maintenance dose,D m)、给药间隔(Dosing interval,d i)、总清除率(Total clearance,CL)和生物利用度(bioavailability,F)已知情况下,某种药物在某个“正常”患者体内的预期平均稳态浓度(Average steady-state concentration,C av)可通过下面的公式计算。
