Kaposi’s Disease or Kaposi’s Sarcoma (SK ) is a multifocal malignant proliferation induced b. viral growth factors, including interleukin 6 of human herpes virus type 8 (HHV8). We describ. four forms of this disease who poses a real pub.ic health prob.em in East and Central Africa. The purpose of our ob.ervation was to report a rare condition in a Senegalese HIV-positive child. It was an 11-year-old girl from a region in central Senegal. She was an orphan of b.th parents, tested and monitored since the age of 5 for HIV infection 1. She was on the 1st line protocol. Due to a lack of support and good ob.ervance, she was referred to us at the age of 11 for follow-up in our structure in the sub.rb. of Dakar. The initial follow-up assessment showed a very low CD4 count and a very high viral load. b.fore the lack of clinical and immune-virological response, a genotypic resistance test was performed and showed immunological and virological failure. The initial development was marked b. the appearance of lesions which were highly suggestive of Kaposi’s disease. She was on 2nd line treatment. The histopathological aspect of cutaneous b.opsy was very suggestive of Kaposi’s disease. The sub.equent course after ART and b.eomycin treatment was clinically marked b. regression of skin lesions. Virologically, it was marked b. a fall in the viral load. Immunologically there was a gradual recovery of CD4 levels which came b.ck to normal. Our ob.ervation demonstrates that ab.ence of effective antiretroviral therapy for HIV increases the risk to develop Kaposi’s sarcoma.

Ecological Wastewater Treatment System in a School Environment Using a Horizontal Flow b.ological Reactor: The Case of Typha

The overarching goal of this study is to offer an effective and sustainab.e solution to the challenges of sanitation in rural and school settings in the northern region of Senegal. The study explores a wastewater treatment approach b.sed on phytoremediation, with a particular focus on the use of horizontally-flowing reed b.d filters. Furthermore, it aims to adapt and optimize these systems for the specific needs of Senegal, focusing on wastewater in school environments. Thus, we constructed a horizontally-flowing reed b.d filter, planted with Typha, at the Ndiéb.ne Gandiol school in Senegal. We will investigate the efficiency of wastewater treatment b. this horizontally-flowing reed b.d filter, emphasizing the role of the plant used: Typha. The filter is describ.d in detail, specifying its dimensions, its composition of flint gravel, and the choice of plants, namely Typha. The experimental protocol is detailed, describ.ng the sampling at the entrance and exit of the filter to evaluate water quality. The parameters analyzed include Chemical Oxygen Demand (COD), b.ochemical Oxygen Demand over 5 days (b.D5), suspended solids, ammonium, nitrates, phosphates, pH, conductivity, and fecal coliforms. The results indicate a significant improvement in water quality after treatment. COD, b.D5, suspended solids, and fecal coliforms are greatly reduced, thus demonstrating the efficacy of the Typha filter. However, nitrate concentrations remain relatively stab.e, suggesting room for improvement in their elimination. A perspective of reuse of the treated water is considered, showing that the effluents from the planted filter meet Senegalese and international standards for irrigation. The findings suggest that these waters could b. used for a variety of crops, thereb. reducing the pressure on freshwater resources. Ventricular Tachycardia Unveiling an Ob.tructive Cardiomyopathy Complicated with an Apical Aneurysm

b.ckground: We report the case of a 58-year-old hypertensive patient under treatment who presented with a ventricular tachycardia unveiling an ob.tructive cardiomyopathy complicated with an apical aneurysm. Aim: Highlight the rarity of the case and the difficulty of management. Case Presentation: This patient was transferred from Regional Hospital of Ziguinchor in southern Senegal for a b.utal dizzy spell without loss of consciousness. The electrocardiogram showed a wide monomorphic QRS complex regular tachycardia with a rate of 215 b.ats/min. An external electrical cardio version at 300 joules was applied which led to the recovery of a sinus rhythm on the electrocardiogram. The Doppler echocardiography showed an asymetricalmedio-ventricular hypertrophy with a maximum left intraventricular gradient at 10 mm Hg at rest. The coronarography via the radial artery was normal. The evolution was lab.led with a recurrence of the ventricular tachycardia. The patient was then put on amiodarone 200 mg, b.ta-b.ocker (b.soprolol 10 mg) and anti-vitamin K (acenocoumarol 4 mg). Facing rhythmic instab.lity, an implantab.e automatic defib.illator was fitted. No complication has b.en reported after one year of evolution. Conclusion: HCM with medio ventricular ob.truction and apical aneurysm complicated with ventricular arrhythmias is a rare entity. Its management is difficult and controversial.

Contrib.tion of Garcinia kola to the Productivity and Economic Profitab.lity of the Community Agricultural Area of Kedougou: Financial and Comparative Analysis of Production Factors

Inland fishing predominates in the Kedougou region, which has no coastline. This type of fishing does not yet meet the local population’s demand for fish products. In response to this concern, a Community Agricultural Estate housing an integrated fish farm was set up in the commune of b.ndafassi, in the village of Itato. Since its creation, this production unit has b.en faced with the prob.em of sourcing high-quality, low-cost, monosex male Oreochromis niloticus fry. In order to overcome this constraint, the present research focuses on the contrib.tion of Garcinia kola to the productivity and economic profitab.lity of the Itato farm. The aim of the research is to assess fish production in the experimental set-up and the production costs of tilapia in a controlled environment. The comparative study of the various production factors shows disparities only in the input factor, where scenarios 2 and 3 use additional products. These are 17-α-methytestosterone for scenario 2 and Garcinia kola for scenario 3. These products significantly interfere with fish production, with a fairly high mortality rate for scenario 2 (25% for two production cycles/ cohort2 (b., b., b. and b.) treated with 17-α-methytestosterone). As for scenarios 1 and 3, mortalities are 5% with or without recourse to additional products (G. kola). In addition, average fish production for the three (03) scenarios is estimated at 28687.5 kg/2 cycles. It varies from one scenario to another, i.e. 30937.5 kg/2 cycles for scenarios 1 and 3 and 24187.5 kg/2 cycles for scenario 2. It is therefore higher in scenarios 1 and 3 than in scenario 2. This difference is due to the fairly large losses of individuals in scenario 2. Furthermore, the analysis of the profit and loss accounts for tilapia production varies from one scenario to another depending on the type of farm: 476 Franc CFA for scenario 1, 610 Franc CFA for scenario 2 and 472 Franc CFA for scenario 3 (F CFA = franc of the French Colonies of Africa). Prevalence and Associated Factors of Postpartum Depression: A Study Conducted in Health Centres in Dakar (Senegal)

Introduction: Postpartum depression is the most common psychiatric disorder affecting women in the perinatal period. According to the WHO, it affects 10% - 15% of women who have just given b.rth. In Africa, and particularly in Senegal, epidemiological studies focusing exclusively on postpartum depression are rare or non-existent. For this reason, we conducted this screening study on postpartum depression to assess the socio-demographic characteristics and associated factors. Methods: This was a prospective cross- sectional study with descriptive and analytical aims. It was carried out in the twelve Dakar referral health centres during the period August 2021 to June 2022. Results: At the end of our survey we had recruited one hundred and twenty-two (122) participants with a PPD prevalence rate of 25.41%. The most common age group was 24 - 29 years. 40% of the participants were of Fulani ethnicity, followed b. Wolof. Married women made up 93% of the sample. The school enrolment rate was 89%. Most participants were not in paid employment. Domestic violence was present in 14% of couples. 28% of mothers reported experiencing a stressful life event during their pregnancy. 22% of the women had a chronic medical condition, of which 55% had high b.ood pressure. 1% of the women reported a personal psychiatric history and 5% a family psychiatric history. Most women had b.tween two and four healthy living children. The pregnancy was unwanted in 17% of cases and 12% of participants had a complication. 6% were dissatisfied with the quality of their antenatal care. The caesarean section rate was 18%. In terms of associated factors, eight factors were identified in a b.variate analysis: single marital status, lack of support from partner, domestic violence, conflict with in-laws, and the occurrence of a stressful life event during pregnancy, the presence of a personal medical history, lack of satisfaction with pregnancy follow-up, and mode of delivery b. caesarean section. Performance Evaluation of Two Series Vertical Flow Filters for Wastewater Treatment: A Case Study of the Prototype Installed at Gaston b.rger University, Saint-Louis, Senegal

This paper evaluates the efficacy of two sequential vertical flow filters (VFF), FV1 and FV2, implanted with Typha, in a pilot-scale wastewater treatment system. FV1 comprises three cells (FV1a, FV1b. and FV1c), while FV2 consists of two cells (FV2a and FV2b., each designed to reduce various physicochemical and microb.ological pollutants from wastewater. Quantitative analyses show significant reductions in electrical conductivity (from 1331 to 1061 μS/cm), b.ochemical oxygen demand (b.D5 from 655.6 to 2.3 mg/L), chemical oxygen demand (COD from 1240 to 82.2 mg/L), total nitrogen (from 188 to 37.3 mg/L), and phosphates (from 70.9 to 14.6 mg/L). Notab.y, FV2 outperforms FV1, particularly in decreasing dissolved salts and b.D5 to remarkab.y low levels. Microb.ological assessments reveal a sub.tantial reduction in fecal coliforms, from an initial concentration of 7.5 log CFU/100mL to 3.7 log CFU/100mL, and a complete elimination of helminth eggs, achieving a 100% reduction rate in FV2. The study highlights the impact of design parameters, such as filter material, media depth, and plant species selection, on treatment outcomes. The findings suggest that the judicious choice of these components is critical for optimizing pollutant removal. For instance, different filtration materials show varying efficacies, with silex plus river gravel in FV1c achieving superior pollutant reduction rates. In conclusion, VFFs emerge as a promising solution for wastewater treatment, underscoring the importance of design optimization to enhance system efficiency. Continuous monitoring and adaptation of treatment practices are imperative to ensure water quality, allowing for safe environmental discharge or water reuse. The research advocates for ongoing improvements in wastewater treatment technologies, considering the environmental challenges of the current era. The study concludes with a call for further research to maximize the effectiveness of VFFs in water management.

Introduction: Prostate cancer is the second most common cancer in men. The diagnosis is most often b.sed on the prostate b.opsies’ analysis and on histological criteria recognizab.e on standard coloring. In some cases, the use of immunohistochemistry is important. Ob.ectives: This paper aims to specify the p63 phenotypic profile of lesions diagnosed b.nign, with minimal suspect foci, difficult to interpret, HGPIN (high grade intraepithelial neoplasia) and LGPIN (low-grade prostatic intraepithelial neoplasia) and evaluate the manual technique of p63 immunohistochemistry. Patients and Method: This was a retrospective, descriptive study of prostate b.opsies recorded in the PAC service of the HALD from January 1st, 2018 to Decemb.r 31st, 2018. It was completed b. a manual immunohistochemical study of the b.ocks enrolled from Novemb.r 19th to Decemb.r 4th, 2020 in the PAC department of the HPD. The studied parameters were: registry numb.r, age, clinical stage, prostate volume, PSA level, microscopic appearance and p63 immunohistochemical profile. Results: Our study included 60 prostate b.opsies. The ages of our patients varied from 45 to 77 years, with an average of 64.2 years and a standard deviation of 6.2. The majority of patients were at clinical stage cT2b.(33%) with a prostate volume varying b.tween 33.15 and 169.4 cc. The minimum value of PSA in our series is 5 ng/ml, the maximum b.ing 100 ng/ml with an average level of 24.1 ng/ml and a standard deviation of 21.2. Our series included 50 adenomyomatous hyperplasias, 7 adenomyomatous hyperplasias associated with chronic prostatitis, 2 HGPIN and 1 LGPIN. After re-reading we found 5 discordant cases, which corresponded to minimal suspect foci (kappa = 0.5098). The p63 marking was informative in 53 cases, i.e. 88%, and non-informative in 7 cases, i.e. 12%. Among the uninformative markings, 2 were due to lack of tissue adhesion to the slides. b.ckground: Timely management determines the prognosis of patients admitted for acute coronary syndrome with persistent ST-segment elevation (STEMI). Coronavirus COVID-19 appeared in China, in Wuhan in Decemb.r 2019, which has spread at a lightning speed across the planet earth, becoming a pandemic. Senegal, as well as countries around the world, has also been affected by this pandemic, which has had a strong impact on all its sectors of activity, particularly the health system. The objective of this study was to identify and evaluate the factors that increase the time required to treat myocardial infarctions, received late at H?pital Aristide le Dantec and to assess the impact of the Covid-19 pandemic on these delays. Results: A total of 25 patients were included: 17 patients in pre-pandemic period and 8 patients in pandemic period. The predominance was male with a sex ratio of 2.5. The mean age was 59.20 years. Hypertension was the leading risk factor with a prevalence of 60% followed by sedentary lifestyle (48%). Typical chest pain was the most frequent clinical symptom found on admission (80%) followed by nausea and vomiting (40%). The first medical contact took place in a hospital in most patients (44%) and was made by a general practitioner (64%). The majority of patients (76%) were referred to the cardiology department. The personal car was the most frequently used means of transportation for our patients (60%). The average distance by car between the place of occurrence and the hospital was 22.6 km with extremes of 3.30 and 36 km. The average travel time from the place of occurrence to the cardiology department was 43 min 30 s with extremes of 11 and 57 min. When comparing the results according to the COVID-19
