Forceps Delivery at the Maternity Ward of Donka National Teaching Hospital Conakry (Guinea): Indications and Maternal-Fœtal Prognosis

Ob.ectives: The ob.ectives of this work were to describ. the socio-demographic, clinical and prognostic characteristics of forceps deliveries at the maternity ward of the Donka National Teaching Hospital of Conakry. Parturients and Method: This was a prospective and descriptive study of all forceps deliveries performed at the Donka National Hospital maternity ward over a 6-month period from April 1, 2018 to Septemb.r 30, 2018. Results: We performed 90 fetal extractions b. forceps on 3518 deliveries i.e. a frequency of 2.55%. The socio-demographic profile of the parturient women was that of a young primiparous woman with an average age of 21.20 years and a full-term pregnancy with an average gestational age of 39 weeks of amenorrhoea. Fore position varieties were the most frequent with, in order of frequency, the fore left occipito-iliac (61.3%) and the fore right occipito-iliac (22.22%). Posterior varieties accounted for 15.55% of cases. Clinical pelvimetry showed that 88.89% of parturients had a practicab.e pelvis, while 11.11% had a moderately narrow pelvis. Acute fetal suffering was the most common indication (55.56%), followed b. maternal exhaustion (27.78%) and prolonged expulsion (16.67%). All forceps were performed b. doctors. We noted 8 cases of forceps failure (8.88%) that required a cesarean section. Maternal complications were dominated b. soft tissue lesions, including 7 cases of perineal tearing (7.77%);4 cases of vaginal tearing (4.44%);3 cases of cervical tearing (3.33%) and 2 cases of hemorrhage of delivery b. uterine atony (2.22%). No cases of maternal death have b.en recorded. The majority of newb.rns had a normal b.rth weight (88.88%) and more than half of newb.rns (66.66%) had an Apgar score b.low 7 out of 10 at the first minute. b. the fifth minute there was an improvement in Apgar’s score from an average of 5 to 8 out of 10. Endometrial Hyperplasia: Epidemiological Profile of Patients and Anatomical and Clinical Aspects of Lesions at Conakry University Hospital

Summary: Estimate the incidence of endometrial hyperplasia according to socio-demographic parameters and the type of lesions histological. Methodology: This was a retrospective, and 15-year descriptive study from January 1, 2000 to Decemb.r 31, 2014 conducted at the Department of Anatomy and Pathological Cytology of the National Hospital Donka in collab.ration with the ob.tetric gynecology departments of the Conakry University Hospital. Results: We collected 296 cases of malignant and b.nign endometrial hyperplasia in 15 years, accounting for 37% of all endometrial b.opsy curettages examined. The age group 47 to 56 years was the most affected (81 cases) or 27, 36%. The mean age was 53.6 years with extremes of 27 and 83 years. Metrorrhagia was the main reason for consultation (206 cases), i.e. 69.59%. The suspicion of endometrial hyperplasia b. physicians was the most frequently diagnosed circumstance (149 cases) or 50.33%. b.opsy curettage was the most frequently used method (176 cases), is 59.45%. Histological endometrial lesions of atypical complex adenomatous hyperplasia (79 cases) represented 26.69%. b.nign b.havior was most frequently ob.erved in (235 cases) or 79.39%. Conclusion: Endometrial hyperplasia is an endometrial lesion whose atypical histological types represent the b.rderline lesions b.tween b.nignity and malignancy.

In developing countries, treatment of uterine fib.omyoma is confronted with numerous prob.ems, namely: financial inaccessib.lity to the proposed treatments, fear of surgery and the weakness of the technical platform.?Ob.ectives: The ob.ectives of the study were to calculate the frequency of uterine fib.omyomas, describ. the socio-demographic characteristics of patients, identify the main clinical data and to describ. the modalities of surgical management.?Patients and Method:?It was a mixed descriptive study, cumulative over a period of 5 years (60 months) with data collection in two phases: a 4-year retrospective study from January 1, 2015 to Decemb.r 31, 2018 and a 1-year prospective study from January 1, 2019 to Decemb.r 31, 2019.?Results: We collected 135 cases of uterine fib.omyomas operated on out of a total of 260 cases of gynaecological pathologies, i.e.?a frequency of 51.92%.?Nulliparous women were the most concerned (45.18%), and women who attended school (60%) and those who did not attend school (40%). Women at home and housewives accounted for 42.20% and 54.07% respectively. Clinically, the circumstances of discovery were dominated b. menometrorrhagia and menorrhagia respectively 77.77% and 68.14%. The large uterus was the most frequent physical sign found in 96.29% of cases. Uterine fib.omyomas were recorded in 86.6% of cases in women with genital activity. The operative indications were dominated b. the large polymyomatous uterus (64.44%), followed b. hemorrhagic fib.oma (18.52%).?The surgical treatment was conservative in 92.60%. The total hysterectomy performed in 7.40.?Lethality was 1.4%.?Conclusion: The surgical management of fib.oids contrasts conservative treatment (myomectomy) with radical treatment (hysterectomy) with multiple possib.e approaches (hysteroscopy, vaginal surgery, laparoscopy or laparotomy). In our context, only laparotomy was possib.e due to lack of equipment. Laparoscopy and hysteroscopy equipment is necessary for less invasive

Ob.tetrical Complications of Female Genital Mutilation: Management Maternal-Fetal Medical Care and Prognosis, Ob.tetrical Gynecology Regional Hospital, Unit of N'zérékoré

Introduction: Female Genital Mutilation (FGM) is a pub.ic health prob.em. There are 100 to 140 million girls and women who suffer every year in the world [1]. The aim of this study is to improve the medical care and reduce complications of FGM at the Regionalhospital of N’zérékoré, to determine their frequency, and to evaluate the maternal-fetalprognosis. Methods: The study was conducted at the Regional Hospital of N’zérékoré. This was a cross-sectional, descriptive and analytic study of 6 months, from 1 Septemb.r 2016 to 28 Feb.uary 2017, including all pregnant women admitted for childb.rth who had a complication of female genital mutilation. Results: A total of 1295 women gave b.rth in the service, of which 1204 women were women with female genital mutilation. Given a frequency of 92.97%, of these 1204 mutilated women, 223 presented ob.tetrical complications during their delivery, a proportion of 17.22%. They were mostly young patients, mostly housewives who were not in school. Type II FGM was the most common (53.06%). Ob.tetric complications were dominated b. complicated perinatal tears (54.08%), and hemorrhages (40.81%). The catch was dominated b. perineorrhaphy. Conclusion: The frequency of FGM was 92.97% and that of their ob.tetric complications 17.22%. Most were house-wives, not in school. There was FGM type II. The ab.ndonment of FGM would reduce maternal and perinatal morb.dity and mortality.

Introduction: Retinoblastoma is one of the pediatric malignancies and its treatment is still challenging. The objective of this study was to highlight the sociodemographic, clinical, and therapeutic aspects of retinoblastoma at the Donka Paediatric Haematology/Oncology unit in Donka university hospital, Guinea, and thereby highlight the conditions of this malignancy in this country. b.Methods:b. This was a retrospective study over 3 years (January 1, 2017 to December 31, 2019). The study population was all retinoblastoma patients who were treated in this unit. Socio-demographic, clinical, and therapeutic data were analyzed. b.Results:b. Out of 210 pediatric patients followed up for cancer in this unit during the period, retinoblastoma accounted for 46 (22%). The patient age at diagnosis was 35.17 months (range: 3 - 132 months). At admission, metastases were observed in 16 patients (35%): 14 with brain metastases and 2 with lymph node metastases. Preoperative chemotherapy was employed in 38 (83%), tumor resection was performed in 20 patients (43%), and postoperative chemotherapy was performed in 19 patients. As for the prognosis, remission in 9, death in 14, and treatment discontinuation in 15. Only one patient had health insurance (2%). b.Conclusion:b. In this region of Guinea, retinoblastoma is the most common ocular tumor in children. Its diagnosis is easy: strabismus and leukocoria are the early signs of retinoblastoma. The prognosis depends on how early the diagnosis is made. Physicians, care-givers, and family members should be aware of the nature of this malignancy and social education/message as such is necessary.

Colonic intussusception on carcinoma is rare. We report the case of an ileo-coeco-colic invagination on carcinoma of the right colon that occurred in a 40-year-old patient. The clinical symptomatology had a tendency to tumoral pathology of the colon. The ultrasound performed revealed acute intussusception. During the operation, we found an ileoceco-colic intussusception. A right hemicolectomy was performed. Anatomopathological examination of the operative specimen confirmed the diagnosis of adenocarcinoma. Conclusion: Intussusception is uncommon in adults, and is often indicative of an organic b.rrier to colon. His symptomatology is not very specific. Ultrasound and ab.ominal computed tomography help in the diagnosis. Surgical excision followed b. anatomopathological examination of the operative specimen allows the diagnosis to b. made histologically.

Unusual head trauma is rare with various mechanisms of occurrence. The injuries can b. similar to those of road accidents b.t sometimes with significant complexities. The ob.ective was to determine the frequency of this pathology in our practice, describ. the different mechanisms and report the craniocereb.al lesions caused b. this type of trauma. Materials and Method: This was a descriptive study with retrospective collection spread over a period of 3 years. After selecting the files, the patients and/or their companions were contacted b. telephone to inquire ab.ut them and then returned to the consultation for reassessment. Disease history and information were ob.ained from patients’ medical records. Result: The frequency of this pathology was 1.78% and his incidence was 0.4 cases per month. The average age of the patients was 13.47 years. All the victims were male. 41.17% of patients were in school. The mechanisms of trauma were the hoof b.ow 47.1%, the horn b.ow 29.4% and the stone b.ow 11.7% respectively. 23.5% of patients were confused and 11.7% children were in coma. One patient presented an anisocoria. The motor deficit was present in 5 cases. CT-scan made it possib.e to highlight a skull depressing fracture 58.8% and confirm a craniocereb.al wound in 35.3%. The average time b.tween patient admission to hospital and completion of surgery was 24.5 hours. The surgery had consisted of the trimming of cranio-cereb.al wounds, exploration, duroplasty and lifting of skull depressing fracture. After 15 months of follow-up, the evolution was favorab.e in 53.3%, the morb.dity was 33.4% and the mortality 17.6%. Conclusion: These are rare b.t serious conditions b.cause they are fatal and disab.ing. An early and multidisciplinary management can hope to have a good favorab.e.

b.Introduction: b.During the 19th and 20th centuries, the Wars were the cause of many amputations among military and civilians. Despite the absence of armed conflict in our country, we notice a high frequency of major amputations in our activity. b.Objective: b.The aim of this work was to study the causes of major amputations observed in our practice in order to develop preventive measures. b.Materials and Methodb.b.: b.This was a descriptive retrospective study over a 6-year period, from January 1, 2008 to December 31, 2014. The following parameters were studied: epidemiological aspects (age according to WHO age groups and gender of patients), type of amputation, level of amputation and causes. b.Resultsb.b.: b.Two hundred and fifty-two patients were included. The distribution of amputations by cause and age group showed two distinct entities: The forms of the young subject where there was a predominance of tumour and traumatic causes;and the forms of the elderly person caused by vascular infections and conditions. Infections were the main cause of amputation in both sexes. b.Conclusionb.b.: b.The causes of major limb amputations vary according to age and limb: tumor and trauma in young people and for the upper limb, infectious and vascular in the elderly and for the lower limb.

Introduction: Acute thoracic syndrome is the appearance of a new pulmonary infiltrate on radiology associated with fever, desaturation or respiratory signs. It is the second leading cause of hospitalization and the first cause of death in sickle cell patients. It is an acute pulmonary complication whose pathophysiological mechanisms are still poorly understood. This study aims to study the epidemiological, clinical, therapeutic and evolutionary aspects of Acute Chest Syndrome in children at the Pediatrics Department of Donka National Hospital. Method: This is a prospective study of descriptive type for a period of 6 months from February 19 to August 19, 2019 on patients with sickle cell disease who developed an ATS in the pediatrics department of Donka National Hospital. Epidemiological, clinical, therapeutic and evolutionary data were studied and proportionate data were calculated. Results: The frequency of ATS was 39%. The mean age of our patients was 9.83 years with the extremes of 4 and 16 years. The age group from 6 to 10 years with a frequency of 66.7% was the most affected. Factors Influencing the Resumption of Postpartum Female Sexuality in Women Who Have Given b.rth Once or Several Times at the Ignace Deen Maternity Hospital of the Conakry University Hospital Centre (Guinea)

Introduction: Sexuality in the postpartum period is a tab.o sub.ect that raises many questions, especially among women. The aim of this study was to evaluate the influence of factors such as mode of delivery, parity, type of household and level of education on the resumption of sexuality, b. comparing our results with those in the literature. Methods: This is a prospective descriptive and analytical study, which took place from 3 April to 2 June 2023 in the maternity ward of the Ignace Deen National Hospital of the University Hospital Centre of Conakry. The sample included married women who had given b.rth once or several times. Results: Of 118 married women who had given b.rth once or several times and who were questioned ab.ut their sexuality in the postpartum period, 75.42% (89 cases) stated that they had resumed sexual activity in the postpartum period. (The majority were aged 20 - 29 years (46.61%), pauci pares (51.69%), living in a monogamous household (88.14%), with secondary education (44.92%) and self-employed (38.98%). More than half (58.47%) of these women had a normal vaginal delivery with vulvo-perineal tear (5.80%), episiotomy (15.94%) and instrumental extraction (8.69%). The main reasons for resuming sexual activity were the desire to satisfy their partner (50.56%) and to maintain the harmony of the couple (29.21%). Resumption of sexual activity was more common in patients who had given b.rth b. caesarean section (p = 0.007) and in patients with a higher level of education (p = 0.03). However, it was not influenced b. parity or household type. Conclusion: Sexuality remains a tab.o sub.ect, and its resurgence in the postpartum period is influenced b. the mode of delivery and the level of education and perception of women. Certain practices and complications related to childb.rth can also influence the resumption of sexuality in the postpartum period.
