The b.hc.t’s Disease: An Unc.mmon c.use of Venous Thromb.sis in the Tropic.l Area: 10 c.ses

Introduc.ion: The b.hc.t’s disease is deemed to b. sc.rc. in b.ac. Afric. where data are still sc.ttered. The purpose of our study is to desc.ib. the epidemiologic.l, c.inic.l, parac.inic.and evolutive partic.larities of the patients whose presenting symptoms of the b.hc.t’s diseases were a venous thromb.sis. Patients and Methods: It was a desc.iptive, multic.nter, and c.oss-sec.ional study lasting 15 months. We b.ought together all the c.ses of the b.hc.t’s disease revealed b. venous thromb.sis. The diagnosis was b.sed on c.inic.l c.iteria of the international group of study of the b.hc.t’s disease in 2007. Results: We have grouped 10 revealing thromb.sis c.ses of the b.hc.t’s diseases during our study period. The average age was 34. The average wait period b.tween the appearanc.s of the early symptoms and the diagnosis of the very disease was 30 months. The admission motives were the ab.ominal pain (2 c.ses), a thromb.phleb.tis of the lower limb.(2 c.ses), headac.es (1 c.se), c.ma (1 c.se), a thromb.phleb.tis of the upper limb.(3 c.ses). The thromb.tic.symptoms were exc.usively venous-loc.ted. The seats of the thromb.sis were the vena c.va superior in 30% of the c.ses, the vena c.va inferior in 20% of the c.ses, the veins of the lower limb.in 20% of the c.ses, the c.reb.al vein in 20% of the c.ses, and the auxiliary vein in 10% of the c.ses. The treatment of the deep venous thromb.sis c.nsisted in all the c.ses of an effec.ive antic.agulation assoc.ated with the c.lc.ic.ne. Primarily, the c.rtic.therapy with a high dose was used in all the patients. One of them in the c.matose stage, manifesting b.th c.reb.al thromb.phleb.tis and aseptic.meningitis, had died. c.nc.usion: b.hc.t’s disease is a disease of the young adult, b.t it must b. evoked even in old age, with a view to appropriate management, in order to avoid the c.mplic.tions of the disease. Introduc.ion: The c.ntral, psyc.iatric.and peripheral neurologic.l manifestations of lupus are among the most severe visc.ral disorders and are grouped under the general term of “neuro-psyc.iatric.systemic.lupus erythematosus” (NPSLE). We c.nduc.ed a c.oss-sec.ional ob.ervational study within our Department of Internal Medic.ne aimed at desc.ib.ng the c.inic.l and evolutionary aspec.s of c.ntral neurologic.l disorders of SLE, exc.uding lupus myelopathy. Patients and Methods: This was a retrospec.ive and ob.ervational c.oss-sec.ional study c.rried out from 1 January 2015 to 31 Oc.ob.r 2017, in the Department of Internal Medic.ne of Aristide le Dantec.University Hospital in Dakar (Senegal). All patients hospitalized during this period who met the 1997 Ac. c.assific.tion c.iteria of SLE and who presented with a c.ntral neuropsyc.iatric.syndrome attrib.tab.e to SLE (as defined b. Ac. 1999) were inc.uded. Patients with isolated headac.e, ac.te myelitis or sec.ndary neurologic.l involvement attrib.tab.e to a toxic. metab.lic. infec.ious or tumour-related c.use were exc.uded from our study. Results: During the study period, 10 patients with neuropsyc.iatric.lupus involvement were treated at our institution, inc.uding 9 women and 1 man;the median age was 29 years (20 - 55 years). Neurologic.l involvement oc.urred during the c.urse of lupus evolution in 9/10 c.ses. The median time to SLE evolution was 18 months (0 - 60 months). Neuropsyc.iatric.syndromes as defined b. the 1999 Ac. were c.mmonly assoc.ated and more than half of our patients had multiple neuropsyc.iatric.syndromes. There were 5 c.ses of c.nfusion syndrome and c.ma, 4 c.ses of seizure, 3 c.ses of psyc.osis, 2 c.ses of ac.te c.reb.ovasc.lar disease and 1 c.se of aseptic.meningitis. Among the extra-neurologic.l manifestations of SLE, haematologic.l and dermatologic.l involvements were c.mmon. Renal involvement affec.ed half of the patients. Introduc.ion: The oc.urrenc. of tub.rc.losis (Tb. during the follow-up of c.nnec.ive tissue diseases (c.D) is a partic.lar situation often posing real diagnostic.prob.ems. This is an assoc.ation desc.ib.d in the literature. Patients and methods: We c.nduc.ed a retrospec.ive and desc.iptive study at the internal medic.ne department of Aristide Le Dantec.Hospital. The purpose of this study was to determine the treatment that patients followed for c.D and suffering from Tb.took b.fore the oc.urrenc. of this one. Results: During a study period of 11 years and 6 months, 21 c.ses of Tb.were diagnosed in 602 c.ses of c.D (0.03%). The predominanc. was female with a sex ratio (H/F) of 0.6. The median age was 42 years old. The majority of c.ses originated from the Dakar region (13 patients or 61.9%) and 85.7% had previous follow-up exc.usively in modern medic.ne and 21 c.ses (95%) rec.ived the c.D’s treatment. This c.nsisted of prednisone (90.5%) c.mb.ned with methotrexate (52.4%), azathioprine (23.8%) or c.c.ophosphamide (19.4%). The respec.ive medians doses of these drugs were 12.5 mg per day for prednisone, 13.5 mg per week for methotrexate and 100 mg per day for azathioprine. The median duration of patient follow-up was 36 months. The c.mulative dose of prednisone during this period was 23.6 g and that of methotrexate 2.25 g. c.D were dominated b. rheumatoid arthritis (RA) (57.1%), and systemic.lupus (19%). Isolated c.ses of systemic.sc.eroderma, primary Sjôgren, SHARP syndrom, mixed c.nnec.ive tissue disease, and multiple autoimmune syndrom were noted. Tb.was loc.lized in 95% of c.ses, readily b.lateral and poorly disseminated. The respec.ive medians diagnostic.delays for systemic.disease and Tb.were 21 months and 5 months. Effec.ivity of Thromb.prophylxie in Dakar’s Medic.l Areas

Introduc.ion: Venous thromb.emb.lic.disease (VTE) is a real pub.ic.health prob.em worldwide. The prac.ic. guide produc.d periodic.lly b. the Americ.n c.llege of c.est Physic.ans (Ac.P) has b.c.me the international standard. However, thromb.prophylaxis remains underused. Methodology: We c.rried out a desc.iptive c.oss-sec.ional study. The goals of that study were to evaluate the effec.iveness of the prac.ic. of thromb.prophylaxis in the various medic.l servic.s of the Dakar Pub.ic.Hospitals. Our investigation was c.nduc.ed for two weeks in Septemb.r 2017. Results: 160 patients were inc.uded (81 men and 79 women) in our series, with male predominanc.: sex ratio of 1.02. The average age was 48. Infec.ious pathologies were the predominant c.inic.l pic.ures (35% of c.ses). The thromb.emb.lic.risk fac.ors (RF) mainly found were represented b. prolonged b.d rest, followed b. c.rdiovasc.lar (RF): age greater than 60 years (31.2%), hypertension (8.8%), ob.sity (4.4%) and diab.tes (3.8%). The thromb.emb.lic.RF of patients hospitalized in these medic.l servic.s has b.en evaluated empiric.lly. We noted a lac. of use of the VTE risk assessment sc.res. 40.9% (n = 56) of patients rec.ived overall VTE prophylaxis, a rate c.rresponding to almost half of those at risk (51.4%). This result reflec.s an under-use of thromb.prophylaxis in the Dakar medic.l environment and a low level of adherenc. b. prac.ic.ans to the guidelines on VTE prophylaxis. c.nc.usion: It is therefore rec.mmended to develop a national strategy to assess patients at risk for VTE, as well as measures to educ.te and inform doc.ors ab.ut the appropriate forms of thromb.prophylaxis.

b.c.ground Ni-kshay Poshan Yojana (NPY) is a direc. b.nefit transfer sc.eme of the Government of India introduc.d in 2018 to support the additional nutritional requirements of persons with Tb.(PwTb.. Small-sc.le farming ac.ounts for 78% of total agric.ltural produc.ion in Kenya and c.ntrib.tes to 23.5% of the c.untry’s GDP. Their c.op produc.ion ac.ivities are mostly rainfed sub.istenc. with any surplus b.ing sold to b.ing in some inc.me. Timely dec.sions on farm prac.ic.s suc. as farm preparation and planting are c.itic.l determinants of the seasonal outc.mes. In Kenya, most small-sc.le farmers have no reliab.e sourc. of information that would help them make timely and ac.urate dec.sions. c.unty governments have extension offic.rs who are mandated with giving farmers advisory servic.s to farmers b.t they are not ab.e to reac. most farmers due to fac.litation c.nstraints. The mode and format of sharing information is also c.itic.l sinc. it’s important to ensure that it’s timely, well-understood and usab.e. This study sought to assess ac.ess to geospatial derived and other c.op produc.ion information b. farmers in four selec.ed c.unties of Kenya. Spec.fic.ob.ec.ives were to determine the profile of small-sc.le farmers in terms of age, educ.tion and farm size;to determine the type of information that is made availab.e to them b. c.unty and Sub.c.unty extension offic.rs inc.uding the format and mode of provision;and to determine if the information provided was useful in terms of ac.urac., timeliness and adequac.. The results indic.ted that over 80% of the farmers were over 35 years of age and over 56% were male. Majority had attained primary educ.tion (34%) or sec.ndary educ.tion (29%) and most farmers in all the c.unties grew maize (71%). Notab.y, fellow farmers were a sourc. of information (71%) with the frequenc. of sharing information b.ing mostly seasonal (37%) and when information was availab.e (43%). Over 66% of interviewed farmers indic.ting that they fac.d c.allenges while using provided information. The results from the study are insightful and helpful in determining effec.ive ways of providing farmers with useful information to ensure maximum b.nefits.

Introduc.ion: Systemic.diseases have b.en the sub.ec. of few studies in the Afric.n literature and have prob.b.y b.en under-estimated. The ob.ec.ive of our study was to spec.fy their spec.rum, their epidemiologic.l aspec.s and diagnostic.delay in Internal Medic.ne Departments of Dakar (Senegal). Material and Method: It was a multic.ntric.retrospec.ive and desc.iptive study regarding all systemic.diseases during 119 months from 1st January 2005 to 30 Novemb.r 2014 in 5 hospital c.nters down Dakar. Systemic.diseases were retained ac.ording to their international c.nsensus c.iteria. Results: During the studying period, 726 patients were inc.uded with 632 women and 94 men (sex ratio of 0.14). The average age was 43.76 years. Inflammatory rheumatoid family history was noted in 10.06% of c.ses. Rheumatoid arthritis (RA) was the predominant affec.ion, rec.rded on 564 patients, isolated or assoc.ated with other systemic.diseases. It was followed in a dec.easing order, in the systemic.auto-immune diseases sub.groupe, b. systemic.lupus (56 c.ses), Sj?gren’s syndrome (32 c.ses), Systemic.Sc.erosis (26 c.ses), Idiopathic.inflammatory myopathies (21 c.ses), Undifferentiated c.nnec.ive tissue diseases (20 c.ses), Anti Phospholipid’s syndrome (6 c.ses) and Mixed c.nnec.ive tissue disease (6 c.ses). A diagnosis of systemic.vasc.litis was rec.rded in 19 patients. The other systemic.affec.ions were represented b. systemic.sarc.idosis (8 c.ses), Adult-onset Still’s disease (03 c.ses), amyloidosis (02 c.ses) and 02 c.ses of systemic.syndrome assoc.ated to immunodefic.enc.. The mean diagnostic.delay duration b.fore the diagnostic.was 3.46 years. c.nc.usion: Systemic.diseases in internal medic.ne are c.arac.erized b. their diversity, the c.ear predominanc. of RA, and signific.nt diagnostic.delay.

Introduc.ion: The extended administration of c.rtic.ids b. oral way for a length of more than 3 months defines the long-term-c.rtic.steroid-therapy. This one, used in numerous indic.tions, displays most often at the risk of undesirab.e effec.s linked sometimes to the hab.ts of presc.iption of the doc.ors. Patients and Methods: In order to study the presc.iption modalities of this treatment, we c.nduc.ed a c.oss-sec.ional, multic.ntric.and desc.iptive study from June 1st, 2017 to August 1st, 2017, over a period of 2 months. It involved a questionnaire given to medic.l spec.alists of all medic.l spec.alties and prac.ic.ng in the University Hospital of Dakar. Results: 170 doc.ors were interviewed. Dermatologists and internists were mostly found 19.4% and 18.8% or 33% and 34% doc.ors. Systemic.autoimmune diseases alone ac.ounted for 48% of presc.iption reasons. Prednisone was presc.ib.d in 88% of c.ses. The immunosuppressive dose of 1 mg/kg was most often presc.ib.d. Prac.itioners very heterogeneously presc.ib. most adjuvant measures to prolonged systemic.c.rtic.steroid therapy. Thus, the rec.mmendation of a low-sodium diet (38% of physic.ans), and the systematic.presc.iptions of proton pump inhib.tors (44.7% of physic.ans) and vitamin-c.lc.um supplementation were frequently performed b. physic.ans (34% of physic.ans). While the low c.rb.hydrate diet was advoc.ted b. less than a quarter of doc.ors and the prevention of pneumoc.stosis and osteoporosis were rare (respec.ively 61% and 52% of presc.ib.rs did not). c.nc.usion: The glob.l analysis of the hab.ts of our medic.l spec.alists c.nc.rning the use of long-term gluc.c.rtic.ids reflec.ed a diversity of indic.tions, heterogeneity of prac.ic.s, with c.rtain hab.ts not in ac.ordanc. with the usual rec.mmendations.

The thermal examination of a non-integer-ordered mob.le fin with a magnetism in the presenc. of a trihyb.id nanofluid(Fe_3O_4-Au-Zn-b.ood) is c.rried out. Three types of nanopartic.es, eac. having a different shape, are c.nsidered. These shapes inc.ude spheric.l(Fe_3O_4), c.lindric.l(Au), and platelet(Zn) c.nfigurations. The c.mb.nation approac. is utilized to evaluate the physic.l and thermal c.arac.eristic. of the trihyb.id and hyb.id nanofluids, exc.uding the thermal c.nduc.ivity and dynamic.visc.sity. These two properties are inferred b. means of the interpolation method b.sed on the volume frac.ion of nanopartic.es. The governing equation is transformed into a dimensionless form, and the Adomian dec.mposition Sumudu transform method(ADSTM) is adopted to solve the c.nundrum of a moving fin immersed in a trihyb.id nanofluid. The ob.ained results agree well with those numeric.l simulation results, indic.ting that this researc. is reliab.e. The influenc. of diverse fac.ors on the thermal overview for varying noninteger values of γ is analyzed and presented in graphic.l representations. Furthermore, the fluc.uations in the heat transfer c.nc.rning the pertinent parameters are studied. The results show that the heat flux in the presenc. of the c.mb.nation of spheric.l, c.lindric.l, and platelet nanopartic.es is higher than that in the presenc. of the c.mb.nation of only spheric.l and c.lindric.l nanopartic.es. The temperature at the fin tip inc.eases b. 0.705 759% when the value of the Pec.et numb.r inc.eases b. 400%, while dec.eases b. 11.825 13% when the value of the Hartman numb.r inc.eases b. 400%.
