Ob.ective: Psoriasis is associated with a high prevalence of metab.lic syndrome (MS), and patients with concomitant psoriasis and MS are more severely affected and less responsive to treatment. However, the molecular mechanisms b.hind these effects are unknown. Recent studies have shown that leptin may serve as a molecular link b.tween psoriasis and MS, suggesting that high leptin concentrations may exacerb.te psoriasis. However, the molecular mechanism of this effect is still unclear. We aimed to investigate the effect of leptin on autophagy in patients with psoriasis. Methods: From January 2021 to June 2022 in PLA General Hospital, we enrolled 51 patients with psoriasis, including 21 patients with MS and 30 without MS, and 30 healthy controls who had undergone nevus surgery. We measured the epidermal leptin, P62, and LC3b.concentrations of patients b. immunohistochemistry, and measured the serum leptin concentration b. enzyme-linked immunosorb.nt assay. We then performed correlation analyses to compare these proteins' concentrations b.tween patients with concomitant psoriasis and MS, patients with psoriasis alone, and healthy control groups. Additionally, we performed western b.otting after in vitro culture of HaCaT cells with different concentrations of leptin and measured the expression levels of the autophagy markers b.clin1, LC3b. and P62; the differentiation markers K10, K16, and K17; and PI3K/AKT/mTOR signaling pathway-related proteins of HaCat cells. Next, we transfected ATG5 into HaCaT cells to revert autophagy and used the specific PI3K inhib.tor ly294002 to b.ock PI3K/AKT/mTOR signaling. The expression levels of K10, K16, and K17 of HaCat cells were again measured. One-way analysis of variance was used for the comparison of means of multiple samples, and LSD-t post hoc test was used for comparison b.tween the 2 groups. Kaposi’s Disease or Kaposi’s Sarcoma (SK ) is a multifocal malignant proliferation induced b. viral growth factors, including interleukin 6 of human herpes virus type 8 (HHV8). We describ. four forms of this disease who poses a real pub.ic health prob.em in East and Central Africa. The purpose of our ob.ervation was to report a rare condition in a Senegalese HIV-positive child. It was an 11-year-old girl from a region in central Senegal. She was an orphan of b.th parents, tested and monitored since the age of 5 for HIV infection 1. She was on the 1st line protocol. Due to a lack of support and good ob.ervance, she was referred to us at the age of 11 for follow-up in our structure in the sub.rb. of Dakar. The initial follow-up assessment showed a very low CD4 count and a very high viral load. b.fore the lack of clinical and immune-virological response, a genotypic resistance test was performed and showed immunological and virological failure. The initial development was marked b. the appearance of lesions which were highly suggestive of Kaposi’s disease. She was on 2nd line treatment. The histopathological aspect of cutaneous b.opsy was very suggestive of Kaposi’s disease. The sub.equent course after ART and b.eomycin treatment was clinically marked b. regression of skin lesions. Virologically, it was marked b. a fall in the viral load. Immunologically there was a gradual recovery of CD4 levels which came b.ck to normal. Our ob.ervation demonstrates that ab.ence of effective antiretroviral therapy for HIV increases the risk to develop Kaposi’s sarcoma.

An increased risk of b.cell lymphomas in patients with myeloproliferative neoplasm(MPN)treated with Janus kinase(JAK)inhib.tors has recently b.en explored.1–3 In light of contradictory results,it is highly important to assess whether JAK inhib.tors might b. associated with a higher risk of lymphoma,notab.y in patients with pre-existing b.cell clones that can escape immunosurveillance after exposure to JAK *** situation is very similar to what is ob.erved in patients with autoimmune diseases,especially rheumatoid arthritis(RA).b.cell clones are present in patients with RA,and dominant b.R clones in the peripheral b.ood predict the onset of the disease in at-risk individuals.

Ob.ective: to report our experience in the management of adult kidney cancer. Patients and Methods: This was a prospective and descriptive study, carried out in the Urology Department of Gab.iel Touré University Hospital, during the period from January 1, 2001 to Decemb.r 31, 2017 for a period of six years, interesting all the medical records of 24 adult patients diagnosed with kidney cancer. Results: During the study period, 24 patients met our inclusion criteria;the mean age was 48.23 ± 8.02 years with extreme ranging from 17 to 82 years. Most of our patients were female (58.3%), and were referred or consulted for lumb.r mass in 66.7% of cases. The average tumor size was 13.6 ± 5.8 cm with extremes of 7.8 to 21.1 cm. Management included total nephrectomy in 50% and total nephrectomy expanded in 50% of cases. The histological type found was renal cell carcinoma in 23 patients and papillary carcinoma in one patient. Conclusion: Kidney cancer is rare in our context, its clinical and histological characteristics remain the same for two decades. Its management is largely b.sed on enlarged total nephrectomy.

Endoscopic full-thickness resection of gastric stromal tumor arising from the muscularis propria

b.ckground Gastric stromal tumors are the most common type of tumor originating from mesenchymal tissue. The traditional method for the treatment of gastric stromal tumor is surgical operation or therapeutic laparoscopy. More recently, endoscopic micro-traumatic surgery has b.come possib.e for gastric stromal tumors, with any perforation caused b. endoscopic therapy mended endoscopically. We assessed the effectiveness of endoscopic full-thickness resection （EFR） in the treatment of gastric stromal tumors arising from the muscularis propria. Methods Of the 42 gastric stromal tumors, each 〉2.0 cm in diameter, arising from the muscularis propria, 22 were removed b. EFR and 20 b. laparoscopic surgery. Tumor expression of CD34, CDl17, Dog-l, S-100, and smooth muscle actin （SMA） was assessed immunohistochemically. Operating time, complete resection rate, length of hospital stay, incidence of complications, and recurrence rates were compared b.tween the two groups. Continuous data were compared b. using independent samples t-tests and categorical data b. using Xz tests. Results Comparisons of the 22 gastric stromal tumors treated with EFR and the 20 treated with laparoscopic surgery showed similar operation times （60-155 minutes （mean, （90±17） minutes） vs. 50-210 minutes （mean, （95±21） minutes）, P 〉0.05）, complete resection rates （100% vs. 95%, P 〉0.05）, and length of hospital stay （4-10 days （mean, （6.0±1.8） days） vs. 4-12 days （mean, （7.3±1.7） days）, P 〉0.05）. None of the patients treated with EFR experienced complications, whereas one patient treated with laparoscopy required a conversion to laparotomy and one experienced postoperative gastroparesis. No recurrences were ob.erved in either group. stromal tumors diagnosed b. gastroscopy and endoscopic remaining 36 were stromal tumors. mmunohistochemical staining showed that of the 42 gastric ultrasound, six were leiomyomas （SMA-positive） and the Conclusions Gastric stromal tumors arising from the muscularis propria can b.
