Methylprednisolone（MP） is currently the only drug confirmed to exhibit a neuroprotective effect on acute spinal cord injury（SCI）. Vitamin C（VC） is a natural water-soluble antioxidant that exerts neuroprotective effects through eliminating free radical damage to nerve cells. Bone marrow mesenchymal stem cells（BMMSCs）, as multipotent stem cells, are promising candidates in SCI repair. To evaluate the therapeutic effects of MP, VC and BMMSCs on traumatic SCI, 80 adult male rats were randomly divided into seven groups： control, SCI（SCI induction by weight-drop method）, MP（SCI induction, followed by administration of 30 mg/kg MP via the tail vein, once every other 6 hours, for five times）, VC（SCI induction, followed by intraperitoneal administration of 100 mg/kg VC once a day, for 28 days）, MP ＋ VC（SCI induction, followed by administration of MP and VC as the former）, BMMSCs（SCI induction, followed by injection of 3 × 10~6 BMMSCs at the injury site）, and BMMSCs ＋ VC（SCI induction, followed by BMMSCs injection and VC administration as the former）. Locomotor recovery was assessed using the Basso Mouse Scale. Injured spinal cord tissue was evaluated using hematoxylin-eosin staining and immunohistochemical staining. Expression of transforming growth factor-beta, tumor necrosis factor-alpha, and matrix metalloproteinase-2 genes was determined using real-time quantitative PCR. BMMSCs intervention better promoted recovery of nerve function of rats with SCI, mitigated nerve cell damage, and decreased expression of transforming growth factor-beta, tumor necrosis factor-alpha, and matrix metalloproteinase-2 genes than MP and/or VC. More importantly, BMMSCs in combination with VC induced more obvious improvements. These results suggest that VC can enhance the neuroprotective effects of BMMSCs against SCI.

Efficient Computation Offloading of IoT-Based Workflows Using Discrete Teaching Learning-Based Optimization

As the Internet of Things(IoT)and mobile devices have rapidly proliferated,their computationally intensive applications have developed into complex,concurrent IoT-based workflows involving multiple interdependent *** exploiting its low latency and high bandwidth,mobile edge computing(MEC)has emerged to achieve the high-performance computation offloading of these applications to satisfy the quality-of-service requirements of workflows and *** this study,we propose an offloading strategy for IoT-based workflows in a high-performance MEC *** proposed task-based offloading strategy consists of an optimization problem that includes task dependency,communication costs,workflow constraints,device energy consumption,and the heterogeneous characteristics of the edge *** addition,the optimal placement of workflow tasks is optimized using a discrete teaching learning-based optimization(DTLBO)*** experimental evaluations demonstrate that the proposed offloading strategy is effective at minimizing the energy consumption of mobile devices and reducing the execution times of workflows compared to offloading strategies using different metaheuristics,including particle swarm optimization and ant colony optimization.

high fat diet dysregulates microRNA-17-5p and triggers retinal inflammation:Role of endoplasmic-reticulum-stress

AIM To elucidate how high diet-induced endoplasmic reticulum-stress upregulates thioredoxin interacting protein expression in Müller cells leading to retinal inflammation. METhODS Male C57Bl/J mice were fed either normal diet or 60% high fat diet for 4-8 wk. During the 4 wk study, mice received phenyl-butyric acid(PBA); endoplasmic reticulum-stress inhibitor; for 2 wk. Insulin resistance was assessed by oral glucose tolerance. Effects of palmitate-bovine serum albumin(BSA)(400 μmol/L) were examined in retinal Müller glial cell line and primary Müller cells isolated from wild type and thioredoxin interacting protein knock-out mice. Expression of thioredoxin interacting protein, endoplasmic reticulum-stress markers, mi R-17-5p m RNA, as well as nucleotide-binding oligomerization domain-like receptor protein(NLRP3) and IL1β protein was *** high fat diet for 8 wk induced obesity and insulin resistance evident by increases in body weight and impaired glucose tolerance. Synthesis and characterization of CNT/Ce-TiO_2 nanocomposite for phenol degradation

An innovative photocatalyst, Carbon nanotube （CNT） supported Ce-TiO2 nanocomposite was successfully synthesized via modi-fied sol gel method and investigated in a batch reactor for abolition of phenol under UV light spectrum. Characterization of catalyst micro-structure and internal properties were done by means of X-ray diffraction （XRD）, Brunauer-Emmett-Teller （BET）, scanning electron micros-copy （SEM）, transmission electron microscopy （TEM） and UV-vis diffuse reflectance spectra （DRS）. Ce doping can inhibit phase transfor-mation from anatase to rutile and eliminate the recombination of electron-hole pairs in the catalyst. The presence of CNT in TiO2 composite can both increase the photoactivity under UV and change surface properties to achieve sensitivity to visible light. The optimum mass ratio of CNT support and cerium （Ce） dopant in TiO2 was the prominent factor to harvest CNT/Ce-TiO2 photocatalyst nanocomposite. The results demonstrated that optimum mass ratio of CNT：TiO2：Ce was 0.02：1.0：0.06, which resulted in the great performance of the photocatalyst to de-grade about 94% of phenol in a 50 mg/L solution in only 3 h. In this paper, dissimilar role of CNT support and Ce dopant in the TiO2 photo-catalysis of phenol was also discussed.

Ezetimbe as potential treatment for cholesterol gallstones: The need for clinical trials

Obesity is associated with insulin resistance, nonalcoholic fatty liver disease (NAFLD) and gallstones. high fat diets (unsaturated fats) rich in cholesterol have been demonstrated to produce not only gallstones but also NAFLD and insulin resistance. Interestingly, a high incidence of gallstones is being reported in association with insulin resistance and NAFLD. Laparoscopic cholecystectomy is the best definitive therapy for symptomatic gallbladder disease. Ezetimibe is a drug that inhibits the absorption of both dietary and biliary cholesterol in the small intestine. Importantly, ezetimibe showed potential benefit not only in treating and preventing gallstones but also in insulin resistance and NAFLD. Further studies are required before the use of ezetimibe for the treatment of gallstones can be advocated.

Management of Acute Malnutrition in Children Aged 0 to 24 Months Boulbinet health Center

Introduction: Malnutrition is a pathological state resulting from the relative deficiency or excess of one or more essential nutrients, whether manifested clinically or detected only by biochemical, anthropometric or physiological analyses. The overall objective was to assess the quality of management of acute malnutrition in children aged 0 - 24 months at the Boulbinet health center. Methodology: This was a prospective descriptive study lasting six (06) months from May 5 to October 5, 2018. The study included all children aged 0 to 24 months. Results: Acute malnutrition in children aged 0 - 24 months accounted for 2.11% of cases. The sex ratio was 1.41 in favor of males. The mean age of our patients was 5 months 7 days, with extremes of 1 month and 6 months. The majority came from Ra toma (40.24%). Exclusive breastfeeding was most common (54.02%). The main clinical signs were: pallor 49.42%, diarrhea 46.67, oral lesions37.96%. SAM represented 89.66% and MAM 10.34%. Most associated pathologies: anemia 49.42% and oral candidiasis 37.93%. In terms of outcome, we recorded 56.32% cures, 20.69% deaths, 18.39% dropouts and 4.60% cures. Conclusion: Improving the quality of care for malnourished children aged 0 - 24 months requires raising awareness among mothers and the general public of the consequences of malnutrition.

The antiretroviral treatment (ART) has significantly reduced the number of new hIV/AIDS infections and related deaths. however, cases of immunovirological discordance (IVD) are found in various locations. The objective of this study was to determine the profile of People living with hIV (PLhIV) with IVD and to identify associated factors. We conducted a cross-sectional study based on the records of PLhIV under ART for at least 6 months, followed at hpital de Jour Donka from 2015 to 2017, and having both viral load (CV) and CD4 T-cell count. Prevalence of IVD was 34.57%, with 23.87% for immunological discordance (ID) and 10.7% for virological discordance (VD). Females were predominant (66.26%), and male gender influenced IVD with a statistically significant difference (p = 0.006) and was associated with VD (p = 0.007). The average age was 38.77 11.30 years. PLhIV were classified at WhO stages 3 and 4 (86.01%). The median initial haemoglobin level was 11.5 g/L [3.2 - 12]. The mean initial CD4 T-cell count was 272.84 cells/mm3 201.6. The median initial viral load (VL) was 147,337 copies/mL [1092 - 31,675,000]. The initial CD4 T-cell count 3 was associated with IVD with a statistically significant difference (p = 0.0009) and correlated with ID (p = 0.000). Prurigo was associated with IVD with a statistically significant difference (p = 0.003). Cerebral toxoplasmosis was not associated with IVD but was associated with ID (p = 0.04). This study allowed us to describe the profile of PLhIV with IVD. The main associated factors were male gender, initial CD4 T-cell count 3, toxoplasmosis, prurigo, and herpes zoster.

Obstructive sleep apnea (OSA) is a complex disorder that consists of upper airway obstruction, chronic intermittent hypoxia and sleep fragmentation. OSA is well known to be associated with hypoxia, insulin resistance and glucose intolerance, and these factors can occur in the presence or absence of obesity and metabolic syndrome. Although it is well established that insulin resistance, glucose intolerance and obesity occur frequently with non-alcoholic fatty liver disease (NAFLD), it is now becoming apparent that hypoxia might also be important in the development of NAFLD, and it is recognized that there is increased risk of NAFLD with OSA. This review discusses the association between OSA, NAFLD and cardiovascular disease, and describes the potential role of hypoxia in the development of NAFLD with OSA.

Beta secretase activity in peripheral nerve regeneration

While the peripheral nervous system has the capacity to regenerate following a nerve injury,it is often at a slow rate and results in unsatisfactory recovery,leaving patients with reduced *** regeneration associated genes have been identified over the years,which may shed some insight into how we can manipulate this intrinsic regenerative ability to enhance repair following peripheral nerve *** lab has identified the membrane bound protease beta-site amyloid precursor protein-cleaving enzyme 1（BACE1）,or beta secretase,as a potential negative regulator of peripheral nerve *** beta secretase activity levels are abolished via a null mutation in mice,peripheral regeneration is enhanced following a sciatic nerve crush ***,when activity levels are greatly increased by overexpressing beta secretase in mice,nerve regeneration and functional recovery are impaired after a sciatic nerve crush *** addition to our work,many substrates of beta secretase have been found to be involved in regulating neurite outgrowth and some have even been identified as regeneration associated *** this review,we set out to discuss BACE1 and its substrates with respect to axonal regeneration and speculate on the possibility of utilizing BACE1 inhibitors to enhance regeneration following acute nerve injury and potential uses in peripheral neuropathies.
