Contralateral eye study of refractive,topographic and aberrometric outcomes after femtosecond assisted Myo Ring implantation and DALK for management of keratoconus

AIM：To evaluate the efficacy of femtosecond laser assisted MyoRing intrastromal corneal implant and deep anterior lamellar keratoplasty （DALK） for management of moderate to advanced keratoconus regarding the degree of changes in visual acuity, refraction, corneal asphericity and ***：A prospective non comparative interventional case study was conducted in Ophthalmology Department,Ain Shams University Hospital in the period from January 2015 to February 2017. The study included 30 eyes of moderate to advanced keratoconus. MyoRing was implanted in one eye （Group Ⅰ） and DALK operation was performed in the contralateral eye of the same patient （Group Ⅱ）.Preoperative and 6mo post-operative uncorrected visual acuity（UCVA）, corrected distance visual acuity （CDVA）, spherical equivalent （SE）, corneal and refractive astigmatisms,keratometry and Q-value using topography images were acquired. Quality of vision was assessed in all eyes including total corneal, anterior corneal high order aberrations analysis at 5 mm pupil size and the Strehl ratio of point spread function （PSF） as an objective measure of ***：Mean postoperative UCVA, CDVA, SE, refractive astigmatism,keratometryreadingsandasphericity were statistically improved compared to preoperative parameters in both groups （P〈0.05）. Significant reduction of all corneal aberrations following both techniques（P〈0.05） was achieved except mean trefoil and mean PSF in Group Ⅰ （P〉0.05）. Postoperative corneal aberrations were significantly lower in Group Ⅱ compared to Group Ⅰ.A statistically significant negative correlation was found in Group Ⅰ between the mean change in CDVA （logMAR）and the mean preoperative and mean postoperative total corneal aberrations root mean square （RMS; r=-0.78,P=0.04）. Also a statistically significant negative correlation was found between mean preoperative coma RMS and mean post PSF （r=-0.86, P=0.01）. Parameters Affecting Energy Consumption for Producing High Carbon Ferromanganese in a Closed Submerged Arc Furnace

The power consumption is considered to be the most important factor affecting the production cost of fer romanganese alloy. Different parameters affecting the energy consumption for industrial production of high carbon ferromanganese HCFeMn were investigated in a closed submerged arc furnace. The analysis of industrial data revealed that the most energy consumed factors were the direct reduction by solid carbon, Boudouard reaction, metal and slag formation, and decomposition of fluxing materials （limestone and dolomite）. To reduce the energy con- sumption and minimize the energy losses in the production process of HCFeMn, it was recommended to use Mn blend with minimum Mn to Fe ratio of 6 and lower SiO2 content or higher basicity. The added coke must be adjusted according to the material balance to prevent the over-coke and to minimize the highly endothermic ＂Boudouard reac tion＂. In addition, it was recommended to work at basic slags with the ratio of （CaO＋MgO） to Si（）2 equal to 1.0- 1.2 instead of much higher slag basicity. Furthermore, the mass losses had to be minimized through adjusting the handling and charging process and to take care of all metal produced.

Safety and efficacy of posterior sub-Tenon＇s carboplatin injection versus intravitreal melphalan therapy in the management of retinoblastoma with secondary vitreous seeds

AIM： To evaluate the safety and efficacy of posterior sub-Tenon＇s carboplatin injection compared to intravitreal melphalan injection in the management of retinoblastoma （RB） with secondary vitreous seeds. The outcome measures were vitreous seeds regression, need for other treatment modalities to achieve ocular salvage and treatment side effects. METHODS： A prospective interventional comparative nonrandomized study included RB eyes developed secondary vitreous seeds during the period of follow up. They subdivided into two groups： study group Ⅰ where posterior sub-Tenon＇s carboplatin （20 mg/2 mL） was injected and study group Ⅱ where intravitreal melphalan （20 μg/0.1 mL） was injected. The injections repeated every 2-4wk. RESULTS： Thirty-three eyes were included in the study. Seventeen eyes （16 patients） in study group Ⅰ and 16 eyes （16 patients） in study group Ⅱ. Ten eyes （30.3%） were completely salvaged following local chemotherapies, Ocular salvage was 23.5% following posterior sub-Tenon＇s carboplatin injection versus 37.5% following intravitreal melphalan raised to 47.1% and 75% with addition of external beam radiotherapy （EBR） with no statistically significant difference between the study groups （P=0.16）. A statistically significant correlation was found between ocular salvage rate and type of vitreous seeds either dust, spheres and clouds （r=0.42, P=-0.015） and eyes harbor new solid tumor growth （r=0.35, P=0.045）. The mean and median follow up periods following local chemotherapy injections were 2.0y in the study group Ⅰand 2.37y in the study group Ⅱ. Few complications were reported： periorbital edema in all eyes and ocular motility disturbances in 13 eyes （76.5%） following posterior sub-Tenon＇s carboplatin injection. Vitreous hemorrhage developed in 2 eyes （12.5%） and localized retinopathy in 5 eyes （31.25%） following intravitreal melphalan. ~ CONCLUSION： Local chemotherapy for treatment of RB with secondary vitreous seeds is safe and can salvage 30.3% of eyes

Safety and efficacy of photodynamic therapy using BCECF-AM compared to mitomycin C in controlling post-operative fibrosis in a rabbit model of subscleral trabeculectomy

AIM： To evaluate the safety and efficacy of cellular photoablation using BCECF-AM [2, T-bis-（2-carboxyethyl） -5-（and-6）-carboxyfluorescein, acetoxymethyl ester mixed isomersl as a method to control postoperative fibrosis in subscleral trabeculectomy （SST） compared to mitomycin C （MMC） in a rabbit model. METHODS： A comparative prospective case-control animal study was conducted. Fourteen rabbits were subjected to SST with intraoperaUve use of wound modulating agents （MMC or BCECF-AM） of the right eye （study groups I and II respectively） and SST without use of intraoperative wound modulating agents for the left eye （control group II）. Two rabbits 4 eyes were considered as control group I with no surgical intervention. BCECF-AM was injected subconjunctivally 30min before surgery followed by intraoperative illumination with diffuse blue light for 10min. Antifibrotic efficacy was established by clinical response and histological examination. Clinical response was assessed by measuring intraocular pressure （lOP） at day 1, 3, 5, 7, 14, 21 postoperatively, Success was defined by 〉20.0% reduction in lOP from the preoperative values without anti-glaucoma medications. RESULTS： The mean percentage of reduction was 35.0% in the study group I with only one eye （14.3%） had 12.5% reduction. The mean percentage of reduction was 28.0% in the study group U with two eyes （28.6%） in study group II had 14.2% reduction each. Regarding the control group II, the mean percentage of reduction was 14.3% with 64.3% eyes had 〈20.0% reduction. There was a highly statistically significant difference between each of the study groups （right eyes） and the corresponding control group II （left eyes） as regards the mean postoperative lOP values started from day 5 in both study groups and this highly significant difference remained so till the end of the follow up period. A Comparative Study of Locality Preserving Projection and Principle Component Analysis on Classification Performance Using Logistic Regression

There are a variety of classification techniques such as neural network, decision tree, support vector machine and logistic regression. The problem of dimensionality is pertinent to many learning algorithms, and it denotes the drastic raise of computational complexity, however, we need to use dimensionality reduction methods. These methods include principal component analysis (PCA) and locality preserving projection (LPP). In many real-world classification problems, the local structure is more important than the global structure and dimensionality reduction techniques ignore the local structure and preserve the global structure. The objectives is to compare PCA and LPP in terms of accuracy, to develop appropriate representations of complex data by reducing the dimensions of the data and to explain the importance of using LPP with logistic regression. The results of this paper find that the proposed LPP approach provides a better representation and high accuracy than the PCA approach.

Corneal sensitivity,ocular surface health and tear film stability after punctal plug therapy of aqueous deficient dry eye

AIM： To evaluate the effect of punctal occlusion using thermosensitive（smart plug） versus silicone plug for management of aqueous deficient dry eye on cornea sensitivity, ocular surface health and tear film ***： A comparative prospective interventiona case study included 45 patients with bilateral severe form of aqueous deficient dry eye. In each patient, the smar plug was inserted in the lower punctum of the right eye which was considered as study group 1 and silicone plug was inserted in the lower punctum of the left eye o the same patient which was considered as study group2. All patients were subjected to careful history taking and questionnaire for subjective assessment of severity of symptoms. Corneal sensitivity, corneal fluorescein rose bengal staining, Schirmer＇s I test, tear film break up time and conjunctival impression cytology were performed pre and 1, 3 and 6mo post plug ***： A statistically significant improvement in subjective and objective manifestations occurred following treatment with both types of plugs（P 〈0.01）The thermosensitive plug caused significant overal improvement, decrease in frequency of application o tear substitutes and improvement of conjunctiva impression cytology parameters in the inserted side（P 〈0.01）. Canaliculitis was reported in two eyes（4.4% following punctal occlusion using thermosensitive plug（study group 1）. Spontaneous plug loss occurred in 21eyes（46.6%） in the silicone plug group（study group 2）.CONCLUSION： Improvement of subjective and objective manifestations of aqueous deficient dry eye occurs following punctal plug occlusion. Thermosensitive plug has good patient＇s compliance with fewer complications and lower rates of loss compared to the silicone plug.

Validation of Calculated Thermal Parameters with Experimental Results in SOFCs

Eleven steel grades were designed to be used as metallic interconnects for Solid Oxide Fuel Cells (SOFC). Low carbon, high chromium steel with different additives of niobium, vanadium, aluminum, molybdenum, silicon, manganese and titanium were produced. Phase transformation temperatures;eutectoid temperature (Ac1) and temperature at which transformation of ferrite to austenite is completed during heating (Ac3) were measured by L75-76 dilatometer. The influence of the alloying elements on transformation temperatures was analyzed using MATLab. Considering the interaction between different alloying elements two equations for predicting Ac1 & Ac3 were obtained. The obtained Ac1 & Ac3 by these equations showed more compatibility than that obtained by traditional ones. In addition, the coefficients of thermal expansion of these steel grades were detected. The influences of chemical composition and temperature on the thermal expansion coefficient were analyzed;the obtained equations were verified to certain extent by using several kinds of steels. The predicted values were in good accordance with the experimental results which proof the validation of calculation model.

This study has been performed to investigate the different parameters affecting on the production of high carbon ferromanganese in closed submerged arc furnace. The analysis of industrial data revealed that using manganese ores with low Mn/Fe ratio necessitates higher amount of Mn-sinter in the charge. Using Mn-blend with higher Mn/Fe ratio reduces the coke consumption and this leads to reducing the electrodes consumption. The recovery of Mn ranges between 70 and 80 %. Much higher basic slag has slight effect on Mn- recovery. However, as slag basicity increases, the MnO- content of slag decreases. The manganese content of produced HCFeMn depends mainly on Mn/Fe ratio of Mn-blend. For obtaining HCFeMn alloy containing minimum 75%Mn, it is necessary to use Mn-blend with Mn/Fe ratio of higher than 6. A model for determination of the amount and composition of off-gases has been derived based on the chemical composition and material balance of the input raw materials and the produced alloy and slag. By using this model, the amount of off-gases was found to increase by increasing both Mn-blend and coke consumption.

Characterization of Egyptian Manganese Ores for Production of High Carbon Ferromanganese

This work aims at studying the reactivity of Egyptian manganese ores to be used in the production of ferromanganese alloys in submerged electric arc furnace. Ores with different manganese content (high-medium and low) were selected and characterized by X-Ray Fluorescence (XRF), X-Ray Diffraction (XRD) and Scanning Electron Microscope (SEM). The main mineralogical compositions in the three ores are pyrolusite (MnO2) and hematite (Fe2O3). Porosity of selected Mn ores was determined. The reactivity of the different ores was carried out through pre-reduction of the selected ores using thermobalance at 900°C and 1100°C and mixture of CO and CO2 gases. The reduction process was done until steady weight. The reduced ores were examined using XRD and SEM. The results showed that pyrolusite in high and medium ores are converted completely to MnO at 1100°C. However, the ore with low manganese content was converted to MnO and Mn3O4. Consequently, it is clear from the results that Mn ores with high and medium MnO2 content are more reactive than those with low MnO2. Therefore, high MnO2 content Mn ores are preferable to get good economic impact during the production of high carbon ferromanganese.

Clinical benefits of biochemical markers of bone turnover in Egyptian children with chronic liver diseases

AIM： To investigate the association between serum insulin-like growth factor 1 （IGF-1）, osteocalcin, and parathyroid hormone （PTH） levels with the etiology and clinical condition of patients with chronic liver disease. METHODS： Eighty children with hepatocellular damage were divided into 3 groups according to the etiology of disease infection： bilharziasis （9 patients）, hepatitis B virus （HBV, 12 patients） and hepatitis C virus （HCV, 29 patients）. The Child score index was found as A in 24 patients, B in 22 patients, C in 4 patients. Thirty healthy children served as control ***, HBcAbIgM, HBcAbIgG, and anti-HCV were detected using ELISA technique. HCV-RNA was measured by reverse transcription polymerase chain reaction （RT-PCR）. Antibllharzial antibodies were detected by indirect haemagglutination test. Liver function tests were performed using autoanalyser. Serum IGF-1, osteocalcin and PTH levels were measured by ELISA technique. Abdominal ultrasonography was also conducted. RESULTS： Serum IGF-1 level was significantly lower in all patient groups with liver diseases, while serum osteocalcin and PTH levels were significantly elevated in patients with HBV and HCV infections compared with the control group. Serum osteocalcin and PTH concentrations were measured with the severity of liver disease from Child A to C. Child A patients unexpectedly showed significantly reduced IGF-1 levels in comparison to patients staged as Child B or C. Serum osteocalcin level was negatively correlated with albumin （14.7 ± 0.54 vs 3.6 ± 0.10, P 〈 0.05）, while that for PTH was positively correlated with total protein （70.1 ± 2.17 vs 6.7 ± 0.10, P 〈 0.05） in patients with HCV infection.
