Management of Intracranial Complications of Sinusitis

Intracranial complications of sinusitis are rare. However, they have an important morbidity and mortality rate, and can be a source of disabling neurological sequelae in the absence of a rapid diagnosis and adequate treatment. We carry a retrospective study of 23 patients having sinusitis with intracranial complications, treated between 1996 and 2011. All patients underwent complete ENT and neurological examination, biological investigations and sinonasal and cerebral CT. An intraveinous large-spectrum antibiotherapy was administered to all patients. Twenty patients underwent surgery. It included evacuation of the intracranial collection, sinus drainage, with or without cranialization of the frontal sinus. Evolution was assessed on clinical biological and radiological criteria. Mean age was 25 years and sex-ratio was 3.6. Neurological signs were the most frequent symptoms. Rhinological signs were essentially purulent rhinorrhea (14 cases) and nasal obstruction (12 cases). Nasal endoscopy showed pus in the middle meatus in 10 cases. On CT, intracranial complications included subdural empyema (11 cases), extradural empyema (7 cases) and brain abscess (5 cases). Associated cerebral thrombophlebitis was noted in 4 cases. Three patients with extradural empyema had had an exclusive medical treatment. All other patients (20 cases) were operated. Clinical and radiological evolution was favorable after initial treatment in 14 cases (60.8%). Six patients required secondary surgery. Two patients have died despite intensive care. The intracranial complications of sinusitis are serious and source of important morbidity and mortality. Management should rapid and adequate, combining effective antibiotic therapy and eventually neurosurgical treatment.

Otosclerosis: Contribution of Computed Tomography and Radio-Anatomical Correlations

Otosclerosis is an osteodystrophia of the otic capsule, responsible for hearing loss by blocking of the stapes footplate. The purpose of this study was to evaluate the reliability of computed tomography (CT) in the positive diagnosis of otosclerosis, to compare the radiological findings to those intraoperatively, and to seek a correlation between imaging data and postoperative audiometric results. This is a retrospective study of 60 patients having otosclerosis, treated in our department between 2009 and 2011. All patients underwent an otoscopic examination, pure tone and vocal audiometry, impedancemetry and petrous CT scan. At petrous CT scan, a footplate thickening was found in 9 cases. Labyrinthine bone hypodensity was noted in 51 cases. Sensitivity of CT in the diagnosis of otosclerosis was 100%. Regarding the operative findings and their correlation with imaging, we found that among the 7 facial canal procidences reported intraoperatively, CT had identified 6, with a sensitivity of 86%. Of the 53 facial nerve in normal position intraoperatively, CT had only identified 34, with a specificity of 64%. Regarding the ossicles, there were 6 cases of abnormalities of the incus, among them 3 were identified on CT, with a sensitivity of 50%. The 54 patients with normal ossicular chain intraoperatively were all identified as such on CT, with a specificity of 100% (p = 0.001). For correlation between imaging and postoperative results, it was found that patients with extended otosclerosis had lower postoperative audiometric improvement than those having localized disease (p < 0.05). Computed tomography is necessary for the diagnosis of otosclerosis. It also helps, with good sensitivity and specificity, seeking for anatomical variants that the surgeon could possibly encounter during surgical procedure. The challenge faced by the visually impaired persons in their day-today lives is to interpret text from *** this context,to help these people,the objective of this work is to develop an efficient text recognition system that allows the isolation,the extraction,and the recognition of text in the case of documents having a textured background,a degraded aspect of colors,and of poor quality,and to synthesize it into *** system basically consists of three algorithms:a text localization and detection algorithm based on mathematical morphology method(MMM);a text extraction algorithm based on the gamma correction method(GCM);and an optical character recognition(OCR)algorithm for text recognition.A detailed complexity study of the different blocks of this text recognition system has been *** this study,an acceleration of the GCM algorithm(AGCM)is *** AGCM algorithm has reduced the complexity in the text recognition system by 70%and kept the same quality of text recognition as that of the original *** assist visually impaired persons,a graphical interface of the entire text recognition chain has been developed,allowing the capture of images from a camera,rapid and intuitive visualization of the recognized text from this image,and text-to-speech *** text recognition system provides an improvement of 6.8%for the recognition rate and 7.6%for the F-measure relative to GCM and AGCM algorithms.

Preparation, Characterization and Statistical Studies of the Physicochemical Results of Series of “B” Carbonated Calcium Hydroxyapatites Containing Mg2+and CO2-3

In this study, series of hydroxyapatites containing Mg2+ and?CO2-3 are prepared by the precipitation method with independently varying concentrations of?CO2-3 and Mg2+. All the compounds are characterized by infrared spectra (IR);powder X-ray diffraction (PXRD) and elemental analysis. The physical analysis results show that the prepared compounds are pure B-type carbonate apatite. The presence of Mg2+ and?CO2-3 in the apatite cause the following effects on its physical properties: a decrease in a-dimension but no changes in c-dimension and a decrease in crystallinity as shown in XDR patterns and IR spectra. The results of the chemical analysis allow us to predict the predominant substitution mechanisms for the CO2-3 and the Mg2+ incorporations in the calcium hydroxyapatites and to calculate their relative contributions x, y and z. Statistical studies of the results “multiple linear regression, analysis of variance (ANOVA) and t-test of the regression coefficients” allow us to determine and to test the mathematical model proposed. Finally, the present study makes it possible to write the general formula for these com-pounds.

Background:Fever of unknown origin(FUO)in developing countries is an important dilemma and further research is needed to elucidate the infectious causes of ***:A multi-center study for infectious causes of FUO in lower middle-income countries(LMIC)and lowincome countries(LIC)was conducted between January 1,2018 and January 1,*** total,15 participating centers from seven different countries provided the data,which were collected through the Infectious DiseasesInternational Research Initiative *** adult patients with confirmed infection as the cause of FUO were included in the *** severity parameters were quick Sequential Organ Failure Assessment(qSOFA)≥2,intensive care unit(ICU)admission,vasopressor use,and invasive mechanical ventilation(IMV).Results:A total of 160 patients with infectious FUO were included in the ***,148(92.5%)patients had community-acquired infections and 12(7.5%)had hospital-acquired *** most common infectious syndromes were tuberculosis(TB)(n=27,16.9%),infective endocarditis(n=25,15.6%),malaria(n=21,13.1%),brucellosis(n=15,9.4%),and typhoid fever(n=9,5.6%).Plasmodium falciparum,Mycobacterium tuberculosis,Brucellae,Staphylococcus aureus,Salmonella typhi,and Rickettsiae were the leading infectious agents in this study.A total of 56(35.0%)cases had invasive procedures for *** mean qSOFA score was 0.76±0.94{median(interquartile range[IQR]):0(0–1)}.ICU admission(n=26,16.2%),vasopressor use(n=14,8.8%),and IMV(n=10,6.3%)were not ***,38(23.8%)patients had at least one of the severity *** mortality rate was 15(9.4%),and the mortality was attributable to the infection causing FUO in 12(7.5%)***:In LMIC and LIC,tuberculosis and cardiac infections were the most severe and the leading infections causing FUO.
