Osteoarthritis(OA)is a complex"whole joint"disease pursued by inflammatory mediators,rather than purely a process of"wear and tear".Besides cartilage degradation,synovitis,subchondral bone remodeling,degeneration of ligaments and menisci,and hypertrophy of the joint capsule take parts in the *** is the hallmark symptom of OA,but the extent to which structural pathology in OA contributes to the pain experience is still not well *** the knee OA,intraarticular(IA)injection(corticosteroids,viscosupplements,blood-derived products)is preferred as the last nonoperative modality,if the other conservative treatment modalities are *** corticosteroid injections provide short term reduction in OA pain and can be considered as an adjunct to core treatment for the relief of moderate to severe pain in people with *** hyaluronic acid(HA)injections might have efficacy and might provide pain reduction in mild OA of knee up to 24 *** for HA injections,the costeffectiveness is an important concern that patients must be informed about the efficacy of these *** more high-quality evidence is needed,recent studies indicate that IA platelet rich plasma injections are promising for relieving pain,improving knee function and quality of life,especially in younger patients,and in mild OA *** current literature and our experience indicate that IA injections are safe and have positive effects for patient ***,there is no data that any of the IA injections will cause osteophytes to regress or cartilage and meniscus to regenerate in patients with substantial and irreversible bone and cartilage damage.

AIM:To evaluate the efficacy and safety of subthreshold micropulse yellow laser(SMYL)in the treatment of chronic central serous chorioretinopathy(CCSC).METHODS:The medical records of 58 eyes of 58 patients with CCSC were reviewed.A 577-nm SMYL system was used for the *** fluorescein angiography was used as the primary method of identifying CCSC,and resolution of subretinal fluid(SRF)evaluated by optical coherence tomography(OCT)and fundus *** macular thickness(CMT),central macular volume(CMV),total macular volume(TMV),subfoveal choroidal thickness(SFCT),subretinal fluid height(SRFH),and subfoveal fluid basement diameter values were measured by spectral domain-OCT(SD-OCT)for all ***:The mean age of the patients was 42.4±9.9(range:20-72)*** mean follow-up was 11.4±8.5(range:6-37)*** BCVA at at the final follow up after treatment was statistically significant from the *** SRF resolution was 12.1%of the eyes in the 1^st month,67.2%of the eyes in the 3rd month and 67.2%of the eyes in the last follow *** initial median CMT,CMV,TMV,and SFCT values before treatment was significantly higher than 3^rd month visit values(P<0.001).In the multivariate analysis performed,age and disease duration were found to be a risk factor for persistent SRF(P=0.017,P=0.016,respectively).CONCLUSION:SMYL treatment provides a significant anatomical and functional improvement and is effective in eliminating SRF in eyes with CCSC.

Effects of contrast media on the hepato-pancreato-biliary system

AIM： To determine the effects of high osmolarity contrast media （HOCM） and iso-osmolar contrast media （CM） application, with or without pressure, on hepato-pancreato-biliary （HPB） system. METHODS： Sixty rats were divided into six equal groups as follows： Group i：（0.9% NaCl, control）, Group 2：（diatrizoate meglumine Na, ionic HOCM, Urographin）, Group 3：（iodixanol, iso-osmolar nonionic CM, Visipaque）; each of which was applied without pressure, whereas the animals of the remaining three groups （1p, 2p, 3p） were subjected to the same CM with pressure. We performed a duodenal puncture and introduced a catheter into the ampulla. After the catheterization, 0.2 mL CM or 0.9% NaCI was injected with or without pressure. Blood samples were taken for biochemical evaluations. The histopathological examinations of liver, common bile duct, and pancreas were performed. RESULTS： There were no significant differences between the six groups for blood amylase, alanine aminotransferases, aspartate aminotransferases, bilirubin levels （P 〉 0.05）. Alkaline phosphatase and γ glutamyl transaminase levels were higher （P 〈 0.05） in the Urographin groups （2, 2p） than the Visipaque groups （3, 3p）, or control groups （1, 1p）. Hepatocyte necrosis, portal area inflammation, and Kupffer＇s cell hyperplasia were higher （P 〈 0.05） in the study groups than the control group. However, there were no significant differences （P 〉 0.05） between HOCM （2, 2p） and iso-osmolar CM （3, 3p） groups. Bile duct proliferation and regeneration in the Urographin groups （2, 2p） were significantly higher （P 〈 0.05） than the Visipaque~ groups （3, 3p） or the control groups （1, lp）. Although CM caused minor damage to the pancreas, there were no statistically significant differences （P 〉 0.05） between the groups. Application of the CM with pressure did not cause additional damage to the HPB system. AIM: To investigate the tolerance development against the relaxant effect of nitric oxide donating drug isosorbide dinitrate (ISDN) and sodium nitropruside (SNP) in internal anal sphincter (IAS) smooth muscle. METHODS: Relaxation responses of ISDN, and electrical fi eld stimulation (EFS) were obtained before and after tolerance induction by ISDN incubation. RESULTS: ISDN (10-7-10-4 mol/L) and SNP (10-8-10-4 mol/L) caused a concentration-dependent relaxation on the basal tonus of the isolated rabbit IAS strips. After a period of 2 h incubation of the 6 x 10-4 mol/L ISDN the relaxation effects of ISDN and SNP did not change compared to control strips. EFS evoked frequency-dependent relaxation in internal anal sphincter smooth muscle and Emax obtained from control strips were not changed in ISDN tolerance-inducing condition. In this study nitrate tolerance was not observed in rabbit IAS smooth muscle. CONCLUSION: This result shows that nitric oxide donating drugs relaxes the internal anal sphincter of the rabbits without the development of tolerance.

AIM： To investigate in vitro effects of propofol, midazolam and dexmedetomidine, which are commonly used anaesthesic or sedatives, on spontaneous contractions of the ileum both in normal rats and those exposed to hyperdynamic peritonitis.METHODS： Spontaneous contractions of isolated ileum muscle segments from sham operated rats and those exposed to peritonitis, were studied in vitro. The amplitude and the frequency of spontaneous contractions of ileum muscle segments were studied after adding dexmetetomidine, propofoi, and midazolam to the organ bath in a cumulative manner.RESULTS： Both amplitude （85.2 ± 6.6 vs 47.4 ± 7.1） and frequency （32.8 ± 4.6 vs 20.2 ± 3.9） of spontaneous contractions in ileum smooth muscle segments were decreased significantly in the peritonitis group compared to the control group （P 〈 0.05）. Dexmedetomidine significantly increased the amplitude of spontaneous contractions （85.2 ± 6.6 vs 152.0 ± 5.4, P 〈 0.05） whereas, propofol （85.2 ± 6.6 vs 49.6 ± 4.8, P 〈 0.05） and midazolam （85.2 ± 6.6 vs 39.2 ± 4.5, P 〈 0.05） decreased it in both control and peritonitis groups. The frequency of spontaneous contractions were significantly decreased by propofol in both control （32.8 ± 4.6 vs 18.2 ± 3.4, P 〈 0.05） and peritonitis groups 20.2 ± 3.9 vs 11.6 ± 3.2, P 〈 0.05）. Dexmedetomidine and midazolam did not cause significant changes in the number of spontaneous contractions in both control and the peritonitis groups （P 〉 0.05）.CONCLUSION： Propofol, midazolam and dexmede- tomidine have various in vitro effects on spontaneous contractions of the rat ileum. While dexmedetomidine augments the spontaneous contraction of the rat ileum, propofol attenuates it. However, the effects of these compounds were parallel in both control and peritonitis groups.

This study aims to predict the undrained shear strength of remolded soil samples using non-linear regression analyses,fuzzy logic,and artificial neural network modeling.A total of 1306 undrained shear strength results from 230 different remolded soil test settings reported in 21 publications were collected,utilizing six different measurement *** water content,plastic limit,and liquid limit were used as input parameters for fuzzy logic and artificial neural network modeling,liquidity index or water content ratio was considered as an input parameter for non-linear regression *** non-linear regression analyses,12 different regression equations were derived for the prediction of undrained shear strength of remolded ***-Forward backpropagation and the TANSIG transfer function were used for artificial neural network modeling,while the Mamdani inference system was preferred with trapezoidal and triangular membership functions for fuzzy logic *** experimental results of 914 tests were used for training of the artificial neural network models,196 for validation and 196 for *** was observed that the accuracy of the artificial neural network and fuzzy logic modeling was higher than that of the non-linear regression ***,a simple and reliable regression equation was proposed for assessments of undrained shear strength values with higher coefficients of determination.

Tuberculous peritonitis in children:Report of nine patients and review of the literature

AIM:To present our experience with tuberculous peritonitis treated in our hospital from 2002-2007. METHODS: We reviewed the medical records of 9 children with tuberculous peritonitis. RESULTS: Nine patients (5 boys, 4 girls) of mean age 14.2 years were diagnosed with peritoneal tuberculosis. All patients presented with abdominal distention. Abdominal pain was seen in 55.5% and fever in 44.4% of the patients. Four cases had coexisting pleural effusion and two had pulmonary tuberculosis with parenchymal consolidation. Ultrasonography found ascites with septation in 7 patients. Two patients had only ascites without septation. Ascitic fluid analysis of 8 patients yielded serum-ascite albumin gradients of less than 1.1 gr/dL. Laparoscopy and laparotomy showed that whitish tuberculi were the most common appearance. Adhesions were also seen in three cases. The diagnosis of peritoneal tuberculosis was confirmed histo-pathologically in 7 patients and microbiologically in two. Two patients had been diagnosed by ascitic fluid diagnostic features and a positive response to antituberculous treatment. All patients completed the antituberculous therapy without any complications. CONCLUSION: Tuberculous peritonitis has to be clinically suspected in all patients with slowly progressive abdominal distension, particularly when it is accompanied by fever and pain. Laparoscopy and peritoneal biopsy are still the most reliable, quick and safe methods for the diagnosis of tuberculous peritonitis.
