AIM:To investigate whether oral glutamine pretreatment prevents impairment of intestinal mucosal integrity during ischemia-reperfusion (I/R) in rats. METHODS: The study was performed as two series with 40 rats in each. Each series of animals was divided into four groups. The first group was used as a control. Animals in the second group were only pretreated with oral glutamine, 1 g/kg for 4 d. The third group received a normal diet, and underwent intestinal I/R, while the fourth group was pretreated with oral glutamine in the same way, and underwent intestinal I/R. Intestinal mucosal permeability to 51Cr-labeled EDTA was measured in urine in the first series of animals. In the second series, histopathological changes in intestinal tissue and plasma endotoxin levels were evaluated. RESULTS: Intestinal I/R produced a significant increase in intestinal permeability, plasma endotoxin level and worsened histopathological alterations. After intestinal I/R, permeability was significantly lower in glutamine- treated rats compared to those which received a normal diet. However, no significant change was observed in plasma endotoxin levels or histopathological findings. CONCLUSION: Although glutamine pretreatment seems to be protective of intestinal integrity, upon I/R injury, such an effect was not observable in the histopathological changes or plasma endotoxin level.

Emotions of Gynecologic Cancer Patients Dealing with Permanent Colostomy:A Qualitative Interview Study

Purpose: This study was conducted to describe emotional state of colostomized patients with gynecologic cancer. Patients and Methods: The study employed a qualitative design from May to October of 2010, in a private university hospitals outpatient gnecologic oncology clinic in Ankara, Turkey. Content analysis with an inductive approach had been used to analyze 32 interviews that served as the data base for this study. Interview form that focused on what gynecologic cancer women felt to living with a colostomy. Results: Findings also reveal that gynecologic cancer patients with colostomy experience many uneasy feelings that affect their emotional state. The content analysis resulted in the development of two thematic units. The first we described as “To experience the consequences of stoma as a gynecologic cancer patients”. The second was “suffering experienced-being challenged the consequences of the stoma”, and the third was “promote self management-normalization of the new condition”. For the psychological support, family and friends’ support has been mentioned as the most important part that can release their anxiety. Conclusions: We concluded that a having stoma was difficult for the women. The emotional state of the women when they get colostomy was typically characterized by fear, and worry about their current process. Body image, self-esteem, social activity, sexuality were the aspects that most affected the patients. Nurses have very important role within holistic approach and empathetic interaction, for determining major problem areas.

Prognostic Value of CD74 and HLA-DR Expressions in Invasive Ductal Breast Cancer

Introduction: Despite the presence of many prognostic and predictive factors, overtreatment remains a major problem in patients with breast cancer. The aim of this study is to investigate the effects of CD74 and HLA-DR expressions on the prognosis of patients who have had a mastectomy for the treatment of breast cancer. Materials and Methods: A retrospective search of medical records was carried out for patients who had surgery for breast cancer at the Department of General Surgery, Akdeniz University School of Medicine, between March 1984 and November 1999. Patients with regular follow-up and necessary data for the study (i.e. patients’ demographics, pathology results, and treatment characteristics) were included in the study. Paraffin blocks of tumor specimens were re-examined with immunohistochemical methods in March 2010 to determine the extent of CD74 and HLA-DR expression and the level of tumor infiltrating lymphocyte (TILs). Results: The mean age and the median duration of follow-up for the 41 participants were 48.29 ± 11.86 years and 125 months (range 115 to 135 months), respectively. Disease-free survival (DFS) in CD74 negative subjects was better than in CD74 positive patients, but the difference was not statistically significant (p = 0.75). Similarly, HLA-DR negative and HLA-DR positive groups showed no statistically significant differences in terms of DFS (p = 0.81). Conclusion: There were positive but insignificant correlations with increased expression of CD74, decreased expression of HLA-DR, and TILs levels. Further studies involving larger sample sizes may provide more insight into these associations.

Radical vs conservative surgery for hydatid liver cysts:Experience from single center

AIM:To compare the efficacy and safety of radical and conservative surgical interventions for liver hydatid ***:The study comprised 59 patients in two groups who had undergone radical and conservative surgical procedures for liver hydatid disease in our department between 2004 and 2009. Preoperative diagnostic tools,medical treatments,demographic and clinical characteristics,postoperative follow-up,and recurrence were compared in both groups. RESULTS:This non-randomized retrospective studyincluded 59 patients who had undergone liver hydatid disease surgery. The radical technique was used in 18 patients (mean age:42.1 ± 13.5 years,seven male,11 female),and the conservative technique was used in 41 patients (mean age:43.5 ± 13.9 years,17 male,24 female). The follow-up period ranged from 3 to 58 mo. Although operative time was significantly shorter in the conservative group (P < 0.001),recurrence was significantly reduced in the radical group (P = 0.045). No statistically significant differences were found in terms of hospitalization duration,cyst count and size,location,postoperative complications,scolicidal solution usage,or follow-up duration between the two ***:The more effective method for pre-venting postoperative recurrence is radical surgery. En-doscopic retrograde cholangiopancreatography for bile leakage in the early postoperative period may decrease the requirement for repeat surgery.

Giant vesical diverticulum:A rare cause of defecation disturbance

Vesical diverticula frequently result from bladder outlet obstructions. However, giant vesical diverticula which cause acute abdomen or intestinal obstruction are very rare. Our review of the English medical literature found 3 cases of bladder diverticula which caused gastrointestinal symptoms. Here, we present a 57-yearold man with a giant diverticulum of the urinary bladder who complained of abdominal pain, nausea and vomiting, constipation, no passage of gas or feces, and abdominal distension for 3 d. A 20 cm x 15 cm diverticulum was observed upon laparotomy. The colonic obstruction was secondary to external compression of the rectum against the sacrum by a distended vesical diverticulum. We performed a diverticulectomy and primary closure. Twelve months postoperatively, the patient had no difficulty with voiding or defecation.

Effects of contrast media on the hepato-pancreato-biliary system

AIM： To determine the effects of high osmolarity contrast media （HOCM） and iso-osmolar contrast media （CM） application, with or without pressure, on hepato-pancreato-biliary （HPB） system. METHODS： Sixty rats were divided into six equal groups as follows： Group i：（0.9% NaCl, control）, Group 2：（diatrizoate meglumine Na, ionic HOCM, Urographin）, Group 3：（iodixanol, iso-osmolar nonionic CM, Visipaque）; each of which was applied without pressure, whereas the animals of the remaining three groups （1p, 2p, 3p） were subjected to the same CM with pressure. We performed a duodenal puncture and introduced a catheter into the ampulla. After the catheterization, 0.2 mL CM or 0.9% NaCI was injected with or without pressure. Blood samples were taken for biochemical evaluations. The histopathological examinations of liver, common bile duct, and pancreas were performed. RESULTS： There were no significant differences between the six groups for blood amylase, alanine aminotransferases, aspartate aminotransferases, bilirubin levels （P 〉 0.05）. Alkaline phosphatase and γ glutamyl transaminase levels were higher （P 〈 0.05） in the Urographin groups （2, 2p） than the Visipaque groups （3, 3p）, or control groups （1, 1p）. Hepatocyte necrosis, portal area inflammation, and Kupffer＇s cell hyperplasia were higher （P 〈 0.05） in the study groups than the control group. However, there were no significant differences （P 〉 0.05） between HOCM （2, 2p） and iso-osmolar CM （3, 3p） groups. Bile duct proliferation and regeneration in the Urographin groups （2, 2p） were significantly higher （P 〈 0.05） than the Visipaque~ groups （3, 3p） or the control groups （1, lp）. Although CM caused minor damage to the pancreas, there were no statistically significant differences （P 〉 0.05） between the groups. Application of the CM with pressure did not cause additional damage to the HPB system. CONCLUSION： Iso-osmolar, non-ionic CM could be more reliable than the ionic HOCM, whereas the application of pressure during the

AIM: To investigate the tolerance development against the relaxant effect of nitric oxide donating drug isosorbide dinitrate (ISDN) and sodium nitropruside (SNP) in internal anal sphincter (IAS) smooth muscle. METHODS: Relaxation responses of ISDN, and electrical fi eld stimulation (EFS) were obtained before and after tolerance induction by ISDN incubation. RESULTS: ISDN (10-7-10-4 mol/L) and SNP (10-8-10-4 mol/L) caused a concentration-dependent relaxation on the basal tonus of the isolated rabbit IAS strips. After a period of 2 h incubation of the 6 x 10-4 mol/L ISDN the relaxation effects of ISDN and SNP did not change compared to control strips. EFS evoked frequency-dependent relaxation in internal anal sphincter smooth muscle and Emax obtained from control strips were not changed in ISDN tolerance-inducing condition. In this study nitrate tolerance was not observed in rabbit IAS smooth muscle. CONCLUSION: This result shows that nitric oxide donating drugs relaxes the internal anal sphincter of the rabbits without the development of tolerance.

AIM： To investigate in vitro effects of propofol, midazolam and dexmedetomidine, which are commonly used anaesthesic or sedatives, on spontaneous contractions of the ileum both in normal rats and those exposed to hyperdynamic peritonitis.METHODS： Spontaneous contractions of isolated ileum muscle segments from sham operated rats and those exposed to peritonitis, were studied in vitro. The amplitude and the frequency of spontaneous contractions of ileum muscle segments were studied after adding dexmetetomidine, propofoi, and midazolam to the organ bath in a cumulative manner.RESULTS： Both amplitude （85.2 ± 6.6 vs 47.4 ± 7.1） and frequency （32.8 ± 4.6 vs 20.2 ± 3.9） of spontaneous contractions in ileum smooth muscle segments were decreased significantly in the peritonitis group compared to the control group （P 〈 0.05）. Dexmedetomidine significantly increased the amplitude of spontaneous contractions （85.2 ± 6.6 vs 152.0 ± 5.4, P 〈 0.05） whereas, propofol （85.2 ± 6.6 vs 49.6 ± 4.8, P 〈 0.05） and midazolam （85.2 ± 6.6 vs 39.2 ± 4.5, P 〈 0.05） decreased it in both control and peritonitis groups. The frequency of spontaneous contractions were significantly decreased by propofol in both control （32.8 ± 4.6 vs 18.2 ± 3.4, P 〈 0.05） and peritonitis groups 20.2 ± 3.9 vs 11.6 ± 3.2, P 〈 0.05）. Dexmedetomidine and midazolam did not cause significant changes in the number of spontaneous contractions in both control and the peritonitis groups （P 〉 0.05）.CONCLUSION： Propofol, midazolam and dexmede- tomidine have various in vitro effects on spontaneous contractions of the rat ileum. While dexmedetomidine augments the spontaneous contraction of the rat ileum, propofol attenuates it. However, the effects of these compounds were parallel in both control and peritonitis groups.
