Clinical and demographic features of patients undergoing videocapsule endoscopy management:A descriptive study

BACKGRoUND video-capsule endoscopy(vCE)is an efficient tool that has proven to be highly useful in approaching several gastrointestinal *** was implemented in Colombia in 2003,however current characterization of patients undergoing vCE in Colombia is limited,and mainly comes from two investigations conducted before the SARS-Cov-2 pandemic *** To describe the characteristics of patients undergoing vCEs and establish the main indications,findings,technical limitations,and other outstanding *** A descriptive study was carried out using data from reports of vCE(PillCam SB3 system)use in a Gastroenterology Unit in Bogotá,Colombia between September 2019 and January *** and clinical variables such as indication for the vCE,gastric and small bowel transit times(GTT,SBTT),endoscopic preparation quality,and limitations were described[n(%),median(IQR)].RESULTS A total of 133 vCE reports were *** were in men with a median age of 70 *** majority had good preparation(96.2%),and there were technical limitations in 15.8%of *** main indications were unexplained anemia(91%)or occult bleeding(23.3%).The median GTT and SBTT were 14 and 30 minutes,*** frequencies of bleeding stigma(3.79%)and active bleeding(9.09%)were low,and the most frequent abnormal findings were red spots(28.3%),erosions(17.6%),and vascular ectasias(12.5%).CoNCLUSIoN vCE showed high-level *** main indication was unexplained *** bleeding was the most frequent *** with artificial intelligence,vCE can improve diagnostic precision and targeted therapeutic interventions.

Mixed neuroendocrine non-neuroendocrine neoplasms(MiNENs)are a hetero-geneous group of malignant neoplasms that can settle in the gastroenteropan-creatic *** are composed of a neuroendocrine(NE)and a non-NE compo-nent in at least 30%of each *** non-NE component can include different histological combinations of glandular,squamous,mucinous and sarcomatoid phenotypes,and one or both of the components can be low-or high grade *** changes in the nomenclature of these neoplasms might lead to great deal of confusion,and the lack of specific clinical trials is the main reason why their management is *** review aims to clarify the definition of MiNEN and analyze available evidence about their diagnosis and treatment options according to their location and extension through careful analysis of the available *** would be important to reach a general consensus on their diagnosis in order to construct a classification that remains stable over time and facilitates the design of clinical trials that,due to their low incidence,will require long recruitment periods.

A Systematic Review of Cannabidiol Effects in Coronary Syndromes: Challenges to Clinical Translation

Background: Myocardial ischemia in addition to other several cardiac syndromes represent a pathological proinflammatory state alongside a complex cellular microenvironment that can be modified by using cannabinoids. Cannabidiol (CBD), a non-psychoactive compound of cannabis has been recently proposed as an immudomodulatory and cardioprotective drug. objectives: In this systematic review we sought to clarify and summarize the clinical and preclinical evidence of potential benefit of the use of CBD in coronary syndromes. Methods: We conducted a systematic search and review according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and Review of Animal Data from Experimental Studies (CAMARADES) guidelines, in the electronic database from PubMed, Web of Science and Scopus up to April 2022 using predefined search terms. Pre-specified exclusion and inclusion criteria were considered, finally 11 articles were chosen to be included for this peer review. Results: Currently there are no good-quality clinical trials with the use of CBD in acute or chronic coronary syndromes. A total of 11 preclinical studies where prescreened and 5 demonstrated reproducible positive cardiovascular outcomes on in-vivo models treated with CBD. Mechanisms of CBD cardioprotection observed: 1) reduction in oxidative stress and inflammation, 2) activation of adenosine receptors and 3) increased expression of angiotensin type 2-receptor. Experimental models included ischemia/reperfusion injury, myocardial infarction, arrhythmias, and metabolic syndrome-like conditions. Conclusion: No clinical recommendation can be issued with the current evidence, on the use of CBD in acute and chronic coronary syndromes. Based on preclinical evidence, we considered there is enough evidence to propose the development of well-designed clinical trials that include CBD in the management of coronary syndromes.

Epidemiology of work-related hand and wrist injuries in a referral center: A descriptive study

BACKGRoUND occupational hand and wrist injuries(oHWIs)account for 25%of work-related accidents in low-and middle-income *** Colombia,more than 500000 occupational accidents occurred in 2021,and although the rate declined to less than 5%in 2020 and 2021,at least one in four accidents involved a hand or wrist *** To describe the oHWIs in workers seen at the emergency room at a second-level hospital in *** An observational study was performed using data from workers who experienced oHWIs and attended a second-level hospital,between June,2020 and May,*** overall frequency of oHWIs,as well as their distribution by sociodemo-graphic,clinical,and occupational variables,are ***,association patterns between sex,anatomical area(fingers,hand,wrist),and type of job were analyzed by correspondence analysis(CA).RESULTS There were 2.101 workers treated for occupational accidents,423(20.3%)were cases of oHWIs,which mainly affected men(93.9%)with a median age of 31 years and who worked mainly in mining(75.9%).oHWIs were more common in the right upper extremity(55.3%)and comprised different types of injuries,such as contusion(42.1%),laceration(27.9%),fracture(18.7%),and crush injury(15.6%).They primarily affected the phalanges(95.2%),especially those of the first finger(25.7%).The CAs showed associations between the injured anatomical area and the worker’s job that differed in men and women(explained variance>90%).CoNCLUSIoN one out of five workers who suffered occupational accidents in Cundinamarca,Columbia had an oHWI,affecting mainly males employed in *** occupational profile is likely to lead to prolonged rehabilitation,and permanent functional *** results might be useful for adjusting preventive measures in cluster risk groups.

As disasters cripple the world’s prospects for sustainable development, protecting the most vulnerable groups exposed to hazards is one of the main challenges facing humanity. owing to the systemic nature of risk and the interactions and interdependencies between upland and lowland systems, healthy and productive mountain households and livelihoods are essential to global sustainability. This paper argues that, building on existing international frameworks, and integrated knowledge and praxis, the development of a global policy agenda should be established to build sustainable peace, sustainable security, and development.

埋地Co_(2)管道不可避免地存在泄漏风险,可能造成极大的经济损失和人员伤亡。因此,深入研究埋地Co_(2)管线泄漏点附近温度场对泄漏定位监测至关重要。运用计算流体动力学(Computational Fluid Dynamics,CFD)软件FLUENT模拟管道不同位置泄漏扩散温度场的变化情况。结果表明,在顶部泄漏时,由于Co_(2)发生相变和射流作用,在泄漏口正上方呈现低温线,且最低温也持续降低。因受到热传递和土壤的影响,低温线无法继续向土壤表层传递。在埋地Co_(2)管道侧向泄漏情形下,当口径小于8 mm时,随着泄漏口径的增大,Co_(2)的泄漏状态更加剧烈,泄漏口喷射出的Co_(2)水平速度矢量也随之增大;当口径达到8 mm时,速度矢量达到最大;当口径大于8 mm时,Co_(2)的泄漏状态再度趋于稳定,而后随着口径增大,速度矢量逐渐减小。

Flowback and early-time production modeling of unconventional gas wells using an improved semi-analytical method

Multiple fractured horizontal wells (MFHWs) currently are the only possible means of commercial production from the low and ultra-low permeability unconventional gas reservoirs. In early production time, flowback fluid, which constitutes of hydraulic water and gas flow within fractures, is collected and analyzed. Flowback analysis has been shown to be a useful tool to estimate key properties of the hydraulic fracture such as conductivity and pore volume. Until date, most tools of flowback analysis rely on empirical and approximate methods. This study presents an improved Green-function-based semi-analytical solution for performance analysis of horizontal gas wells during flowback and early production periods. The proposed solution is derived based on coupling the solutions of two domains: a rigorously derived Green’s function-based integral solution for single-phase gas flow in matrix, and a finite-difference, multiphase solution for gas–water two-phase flow in the fracture. The validity of proposed semi-analytical solution is verified by finely gridded numerical models built in a commercial simulator for a series of synthetic cases considering a variety of fluid and reservoir property combinations, as well as various different production constraints. Comparisons against available empirical and approximate methods are also provided for these cases.

Deep learning for land use and land cover classification from the Ecuadorian Paramo.

The paramo,plays an important role in our ecosystems as They balance the water resources and can retain substantial quantities of *** research was carried out in the province of Tungurahua,specifically the Quero *** aim is to develop a classification of the land use land cover(LULC)in the paramo using satellite imagery using several classifiers and determine which one obtains the best performance,for which three different approaches were applied:Pixel-Based Image Analysis(PBIA),Geographic object-Based Image Analysis(GEoBIA),and a Deep Neural Network(DNN).various parameters were used,such as the Normalized Difference vegetation Index(NDvI),the Bare Soil Index(BSI),texture,altitude,and *** classes were used:paramo,pasture,crops,herbaceous vegetation,urban,shrubrainland,and forestry *** data was obtained with the help of onsite technical experts,using geo-referencing and reference *** the models used the highest-ranked was DNN with an overall precision of 87.43%,while for the paramo class specifically,GEoBIA reached a precision of 95%.
