Use of oral vancomycin in children with autoimmune liver disease:A single centre experience

BACKGROUND Previous reports showed some beneficial effect of oral vancomycin treatment(OVT)in children with primary sclerosing cholangitis;conversely,the experience in patients with other autoimmune liver diseases(AILD),including autoimmune hepatitis(AIH)and autoimmune sclerosing cholangitis(ASC),is *** To assess the response to immunosuppressive treatment(IS)and to OVT in children diagnosed with *** Retrospective study of children diagnosed with AIH(normal biliary tree at cholangiography)and ASC(abnormal biliary tree at cholangiography)in the last 10 *** underwent standard immunosuppressive therapy(IS),but nonresponders received also *** remission[normal aspartate aminotransferase(AST)]and immunological remission(normal IgG and negative autoantibodies)rates and Sclerosing Cholangitis Outcomes in Pediatrics(SCOPE)index were assessed and compared during the follow *** 75 children were included[69%female,median age 10.5 years(5.6-13.4 years),AIH=54,ASC=21].Sixty-three patients(84%,AIH=52,ASC=11)were treated with standard IS and 61 achieved biochemical remission,whereas 12 not responding to IS[16%,F=75%,median age 13.5 years,(12.2-15.7),10 with ASC]required OVT and 8 achieved biochemical *** OVT increased the biochemical remission rate of the whole group of AILD patients from 81%(61/75)to 92%(69/75).Median values of AST,alanine aminotransferase(ALT)and gamma-glutamyl transferase(GGT)decreased significantly after OVT start(P<0.05).Complete normalization of livers enzymes(AST,ALT and GGT)was observed in 6/12 patients(50%).Decrease in SCOPE index score was reported in 5/12 patients(42%).At last follow up(median of 4.4 years,range 0.6-13.8 years)all 75 patients are alive,6(8%,1 with ASC)successfully discontinued medications,1(with ASC)required liver *** Children with AIH and ASC respond well to IS *** may represent a valuable treatment option to achieve biochemical remission in patients

BACKGROUND Intestinal ischemia has been described in case reports of patients with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)disease(coronavirus disease 19,COVID-19).AIM To define the clinical and histological,characteristics,as well as the outcome of ischemic gastrointestinal manifestations of SARS-CoV-2 *** A structured retrospective collection was promoted among three tertiary referral centres during the first wave of the pandemic in northern ***,radiological,endoscopic and histological data of patients hospitalized for COVID-19 between March 1st and May 30th were *** diagnosis was established by consecutive analysis of all abdominal computed tomography(CT)scans *** Among 2929 patients,21(0.7%)showed gastrointestinal ischemic manifestations either as presenting symptom or during *** CT showed bowel distention in 6 patients while signs of colitis/enteritis in *** patients presented thrombosis of main abdominal ***,when feasible,confirmed the diagnosis(6 patients).Surgical resection was necessary in 4/21 *** tissue examination showed distinctive features of endothelial inflammation in the small bowel and *** hospital stay was 9 d with a mortality rate of 39%.CONCLUSION Gastrointestinal ischemia represents a rare manifestation of COVID-19.A high index of suspicion should lead to investigate this complication by CT scan,in the attempt to reduce its high mortality *** shows atypical feature of ischemia with important endotheliitis,probably linked to thrombotic microangiopathies.

Comparison of Air Pollution−Mortality Associations Using Observed Particulate Matter Concentrations and Reanalysis Data in 33 Spanish Cities

Maxillofacial Surgery Education: Where Is It Heading?

Oral and maxillofacial surgery have a long history dating back to ancient civilizations such as the Egyptians. Since then, surgeons have treated craniofacial trauma and alveolodental fractures. In 1846, Hullihen performed a mandibular body osteotomy to correct prognathism, marking a significant milestone in the field. Over time, the specialty has evolved significantly. The field of craniofacial surgery has undergone several phases, including its involvement in conflicts during world wars and the treatment of craniofacial fractures and discrepancies. The process of learning and teaching specialties has evolved differently in various countries throughout the 20th century. Currently, there is no uniformity in the training and registration of specialized professionals, resulting in distinct scenarios. Considering the accessibility of scientific knowledge through technology and globalization, it is crucial to provide an objective overview of the current state of education in the field. This paper focuses on effective professional training as the primary subject of residency courses in this specialty. A proposal is presented for education, emphasizing the significance of providing quality professional training that is tailored to the specific legislation of each country.

Inflammatory fibroid polyps in children:A new case report and a systematic review of the pediatric literature

AIM: To study that inflammatory fibroid polyps(IFPs) in children are extremely uncommon tumors that may occur throughout the gastrointestinal tract. METHODS: A systematic review of the pediatric literature and a report of a new case of IFP is also pres-ented. The Pub Med database was searched for original studies on pediatric IFPs since 1960, according to "Preferred reporting items for systematic reviews and meta-analyses" guidelines for systematic reviews. RESULTS: Five studies were finally enclosed, encompassing 6 children with IFPs(mean age 64 mo). Tumors were located in the stomach(2 patients), in the small bowel(2 patients), in the rectum(1 patient) and in the colon(1 patient). Open surgery was performed in all patients and complete excision of the mass was achieved in all cases. All patients are alive and free of symptom. Authors described a further case of a 3-year-old boy with a large duodenal IFP, in whom the tumor was removed by "en block resection". The presence of IFP throughout the gastrointestinal tract and its variable clinical appearances make it difficult to diagnose. An accurate pre-operative assessment is fundamental in order to differentiate IFP from other more aggressive gastrointestinal tumor, enabling unnecessary demolitive surgery. CONCLUSION: When complete resection of the IFP is achieved, the prognosis is excellent.

Tree height (H) in a natural stand or forest plantation is a fundamental variable in management, and the use of mathematical expressions that estimate H as a function of diameter at breast height (d) or variables at the stand level is a valuable support tool in forest inventories. The objective was to fit and propose a generalized H-d model for Pinus montezumae and Pinus pseudostrobus established in forest plantations of Nuevo San Juan Parangaricutiro, Michoacan, Mexico. Using nonlinear least squares (NLS), 10 generalized H-d models were fitted to 883 and 1226 pairs of H-d data from Pinus montezumae and Pinus pseudostrobus, respectively. The best model was refitted with the maximum likelihood mixed effects model (MEM) approach by including the site as a classification variable and a known variance structure. The Wang and Tang equation was selected as the best model with NLS;the MEM with an additive effect on two of its parameters and an exponential variance function improved the fit statistics for Pinus montezumae and Pinus pseudostrobus, respectively. The model validation showed equality of means among the estimates for both species and an independent subsample. The calibration of the MEM at the plot level was efficient and might increase the applicability of these results. The inclusion of dominant height in the MEM approach helped to reduce bias in the estimates and also to better explain the variability among plots.

The treatment of nonhealing and chronic cutaneous wounds still needs a clinical advancement to be *** mesenchymal stem cells(MSCs),obtained from different sources,and their secretome derived thereof(especially exosomes)can activate signaling pathways related to promotion of cell migration,vascularization,collagen deposition,and inflammatory response demonstrating prohealing,angiogenetic and anti-scarring *** the other hand,biodegradable biomimetic scaffolds can facilitate endogenous cell attachment and proliferation as well as extracellular matrix *** this Review,we revise the complex composites made by biomimetic scaffolds,mainly hydrogels,and MSC-derived exosomes constructed for cutaneous wound *** demonstrate that there exists a synergistic action of scaffolds with encapsulated exosomes,displaying a sustained release profiles to facilitate longlasting healing *** can be envisioned that dressings made by biomimetic hydrogels and MSC-derived exosomes will be clinically applied in the near future for the effective treatment of nonhealing and chronic wounds.

In recent years,hyper-complex deep networks(such as complex-valued and quaternion-valued neural networks–QVNNs)have received a renewed interest in the *** find applications in multiple fields,ranging from image reconstruction to 3D audio *** to their real-valued counterparts,quaternion neural networks require custom regularisation strategies to avoid *** addition,for many real-world applications and embedded implementations,there is the need of designing sufficiently compact networks,with few weights and ***,the problem of regularising and/or sparsifying QVNNs has not been properly addressed in the literature as of *** this study,the authors show how to address both problems by designing targeted regularisation strategies,which can minimise the number of connections and neurons of the network during *** this end,they investigate two extensions of ℓ1 and structured regularisations to the quaternion *** the authors’experimental evaluation,they show that these tailored strategies significantly outperform classical(realvalued)regularisation approaches,resulting in small networks especially suitable for low-power and real-time applications.
