The objective of this paper is to propose and illustrate feasibility, approach, validation as per prevailing codes and design standards, specification to rehabilitate bridge pier cap by using Ultra-High Performance Concrete (UHPC). The evaluation of existing pier caps and bearings indicates that the complete removal of existing bearing is undesirable due to 1) massive size of bearing 2) difficulty in cutting through the thick components of existing bearing 3) deeply anchored lower shoe of existing bearings 4) huge cost and time required to erect temporary support system for superstructure to facilitate the construction of new piers. To overcome these difficulties, UHPC could be cast around the lower shoe up to the existing bearing pin. This UHPC cast could be used to support jacks and temporary bearings. The new low height permanent bearing could then be installed after removing the upper shoe of the existing bearing. In the present research, first properties of UHPC are summarized followed by evaluation of case studies to check feasibility of the solution to rehabilitate pier cap by using UHPC. The complex load paths in pier cap are idealized by using validated strut and tie model as per prevailing AASHTO LRFD Bridge Design Specification.

BACKGROUND Third epidermoid tumors are a rare finding. The appearance of these tumors often makes them difficult to diagnose, and thus they require multimodality imaging. CASE SUMMARY A 48-year-old male patient reported to our hospital with complaints of vomiting and severe headache. The patient also complained of involuntary micturition for the past five days. We used a combination of computed tomography (CT) and magnetic resonance imaging (MRI) imaging modalities to confirm the presence of a malignant epidermoid cyst arising from the third ventricle. A contrastenhanced CT of the head demonstrated minimal perilesional enhancement while an MRI revealed a large, lobulated and septated T2 hyperintense mass arising from the third ventricle. The maximum size of the lesion measured 73 mm × 65 mm × 64 mm in size. CONCLUSION Malignant epidermoid arising from the third ventricle in an adult male was reported using a combination of CT, MRI, and MR spectroscopy.

Efficacy of Magnetic Resonance Imaging in BIRADS 3, 4 and 5 Patients Detected on Full Field Digital Mammography: Our Experience

Aim of the study: To perform Dynamic contrast enhanced MRI of breast in patients with positive findings (BIRADS 3, 4 and 5) detected on screening mammography;to correlate the findings of digital mammography and contrast enhanced MRI of breast with histopathological examinations. Settings and Design: A prospective observation study was conducted at a single centre, i.e. HCG Manavata Cancer Centre. Materials and Methods: Screening mammography was performed on patients with age > 40 years and on patients with age 35 - 40 years having positive family history. The positive mammography was reported and the lesions classified according to BIRADS criteria for mammography. Results: Mammographic examination of the breast lesions yielded an overall sensitivity of 97.67% and a specificity of 85.71%. In our study we combined both morphologic and dynamic parameters and its modification into BIRADS category for lesion classification. The sensitivity of MRI examinations was 97.67% while the specificity was 71.43%. Spiculated margins were encountered only in malignant lesions (p = 0.0006). Statistical correlation was obtained between the pathologically proven benign and malignant lesions regarding their enhancement pattern with p value of Conclusion: As per the results, dynamic contrast MRI had high sensitivity but limited specificity. We did not find any significant difference between FFDM and MRI in terms of diagnostic accuracy. The use of DWI showed high specificity at cut off point of ADC value—0.85 mm2/s. Thus, DWI can be used in addition of morphological and dynamic kinetic characteristics to increase specificity of MRI.

A cantilever-structured magneto-mechano-electric(MME)generator comprising a magnetoelectric composite with a magnet proof mass is a potential candidate for powering autonomous wireless sensor ***,the concept of a magnetic flux concentrator(MFC)to enhance the output performance of the MME generator by focusing the ultralow-intensity magnetic field into the MME generator was ***,the MFC-concentrated magnetic flux mostly focused on the end tip of the MME cantilever rather than at the magnet proof mass located on the cantilever *** that the torque generated by the magnet proof mass contributes more than half of the output power of an MME generator,optimizing the volume and position of the proof-mass with MFC is crucial for better ***,a smaller proof-mass is desirable for the long-term reliability of cantilevertype ***,we investigated the effect of the position and weight(volume)of the magnet proof mass with respect to the MFC on the output performance of the MME generator through finite element analysis and *** MME generator with the lighter magnet proof mass at the optimized position generated a maximum power of 5.35 mW under a 10 Oe magnetic field,which was 210%of that of the MME configuration used in our previous ***,the MME generator showed broadband characteristics around the practical frequency of 60 Hz,which could provide more freedom to design the harvester with high performance.

First contact investigations and compliance to treatment in patients with uveitis

AIM:To ascertain the pattern of investigations at first contact in uveitic patients and evaluate compliance to ***:An observational study comprised of 201 uveitic patients presenting for the first time to our centre from January 2019 to June *** information regarding systemic investigations undertaken by specialists at the time of first contact and the cost of these investigations were reviewed on the first visit to our *** with the treatment was determined and reasons behind non-compliance were evaluated on the first follow-up in patients who had no improvement in clinical signs and ***:The mean age of the study group was 35.35±14.1y and gender composition was 59.7%males and 40.3%*** uveitis was observed in 45.3%of patients,intermediate uveitis in 31.8%of patients,posterior uveitis in 14.9%of patients and panuveitis in 8.0%of *** with a systemic disease was evident in 17.9%of *** compared with standard guidelines and uveitis patterns,systemic investigations were identified to be relevant only in 38.3%of ***-compliance to treatment was documented in 22.4%of *** reasons for non-compliance were inadequate counselling by the treating physician about treatment in 26.7%of patients and a busy schedule at work/school in 22.2%of ***:Significant number of investigations performed at first contact is found to be contrary to standard guidelines and are not contributory to the *** a quarter of patients in this study are found to be non-compliant with the *** is more challenging to achieve in school-going children and working *** availability of comprehensive,periodically updated,evidence-based guidelines on the role of investigations and the use of trained counsellors may help to channelize proper evaluation and improve compliance to treatment,respectively,in patients with uveitis.

Although otitis media poses a serious health risk in developing countries,being a frequent occurrence in children below five years of age,parental awareness and practices about otitis media have not been adequately *** cross-sectional study was undertaken in Navi Mumbai schools,from October 2019 to December 2019,wherein parents with children<5 years of age answered a questionnaire which gathered data on their knowledge,attitude and practices about otitis media in their ***-square tests,Cramer’s V were used to study association between gender,age-group and education of parents with their knowledge,practice and attitude about otitis *** of 425 valid responses,overall most parents displayed adequate knowledge(77%),positive attitude(61%)and good care-seeking practices(70%).There was a positive correlation of knowledge and practice with level of *** study shows acceptable levels of knowledge,attitude and practices in parents about otitis *** who did not seek treatment from health centre attributed the behaviour to poverty,ignorance and lack of health *** positive correlation of knowledge and practices with level of education highlights the importance of role of education in modifying parental awareness and care seeking *** findings call for a need to further strengthen community-based healthcare and improve parental confidence in healthcare services for early detection and adequate treatment of otitis media.

Climate change-induced heat stress combines two challenges:high day-and nighttime temperatures,and physiological water deficit due to demand-side drought caused by increase in vapor-pressure *** is one of the major factors in low productivity of maize in rainfed stress-prone environments in South Asia,affecting a large population of smallholder farmers who depend on maize for their sustenance and *** International Maize and Wheat Improvement Center(CIMMYT)maize program in Asia,in partnership with public-sector maize research institutes and private-sector seed companies in South Asian countries,is implementing an intensive initiative for developing and deploying heat-tolerant maize that combines high yield potential with resilience to heat and drought *** the integration of novel breeding tools and methods,including genomics-assisted breeding,doubled haploidy,fieldbased precision phenotyping,and trait-based selection,new maize germplasm with increased tolerance to heat stress is being developed for the South Asian *** a decade of concerted effort has resulted in the successful development and release of 20 high-yielding heat-tolerant maize hybrids in CIMMYT genetic *** public–private partnerships,eight hybrids are presently being deployed on over 50,000 ha in South Asian countries,including Bangladesh,Bhutan,India,Nepal,and Pakistan.
