目的　用 1例有 3种免疫球蛋白分泌的多发性骨髓瘤 (mm)患者免疫球蛋白重链 (IgH)基因标志探讨mm究竟是由几个克隆组成。方法　患者血清蛋白电泳发现 3个单克隆免疫球蛋白峰 ;RT PCR采用IgHV 6个家族 (VH1～VH6)正向引物 ,通用J区反向引物 ,IgH恒区Cμ、Cδ、Cγ、Cα和Cε 5种特异性反向引物。采用不同的引物组合进行PCR。获得的免疫球蛋白重排基因经克隆后 ,进行测序分析。结果　患者用VH3家族引物和通用J区引物获得一条边缘清晰的浓染条带 ,表明患者骨髓瘤细胞是由IgVH3片段组成可变区。用IgVH3和恒区Cμ、Cγ、Cα特异性引物也可获得相应的PCR产物条带 ,但是不能用引物VH1、VH2、VH4、VH5和J以及VH3和Cδ、Cε获得 ,表明患者有Igm、IgG、IgA基因重排 ,因此外周血血清有相应免疫球蛋白产物。克隆用Cμ、Cγ、Cα引物获得的PCR产物并扩增 ,然后测序比较 ,3种产物除恒区片段不同外 ,其重链基因的VH J重排序列完全一致。表明该mm患者血清出现的 3种免疫球蛋白IgG、IgA、Igm的可变区是完全一致的。结论　尽管从蛋白角度分析该mm细胞似乎是寡克隆性的 ,但是基因序列分析的结果表明其恶性克隆仍然是单克隆性起源。

Background: Little is known about the management of elderly patients with congstive heart failure(CHF) although they represent the majority of the CHF population. Therefore, the TImE-CHF study was set up(1) to evaluate the medical management of very old patients(≥75 years) with CHF compared with younger patients(60-74 years),(2) to compare an intensified with a standard treatment approach, and(3) to differentiate between systolic and diastolic dysfunction(ejection fraction ≤45%vs >45%). methods: In a prospective single-blinded multicenter trial, 824 symptomatic patients, CHF hospitalization within the last year and elevated nt-BNP, are randomized to an intensified versus a standard medical therapy. Treatment strategies follow the published guidelines with the aim to reduce symptoms to NYHA class ≤II(standard) or, additionally, nt-BNP levels below twice the upper limit of normal(intensified). The primary end points are 18-month hospitalization-free survival and quality of life. Results: By the end of 2004, 297 patients have been included, 147 randomized to intensified and 150 to standard therapy. mean age in the older age group was 82±4 years(n=174) and 69±4 years in the younger group(n=123), respectively. Ejection fraction was >45%in 26%and 10%, respectively. Significant comorbidities were present in 93%of patients. Conclusion: TImE-CHF will be the first prospective randomized trial to comprehensively study the management of elderly patients with CHF. It will provide unique information comparing two treatment strategies in two age groups irrespective of ejection fraction regarding prognosis, quality of life, as well as resource utilization and costs.

As part of the PRIDE-PRD2006 intensive campaign, atmospheric boundary layer(ABL) measurements were performed in Qingyuan, Panyu, and Xinken over the Pearl River Delta(PRD) on 1–30 July 2006. During the summer, the surface winds over the PRD are generally controlled by the south, usually with vertical wind shear at a height of approximately 800 m. Subsidence and precipitation from a tropical cyclone affects the air quality of the PRD. Under subsidence, wind speed in the ABL and the height of the ABL decrease and result in high-level concentrations. When the background wind speed is small or calm, the wind profile in Panyu and Xinken changes dramatically with height,which is perhaps caused by local circulation, such as sealand breezes. To better understand the ABL of the PRD,simulations that used the Weather Research and Forecasting(WRF) mesoscale model were utilized to analyze the ABL characteristics over the PRD. Based on three types of weather condition simulations(i.e., subsidence days, rainy days, and sunny days), the WRF model revealed that the simulated temperature and wind fields in these three cases were moderately consistent with the measurements. The results showed that diurnal variations of the ABL height on subsidence days and sunny days were obvious, but diurnal variations of the ABL height on rainy days were not *** ABL is obviously affected by local circulation, and the ABL features are different at various stations. A simulation focused on a high pollution episode during the subsidence days on 12–15 July 2006, occurred under high-pressure conditions, accompanied by the tropical cyclone "Bilis". A comparison of the simulated vertical wind fields and temperature structure with the ABL measurements at Xinken, Panyu, and Qingyuan stations found that the modeled and measured atmospheric fields revealed two different types of ABL characteristics over the PRD. Aims: In children with convulsive status epilepticus (CSE)with fever, to deter mine the likelihood of acute bacterial meningitis (ABm), the proportion that are treated with antibiotics, and the proportion that have diognostic CSF sampling. methods: Patients with an incident episode of CSE with fever were identified as part of an ongoing prospective population based study of CSE in childhood. Resu lts: There were 49 incident cases of CSE in the first six months. Ascertainment was 96%. Twenty four had CSE with fever, 16 had early parenteral antibiotics, n ine had diagnostic CSF sampling, and four had ABm. The population risk of ABm in CSE with fever was significantly higher than that of short seizures with fever (17%v 1.2%). Conclusions: The classical symptoms and signs of ABm.m.y be abse nt in CSE with fever. A high index of suspicion for ABm in the child with CSE wi th fever is paramount. The most appropriate management is suggested to be early parenteral antibiotics and a lumbar puncture when there are no contraindications .

Objective: To differentiate between primary and secondary bilateral synchrony( PBS and SBS, respectively) in generalized spike-and-wave discharges (GSWD),we applied independent component analysis (ICA) to GSWD, and analyzed the character istic patterns of independent components. methods: EEGs from 19 patients with GS WD (9 PBS patients, 10 SBS patients)were studied. Thirty GSWD epochs were selected and concatenated to construct an EEG datamatrix that was subjected to ICA. Se lected independent components were localized by mapping them on a spherical mode l of the head by means of brain electrical source analysis (BESA) to define dipo le ***: Epileptic components of GSWD were clearly separated by the I CA algorithm in all patients. Between one and three components per patient were responsible for GSWD. In PBS patients, 70.6%of the independent components had d ipole sources within the dorsolateral frontal region, and 56.5%of the independe nt components in SBS patients were within the medial frontal region (P=0.012). T he orientation of all of the independent components in PBS patients was radial, whereas 47.8%of the independent components in SBS patients were tangential;this difference was statistically significant (P=0.001). Conclusions:Spatiotemporal decomposition of GSWD by using ICA might be helpful for differentiating SBS from PBS. Significance: The localization of the dipole sources of the independent co mponents may provide insight into the pathophysiological origins of GSWD.

Background. Cutis laxa is an uncommon entity characterized by laxity of the sk in, which hangs in loose folds, producing the appearance of premature aging. It can be subdivided into congenital and acquired. This latter variant is rare and the skin involvement varies from generalized to localized. We report a case of a localized acquired cutis laxa confined to the face, without preceding inflammat ory lesions or systemic compromise. Four similar cases have been reported to dat e. The etiology remains unknown and there is no definitive treatment. methods. A 27-year-old White woman came to our hospital with a wrinkled face, pendulous earlobes and drop eyelids. Changes began 5 years prior, and she appeared much ol der than her age. Results. Histological analysis and ultrastructural examination of skin biopsy revealed reduction and fragmentation of elastic fibers, confirmi ng the diagnosis of cutis laxa. No systemic involvement was diagnosed. The patie nt was submitted to plastic surgery for repair, with satisfactory results to dat e. Conclusions. Acquired localized cutis laxa confined to the face without prece ding inflammatory lesions is extremely rare. The etiology remains unknown. Clini cal features and histopathologic findings confirm the diagnosis. Surgical repair seems to be the only therapeutic choice, but the results are variable and tempo rary.

＜正＞It is well known that obstructive sleep apnea and associated intermittent hypoxemia cause deficit in learning and m.m.ry in *** had shown previously that intermittent hypoxia（IH） impairs long-term potentiation（LTP） in hippocampal C A1 region of mice undergoing 7 d and 14 d of IH *** this study we examined the effect of IH on the expression of brain-derived neurotrophic factor（BDNF）,which is known to play a critical role in *** groups of adult male mice were exposed to 3 d IH,7 d IH,14-day IH and nomoxia environment *** paradigm of IH consists of cycles of oxygen levels between 10%and 21%every 90 s（40 cycles/h） during the light phase for 8 *** levels of the free mature neurotrophic factors BDNF,and also those of nt-3,nt-4/5 and nerve growth factor（NGF） in the hippocampi were determined by enzyme linked immunosorbent assay（ELISA）.Significant decrease in free mature BDNF expression was found in both 7 d IH group（7 mice,77.8±23.0%,P＜0.05） and 14 d group（8 mice,43.9±12.1%,P＜0.001）,compared with the control group,while there was no significant change in the 3-day group（8 mice,94.4±21.6%,P＞0.05）.Similar trends were observed in the case of nt-3 and NGF while there was no significant change in the level of nt-4/*** is known that IH decreased the expression of phosphorylated cAmP response element binding protein（CREB）.Our results support the notion that CREB-dependent transcription of genes,including BDNF,is impaired in IH,and which may underlie the impaired LTP in intermittent hypoxia.
