Searching for a model of Good Leadership in the Light of Exodus 18:12-27 and African Culture

It is an undeniable fact that the success or the failure of any given society or community of human beings depends largely on who controls the affairs of that group.A leader without a clear vision and setting objectives simply drags the group being led into turmoil,while a good leadership paves the way to upward movement of a group of human beings under such a *** for a model of good leadership becomes then a serious need in any human society since success is desired by *** study has therefore searched for a recommendable or model of good leadership in the light of Exodus 18:12-27 and African *** biblical text was studied as a narrative peace dealing with the change of leadership style by moses,the ancient leader of the *** changed from the sole-actor style of leadership to a collaborative and democratic type due to the good advice given to him by Jethro,his *** type,in which there was decentralisation of leadership authority,was found to be beneficial to all involved,because the work of the leader(moses)was made lighter and the led got quicker *** concepts of leadership were also studied,and it was found out that the African/Igbo democratic style is highly recommendable because it is caring;it has good“listening ears”and excludes dictatorship,without degenerating to the laissez-faire type of *** the various types of leadership styles examined,the democratic,collaborative type ranks first as *** Jesus-model of leadership as entailing love and service rather than exploitation,as well as the transparency model of leadership offered by *** in the nt,present themselves as models of good leadership.A good leader serves the interest of the group under his/her charge and not his or her selfish and nepotistic interest.

＜正＞Background:5’-Nucleotidases play a critical role in nucleotide pool balance and the metabolism of nucleoside analogs such as gemcitabine and cytosine arabinoside（AraC）.Previously study performed an expression array association study with gemcitabine and AraC cytotoxicity using 288 human lymphoblastoid cell lines. One gene that was significantly associated with gemcitabine cytotoxicity was a nucleotidase family m.m.er,nt5C2L1. The purpose of this study was to determine the effectiveness of a fetal anatomy survey in conjunction with first-trimester nuchal translucency (nt) screening ultrasound in an unselected pregnant population performed by sonographers. Study design: This was a prospective observational study of women presenting for nt screening for chromosomal defects. An anatomic survey was performed after a standardized protocol with a maxim.m.scan time of 30 minutes. Results: Three hundred twenty-five singleton fetuses (11+ 0 to 13+ 6 weeks)- were examined and pregnancy outcome was available for 300 (92.3% ) of cases. In 89 (24.6% ) cases, transvaginal ultrasound was performed. The following fetal structures were seen in 95% of cases: cranium and intracranial anatomy, face, cord insertion, stomach, and all 4 limbs. The bladder was visualized in 89.5% of cases, the cardiac 4 chamber view in 84% , and the spine in only 45% of cases. Complete anatomy was seen in 109 (33% ). Crown rump length greater than 55 mm and sonographer experience were important factors contributing to anatomic visibility. Of a total of 6 congenital defects in this cohort, 1 was detected in the first trimester (neural tube defect), 4 at the 18- to 20- week anatomic scan, and 1 postnatally. Conclusion: A complete anatomy survey was successful in 33% of first-trimester fetuses in a time-limited sonographer based screening program. Since some anomalies are not evident in the first trimester, the 18- to 20- week scan remains the gold standard.

Purpose: To determine the concentration of group IIA phospholipase A2 (GIIAPLA 2) in tears of patients with ocular rosacea, and to compare it with GIIAPLA2 con centration in tears of age-matched healthy controls. methods: The GIIAPLA2 conc entration in tears was measured with a time-resolved fluoroimmunoassay in 21 pa tients with ocular rosacea (mean age 55.6 ±9.2 years) and in 21 normal subjects (mean age 53.4 ±8.2 years). Conjunctival brush cytologywas carried out and eos inophils, neutrophils, lymphocytes, squamous epithelial cells, columnar epitheli al cells, metaplastic changes and goblet cells were calculated separately. Resul ts: The GIIAPLA2 concentration in tears was statistically significantly lower in patients with ocular rosacea(31.0±18.4 μg/ml, p=0.0099) and, more spe- cifically, in patients who had dry eye (25.8±15.1 μg/ml, p=0.0034), compared to that in normal controls. There was no correlation between the GIIAPLA2 conte nt of tears and the conjunctival cells collected by the brush cytology. Conclusi on: The tears of patients with dry eye symptoms due to ocular rosacea have decre ased GIIAPLA2 content. The pathogenic importance of this finding is discussed.

Elevated plasma levels of B-type natriuretic peptide(BNP) and N-terminal pro-BNP(nt-pro-BNP) are seen in the setting of cardiac ischemia and are associated with adverse outcomes in patients with coronary artery disease. The mechanisms leading to natriuretic peptide elevation in patients with coronary artery disease, including the contribution of coronary atherosclerosis itself, have not been fully elucidated. measurement of nt-pro-BNP, electron beam computed tomography, and cardiac magnetic resonance imaging were performed in 2,445 subjects from the Dallas Heart Study who were free of heart failure and renal insufficiency. Electron beam computed tomography determined coronary artery calcium scores were categorized as none(mild(≥ 10 tomoderate(≥ 100 tont-pro-BNP levels increased significantly across increasing coronary artery calcium score categories(pmultivariate models adjusted for age, gender, race, body mass index, hypertension, history of myocardial infarction, angina, angiotensin-converting enzyme inhibitor use, β -blocker use, left ventricular(LV) ejection fraction, and LV mass, higher coronary artery calcium scores remained independently associated with higher log nt-pro-BNP levels(p=0.03). This association persisted in similar models excluding patients with low LV ejection fractions, LV hypertrophy, angina pectoris, and a history of myocardial infarction. In conclusion, these findings support the hypothesis that coronary atherosclerosis may directly influence the activation of the cardiac neurohormonal system.

Achalasia has been described following fundoplication and is attributed to vag al nerve damage during surgery. Similarly,other traumatic events to the distal e sophagus may be linked to the development of achalasia. Operative and nonoperati ve trauma as a possible factor in the development of achalasia was studied. A re trospective analysis of patients with achalasia(n = 64) at our institution was p erformed. Collected data included age, gender, symptoms, and history of operativ e and nonoperative traumatic events. Comparisons were made to a group of patient s with similar symptoms but normal esophageal manometry (n = 73). Achalasia was diagnosed by manometry in 125 patients over a 6-year period. All patients with complete medical records (n = 64) were studied. A history of operative or nonope rative trauma to the upper gastrointestinal tract prior to the development of sy mptomatic achalasia was present in 16 of 64 (25%). Significantly fewer patients (9.5%) wi th symptoms of dysphagia, but normalmanometry and upper endoscopy, had precede nt trauma to the upper gastrointestinal tract (P ma occurred in motor vehicle accidents. Cases of operative trauma include d coronary artery bypass surgery (n = 4), bariatric surgery (n = 2), fundoplicat ion(n = 3), hea rt/lung transplantation (n = 1), and others (n =5). Pa tients with proven achalasia and a history of trauma were more likely to have chest pain (RR, 4.5; P = 0.012) but less likely to have regurgitation (RR, 0.51; P = 0.01) or nausea/vomiting(RR, 0.0; P = 0.27) than those without a history of antecedent trauma. In this series, significantly more patients with achalasia h ad a history of preceding trauma than did patients with similar symptoms and nor mal esophageal manometry. Following trauma, patients may be at increased risk fo r developing achalasia, possibly from neuropathic dysfunction due to vagal nerve damage. Our report describes a previously healthy 10- year-old female whow as seen for urticarial plaques andmild loss of appetite. An initial laboratory workup re vealed an elevated leukocyte count of 30,000/μ L and a peripheral eosinophil co unt of 22,500/μ L.A skin biopsy showed a marked hypersensitivity tissue respons e with abundant eosinophils. Further investigation of her peripheral eosinophili a uncovered Giardia lamblia in a stool sample. Despite treatment with the anti- parasitic agent furazolidone, the patient’s urticarial plaques, leukocyte cou nt, and peripheral eosinophil count remained unchanged. A bone marrow biopsy con firmed a diagnosis of acute lymphoblastic leukemia (ALL). ALL with hypereosinoph ilia (ALL/Eo) represents a rare and distinct subset of ALL, with more than 30 ca ses documented in the literature. Our discussion summarizes the clinical aspects of this disease and reviews the reported dermatological manifestations of ALL/E o.

Knowing the magnetic field distribution in the magnetotail current sheet(CS) is essential for exploring magnetotail *** this study,using a joint dataset of Cluster/TC-1,the radial profile of the magnetic field in the magnetotail CS with radial distances covering 8nt geomagnetic activity states(i.e.,AE≤100 nt for quiet intervals while AE > 100 nt for active times)and solar wind parameters are statistically *** new findings demonstrate that,independent of the activity state,the field strength and B component(GSm coordinates) start the monotonic increase prominently as r decreases down to 1.5R,which means the dipole field starts to make a significant contribution from *** least in the surveyed radial range,the B component is found to be weaker in the midnight and dusk sectors than that in the dawn sector,displaying a dawn-dusk *** occurrence rate of negative B in active times also exhibits a similar asymmetric distribution,which implies active dynamics may occur more frequently at midnight and dusk *** comparison with that in quiet intervals,several features can be seen in active times:(1) a local B minim.m.between10.5mponent around the midnight region is generally stronger and experiences larger fluctuations,and(3) a sharp positive/negative-excursion of the B component occurs at the dawn/dusk flank regions inside rmeters revealed that the B component is generally stronger under higher dynamic pressure(P > 5 nPa),which may support the dawn-dusk squeezing effect as presented by miyashita et al.(2010).The CS B is generally correlated with the interplanetary magnetic field(ImF) B component,and the correlation quality is found to be better with higher penetration coefficient(the ratio of CS B to ImF B) when ImF B is *** implications of the present results are discussed.

Background: To assess the predictive value of prognosis of different computed tomography (CT) features and National Institutes of Health Stroke Scale score (N IHSS) in acute extensive middle cerebral artery (mCA) infarct. methods: Fifty fi ve patients with acute extensive mCA infarct had the CT performed within 24 hour s of the onset of symptoms. A total of 11 CT features were analysed. The age dis tribution, presence of risk factors, presence of individual CT feature, the tota l CT score, and the NIHSS were correlated with the 30 day ***: Sin gle explanatory variable analysis showed NIHSS, presence of midline shift, midli ne shift of more than 1 cm, extent of infarct, presence of hydrocephalus, efface ment of subarachnoid space/cella media, attenuation of corticomedullary differen tiation, and total CT score were associated with the 30 day mortality. Both exte nt of infarct >67%and attenuationof corticomedullary differentiation gave a sen sitivity and specificity of 93%and 95%, respectively, for the prediction of su rvival. Logistic regression analysis showed that the extent of infarct and NIHSS were the only independent ***: CT features and admission NIH SS are important parameters for prediction of survival in extensive mCA infarct.

Background: Tree species represent 20% of the vascular plant species worldwide and they play a crucial role in the global functioning of the biosphere. The mediterranean Basin is one of the 36 world biodiversity hotspots, and it is estimated that forests covered 82% of the landscape before the first human impacts, thousands of years ago. However, the spatial distribution of the mediterranean biodiversity is still imperfectly known, and a focus on tree species constitutes a key issue for understanding forest functioning and develop conservation strategies. methods: We provide the first comprehensive checklist of all native tree taxa (species and subspecies) present in the mediterranean-European region (from Portugal to Cyprus). We identified some cases of woody species difficult to categorize as trees that we further called "cryptic trees". We collected the occurrences of tree taxa by"administrative regions", i.e. country or large island, and by biogeographical provinces. We studied the species-area relationship, and evaluated the conservation issues for threatened taxa following IUCN criteria. Results: We identified 245 tree taxa that included 210 species and 35 subspecies, belonging to 33 families and 64 genera. It included 46 endemic tree taxa (30 species and 16 subspecies), mainly distributed within a single biogeographical unit. The countries with the highest tree richness are Greece (146 taxa), Italy (133), Albania (122), Spain (155), macedonia (116), and Croatia (110). The species-area relationship clearly discriminated the richest central-eastern (Balkans) and northern (Alpine and Cevenno-Pyrenean) biogeographical provinces, against the five western provinces in the Iberian Peninsula. We identified 44 unrecognized "cryptic trees", representing 21% of the total trees. Among the 245 taxa identified, 19 are considered to be threatened (15 CR+EN+VU) or near threatened (4 nt) by IUCN.
