Very little is known about the impact of psychosocial stress on underlying biological mechanisms in African American lupus patients, although African American women display the highest rates of lupus. Due to the exposure of African Americans to a unique trajectory of stressors throughout the life course, it may be critical to understand the relationship between psychosocial stress and underlying biological mechanisms that influence disease activity and pathology in this high risk group. To begin to fill this research void, an evidence-based self-management program was piloted among a cohort of African American lupus patients participating in a SLE database project at the medical University of South Carolina (mUSC). To assess disease activity, during each clinic visit, a history is obtained, and physical examination, phlebotomy, and urine collection are performed. SLE Disease Activity Index (SLEDAI) and Systemic Lupus International Collaborating Clinics/American College of Rheumatology (SLICC/ACR) Damage Index (SDI) scores are assessed at each visit. Disease data corresponding with data collection timeframes for each participant were extracted from the mUSC SLE Database to assess the effectiveness of the program. Several differences were observed between the intervention and control groups on symptoms pertaining to lupus activity, and many of these differences had large effect sizes. Our findings can be rapidly translated into improved delivery of health care and targeted trials/interventions with relevance to health disparities, and if widely implemented, morbidities and mortality related to lupus could be drastically reduced in African-Americans.

Objective: Very little is known about the impact of psychosocial stress on African American lupus patients. Due to the exposure of African Americans to a unique trajectory of stressors throughout life, it may be critical to understand the relationship between psychosocial stress and underlying biological mechanisms that influence disease activity and pathology in this high risk group. methods: The Balancing Lupus Experiences with Stress Strategies (BLESS) study piloted the validated “Better Choices, Better Health” Chronic Disease Self-management Program (CDSmP) in 30 African-American lupus patients participating in the SLE Clinic Database Project at the medical University of South Carolina (mUSC). measures of psychosocial and biological indicators of stress were collected in all of the patients in each of the study conditions before and after intervention activities, as well as four months’ post-intervention, to assess the effectiveness of the program in reducing perceived and biological indicators of stress. Results: Participation in the workshops had large effects upon depression (d = 1.63 and d = 1.68), social/role activities limitations (d =1.15), health distress (d = 1.13 and d = 0.78), fatigue (d = 1.03), pain (d = 0.96), and lupus self-efficacy (d = 0.85). Neither the differences in cortisol or DHEA levels pre- and post-intervention were found to be significantly different between intervention participants and controls. Conclusion: The intervention workshops acted to reduce perceived stress and improve quality of life. Our findings imply that comparable, if not more significant gains in relevant health indicators are possible in African American patients when provided the opportunity to participate in CDSmP’s.

OBJECTIVE: We sought to assess the accuracy of endometrial thickness measure me nt in the diagnosis of endometrial cancer in patients with obesity, diabetes, an d hypertension and to evaluate whether patient characteristics influence endomet rial thickness irrespective of the final diagnosis. mETHODS: This was a prospect ive study of women not using hormone replacement therapy who presented with post -menopausal bleeding at 8 hospitals in The Netherlands. All women underwent tra nsvaginal ultrasonography and, in the event that the endometrial thickness (doub le layer) was more than 4 mm, subsequent endometrial sampling. The performance o f endometrial thickness measurement in the diagnosis of atypical hyperplasia and endometrial cancer was evaluated in subgroups of patients with diabetes, hypert ension, and obesity by using receiver operating characteristic analysis. RESULTS : Overall, we included 594 consecutive women, of whom 62 (10%) had endometrial carcinoma and 6 (1%) had atypical hyperplasia. In these women, transvaginal ult rasonography had an area under the receiver operating characteristic curve of 0. 87 (standard error [SE] 0.03). In the absence of (pre) malignancy, women with di abetes or obesity were found to have thicker endometria than women without these risk factors, whereas in women with a (pre) malignancy, this difference was not present. The area under the receiver operating characteristic curve decreased t o 0.74 (SE 0.05) and 0.75 (SE 0.07) in diabetic women and obese women, respectiv ely. The presence or absence of hypertension had no impact on the accuracy of transvaginal ultrasonography. CONCLUSION: In view of the decreased diagnostic accuracy in diabetic women and obese women, the clinical value of transvaginal e ndometrial thickness measurement in these women is questionable.

Pancreatitis in children is less common than in adults and is mostly related t o trauma, infection and anatomical anomalies. Gallstone pancreatitis is an even rarer entity in infants and children. We present an unusual case of gallstone pa ncreatitis in a 17-month-old girl who presented with jaundice of 1 month durat ion. The laboratory studies showed conjugated hyperbilirubinaemia with associate d acute pancreatitis and pseudo-hyponatraemia secondary to the markedly elevate d lipid profile. Further evaluations revealed obstruction at the Ampulla of Vate r with no evidence of a choledochal cyst and no mass lesion seen in the pancreas . The patient initially underwent urgent decompressive cholecystostomy with intr aoperative cholecystography. When the biochemical parameters improved, the patie nt underwent formal cholecystectomy with common bile duct exploration, extractio n of multiple impacted stones in the ampullary region and Fogarty balloon sphinc terotomy. The post-operative course was uneventful. Conclusion:Acute gallstone pancreatitis in children may present as jaundice or abdominal pain. Recognition, early diagnosis and surgical intervention are the mainstay for a good outcome.

[1]On the basis of the multipoint magnetic observations of Cluster in the region 15-19 R downtail,the magnetic field structure in magnetotail current sheet(CS) center is statistically *** is found that the B component(in GSm coordinates) is distributed mainly within |B|nt,while the B component is mostly positive and distributes mainly within 10 *** plane of the magnetic field lines(mFLs) is mostly vertical to the equatorial plane,with the radius of curvature(Rc) of the mFLs being directed earthward and the binormal(perpendicular to the curvature and magnetic field direction) being directed azimuthally *** curvature radius of mFLs reaches a minim.m.R,at the CS center and is larger than the corresponding local half thickness of the neutral sheet,h. In addition to clinical risk markers, indices of left ventricular(LV) systolic function are valuable prognostic markers after acute myocardial infarction(mI). Previous studies have also suggested that LV diastolic function may contribute with prognostic information. The present study assessed whether this assumption applies to a large population of patients with acute mI who underwent thrombolyt ic therapy.520 out of 608 patients participating in the ATTenuation by Adenosine of Cardiac Complications (ATTACC) study, with an ST-elevation acute mI underwe nt two-dimensional and Doppler echocardiographic examination at 4 (range 2-10) days after admission. During the follow-up period of 31 (S.D.±11) months, car diovascular death occurred in 57 (11%) patients, nonfatal acute mI occurred in 77 (15%), and 124 (24%) patients suffered a combined cardiovascular end-point (either nonfatal acute mI or cardiovascular death). Univariate regression analy sis showed that all indices of LV systolic function predicted cardiovascular dea th and combined cardiovascular end-points. Regarding LV diastolic function only a restrictive filling pattern predicted cardiovascular death. In a multistep mu ltivariate regression analysis in which the variables were introduced in a hiera rchic order age, history of systemic hypertension, wall motion score index (WmSi ), and history of previous mI and diabetes mellitus were independent predictors of cardiovascular death. A history of systemic hypertension or congestive heart failure were independent predictors of nonfatal acute mI, while a history of sys temic hypertension, wall motion score index and diabetes mellitus independently predicted combined cardiovascular *** results of this study confirme d that clinical risk indicators and LV systolic function were the most important independent predictors of cardiovascular death and combined cardiovascular end -points. Introduction: Alcohol usage has largely been seen as a risk factor for the development of sexual dysfunction as well as erectile dysfunction. Others have reported that prolonged alcohol usage and abuse is compatible with normal sexual function in the absence of endocrinological problems as well as hepatic dysfunction. About seventy five (75) percent of alcoholics have various sexual difficulties with improvements in sexual functions occurring after treatment of alcoholism and psychosexual therapy. It is evident from the various reports over the years that mild and occasional alcohol usage is not as much implicated in the causation of SD and its other forms as heavy, addictive or dependent alcohol usage. Alcohol usage has also long been linked to hypogonadism, testicular atrophy as well as leydig cell toxicity. Alcohol induced hypogonadism has been reported to resolve after withdrawal of alcohol use. Since both diabetes and alcohol usage have been strongly associated with both hypogonadism and sexual dysfunction, it is logical to expect that diabetics who frequently consume alcohol will have a worsened hypogonadal state and sexual function. This research therefore seeks to provide evidence of an association between alcohol consumption in diabetics and a worsened sexual dysfunction in comparison to diabetics who did not consume alcohol. methods: Type II diabetic patients attending the Diabetic Clinic at the maamobi General Hospital between the periods of January 2010 and march 2011 were consecutively recruited for this study. Diabetics with other known endocrinological diseases and physical disabilities were excluded from the study. Sexual function was assessed using the GRISS-m. Early morning fasting samples were used in lipid and testosterone profile assays. Results: Study participants who consumed alcohol recorded higher levels of triglycerides and LDL-Cholesterol.
