Alpha-fetoprotein (AFP)-producing colorectal cancer is extremely rarely reported until now. All of the reported cases harboring synchronous hematogenous spread including liver and/or lung metastasis had a poor prognosis and died within 12 mo. We here describe a 71-year old man with AFP-producing colon cancer who presented with an unusual bulky lymph node metastasis instead of hematogenous spread. He underwent adjuvant chemotherapy in addition to curative surgical resection, which prolonged his survival.

Primary hepatic leiomyosarcoma is a particularly rare tumor with a poor prognosis. Curative resection is currently the only effective treatment, and the efficacy of chemotherapy is unclear. This represents the first case report of a patient with primary hepatic leiomyosarcoma co-existing with metastatic liver carcinoma. We present a 59-year-old man who was diagnosed preoperatively with rectal cancer with multiple liver metastases. He underwent a curative hepatectomy after a series of chemotherapy regimens with modified FOLFOX6 consisting of 5-fluorouracil, leucovorin and oxaliplatin plus bevacizumab, FOLFIRI consisting of 5-fluorouracil, leucovorin and irinotecan plus bevacizumab, and irinotecan plus cetuximab. One of the liver tumors showed a different response to chemotherapy and was diag-nosed as a leiomyosarcoma following histopathological examination. This case suggests that irinotecan has the potential to inhibit the growth of hepatic leiomyosarcomas. The possibility of comorbid different histological types of tumors should be suspected when considering the treatment of multiple liver tumors.

An Efficient Liver-Segmentation System Based on a Level-Set Method and Consequent Processes

This paper presents an efficient liver-segmentation system developed by combining three ideas under the operations of a level-set method and consequent processes. First, an effective initial process creates mask and seed regions. The mask regions assist in prevention of leakage regions due to an overlap of gray-intensities between liver and another soft-tissue around ribs and verte-brae. The seed regions are allocated inside the liver to measure statistical values of its gray-intensities. Second, we introduce liver-corrective images to represent statistical regions of the liver and preserve edge information. These images help a geodesic active contour (GAC) to move without obstruction from high level of image noises. Lastly, the computation time in a level-set based on reaction-diffusion evolution and the GAC method is reduced by using a concept of multi-resolution. We applied the proposed system to 40 sets of 3D CT-liver data, which were acquired from four patients (10 different sets per patient) by a 4D-CT imaging system. The segmentation results showed 86.38% ± 4.26% (DSC: 91.38% ± 2.99%) of similarities to outlines of manual delineation provided by a radiologist. Meanwhile, the results of liver segmentation only using edge images presented 79.17% ± 5.15% or statistical regions showed 74.04% ± 9.77% of similarities.

Life Adjustments of Elderly Hemodialysis Patients for Continuing to Attend Dialysis Facility

This study aimed to elucidate elderly patients’ recognition of and life adjustment to attending dialysis facilities, and then to discuss factors for enabling such patients to continue attending dialysis facilities. Semi-structured interviews were conducted with 15 participants and interview data were analyzed using the modified grounded theory approach. The study was approved by the Ethical Review Board of Niigata University School of Medicine. The interview data yielded 21 concepts, comprising 2 categories and 7 subcategories. Patients who find themselves in a “Difficult Lifestyle” often try to maintain their health and lifestyle by using required support for hemodialysis patients. Factors for continuing to attend dialysis facilities were the ability to cope with the Difficult Lifestyle and to adjust to Hemodialysis as a Life Priority. It is thought that switching from a model of economic security to one of providing living support will be effective for maintaining elderly dialysis patients’ life with only limited expenses. Additionally, it is necessary to consider the utilization of existing services.

The marine area of Japan, including territorial waters and the exclusive economic zone, is the sixth largest in the world at about 4,470,000 km2. Therefore, it is becoming necessary to establish appropriate means of transportation other than ships in order to utilize the area efficiently. In this respect, ultra-light seaplanes are attracting attention from the viewpoint of protecting the natural environment. Accordingly, JRPS （Japan Reinforced Plastics Society） is currently developing FRP （fiber-reinforced plastic） floats for such planes. In this study, we conducted simulations of seaplane behavior during alighting by using the smoothed particle hydrodynamics method, which is one of the functions in the PAM-CRASH solver, and we present the observed trend in the vertical acceleration of the floats as a first step toward deriving the impact force from analytical data.

Age and Gender Differences in the Step Test with Stipulated Tempos

This study examined differences in tempos of a step test with a stipulated tempo by age levels and gender. Subjects were 316 healthy males and females in the age range of 10 - 80 years. They performed the step test twice for 20 seconds, stepping alternately while a beeping metronome was adjusted to 40 bpm, 60 bpm, and 120 bpm. It was assumed that if the total time discrepancy between the tempos when the foot was grounded was small, people could adjust their steps to different tempos well. The results of a three-way ANOVA showed a significant age difference at 40 bpm. A multiple comparison showed that the total time discrepancy was smaller in 10 - 50 years old than in 60 - 80 years old in males and females. The total time discrepancy at 40 bpm was larger than that at 120 bpm in 10 - 50 years old, and larger in the order of 40, 60, and 120 bpm in 60 - 80 years old. The total time discrepancy showed insignificant gender differences at all ages and tempos. In conclusion, people over 60 years were less able to adjust their steps to a slow tempo than people younger than 50 years. Further, it was more difficult for people under 60 years to adjust their steps to a slower tempo, and a difference in tempos in people over 70 years largely affected their step movements.

Survey of Annual and Seasonal Fungal Communities in Japanese <i>Prunus mume</i>Orchard Soil by Next-Generation Sequencing

Fungi play a vital role in the management of soil environment. Although various fungal communities are found in soil, it is difficult to determine the fungal community structure in soil. In this study, we conducted a comprehensive survey of fungal communities in Japanese Prunus mume orchard soil from 2010 to 2012 growing seasons using next-generation sequencing technology. Fungal DNA was directly extracted from the soil samples and the internal transcribed spacer 1 region was amplified by PCR and sequenced. We identified 34,826 fungal clone sequences from the soil samples. The fungal clones were sorted into 2132 operational taxonomic units and a majority of the discriminated clone sequences were classified as Ascomycota and Basidiomycota. The number of fungal species belonging to Ascomycota showed increases in June in the three growing seasons. That belonging to Glomeromycota showed increases in August in the three growing seasons. As Ascomycota fungi are wood decomposers and saprotrophs, the results suggested that the number of plant pathogenic fungi increased in Japanese P. mume orchard soil in June. These findings show for the first time the annual and seasonal fungal community structures in Japanese P. mume orchard soil, and are expected to provide valuable clues for improvement when planting new P. mume trees in Japanese orchards.

Comparison of strength values and laterality in various muscle contractions between competitive swimmers and untrained persons

Competitive swimmers may gain a specific training effect as the result of long term exercise in the water. This study aimed to compare isometric, non-isokinetic and isokinetic muscle strengths in competitive swimmers and untrained persons. Twelve young male adults without exercise experience for over three years and twelve swimmers with over 10 years of competitive swimming experience performed various strength tests. Non-isokinetic tests were evaluated using one repetition of maximum half squat, vertical jump, and drop jump. Isometric and isokinetic (60 and 180 deg/s) muscle strengths were measured by both legs in knee extension and flexion. There were no significant differences between non-isokinetic and isometric muscle strengths of both groups. On the other hand, all isokinetic parameters in both angular velocities were significantly larger in competitive swimmers. There was significant laterality of isokinetic strength in untrained persons, but not in competitive swimmers. In addition, right and left differences of isokinetic strength tended to be smaller in competitive swimmers. In conclusion, competitive swimmers tended to be superior only in isokinetic strength, which is a similar muscle contraction in the water, and have less right and left differences.

Statins as a Candidate of Drugs for Intracranial Aneurysm Treatment

The treatment for intracranial aneurysm (IA) is socially important because of poor outcome posed by subarachnoid hemorrhage after rupture. Further, the incidence of IAs in general public is high and, indeed, in developed countries many IAs are incidentally found through brain check. However, to date, options for treatment of IAs to prevent rupture are quite limited only to surgical procedures such as microsurgical clipping and endovascular coiling. Taking into account unavoidable risks of complication from surgical interventions and numerous aneurysm careers without treatment, less invasive medical therapies should be established. In human IA lesions, the presence of inflammatory responses, such as expressions of pro-inflammatory mediators and infiltration of inflammatory cells, have been reported, which suggests the involvement of inflammatory responses in the pathogenesis of IA. Recent experimental studies using rodent models have revealed the crucial role of inflammatory responses mediated by NF-κB activation in IA formation and progression and supported the notion that IA is an inflammatory disease affected intracranial arteries. To find out a candidate drug for IA treatments, the effect of several drugs with anti-inflammatory and anti-NF-κB actions on IA progression has been examined using rodent models and revealed the excellent inhibitory effect of statins (HMG-CoA reductase inhibitors) on IA progression. Based on these findings, the case-control study was recently carried out enrolling patients with unruptured or ruptured IAs to examine the effect of statin usage on rupture. This study revealed the significant differences in the ratio of statin usage between two groups and notably the remarkable reduction of risk of rupture of pre-existing IAs under statin usage at the adjusted odds ratio of 0.30.
