Drug-eluting beads transarterial chemoembolization for hepatocellular carcinoma: Current state of the art

Transarterial chemoembolization(TACE) represents the current gold standard for hepatocellular carcinoma(HCC) patients in intermediate stage. Conventional TACE(c TACE) is performed with the injection of an emulsion of a chemotherapeutic drug with lipiodol into the artery feeding the tumoral nodules, followed by embolization of the same vessel to obtain a synergistic effect of drug cytotoxic activity and ischemia. Aim of this review is to summarize the main characteristics of drug-eluting beads(DEB)-TACE and the clinical results reported so far in the literature. A literature search was conducted using Pub Med until June 2017. In order to overcome the drawbacks of c TACE, namely lack of standardization and unpredictability of outcomes, non-absorbable embolic microspheres charged with cytotoxic agents(DEBs) have been developed. DEBs are able to simultaneously exert both the therapeutic components of TACE, either drug-carrier function and embolization, unlike c TACE in which applying the embolic agent is a second moment after drug injection. This way, risk of systemic drug release is minimal due to both high-affinity carrier activity of DEBs and absence of a time interval between injection and embolization. However, despite promising results of preliminary studies, clear evidence of superiority of DEB-TACE over c TACE is still lacking. A number of novel technical devices are actually in development in the field of loco-regional treatments for HCC, but only a few of them have entered the clinical arena. In absence of well-designed randomized-controlled trials, the decision on whether use DEB-TACE or c TACE is still controversial.

Mixed Convection in Viscoelastic Boundary Layer Flow and Heat Transfer Over a Stretching Sheet

An investigation is carried out on mixed convection boundary layer flow of an incompressible and electrically conducting viscoelastic fluid over a linearly stretching surface in which the heat transfer includes the effects of viscous dissipation,elastic deformation,thermal radiation,and non-uniform heat source/sink for two general types of non-isothermal boundary *** governing partial differential equations for the fluid flow and temperature are reduced to a nonlinear system of ordinary differential equations which are solved analytically using the homotopy analysis method(HAM).Graphical and numerical demonstrations of the convergence of the HAM solutions are provided,and the effects of various parameters on the skin friction coefficient and wall heat transfer are *** addition it is demonstrated that previously reported solutions of the thermal energy equation given in[1]do not converge at the boundary,and therefore,the boundary derivatives reported are not correct.

This brief article reviews a very particular and quite narrowfield,namely what has been done and what is needed to know for tannin adhesives for wood panels to succeed *** present fashionable focus on bioadhe-sives has led to producing chemical adhesive formulations and approaches for tannin adhesives as a subject of academic ***,as good and original they might be,are and will still remain a rather empty aca-demic exercise if not put to the test of real industrial trials and industrial *** will remain so without the“little”secrets and techniques outlined here that show that there is a great gap between developing an adhesive formulation in the laboratory and the hard reality to make it work where it does really count,in its industrial *** outlines the fact that even more modern and excellent,newly developed bioadhesive formulations might well miserably fail once tried in the industry if the problems that always arise in their upgrading are not identified and solved,and solved *** also outlines the fact that not only must costs always be taken into account and that a practical and possibly easy-to-handle approach must always be used,but too expensive or complex and unyielding adhesive systems are also often shown to be unusable or unsuitable in industry.

Magnetic field assisted fluidization:A modified Richardson-Zaki equation

Magnetic particles can be uniformly fluidized by coupling the gas flow with an externally imposed magnetic field. Interparticle forces generated by the magnetic field cause aggregation of the particles in chain-like structures preferentially oriented along the magnetic field lines. In the present paper, we study the implications of the formation of these special types of aggregates on the empirical Richardson-Zaki （RZ） equation, originally proposed to describe the expansion of fluidized beds of non-aggregated particles. We have addressed two important issues, namely the flow regime, which is a function of the size of the aggregates, and the effect of shape and orientation of the chain-like aggregates with respect to gas flow on fluid drag. We propose a modified RZ equation （MRZE） in which the velocity scale, given by the terminal settling velocity of the individual aggregates, and the RZ exponent are predetermined as a function of the chain length. The chain length depends on the ratio of the magnetic energy to gravitational energy, and is estimated from the magnetic field intensity, and particle magnetization, size and density. Predictions of the MRZE are successfully compared with published results in the literature on the expansion of magnetic particles in the presence of externally applied magnetic fields.

Phenomenological model for torsional galloping of an elastic flat plate due to hydrodynamic loads

This study investigates the torsional galloping phenomenon, an instability type flow-induced oscillation, in an elastic stru-cture due to hydrodynamic loads into the water current. The structure applied here is a rectangular flat plate with an elastic axis in its mid-chord length. The elasticity is provided by torsion spring. The flat plate has only one degree of freedom which is rotation in pure yaw about its axis. It is observed that as the current speed is higher than a critical velocity, the flat plate becomes unstable. The instability leads to torsional galloping occurrence, as a result of which the flat plate begins to yaw about the elastic axis. By testing two different chord lengths each with several torsion spring rates, the flat plate behavior is investigated and three different responses are recognized. Then, a phenomenological model is developed with the original kernel in the form of the van der Pol-Duffing equa-tion. The model explains these three responses observed experimentally.

Stability analysis of the fluttering and autorotation of flow-induced rotation of a hinged flat plate

This work describes investigations performed on the interaction of uniform current and freely rotating plate about a fixed vertical axis. Fluttering and autorotation are two different motions that may occur during the flow induced rotation. The dimensional analysis proves that the motion in flow induced rotation motion is governed essentially by the dimensionless moment of inertia and Reynolds number. Certain combinations define the stability boundaries between fluttering and autorotation. Fluttering is oscillation of body about a vertical axis and the autorotation is a name given to the case when the body turns continuously about the vertical axis First, the loads and moment coefficients are calculated by experiments and streamline theory for different angles of attack for a fixed fiat plate. Then for dynamic case, a bifurcation diagram is presented based on experiments to classify different motion states of flow induced rotation. Finally, a dynamical model is proposed for stability analysis of flow induced rotation of a flat plate.

Effectiveness of different biochars in remediating a salt-affected Luvisol in Northeast Brazil

Biochar has been proposed as a soil amendment to reclaim salt-affected ***,the evaluation of the different biochar types in these soils is essential due to the significant variation among biochar *** this study,biochars from sugarcane bagasse(SB),orange bagasse(OB),and corncob(CB)were applied alone and in the presence of gypsum(G)to test their potential to remediate a highly degraded saline-sodic soil in Brazil´s Northeast *** that,we conducted a laboratory column-leaching experiment and a greenhouse *** the column-leaching test,we analyzed the sodium adsorption ratio(SAR),the electrical conductivity of the saturation extract(ECe),the concentrations of major ions,and the exchangeable sodium percentage(ESP).Maize plants were grown in the leached soil in a ***-and corncob-derived biochars were more efficient than gypsum in reducing the soil salinity,bringing the indicators below threshold values,and optimizing soil reclamation *** ECe,SAR,and ESP were reduced to 3.42 dS m−1,1.64(mmolc dm−3)0.5 and 4.86%,respectively,in the SB treatment;and to 3.19 dS m−1,0.88(mmolc dm−3)0.5 and 2.53%,respectively,in the CB *** bagasse biochar did not effectively reduce the salinity *** biochar treatments improved seed germination and plant *** SB,CB,SBG,and CBG treatments increased plant height(478–558%),shoot mass(783–983%),and root mass(386–500%),***,the application of SB and CB is an effective alternative to remediate saline-sodic soils and to reduce the impact of soil salinity in the environment.

Second-look surgery plus hyperthermic intraperitoneal chemotherapy for patients with colorectal cancer at high risk of peritoneal carcinomatosis:Does it really save lives?

The treatment of peritoneal carcinomatosis (PC) of colorectal origin with cytoreductive surgery(CRS) plus hyperthermic intraperitoneal chemotherapy (HIPEC) has a 5-year recurrence-free or cure rate of at least 16%, so it is no longer labeled as a fatal disease, and offers prolonged survival for patients with a low peritoneal carcinomatosis index. Metachronous PC of colorectal origin is so predictable that there is a model which has been used to successfully determine the individual risk of each patient. Patients at risk are clearly identified; those with the highest risk have small peritoneal nodules present in the first surgery (70% probability of developing PC), ovarian metastases(60%), perforated tumor onset or intraoperative tumor rupture(50%). Current clinical, biological and imaging techniques still lack sufficient sensitivity to diagnose PC in its initial stages, when CRS plus HIPEC has a greater impact and a higher cure rate. Second-look surgery with HIPEC or prophylactic HIPEC at the time of the first intervention have been proposed as means of preventing and/or anticipating clinical or radiological relapse in at-risk patients. Both techniques have shown a significant decrease in peritoneal relapses and should be considered essential weapons in the management of colorectal cancer.

Strategies for the analysis of single-tree plot experiments in Eucalyptus plantations

Litter production and leaf nutrient concentration and remobilization in response to climate seasonality in the central Amazon

Litterfall is the largest source of nutrients to for-est soils of tropical ***,variability in lit-terfall production,nutrient remobilization,and changes in leaf nutrient concentration with climate seasonality remain largely unknown for the central *** study meas-ured litterfall production,leaf nutrient remobilization,and leaf area index on a forest plateau in the central *** was measured at monthly intervals during 2014,while nitrogen,phosphorus,potassium,calcium and mag-nesium concentrations of leaf litter and canopy leaves were measured in the dry and rainy seasons,and remobilization rates *** area index was also recorded in the dry and rainy *** litterfall varied from 33.2(in the rainy season)to 87.6 g m^(-2) in the dry season,while leaf area index increased slightly in the rainy *** seasonality had no effect on concentrations of nitrogen,calcium,and magnesium,whereas phosphorous and potassium responded to rainfall seasonality *** phosphorous increased,potassium decreased during the dry *** seasons,nitrogen,potassium,and phosphorous decreased in leaf litter;calcium increased in leaf litter,while magnesium remained unaffected with leaf ***,the five nutrients had similar remobilization rates over the *** absence of climate seasonality on nutrient remobilization suggests that the current length of the dry season does not alter nutrient remobilization rates but this may change as dry periods become more prolonged in the future due to climate change.
