AIM: To improve the results of New therapeutic strategies in hepatocellular carcinoma (HCC). We have conducted a phase Ⅱ study with pegylated liposomal doxorubicin (PLD), 5-fluorouracil (5FU) and folinic acid (FA). METHODS: Thirty-one patients with hystologically- confirmed, inoperable HCC, received combination chemotherapy with PLD 25 mg/mq on d 1, 5FU 1200 mg/mq in 48 h continuous infusion, and oral FA 30 mg on d 1 and 2 every 3 wk until disease progression or intolerable toxicity. RESULTS: The median age was 65 years (range 41-82) and 28 patients were hepatitis C virus seropositive (90%). The majority of patients were Child-Pugh Class B (55%). Two patients showed a partial response (PR), and 16 had stable disease (SD). With a median follow-up of 14 mo, the median time to progression of all evaluable patients was 4 mo (95% CI 1.7-7). Median overall survival was 9 mo (95% CI 3-24 mo). After 1 year, 9 of 18 PR/SD patients were alive. Chemotherapy was well tolerated. CONCLUSION: PLD/FU/FA combination seems capable of achieving durable stabilization of HCC. The manageable toxicity supports a role for combination with other anticancer agents.

AIM:To evaluate retrospectively the efficacy of rituximab plus chemotherapy in gastric diffuse large B cell lymphoma(DLBCL).METHODS:Sixty patients(median age:58 years)with histologically confirmed gastric DLBCL treated at four Italian institutions between 2000 and 2007,were included in this *** were selected by stage (Ⅰ-Ⅳ,Lugano staging system),European Cooperative Oncology Group performance status(0-2)and treatment *** strategies were chemotherapy alone(group A,n=30)[scheduled as cyclophosphamide,doxorubicin,vincristine and prednisone (CHOP)and CHOP-like],and chemotherapy combined with rituximab(group B,n=30).The primary end point of the study was complete response(CR)rate;the secondary end points were disease-free survival (DFS)at 5 years and overall survival(OS).RESULTS:Median follow-up was 62 mo(range:31102 mo).We observed a significant difference between the two groups(A vs B)in terms of CR[76.6%(23/30) vs 100%,P=0.04)and DFS at 5 years[73.3%(22/30) vs 100%,P=0.03).To date,19 group A(63.3%) patients are alive and 11 have died,while all group B patients are *** significant differences in toxicity were observed between the two ***:Rituximab in combination with chemotherapy improves CR rate,DFS and *** prospective trials are needed to confirm our results.

Helicobacter pylori (H. pylori) is a widespread pathogen infecting about 40% of people living in urban areas and over 90% of people living in the developing regions of the world. H. pylori is well-documented as the main factor in the pathogenesis of peptic ulcer disease, chronic gastritis, and gastric malignancies such as cancer and mucosa-associated lymphoid tissue-lymphoma; hence, its eradication is strongly recommended. The Maastricht IV consensus, which focused on the management of H. pylori infection, set important new strategies in terms of treatment approaches, particularly with regards to first- and second-line treatment protocols and led to improved knowledge and understanding of H. pylori resistance to antibiotics. In recent years, various fluoroquinolone-based protocols, mainly including levofloxacin, have been proposed and effectively tested at all therapeutic lines for H. pylori eradication. The aim of the present paper is to review the scientific literature focused on the use of fluoroquinolones in eradicating H. pylori.

In recent years,a number of case reports and clinical studies have highlighted the risk of hepatitis B and C virus reactivation in patients with inflammatory bowel disease who are treated with immunosuppressive *** cases of viral hepatitis reactivation that have been reported are characterized by a wide range of clinical manifestations,from viremia without clinically relevant manifestations to fulminant life-threatening *** development and dissemination of biological immunosuppressive drugs have led to a significant increase in the number of reports of interest to physicians in a variety of clinical *** this topic,there have been a number of published guidelines and reviews that have collected the available evidence,providing recommendations on prophylactic and therapeutic strategies and methods for monitoring patients at ***,it should be noted that,to date,very few clinical studies have been published,and most of the recommendations have been borrowed from otherclinical *** published studies are mostly retrospective and are based on very heterogeneous populations,using different therapeutic and prophylactic regimens and obtaining conflicting ***,it seems clear that it is desirable to concentrate our efforts on prospective studies,not conducting further reviews of the literature in the continued absence of new evidence.

Substrate Type Affects Growth, Yield and Mineral Composition of Cucumber and Zucchini Squash

The expansion of hydroponics in many countries of the world in the last few decades may be ascribed to the ability of soilless growing systems to avoid various problems arising from the use of the soil. Cucumber （Cucurnis sativus L.） and zucchini （Cucurbita pepo L.） plants were grown in closed-soilless culture under unheated-greenhouse conditions at the experimental farm of University of Tuscia, Central Italy to evaluate the effects of four substrates （rockwool, pumice, perlite, and cocofiber） on growth, yield and plant mineral composition. For both cultures, plants grown in cocofiber, perlite and pumice yielded more than those grown in rockwool. The better temperature regime in cocofiber, perlite and pumice was due to the greater thermal inertia compared to rockwool slabs. Thc concentration of N in zucchini and cucumber leaves was significantly higher in cocofiber, perlite and pumice in comparison to the rockwool treatment. The concentration of K was significantly affected by the substrate only for the zucchini squash with the highest value recorded on the organic substrate （cocofiber）, whereas the Ca concentration was significantly influenced by the growing media only for cucumber with the highest value observed on pumice. Finally, the lowest Mg concentration in leaf tissue was observed on plants grown with the rockwool substrate for both zucchini squash and cucumber. We can conclude that cocofiber, perlite and pumice are suitable for zucchini and cucumber production in closed soilless system, whereas the use of rockwool is more suitable for crops grown under heated greenhouse conditions.

Optical vortices are structures of the electromagnetic field with a spiral phase ramp about a point-phase singularity,carrying orbital angular momentum(OAM). Recently, OAM has been imprinted to short-wavelength radiation through high-order harmonic generation(HHG), leading to the emission of attosecond twisted beams in the extreme-ultraviolet(XUV) regime. We explore the details of the mapping of the driving vortex to its harmonic spectrum. In particular, we show that the geometry of the harmonic vortices is convoluted, arising from the superposition of the contribution from the short and long quantum paths responsible of HHG. Finally, we show how to take advantage of transverse phase-matching to select twisted attosecond beams with different spatiotemporal properties.

In May 2022, the UK International Health Regulations National Focal Point notified World Health Organization of 176 cases of severe acute hepatitis of unknown etiology in children under 10 years of age. From that moment on, cases of severe acute hepatitis of unknown origin in children began to be reported in several countries. As of June 17, 2022, a total of 991 cases had been reported in 35 countries worldwide, 50 children needed a liver transplant and 28 patients died. According to information published by ECDC, 449 cases have been detected in 21 EU countries. The children were between 1 month and 16 years of age. Adenovirus was detected in 62.2% of the analyzed samples. So far, the cause of these cases is unknown and many hypotheses remain open, but hepatitis A-E viruses and COVID-19 vaccines have been ruled out. A possible hypothesis has been published to explain the cause of these cases of severe hepatitis, according to which it could be a consequence of adenovirus infection in the intestine in healthy children previously infected with SARS-CoV-2. No other clear epidemiological risk factors have been identified to date. Thus, at this time, the etiology of the current cases of hepatitis remains under active investigation.

Molecularly imprinted polymers （MIP） are receiving increasing attention thanks to their robustness, stability, and inexpensive manufacture compared with their bio-analogues such as antibodies. The molecular imprinting process can be defined as the generation of molecular recognition sites in a synthetic polymer. The template-derived sites created within a polymeric matrix allow MIPs （often referred as plastic antibodies） to selectively recognize and bind to the target molecule. Therefore, MIPs can be used in sensors and in separation and diagnostics. Owing to their size and functional properties, MIP nanoparticles （NPs） can potentially be used in biomedicine, but comprehensive analysis of their interaction with cells and in vitro toxicological tests must be performed first. Herein, we report the synthesis of bare and core-shell imprinted NPs using an innovative solid-phase approach and the toxicological evaluation of such NPs in different cell lines （HaCaT, MEFs, HT1080, and macrophages）. We also evaluated the influence of the protein corona on particle stability, the internalization of NPs in cells, and the influence of various surface coatings. Studies on the metabolic effects of imprinted NPs on fibroblasts showed that bare MIPs do not alter cell metabolism, whereas some issues arise when specific particle coatings are used. Furthermore, in vitro cytokine release studies revealed that macrophages were not activated in the presence of the MIPs evaluated in this study. The results suggest that MIP NPs are biocompatible, paving the way for their in vivo application.

This work presents a moving mesh methodology based on the solution of a pseudo flow *** mesh motion is modeled as a pseudo Stokes problem solved by an explicit finite element projection *** mesh quality requirements are satisfied by employing a null divergent velocity *** methodology is applied to triangular unstructured meshes and compared to well known approaches such as the ones based on diffusion and pseudo structural *** of the test cases is an airfoil with a fully meshed domain.A specific rotation velocity is imposed as the airfoil boundary *** other test is a set of two cylinders that move toward each other.A mesh quality criteria is employed to identify critically distorted elements and to evaluate the performance of each mesh motion *** results obtained for each test case show that the pseudo-flow methodology produces satisfactory meshes during the moving process.

As a consequence of the habituation to low levels of dichlobenil （DCB）, cultured maize cells presented an altered hemicellulose cell fate with a lower proportion of strongly wall-bound hemicelluloses and an increase in soluble extracellular polymers released into the culture medium. The aim of this study was to investigate the relative molecular mass distributions of polysaccharides as well as phenolic metabolism in cells habituated to low levels of DCB （1.5 μM）. Generally, cell wall bound hemicelluloses and sloughed polymers from habituated cells were more homogeneously sized and had a lower weight-average relative molecular mass. In addition, polysaccharides underwent massive cross-linking after being secreted into the cell wall, but this cross-linking was less pronounced in habituated cells than in non-habituated ones. However, when relativized, ferulic acid and p-coumaric acid contents were higher in this habituated cell line. Feasibly, cells habituated to low levels of DCB synthesized molecules with a lower weight-average relative molecular mass, although cross-linked, as a part of their strategy to compensate for the lack of cellulose.
